Reliability of the alcohol and drug modules of the Alcohol Use Disorder and Associated Disabilities Interview Schedule--Alcohol/Drug-Revised (AUDADIS-ADR): an international comparison.

The purpose of the present study was to examine the test-retest reliability of the alcohol and drug modules of the AUDADIS-ADR in three sites: Bangalore, India, Jebel, Romania and Sydney, Australia. The overall reliability of ICD-10, DSM-IV and DSM-III-R dependence diagnoses was found to be good to excellent for each substance, including alcohol, for each time frame, regardless of whether the total sample or user subsample figured into the calculations. Reliability associated with corresponding harmful use and abuse diagnoses were mixed, but generally lower. Reliability statistics for Bangalore were generally lower than those of the Jebel and Sydney sites, particularly for alcohol diagnostic criteria. Implications of these results are discussed, in conjunction with results from the discrepancy interview protocol analyses within sites, in terms of future revisions to the AUDADIS-ADR and its training procedures tailored to developing countries.

Antiradiation compounds. 24. Stability of copper (II) complexes of bis(methylthio) and methylthio amino derivatives of 1-methylpyridinium- and 1-methylquinolinium-2-dithioacetic acids in the presence of amino acids, simple peptides, and a protein.

The radiation-protective bis(methylthio) and methylthio amino derivatives of 1-methylpyridinium- and 1-methylquinolinium-2-dithioacetic acids, a series of non-thiol sulfur-containing compounds, have been examined for their ability to complex Cu(II) ion and remain stable in the presence of alpha-amino acids, peptides, and a protein. Copper-binding stability constants were found that were considered sufficiently large to be capable of existence under cellular conditions, with log K beta values in the range of 13-15. Copper-binding constants for alpha-amino acids are comparable to those for the radioprotective ligands, but overall binding constants for simple peptides are usually 10(4)-10(6) lower in magnitude. Essentially no change was found in the UV spectra of the Cu(II) complexes in the presence of alpha-amino acids and simple peptides, and the binding capacity to bovine serum albumin was quite low for two Cu(II) complexes of the best protective ligands, with values of 10(-6)-10(-7) M. The possibility that these radioprotectors may act in vivo as copper complexes, possibly as superoxide dismutase mimics, is thus feasible. The magnitude of the copper-binding stability constants, the lack of copper exchange between the radioprotective ligands and amino acids or peptides, and the very low tendency of the copper complexes of the radioprotective ligands to bind to a protein indicate the potential of the complexes to contribute to the radioprotective ability of the ligands.

Stress fracture of the pubic ramus in female recruits.

During a four-month period we observed 12 stress fractures of the inferior pubic ramus in 11 military recruits undergoing basic training. Eleven of these were in women. This high number was considered to be caused by the introduction of mixed training of male and female recruits; this forces women to increase their stride length when marching. The presenting symptom was chronic groin pain which failed to settle with rest, and the clinical diagnoses were confirmed by radiographs in all but one patient who was diagnosed by 99mTc bone scanning. Since the required stride length has been reduced there have been no new cases of stress fracture of the pelvis.

Patterns of attractor dimensions of sleep EEG.

Low dimensional chaos is a property of many physiological oscillatory systems including the brain. Time series of sleep EEG records have been analyzed in the framework of recent developments in nonlinear dynamics. One of the characteristics of a chaotic time series is its attractor dimension. The running attractor dimension of a chaotic time series may reflect changes in states more accurately than manually scored records. In the present study the attractor dimensions of consecutive EEG segments of five sleep records were analyzed. The block of the EEG segment (window) was shifted by various lengths along the entire sleep data of each subject thus producing a running attractor dimension curve for each record. The attractor dimension values for different sleep stages were significantly different. The pattern of the running attractor dimension closely matched the scored hypnograms in these five sleep records.

Climate and admissions for mania in the tropics.

Seasonality of admissions for mania has been commonly reported. Most reports are from temperature zones. We analyzed admissions for mania covering a 9 year period and looked for any seasonal trends using existing methods. No seasonal variation was found. The hospital lies in the tropical zone with a relatively constant climate. This may influence the seasonal pattern of relapses in affective disorder.

Seasonal relapses in affective disorder in the Tropics: a prospective follow-up of 12 patients.

Seasonal relapses of affective disorder are known. We report 12 patients who had season-linked relapses during a prospective follow-up period of 4 years. There were both winter and summer relapses of mania and depression. The centre is in the tropical zone, with lesser variation of sunshine and temperature than in more extreme latitudes. This may inference the pattern of relapse in affective disorder. Differences in relapses between tropical and temperate zones need to be investigated.

A few points in the management of juvenile nasopharyngeal angiofibroma.

A few points based on our experience on 100 cases of nasopharyngeal angiofibroma are described, which are found very useful in the successful removal of nasopharyngeal fibroma. A new technique for the control of haemorrhage by a muscle graft immediately after the removal of an angiofibroma is advocated.

Valvular glottic obstruction following extubation.

A case of glottic valvular obstruction following extubation is reported. The cause was bilateral dislocation of arytenoids due to coughing on the tracheal tube, which resolved automatically following a further bout of coughing.

Zonal mitotic response in the adrenocortical tissue of the juvenile female duck to sex hormones.

The zonal response of the adrenocortical tissue to sex hormones was evaluated in the female duckling by mitotic (with colchicine) and histological studies. Estrogen (1 mg per bird daily for 10 days), progesterone (1 mg per bird daily for 10 days) or estrogen plus progesterone (0.75 mg plus 0.25 mg respectively per bird daily for 10 days) selectively inhibited mitosis in the central zone of the adrenocortical tissue. Histologically, estrogen or estrogen plus progesterone produced hypertrophy in the subcapsular zone, while progesterone caused atrophy in the central zone of the cortex. It is suggested that both the sex hormones have zonal action mitotically as well as histologically in the adrenocortical tissue of the female duckling.

Lignocaine pretreatment for suxamethonium. A clinicobiochemical study.

Lignocaine pretreatment (2 mg/kg) significantly restricted the increase in serum potassium and decrease in serum calcium caused by suxamethonium. Suxamethonium muscle pains occurred in only 8% of patients who received lignocaine just before induction of anaesthesia. The incidence of muscle pains was 45% in those patients who were not given lignocaine.

Calcium gluconate pretreatment for prevention of succinylcholine-induced myalgia.

The effects of calcium gluconate pretreatment on succinylcholine-induced postoperative muscle pain and on the changes in serum potassium and calcium levels were studied in 80 patients undergoing elective surgery who were to become ambulatory on the same evening. A test group of 40 patients was given 10 ml of 10% calcium gluconate intravenously immediately before induction of anesthesia with thiopental followed by succinylcholine for tracheal intubation. A control group of 40 patients was given thiopental and succinylcholine without calcium gluconate. Muscle pains were graded according to severity as nil, mild, moderate, and severe. The incidence of postoperative muscle pains was significantly less (5%) in the test group than in the control group (45%). In control patients who subsequently developed muscle pain, serum levels of potassium were significantly increased above preinduction levels 2 min after succinylcholine was administered and serum calcium levels were significantly decreased. In control patients who did not develop postoperative myalgia, there were no significant changes in serum levels of either calcium or potassium. This was also true for patients pretreated with calcium gluconate. Therefore, calcium gluconate pretreatment reduces both the increase in serum potassium and the decrease in serum calcium associated with succinylcholine, and decreases the incidence and severity of postoperative myalgia.

Age-related changes in the orientation and particle size of the mineral phase in human femoral cortical bone.

It has previously been observed that after the third decade of life the tensile strength of bone decreases faster than its density [1], indicating that some microstructural change occurs in bone with advancing age. In order to try to elucidate this observation, the orientation and particle size of the mineral phase of bone have been studied with respect to age. Samples were taken from femora varying in age from 13 to 97 years. The results show that the degree of preferred orientation does not vary with age after the first decade of life. However, the percentage of small particles increased up to the fourth decade of life and thereafter decreased, a pattern of change similar to that observed for bone strength. It would seem to indicate that an increase in mineral particle size is a factor that contributes to the loss of bone strength and that an increased percentage of bone strength and that an increased percentage of large particles in the later years of life could, along with osteoporosis, help to explain the increased fragility in the bones of the elderly.
