Somatostatin-like immunoreactive axon terminals on oxytocin-like immunoreactive neurons in the paraventricular nucleus of the rat hypothalamus.

By double label immunohistochemistry, we studied the synaptic relation between somatostatin (SS) and oxytocin (OT) or vasopressin (VP) neurons in the paraventricular nucleus (PVN) of the rat hypothalamus. In the light microscopic observation, SS-immunoreactive (ir) fibers were seen very frequently in contact with OT-ir neurons, but not with VP-ir neurons. Immunoelectron microscopic analysis revealed that somata and their dendrites of OT-ir magnocellular neurons were in synaptic contacts with SS-ir axon terminals. No SS-ir synaptic terminals were found on VP-ir magnocellular neurons. The results suggest that OT-containing magnocellular neurons in the PVN are under regulatory influences of SS-containing neurons.

Therapeutics of X-linked hypophosphatemic rickets.

X-linked hypophosphatemia, the most common form of familial rickets, is conventionally treated with 1,25-dihydroxyvitamin D3 (5-50 ng/kg per day) plus phosphate supplementation (70-100 mg/kg per day). However, nephrocalcinosis is noted in many children treated with this therapy. Whether to treat or not and whether such treatment should be continued into adulthood or in pregnancy are unsettled questions. This article reviews these controversies and provides current recommendations.

Mutual synaptic associations between neurons containing neuropeptide Y and neurons containing enkephalin in the arcuate nucleus of the rat hypothalamus.

We determined reciprocal synaptic associations between neurons containing neuropeptide Y (NPY) and neurons containing enkephalin-8 peptide (Enk-8) in the arcuate nucleus (ARC) of the rat hypothalamus by using a double-staining immunocytochemical method. Animals treated with an intraventricular administration of colchicine and animals treated with a hemicomplete cut of the medial basal hypothalamus were used. We principally labeled NPY with silver-gold particles and Enk-8 with diaminobenzidine (DAB) chromogen but in part labeled reversely the two antigens to obtain better visualization of Enk-8 cell bodies. Neurons immunoreactive for NPY were found in the mediobasal area, while those for Enk-8 were located somewhat laterally. Their fibers, however, were densely distributed throughout the ARC in a similar fashion. Electron microscopically, we found synaptic junctions between axonal terminals of Enk-8 neurons and perikarya of NPY neurons and between axonal terminals of NPY neurons and perikarya of Enk-8 neurons. In addition, we found synaptic junctions between immunoreactive Enk-8 cell bodies and processes and also between immunoreactive NPY cell bodies and processes. These interneuronal associations may contribute to the interaction of information coming in or going out from this nucleus.

Bone marrow findings in malignant histiocytosis and/or malignant lymphoma with concurrent hemophagocytic syndrome.

We examined bone marrow specimens from 19 patients with malignant histiocytosis (MH) and/or malignant lymphoma (ML) with concurrent hemophagocytic syndrome (HS) who suffered from high fever, hepatosplenomegaly, liver dysfunction, profound cytopenia, and erythrophagocytosis. There was little lymph-node enlargement or no tumor formation. The neoplastic cells in 3 patients exhibited histiocytes/macrophages phenotype with positive reactions for fluoride-sensitive nonspecific esterase, lysozyme and CD68 (KP1). Twelve other patients showed a T-cell (CD3) phenotype, in which 5 patients expressed CD30 (BerH2) as well. B-cell characteristics with CD20 (L26), CIg. nu lambda and gamma kappa were manifest in 2 patients, but indeterminate markers were found in the 2 remaining patients. Eighteen patients showed an infiltration of large neoplastic cells mainly with noncohesive interstitial growth pattern, ranging from 1.7% to 74.2% of the nucleated cells in the bone marrow. A large number of histiocytes/macrophages and dendritic cells was diffusely observed in 15 patients. Severely decreased hematopoiesis in all three series of hematopoietic cells was found in 16 patients. Bone marrow infiltration by the neoplastic cells and numerous reactive cells with erythrophagocytosis appears to be an important factor of profound cytopenia in patients of MH and/or ML with HS. The infiltrating pattern of the neoplastic and reactive cells in the bone marrow of MH and/or ML with HS was different from that of other types of peripheral T-cell ML, B-cell ML in high grade malignancy, and Hodgkin's disease. Cell characteristics and lineage of the neoplastic cells in MH and/or ML with HS are also discussed in this study.

Autopsy findings in 47 cases of adult T-cell leukemia/lymphoma in Miyazaki prefecture, Japan.

To identify factors that might improve the prognosis of adult T-cell leukemia/lymphoma (ATL), we reviewed data on 47 autopsied cases of ATL with reference to the complications and cause of death. The primary cause of death was respiratory insufficiency due to pulmonary infection. Respiratory insufficiency was also attributed to the diffuse alveolar damage and pulmonary fibrosis resulting from chemotherapy given and oxygen. About 90% of the cases had infections with one or more pathogens. Cytomegalovirus (CMV) was the most frequent pathogen involved in 35/47 (74.5%) while fungal infections were also commonly seen in 25 of the 47 cases (53.2%). Of these, 17 (70%) had pulmonary aspergillosis. Other neoplasias were present in 10 of the 47 cases, while hypercalcemia was evident in 21 patients. These findings suggest that the prevention and treatment of nosocomial infections and of drug-induced pulmonary toxicity may improve the prognosis and quality of life of ATL patients.

Sinonasal malignant lymphoma of natural killer cell phenotype associated with diffuse pancreatic involvement.

We report a case of sinonasal lymphoma with a natural killer (NK) phenotype. This 40-year-old man was admitted to our hospital because of left nasal obstruction. Physical examination and computed tomography of the skull revealed a tumor in the left nasal cavity and maxillary sinus. Histopathological examination revealed a diffuse proliferation of pleomorphic lymphoid cells. Imprint cytology showed that tumor cells contained some azurophilic granules, and expressed CD2, CD8, CD16, CD56 and HLA-DR antigens with little expression of other lymphoid or myeloid markers. Southern blot analysis revealed germline configuration for immunoglobulin heavy chain and T-cell receptor genes. These findings indicated that these cells were in fact NK cells. The patient's enlarged pancreas was also involved by lymphoma and the pattern of involvement simulated that seen in primary pancreatic lymphoma. Ulcerative colitis (UC) was also present, a rare finding in this disorder.

[Autonomic innervation of feline laryngeal glands: an immuno-electronmicroscopic study].

Autonomic innervation of feline laryngeal glands was investigated immunoelectronmicroscopically using antisera for vasoactive intestinal polypeptide (VIP), neuropeptide Y (NPY), and tyrosine hydroxylase (TH). A relatively dense population of immunoreactive (ir) VIP fibers was found in the vicinity of the basement membranes of glandular and myoepithelial cells. Some VIP -ir fibers contacted the basement membrane and some of the fibers penetrated the adjoining glandular cells intercellularly without making synaptic contacts with them. Ir-NPY and -TH fibers were less abundant than VIP-fibers and were distributed beneath the basement membranes of myoepithelial cells. These fibers were not encountered between glandular cells. The estimated ratio of ir-VIP, -TH and -NPY fibers was 20:4:1, based on the density of fibers in glandular tissues. Because this value was equal between serous and mucous glandular cell, these two tissues may receive the same pattern of autonomic innervation.

Neuronal associations in the rat suprachiasmatic nucleus demonstrated by immunoelectron microscopy.

The synaptic associations of neurons in the suprachiasmatic nucleus (SCN) of rats were examined by single immunolabeling for somatostatin (SRIH) and arginine vasopressin (AVP), and double immunolabeling for SRIH plus AVP and vasoactive intestinal polypeptide (VIP) plus AVP. Single immunolabeling showed that SRIH neurons, which displayed some somatic and dendritic spines, formed synaptic contacts with immunonegative and positive axon terminals. AVP neurons also formed synaptic contacts with both immunonegative and positive axon terminals. The immunonegative terminals contained small, spherical clear vesicles or flattened clear vesicles. A few immunopositive AVP fibers made synapses with immunonegative somatic or dendritic spines. Double immunolabeling showed synaptic associations between SRIH axons and AVP cell bodies or dendritic processes, and between AVP axons and the somata or dendrites of SRIH neurons. These findings suggest a reciprocal relation between the two types of neurons. Synaptic contacts between AVP neurons and VIP axon terminals were also demonstrated. Previously, we found synapses between SRIH axons and VIP neurons. Thus SRIH neurons appeared to regulate AVP and VIP neurons. On the basis of these findings, two possible oscillation systems of the SCN are proposed.

Synaptic associations between oxytocin-containing magnocellular neurons and neurons containing corticotropin-releasing factor in the rat magnocellular paraventricular nucleus.

In the paraventricular nucleus (PVN) of the rat hypothalamus, we determined synaptic associations between oxytocin (OXT)-containing magnocellular neurons and parvocellular neurons containing corticotropin-releasing factor (CRF) by using a double immunolabeling technique in 7 animals. In single vibratome sections of the hypothalamus, immunoreactive CRF and OXT were labeled with silver-gold particles and diaminobenzidine (DAB) chromogen, respectively. By light microscopy CRF-containing fibers appeared to be black dots, some of which encircled magnocellular perikarya labeled with brown DAB chromogen in the PVN. By electron microscopy we discriminated OXT neurons having fine DAB-chromogen particles distributed throughout the cytoplasm and on large secretory granules from CRF neurons having dense coarse particles of silver-gold. Occasional CRF axons terminated on perikarya or dendritic processes of OXT neurons, making synaptic contacts. The terminals which were characterized by having clusters of small clear vesicles and a few dense core vesicles showed equal thickenings of pre- and postsynaptic membranes at the synaptic junctions.

Immune status of children on continuous ambulatory peritoneal dialysis.

Immunological parameters including serum IgG, IgA and IgM, lymphocyte phenotypes (CD3, CD4, CD8, HLA-DR+CD3-), natural killer cell activity and lymphocyte proliferation with phytohaemagglutinin were assessed in 10 children on continuous ambulatory peritoneal dialysis (CAPD) and 10 control subjects. Live vaccines were injected into 6 of the 10 children on CAPD (4 had a combined measles-mumps-rubella vaccine and the other 2 mumps vaccine). Serum antibody titres to these viruses were measured before and after vaccination. The serum IgG level was statistically lower in the CAPD group than in the control group (P less than 0.01), but there was no difference in the percentage of HLA-DR+CD3- cells and in the ratio of CD4 to CD8 between the two groups. There were no differences in the other parameters between the two groups. All of the 6 vaccinated children seroconverted, and serious side effects were not noted. Our results suggest that children on CAPD have no significant immune impairment.

Human erythropoietin in children undergoing continuous ambulatory peritoneal dialysis.

The effect of intravenous recombinant human erythropoietin (rHuEPO) by two different dosage regimes was evaluated in 12 anemic children on continuous ambulatory peritoneal dialysis (CAPD). Eight patients (group A) were treated once a week at a mean dose of 89 U/kg for 24 weeks. The other four patients (group B) were treated three times a week at a mean dose of 260 U/kg for 8 weeks, and then once a week at a mean dose of 88 U/kg for the next 16 weeks. In group A, the hematocrit (Ht) increased gradually and significantly from 19.9 +/- 2.1% to 28.7 +/- 1.5% at the 24th week, while in group B, it rapidly increased from 17.9 +/- 2.5% to 30.2 +/- 4.3% at the 8th week, and thereafter was maintained at a level of approximately 30%. However, there was no significant difference in the Ht between the two groups. In two of the four patients in group B, blood pressure increased from 136/87 mmHg to 168/102 mmHg and from 106/70 mmHg to 132/92 mmHg, at the 4th and the 8th weeks respectively, related to a rapid rise of the Ht. Neither antibody to rHuEPO nor other sides effect were evident. Gradual improvement of anemia by once-a-week intravenous administration of rHuEPO is advantageous for the quality of life in children on CAPD.

[A case report of an acute promyelocytic leukemia patient showing remarkable thrombopoiesis prior to granulopoiesis by hM-CSF treatment after chemotherapy].

We report a case of an acute promyelocytic leukemia patient who showed remarkable thrombopoiesis prior to granulopoiesis by human macrophage colony-stimulating factor (hM-CSF) treatment after chemotherapy. A 50 year-old man was diagnosed as acute promyelocytic leukemia (FAB classification: M3) with t(15;17). He received two courses of induction therapy with daunorubicin and cytarabine followed by consolidation therapy with cytarabine and mitoxantrone. After each course of chemotherapy. hM-CSF (8 x 10(6) units, daily) was given for 14 days. After each treatment with hM-CSF, the increase in number of platelets preceded increase of granulocytes and reticulocytes. When serum levels of granulocyte-macrophage-colony stimulating factor (GM-CSF), granulocyte-colony simulating factor (G-CSF), interleukins-3 (IL-3) and -6 (IL-6) were measured, only that of G-CSF was increased after hM-CSF treatment.

[Clinical evaluation of bone marrow fibrosis in non-Hodgkin's lymphomas].

A study was performed in 62 patients with non-Hodgkin's lymphoma (ATL included) who were hospitalized between 1985 and 1991 and who underwent bone marrow aspiration and biopsy before or after treatment or at the time of recurrence. The relationships between the extent of bone marrow fibrosis and other prognostic factors as well as the effects of chemotherapy and bone marrow fibrositic prognosis were examined. The results were as follows: 1) The periods of survival in patients with high degree of fibrosis in bone marrow were significantly shortened. 2) The extent of bone marrow fibrosis significantly correlated with the presence or general symptoms, bone marrow invasion, and blood levels of LDH and Ca on blood biochemical examinations. 3) Effective therapy reduced collagen fibers as well as reticulin fibers in bone marrow. 4) Sixty-four percent of relapsing cases showed increase of bone marrow fibrosis. These results suggest that understanding the state of bone marrow fibrosis over the clinical course may give a good guide of indication the prognosis of malignant lymphoma.

Existence of mutual synaptic relations between corticotropin-releasing factor-containing and somatostatin-containing neurons in the rat hypothalamus.

Light microscopic studies of vibratome sections, which were double-immunostained for corticotropin-releasing factor (CRF) and for somatostatin (SS), suggested the presence of reciprocal synaptic relations between neurons containing immunoreactive (ir) CRF and those containing ir SS in the parvocellular paraventricular nucleus (parvo-PVN) and in the anterior periventricular area (APV) of the rat hypothalamus. In the sections the peptides included in neuronal fibers were labeled black with silver-gold particles, and the peptides included in neuronal cell bodies were labeled brown with diaminobenzidine (DAB). Thereby the brown cell bodies appeared to be surrounded by several black nerve terminals. In electron microscopic studies, the labeling was mostly performed in reverse fashion, because of the convenience for observing the ultrastructural details of the nerve terminals. The neuroplasm of the postsynaptic perikarya and dendrites was labeled with gold-coated silver grains, while the presynaptic axonal terminals were shown with scattered DAB particles. Granular structures in the perikarya or axonal terminals were labeled distinctively. The synaptic morphology appeared to be either symmetric or asymmetric connections. Then we found synaptic connections between presynaptic ir SS containing fiber terminals and postsynaptic ir CRF containing perikarya in the parvo-PVN, and those ir CRF containing fiber terminals and ir SS containing perikarya in the APV. The existence of such a reciprocal association between CRF and SS neurons may suggest that these neuronal systems intervene among different functional systems in the hypothalamus.

Asymptomatic isolated microhaematuria: natural history of 136 children.

In a mass screening programme, 251 children with isolated microhaematuria were detected. Of these 251 children, 115 were excluded from the study because of microhaematuria secondary to a specific cause. The remaining 136 children were diagnosed as having asymptomatic isolated microhaematuria (ASH). Of these 136 children, 23 had evidence of urinary abnormalities in their family members. Red blood cell casts were evident in 31 children at their initial visit or during the follow-up period. Ten children had one or more episodes of macrohaematuria during the study. Renal biopsy was performed in 19 children because of indications of glomerular disease, and 13 of these 19 children had mild to moderate glomerulonephritis. None of these 136 children developed hypertension or renal impairment after a mean period of 7.4 years (range 6-13 years). Thirty-five children had normal urinary findings within 6 years of their initial visit, and 100 have had persistent microhaematuria without proteinuria throughout the follow-up period. The other child had microhaematuria with proteinuria greater than 1 g/m2 per day at the end of the study. This study suggests that the prognosis of ASH is good, and that renal biopsy is not indicated for children with ASH.

Genotypic and cytogenetic study of acute myelocytic leukemia and chronic myelocytic leukemia in blast crisis: specific delta rearrangement pattern does not involve J delta gene locus.

We have analysed the configuration of immunoassociated genes and the karyotypes of 30 patients with acute myelocytic leukemia (AML) and 10 with chronic myelocytic leukemia in blast crisis (CML-BC). In AML, the frequencies of T-cell receptor (TcR) beta, gamma, and delta chain and immunoglobulin heavy and light chain gene rearrangements were 4.2%, 19%, 8%, 10.7% and 10.5%, respectively. In CML-BC, they were 10%, 20%, 40%, 50% and 0%, respectively. Nine patients had abnormalities in chromosome 2, 7 or 14, upon which immunoassociated genes are located. There seems to be no apparent relationship between these chromosome abnormalities and gene rearrangements. In all patients but one (5/6), the delta rearrangement was accompanied by other immunoassociated gene rearrangements. Molecular size analysis revealed specific delta rearranged band(s) (19.5 kb-BamHI and/or 6.9 kb-EcoRI), as commonly detected in B-acute lymphocytic leukemia (ALL). All the patients with the delta rearranged band, however, had a germline configuration of J delta gene loci, suggesting a DD or V(D)D (probably V delta 2(D)D) pattern. This study also indicates that the delta rearrangement is specific in AML or CML-BC and distinct from that in early T leukemia/lymphoma.
