[End-to-side anastomosis made with a short circulatory interruption in the receiving vessel. Experimental model in the rat].

End to side microsurgical anastomoses between vessels of small calibre require interruption of blood flow of about 25-40 min. In terminal vascular beds this time may be incompatible with the cellular survival. The authors refer their experience in rats with a partly original experimental technique requiring less time of clamping the receiving vessel (about 3-4 min), as the main part of the anastomosis is performed between the wall of the donor vessel and the adventitia of the receiving vessel, without interruption of blood flow in this latter. Histological examinations, performed 21 days later, reveal a patency rate of 60%.

[Fogging effect in the ischemic cerebral infarction to computerized tomography (author's transl)].

We have examined, with systematic serial computerized tomography (CT) studies, 6 patients with persistent ischemic cerebral infarction. The CT of the ischemic cerebral infarction shows, during the third week, an isodense area. We have called this phenomenon "fogging effect" because at this effect and the CT report could be negative. The possibility of a false negative can be avoided by a control study with contrast medium: the infarction has an intensive contrast enhancement.

[Myelography with B 15000, a new nonionic water-soluble contrast medium. Technical aspects and preliminary clinical experiences (author's transl)].

This is a report of myelography findings obtained in 50 patients with B 15000 (Iopamidol), a new non ionic water-soluble contrast medium. The procedure was easily carried out with a tilting table wired to a TV image amplifier. The dose of contrast medium injected intrathecally was 10 divided by 15 cm3 of a preconstituted solution containing iodine 200 or 300 mg/cm3. There were no accidents or major complications. The myelograms obtained with the test material afforded accurate diagnosis of intramedullary and extramedullary lesions, and signally of intervertebral disk protrusion and hernia.

Primary sellar chondromas.

Two cases of chondromas arising primarily from the sella turcica are reported. The eight cases reported in the literature are reviewed. The basic clinical and radiological findings of these tumors and the difficulties of diagnosis and treatment are discussed.

The process of identity in twins.

The Blacky Picture Test was administered to a sample of 12 pairs of male MZ twins, aged 17 to 19 years, and to an equal number of singletons. A score for 30 factors was assigned to each protocol. By comparing the scores of the two groups using the Mann-Whitney U test, only one significant difference emerged, relative to the factor concerned with positive identification and fraternal relationship. Twins chose the sibling figure as the identification object significantly less often than singletons did. A more qualitative elaboration was undertaken in order to interpret this result. From the final data, the precariousness of the sense of identity in the twins comes out indirectly in their passive and conformistic acceptance of the paternal model. It would seem that the twin is induced by his relationship with his cotwin, seen as the origin of this uncertainty as to his own identity, to adhere closely to the paternal image.

Hemangioma of the temporal muscle.

BACKGROUND: Hemangiomas are benign vascular tumors. Because less than 1% of all hemangiomas are intramuscular, only 8 cases of temporal muscle hemangioma have been described to date. This is a case study of a 13-year-old girl who was referred to our institution because of a soft swelling located in the left temple that has enlarged progressively since birth. METHODS: CT scan, angiography and MRI showed a tumor mass lying in the temporal muscle, with homogeneous contrast enhancement. No tumor blush or feeding arteries were detected. At surgical exploration, the tumor appeared to be well demarcated. It was totally excised, sparing the surrounding temporal muscle, which did not present any sign of infiltration. Histopathologic examination showed the lesion to be a cavernous hemangioma. RESULTS: The cosmetic result was excellent, and MRI after 1 month and 2 years showed complete absence of the lesion and no evidence of recurrence. CONCLUSIONS: Although this type of tumor may be treated by various methods surgical excision yields the best results in the short and the long term. The surrounding tissue is spared as much as possible when no signs of infiltration are noted at operation, especially when involving small and functionally important muscles, as in our case.

Magnetic Resonance angiography of the pulmonary veins before and after radiofrequency ablation for atrial fibrillation.

PURPOSE: To study the usefulness of magnetic resonance angiography (MRA) in imaging of the pulmonary veins (PV) before and after radiofrequency ablation procedures in patients with atrial fibrillation. MATERIALS AND METHODS: Between July 2002 and April 2003, 50 patients with atrial fibrillation underwent MRA prior to ablation; 18 patients also underwent post-procedure MRA. Images were acquired with 3D-spoiled gradient echo sequences after intravenous administration of the paramagnetic contrast medium gadopentetate dimeglumine; an automatic triggering device was used to start the angiographic sequence (Smartprep, General Electric Medical Systems). Postprocessing was performed with maximum intensity projection (MIP) and virtual endoscopy (VE) software (Navigator, GEMS). RESULTS: The venoatrial junction was visualised with MRA VE in 49 of 50 patients (98.0%). Twenty-seven patients out of 49 (55.1%) had two PV ostia on both sides, 13 (26.5%) had two right ostia and a single common left ostium, 5 (10.2%) had supernumerary PV and 4 (8.2%) had both a supernumerary right PV and a single common left ostium. Flythrough navigation showed the number and spatial arrangement of second-order PV branches in 48 out of 49 patients (98.0%). In postablation examinations, mild stenosis was detected with MIP and VE in 17 out of 83 PV examined (20.5%). CONCLUSIONS: This study confirms the clinical value of magnetic resonance imaging for visualising PV ostia in patients undergoing radiofrequency ablation for atrial fibrillation. Before the ablation procedure, MRA allows an accurate evaluation of PV number, shape and size; after the procedure, MRA is useful in screening for post-ablation stenosis and describing the location and severity of stenosis when present.

Intracerebral haemorrhage: criteria for prognosis on the grounds of clinical and CT data.

The Authors report analyses on 83 cases of brain haemorrhage which were conducted over a period of 6 years. On the basis of previous studies made by the same authors, a selection was made from patients whose survival index rating did not exceed 8.1. These patients were treated only with medical and reviving therapies. In this work we attempt to formulate some considerations regarding the prognosis of brain haemorrhage on the basis of clinical and tomodensitometric data.
