Do the tumor cells of hepatocellular carcinomas dislodge into the portal venous stream during hepatic resection?

BACKGROUND: The current study was undertaken to investigate whether or not tumor cells are dislodged into the portal venous stream during hepatic resection for hepatocellular carcinomas. METHODS: A catheter was placed using echo guidance into the portal branch through the mesenteric vein in 31 patients. Cytologic examinations were done on multiple blood samples at various operative stages. RESULTS: Tumor cells were recovered in 7 of 31 patients in whom the tumor sizes were more than 5 cm and portal invasions were found microscopically and/or macroscopically. By contrast, the remaining 24 tumors were less than 5 cm in size and showed negative portal invasions. Recovery of the tumor cells was found, not during the earlier operative stage of mobilization or rotation of the hepatic lobe, but during the later stages of hilar dissection or hepatic parenchymal dissection. CONCLUSIONS: The portal pedicles should be divided before hepatic dissection in segmentectomy and lobectomy to lessen the chance of dissemination of intravasated tumor cells.

Diagnosis of cerebral amyloid angiopathy by enzyme-linked immunosorbent assay of cystatin C in cerebrospinal fluid.

An abnormally low level of cystatin C in the cerebrospinal fluid is a diagnostic marker for the hereditary form of brain hemorrhage associated with amyloidosis that was first identified in Iceland. We developed an assay for cystatin C to use in the diagnosis of patients with cerebral amyloid angiopathy and brain hemorrhage. This test consists of a sandwich enzyme-linked immunosorbent assay using monoclonal mouse anticystatin C and polyclonal rabbit anticystatin C antibodies. The cystatin C level was assayed in cerebrospinal fluid samples from 29 patients with brain hemorrhage and 45 control patients with other neurological diseases. Fifteen patients with brain hemorrhage showed low cystatin C levels (less than or equal to 70 ng/ml) in a clinical setting in which the positive and negative findings were compatible with a diagnosis of cerebral amyloid angiopathy. Immunohistological examination of brain tissue obtained by biopsy from two of the 15 patients confirmed the diagnosis of cerebral amyloid angiopathy and identified the deposition of cystatin C and beta-protein. This enzyme-linked immunosorbent assay is simple to perform and may be useful for investigating patients suspected of having cerebral amyloid angiopathy with brain hemorrhage and the deposition of cystatin C.

Prognostic factors after hepatectomy for hepatocellular carcinomas. A univariate and multivariate analysis.

The current study determines the prognostic factors after hepatectomy for hepatocellular carcinomas. The 295 patients who underwent hepatectomy from 1973 through 1987 were included for a univariate and a Cox multivariate analysis. The favoring conditions were determined as follows. The essential requirements are (1) the absence of tumor thrombi; (2) no intrahepatic metastasis, but even when present, it should be close to the main tumor and removed with a massive resection; and (3) retention rate of indocyanine green dye (ICG) at 15 minutes should be within 14 +/- 4.2% (M +/- SD) to allow that resection. The desired requirement is that the tumor size should preferably be less than 5 cm; a wider free margin from tumors (greater than or equal to 1 cm) is recommended, but not determining factor. The eligible patients, having no thrombi, no intrahepatic metastasis, a tumor size of 5 cm or less, negative surgical margin (greater than or equal to 1 cm), had achieved a 5-year survival of 78%. In conclusion, resection therapy is the first option for patients with those requirements.

[Assessment of alveolar bone changes with gamma-ray absorptiometry--on the analysis of detectability of a measuring system].

It is important to determine the changes in alveolar bone during periodontal treatments. At present, radiography is widely used to determine the changes. But small changes in alveolar bone cannot be detected on X-ray films. To detect these small changes, a direct observation system using gamma-ray from 133Ba was considered. Using a multi-channel analyzer, gamma-ray absorption through the bone was detected in this method. This method was compared with densitometric measurement on the films using sliced animal bone. The newly developed method detected the bone changes more accurately. To evaluate the influence of soft tissue, Mix-D was used in both measurements. 133Ba absorptiometry showed that soft tissue did not influence the measurement more than the densitometric method.

[Diagnosis of cerebral amyloid angiopathy with cerebral hemorrhage by enzyme linked immunosorbent assay (ELISA) of cystatin C in the cerebrospinal fluid].

The lower level of cystatin C in cerebrospinal fluid (CSF) is one of the useful diagnostic markers of hereditary cerebral hemorrhage with amyloidosis in Iceland. We attempted to establish an assay to determine the level of cystatin C in CSF for diagnosis of CAA due to the deposition of cystatin C in CSF for diagnosis of CAA due to the deposition of cystatin C. We carried out the sandwich enzyme immunosorbent assay with the use of monoclonal mouse anti-cystatin C and polyclonal rabbit anti-cystatin C antibodies. CSF from nine cases of cerebral hemorrhage and fifty reference cases with other neurological diseases were examined. Four patients with cerebral hemorrhage showed a low level of cystatin C and clinical manifestations suggestive of CAA. Our study showed the feasibility of using ELISA for the diagnosis of cerebral amyloid angiopathy that causes cerebral hemorrhage with the deposition of cystatin C.

Fast-growing root nodule bacteria produce a novel polyamine, aminobutylhomospermidine.

Polyamines in various root nodule bacteria including Bradyrhizobium japonicum, Rhizobium fredii, R. leguminosarum, R. meliloti and R. loti were identified by capillary gas chromatography. Homospermidine was the polyamine present in highest concentration in all the rhizobia tested. In addition to putrescine and homospermidine, fast-growing type of rhizobial cells contained a novel polyamine, aminobutylhomospermidine, NH2(CH2)4NH(CH2)4NH(CH2)4NH2. The unusual tetraamine was not found in the cells of slow-growing type of rhizobia throughout their growth period, indicating a difference in polyamine metabolism between fast-growing type and slow-growing type of root nodule bacteria.

[Criteria of curability in the resection therapy for hepatocellular carcinoma].

The present study was aimed at determining a criteria for curability of hepatic resection in the treatment of hepatocellular carcinomas. The 239 patients hepatectomized from 1973 through 1987 were included for analysis. The patients with the tumor thrombi(V) or intrahepatic metastasis(IM) scattered over the unilateral lobe(IM2) showed very poor prognosis in spite of macroscopically complete resections. Consequently, these conditions were categorized into non-curative group(NC). The 5-year survival rate of the tumors less than 2 cm was 42%, which was much better than that of 5 cm or more group but no better than that of 2 to 5 cm group. When the prognosis was compared by an extent of resection among the patients with the tumors of less than 5 cm in size and V(-), the patients with lobectomy showed a 5-year survival rate of 84%, much better than 45% of those with segmentectomy or less range. The surgical margin (SM) negative patients showed a better prognosis than the SM positive patients with a slight difference. The tumor backgrounds of the 39 patients who survived three years with disease free condition were as follows: V(-) in all, no intrahepatic metastasis(IM0) in 79%, no IM2, massive resection for all IM1 in whom IM was confined to the area close to the main tumors, a tumor size of 5 cm or less in 82%, SM(-) in 62%. From the findings described above, we defined a complete resection for V(-) and IM0-1 as a curative condition, of which absolute curative resections(AC) corresponded to the patients with the tumors of 5 cm or less and SM(-), and relative curative ones(RC) to the remainders.(ABSTRACT TRUNCATED AT 250 WORDS)

Correlation between carotid blood flow and brain atrophy in patients with multi-infarct dementia.

The relation between carotid blood flow measured by Doppler spectrum analysis and brain atrophy on computed tomography (CT) was studied in 22 subjects with multiple lacunar cerebral infarctions. The subjects were divided into two groups, 7 patients with multi-infarct dementia (MID) (mean age 73 years) and 15 nondemented lacunar stroke subjects (NDLS) (mean age 66 years), according to DSM III criteria. All subjects had a score of 7 points or more on Hachinski's ischemic score and showed no carotid artery stenosis. Systolic peak frequency of the common carotid artery (CCA) was measured by Doppler spectrum analysis (Angioscan II). Brain atrophy was measured quantitatively on CT images by two-dimensional measurement using a digitizer. Peak frequencies were lower in MID than in NDLS. Brain atrophy was more severe in MID than in NDLS. There was a significant correlation between peak frequencies and brain atrophy in all subjects. These results indicate that CCA blood flow may reflect brain function in patients with multiple lacunar infarctions.

[Lobar intracerebral hemorrhage secondary to cerebral amyloid angiopathy: a clinicopathologic study of three operated cases].

Three operated cases of lobar intracerebral hemorrhage (LICH) related to cerebral amyloid angiopathy (CAA) were studied clinicopathologically. They constituted about 8% of all LICH cases (n = 37) operated upon in our institute (DUSM) during the past 3 years. Case 1, 2 and 3 aged 71, 67 and 73 years, respectively. There were 2 males (Cases 1 & 2) and 1 female (Case 3). Only one case (Case 3) had both hypertension and dementia before hemorrhage. In all 3 cases, neurologic symptoms deteriorated after admission. The hematoma involved the right temporo-parietal in 1 (Case 1), the right parieto-occipital in 1 (Case 2) and the left fronto-parietal region in 1 (Case 3). Case 1 developed a new hematoma in the right occipital lobe on the day following surgery. On CT, the hematoma was multilobular in shape and located very superficially extending to the subarachnoid space in all cases. There was no abnormal enhancement in and around the hematoma upon contrast infusion. Angiography showed only an avascular mass sign in case. At surgery, the hematoma was extruded onto the cortical surface in all cases. The surgical outcome was good in 2 (Cases 1 & 2) and fair in 1 (Case 3). Removed hematomas, solid nodular tissues and adjacent brain tissues were examined histologically using hematoxylin and eosin, Azan-Mallory, elastica van Gieson, silver and Congo red stains. Arteries in the hematoma wall, the subarachnoid space and the adjacent brain parenchyma were intensely stained with Congo red and showed birefringence on polarized light.(ABSTRACT TRUNCATED AT 250 WORDS)

Formation of two major nicotine metabolites in livers of guinea pigs.

Using antibody against NADPH-cytochrome P-450 reductase and several effectors of cytochrome P-450 and FAD-containing monooxygenase, we investigated nicotine metabolites formed by these two enzymes. When [3H]nicotine was metabolized by the combination of liver microsomes of guinea pigs and partially purified aldehyde oxidase, three distinct spots corresponding to nicotine, cotinine and nicotine-1'-oxide were observed on fluorograms of thin-layer chromatography. Antibody against NADPH-cytochrome P-450 reductase inhibited the formation of cotinine but not nicotine-1'-oxide. Metyrapone and n-octylamine inhibited the cotinine formation, while methimazole prevented the formation of nicotine-1'-oxide. These results show that microsomal electron transport systems participate in the formation of nicotine-1'-oxide and strongly suggest the involvement of FAD-containing monooxygenase in the formation of nicotine-1'-oxide.

Determination of 15NH3 by gas chromatography-mass spectrometry. Application to the measurement of putrescine oxidation by human plasma.

A convenient and sensitive method for the determination of 15NH3 has been developed. Ammonia was purified from sample solutions by a modified microdiffusion method, derivatized with pentafluorobenzoyl chloride to pentafluorobenzamide (PFBA) and determined by gas chromatography-mass spectrometry using a multiple ion detector. PFBA was eluted from the gas chromatographic column within 2 min and resulted in a simple mass fragmentation pattern. The 15N/14N ratio was accurately determined with picomole amounts of PFBA by measuring the molecular ions of PFBA and [15N]PFBA. The method was applied to the assay of putrescine oxidation by human plasma. 15NH3 was produced by incubating 15N-labelled putrescine with plasma. The 15NH3 production was time dependent and strongly inhibited by the addition of aminoguanidine, an inhibitor of diamine oxidase. Exceedingly high 15NH3 production from [15N]putrescine was observed in the plasma from pregnant women. In contrast, only trace amounts of 15NH3 were formed in the plasma from normal men and non-pregnant women. The method seems to be applicable to various biological systems that produce ammonia as a metabolic product.

Distribution and Metabolism of sym-Homospermidine and Canavalmine in the Sword Bean Canavalia gladiata cv Shironata.

The unusual polyamines, sym-homospermidine (homoSPD) and canavalmine (CAN), were found in the seed of Canavalia species such as C. gladiata, C. ensiformis, and C. brasilensis, but not in those of other leguminous crops. To examine the distribution and metabolism of homoSPD and CAN in sword bean, C. gladiata cv Shironata, polyamine analysis was carried out throughout the life cycle of this plant. During seed germination, putrescine (PUT), spermidine (SPD), and spermine (SPM) were accumulated in the radicle and hypocotyl. HomoSPD and CAN were, however, maintained at very low levels over a 6-day period of germination. In nodulated sword bean plants, a large quantity of homoSPD was found in the root nodule. CAN was detected exclusively in the senescent nodule at very low concentrations. These polyamines were not detected in any other organs including root, stem, leaf, vine, flower, and pod, while PUT, SPD, and SPM were always found in those organs. As plants reached the reproductive stage, homoSPD and CAN appeared in the immature seed and their concentrations increased as seed formation progressed. By contrast, the level of SPM continuously decreased during seed development. In developing seeds, considerable accumulation of canavanine, an analog of arginine, which is a precursor in polyamine biosynthesis, was also observed.
