Lipoma of the sylvian region.

An exceptional case of lipoma of the sylvian region is described and the other seven reported cases are reviewed. The sylvian fissure is the most rare site of intracranial lipomas. Sylvian lipomas may be asymptomatic or present with epileptic seizures due to irritation of the cortex of the sylvian fissure. Only two previous cases have been diagnosed during life by computerized tomography and operated on; the deep location of these lipomas and their adherences to the sylvian cortex and the branches of the middle cerebral artery make radical removal impossible and dangerous.

Hearing loss associated with progressive ataxia (Lichtenstein-Knorr disease?). Report of a sporadic case with peculiar neuroradiological findings.

A sporadic case of hearing loss with ataxia, clinically suggesting Lichtenstein-Knorr disease, is described. Magnetic Resonance (MR) shows an involvement of the white matter, which appears diffusely reduced and abnormal, together with a peculiar tickening of the grey matter. The possible relationships with leukodystrophies and with generalized cortical dysplasias are discussed. To our knowledge, MR imaging has never been performed in Lichtenstein-Knorr disease.

Steroid-induced disappearance of primary central nervous system lymphoma. Clinical, neuroradiological and pathological findings.

A 73-year-old man presented a progressive neurologic syndrome with impairment of multiple cranial nerves, ataxia and limb weakness. Neuroradiological evaluation showed multiple supra and infratentorial lesions, which completely disappeared after steroid treatment on two occasions. These neuroradiological modifications were closely correlated with clinical picture and CSF findings. Post-mortem examination showed multifocal primary CNS immunoblastic large cell lymphoma. Immunohistochemistry showed that the tumor was of B-cell origin.

Effectiveness of a single injectable dose of bromocriptine long acting in the treatment of macroprolactinomas.

The aim of the present study was to investigate the effect of a new galenic injectable form of bromocriptine (Parlodel LA), characterized by sustained release of bromocriptine for 4-6 weeks, in 3 patients with prolactin-secreting macroadenomas through a 42-day follow-up. The following parameters were considered: decrease of plasma prolactin levels, reduction of tumor size studied by seriate CT scan controls and recovery of eventual visual fields impairments. Parlodel LA induced a sharp decrease of plasma prolactin levels for at least 42 days, without reaching normal values. This treatment also caused a rapid and marked shrinkage of tumors in all patients. Moreover, in two patients with constriction of visual fields a clear and progressive improvement in one and only a slight amelioration in the other were observed. There were almost no side effects except for mild nausea in one patient and transient orthostatic in another. These results demonstrate that Parlodel LA produces a long-lasting reduction of plasma prolactin levels and a rapid shrinkage of the tumor with almost no side effects.

[MR vs CT: which is the diagnostic advantage in neuroradiology].

The comparative adequacy was studied of the two main neuroradiological imaging methods, MR imaging and CT, through the analysis of 1500 MR examinations with a preliminary CT study. The comparison was made by giving a score to the diagnostic contribution of MR imaging, especially as far as changes in the therapeutical approach were concerned. The results varied according to the various pathologies, to the different anatomical regions explored, and to the site of the masses. The overall score was higher in the spine and the spinal cord than in brain pathology. In brain neoplastic pathology, eg, the diagnostic capabilities of CT and MR imaging were similar in most cases, especially in supratentorial locations, while MR imaging was clearly superior in the spinal cord district. In infratentorial location, the better anatomical details make MR imaging the method of choice. In conclusion, CT confirms its accuracy in the detection of brain pathology, while MR imaging proves more sensitive in white matter diseases, and in particular locations, e.g. the posterior fossa. In the spine and spinal cord districts, the very high average score mainly confirms MR capability to demonstrate spinal canal normal and pathological content. Moreover, the important role of MR imaging is stressed in pediatric pathology, together with CT primary role in emergency cases.

Cerebral edema associated with meningiomas.

Fifty patients with intracranial meningiomas have been retrospectively examined, and the cerebral edema on computed tomography scan has been correlated with the clinical evolution, size, location, and histological features of the tumor. The degree of brain edema was found to be related to the clinical evolution and the size of the tumor, whereas the histological features were less significant. These results are discussed after reviewing the data of five other series in the literature of this subject. A correlation between the tumor steroid receptors and edema is suggested.

Craniopharyngioma of the cranial base and nasopharynx.

A rare case of craniopharyngioma extending to the cranial base and nasopharynx is reported and eight other cases in the literature are reviewed. The embryology and the clinical features of these tumors are discussed. Tomograms of the skull and computed tomography are the most useful radiological tools of investigation. A nasoseptal or transpalatal approach to surgery, which has been performed in reported cases of cystic nasopharyngeal craniopharyngiomas, was not performed on our patient because of the hard consistency and the diffusely infiltrating aspect of the tumor.

A computerised tomographic scan study of Luque loops and the progression of vertebral fusion in the surgical treatment of scoliosis.

The authors report the results of a study, using CT scan, on 15 patients with scoliosis treated by the Harrington-Luque method. The patients were monitored periodically from the 3rd to the 30th month after operation. Each patient was monitored only once in order to limit the risk of ionizing radiation. In all the follow-ups it was observed that the metallic loops were in close contact with the laminae, which did not show any signs of disturbances. The medullary canal appeared to be of normal dimensions. Vertebral fusion was evident at the first 3 months follow-up and at this time partial corticalization also commenced. In successive follow-ups, fusion progressed steadily, and after 8 months it was completely corticalized. This progression continued even after 18 months so that at 30 months the fusion was such that the distractor was completely surrounded.

[Experimental giant arterial aneurysm. Resection and reconstruction of the arterial wall].

The authors describe 3 experimental techniques for the treatment of giant arterial aneurysm, based on removal of the aneurysmal pouch and reconstruction of the arterial wall. The aneurysm is realized with a venous graft implanted with end-to-side anastomosis on the common carotid artery on the rabbit. In the first experimental model it is resected along its basis; the arterial wall is reconstructed by means of a patch of dural substitute (dacron coated with silicone rubber) fixed with biological glue (human fibrine). In the second model, the experimental aneurysm is hooded with a venous graft sutured to the adventitia of the artery with four angular stitches and biological glue. The artery is then clamped, the aneurysm is resected along its basis. The median incision in the venous graft is sutured, restoring the continuity of the arterial wall. In the third model, a small artery is implanted at the bottom of the aneurysmal sac, realizing a condition similar to a collateral vessel arising from an aneurysm. In this case, too, the aneurysm is hooded by a venous graft fixed only to the adventitia of the arterial wall with the above mentioned technique. The main and the collateral vessels are clamped, and this latter is severed; the aneurysm is resected along its basis; the median incision in the venous graft is sutured, restoring the continuity of the arterial wall; the collateral vessel is re-implanted along the suture line. These techniques aim to realize the reconstruction of the arterial wall without stenosis and shortening the clamping time.(ABSTRACT TRUNCATED AT 250 WORDS)

Cystic lesions associated with meningiomas.

Three cases of meningiomas associated with large cysts are reported, and 58 other cases from the literature are reviewed. The etiology of these cysts is analyzed on the basis of surgical and pathological findings. The preoperative diagnosis of large computed tomographic hypodensities associated with meningiomas and the differential features from other cystic tumors are discussed. The surgical aspect of the solid tumor and particularly the absence of a capsule separating the tumor from the cyst are usually decisive in confirming the intratumoral nature of the cyst. In questionable cases a histologic evaluation of the cyst walls may be necessary.

Arachnoid cyst of the quadrigeminal cistern.

Arachnoid cysts of the quadrigeminal region are rare. We report two new cases. The first is a 29-year-old woman, who had a recurrence five years after sub-total removal of the cyst wall. Initially it was communicating and was treated by a ventriculo-atrial shunt; later it became noncommunicating and direct drainage of the cyst cavity was required. The second was a 25-year-old female, successfully treated by wide removal of the cyst wall thus creating a large communication with the subarachnoid space. Diagnosis of these lesions, as regards either morphological or functional features, nowadays rests largely on CT. Choices about treatment are extremely difficult. The most radical one, namely, complete removal of the capsule, is not without risks and disadvantages. More prudent methods, such as partial removal of the capsule and/or drainage of the cyst and the ventricles, seem on the whole to be preferable, but careful pre- and post-operative evaluation of CSF dynamics are essential for a satisfactory and long-lasting outcome.

Handicap and rehabilitation: two types of information upsetting family organization.

This article aims to show that the birth of a physically handicapped child should be regarded as powerful information, compelling the family to reorganize. The three subsystems most frequently challenged by the onset of a handicap are examined: the extended family, the couple, and the siblings. Further potent information involving the whole family arises in the rehabilitation program by placing demands on parents that may radically upset the existing equilibrium, and inevitably entailing a highly significant relationship with the child's therapists. Rehabilitation staff should be adequately trained to monitor the various phases during which family members adjust to both these types of information (handicap and rehabilitation) in order to ward off the development of dysfunctional games. A family therapy approach is appropriate in the all too many cases in which a rigid dysfunctional game has already set in such that the handicapped member is playing the role of identified patient.
