[Gastrointestinal stromal tumors: clinical considerations].

Gastrointestinal stromal tumors (61ST) are an heterogeneous group of non epithelial tumors of the gastrointestinal tract. They are peculiar to extreme cellular variability and uncertain malignancy. Gist are rare tumors that arise from primitive mesenchymal cells located in all gastrointestinal tract. Till now they are object of discussion about their origin, diagnostic standards, prognostic factors, histopathological classification. They are more frequently in over 40 years old people without difference in two sex, but they can appear in the child too and in the young man suffering from HIV. The authors relate two cases of recent observation, and discuss on the biological behaviour of these rare tumors.

Anastomosis, Surgical↗

[Pilonidal sinus: an retrospective analysis of 205 cases].

Sinus pilonidalis or the sacrococcigea cyst, normally is considered a "small surgical pathology" and actually it is controversial disease regarding its pathogenesis and treatment. Today the congenital hypothesis of this pathology is not considered. The etiopathogenesis exclude vestigia embryonic origin and consider it an inflammatory disease. Traumatism of the sacral region, hirsutism, hormonal secretion stimulating, and furrow sacral furrow cutaneous, explain the prevalence of this pathology in the male sex and in the puberale age. It usually appears after the pubertà and extension one various frequency of presentation in males sex (1.3%) regarding females (0.11%). The symptomatology can be rare or introduce itself in violent and intense way with on abscess of the region, or with only or multiple fistulas orifices from which exit materialclarly purulent. Normally cystic cavity is one, situated in the subcutaneous tissue with annexed inflammatory phenomena. This cavity to times communicates with the skyn fistulating. The treatment of election is surgical and must weighted and be regulated in relation to the entity of the lesion and its inflammatory picture. Sometimes the medical therapy with anti-inflammatory and antibiotic is not control the symptomatology and it need the surgical treatment.

Abrikossoff's tumour: report of a rare case in anal perianal region.

The authors report a rare case of a 47-year old man suffered from Crohn's disease and depression with multiple prominent lesions in the anal and perianal region. The biopsy of these lesions showed the presence of Abrikossoff's tumour. This tumour is very rare in the anal region and usually the lesions are small (0.5-3 cm) and solitary. The authors report this case because they considered it an interesting case for the localization and the appearance of the lesions.

[The tension-free hernioplasty for primary inguinal hernias: personal experience].

The Authors report their experience in the treatment of primary inguinal hernias using Trabucco's tension-free technique. Since April 1998 to June 2002 four hundred and fifteen patients (399 men and 16 women) were treated in our Department for primary inguinal hernia. The results demonstrate the efficacy of this technique in reducing post-operative complications and in preventing relapses. The reduction of costs is related to a shorter hospitalization and an earlier return to work.

[Intraparotid lesions in HIV infection].

HIV-related lesions of the parotid region. A case of parotid enlargement in a 43-year-old male with unsuspected HIV infection is reported. Lesions consisted in lymphonodal changes, epi-myoepithelial islands and cysts lined by squamous epithelium. It is suggested that it is possible to recognize an unsuspected HIV infection, when the first clinical involvement is in the parotid region.

Acquired Immunodeficiency Syndrome↗

[Maternal-fetal transmission of infection with hepatitis B virus: evaluation of viral markers in maternal and fetal biological materials and relation with the vaccine response].

From July 1984 to September 1987, 981 women at third trimester of pregnancy were screened for HBsAg. 26 women were identified as being HBsAg carrier. The study of HBV markers and anti-HBV antibodies was conducted on these women and their offspring to evaluate the presence of intrauterine infection, and the newborns response to passive active immunization in relationship to their markers status during pregnancy. HBsAg, HBeAg, anti-HBe and anti-HBc were assayed on the plasma drawn form the mother, on the amniotic fluid drawn by transabdominal amniocentesis and on funicolar blood samples drawn immediately after delivery. IgM anti-HBc were assayed on amniotic and funicolar samples. HBsAg, anti-Hbc and anti-Hbe were present in 42.8%, 100% and 50% of amniotic samples; whereas the percentage of the same markers in funicolar samples were 50% for HBsAg and 100% for anti-HBc and anti-HBe. In no amniotic or funicolar samples were IgM anti-HBc antibodies present. Anti-HBs, anti-HBc and anti-HBe were assayed on the newborns at 2, 16, 12, 18 months to evaluate the response to immunization. Response to passive-active immunization was protective in all newborns independently from their antigenic status during intrauterine life. Anti-HBc antibodies were cleared within 18 months from delivery, while anti-HBs got a protective title within 6 months from delivery, persisting in 88.8% of cases at 18 months.(ABSTRACT TRUNCATED AT 250 WORDS)

Solitary left ventricle metastasis by renal cell carcinoma with sarcomatoid features.

A 64 year-old male presented with a mass of the left kidney, 9 cm across. A radical nephrectomy was performed and the pathological examination of the surgical specimen revealed a large papillary renal cell carcinoma (RCC). Five weeks after surgery, the patient died because of progressive cardiac failure. At autopsy, a whitish-gray subendocardial mass, measuring 6 cm in its main diameter, was discovered in the left ventricle. Histologically, the tumour, consisted of interlacing bundles of spindle cells, showing large vesicular nuclei, with prominent nucleoli. Rare gland-like structures lined by neoplastic cells were occasionally found. Neoplastic cells were focally immunoreactive to anti-cytokeratin and anti-epithelial membrane antigen antisera. The diagnosis was that of cardiac involvement by RCC with sarcomatoid features. Cardiac metastases by RCC are rare and their incidence ranges from 1.3% to 4.2%. In the present case, the sarcomatoid appearance of the cardiac lesion raises the problem of the differential diagnosis with primary cardiac sarcoma. The clinico-pathological features of this case are discussed and the literature on this topic is reviewed.

The "Tor Vergata" epidemiological blood pressure study. Hereditary and environmental factors.

The aim of this study was to evaluate the influence of hereditary and/or environmental factors on blood pressure patterns. The study group consisted of 1002 individuals. Comparison of blood pressure values among groups was performed by the Bravais-Pearson r correlation coefficient. Results among the different groups were: p < 0.001 in the parents-offspring group (n = 413) for both systolic (SBP) and diastolic blood pressure (DBP); p < 0.001 in the father-offspring group (n = 382) for SBP only; p < 0.001 in the mother-offspring group (n = 389) for both SBP and DBP; NS in the father-son group (n = 199) for any blood pressure value; p < 0.01 in the father-daughter group (n = 183) for SBP only; p < 0.001 in the mother-son and daughter groups (n = 201 and n = 188 respectively) for both SBP and DBP. Family aggregation analysis disclosed p < 0.01 and p < 0.05 for SBP and DBP respectively in the mother-her sibling group. No significant values were found in the father-his sibling group or between offspring and either their paternal or maternal uncles/aunts. Conversely, a significant correlation (p < 0.001) was found between spouses (fathers and mothers) for both SBP and DBP. These results support the hypothesis that both hereditary and environmental factors exert their influence on blood pressure patterns. We performed both univariate and bivariate statistical analyses. The latter included a) variable separation, b) independent variables, and c) variable correlation. Bivariate analysis of variable correlation proved to be the most suitable.

[Primary volvulus of the small intestine: vascular-like acute abdomen].

The Authors discuss etiology, clinical picture, diagnostic and therapeutic possibilities of intestinal volvulus, an uncommon disease in Europe, thinking of a case of primitive small intestine volvulus, recently observed, and considering the literature. The Authors have come to the conclusion that in all the cases of intestinal occlusion, in emergency hospitalization, it is important to suspect the intestinal volvulus and to operate on the patient urgently to avoid the raise of postoperative mortality in all the cases complicated with intestinal gangrene.
