[Relationship between human papillomavirus and oncogenes (c-myc, N-myc) amplification in human cervical cancers].

The human papillomavirus detection and oncogenes amplifications were studied on DNAs from fifteen cervical cancers. We detected HPV16 and HPV18 using Southern blot hybridization and polymerase chain reaction (PCR) technique. The positive subjects of HPVs were eight cases (53%) observed by Southern blot hybridization and fourteen cases (93%) by PCR technique. The gene amplifications of oncogenes (c-myc and N-myc) were analysed by slot-blot method and were observed in c-myc but not in N-myc. The "LARGE" gene amplification (more than five fold) in c-myc was observed in one case (7%) and the "SMALL" gene amplifications (less than five fold) were observed in six cases (40%) in human cervical cancers. Although one of five cases (20%) with HPV16 was present c-myc gene amplification, all of three cases (100%) with HPV18 were found c-myc gene amplifications. In two out of three cases obtained more than three fold c-myc gene amplifications, HPV were not detectable. It is suggested that the negative correlation between gene amplification and numbers of HPV copies exist in advanced cervical cancers.

Identification of the intracytoplasmic region essential for signal transduction through a B cell activation molecule, CD40.

CD40 is a 45-kDa glycoprotein expressed on human B lineage cells. Anti-CD40 induces the proliferation of B cells and the extracellular region of CD40 is related to those of a certain kind of growth factor receptors. Therefore, it has been proposed that CD40 might be a receptor for a molecule involved in the growth regulation of B cells. The cDNA coding for CD40 was transfected into the murine B lymphoma cell line M12 and the murine thymoma cell line EL4. The growth of both M12 and EL4 transfectants expressing human CD40 was inhibited by anti-CD40. Phorbol 12-myristate 13-acetate (PMA) augmented the growth inhibitory effects of anti-CD40 on transfectants. The CD40 molecule was constitutively phosphorylated not only in human tonsil B cells but also in transfectants expressing CD40. PMA augmented the phosphorylation of CD40 in these cells. These results indicate that in spite of the growth inhibitory effect of anti-CD40, the augmentative effect of PMA is conserved in CD40+ transfectants and suggest that the transfectant might be useful for the study of signal transduction mechanism through CD40. To investigate which part of the CD40 molecule is important for signal transduction, transfectants expressing mutant CD40 cDNA were established and their growth response to anti-CD40 was evaluated. The mutant molecule, which had an Ala for Thr substitution at position 234, and the deletion mutants lacking Thr234 were inactive in growth signal transduction, indicating that Thr234 itself or the region around Thr234 is essential for signal transduction through CD40.

Measurement of digital arterial pressure in patients with essential hypertension.

In 8 normotensive volunteers, digital blood pressure was determined, and the value was compared to direct brachial arterial pressure and to that measured by a conventional, indirect Korotkoff-method in the upper arm. The correlation coefficient was determined by changing blood pressure with intravenous administration of vasoconstrictors, sublingual nitroglycerin, and stress tests. Although the absolute value of the digital blood pressure was slightly lower than direct arterial pressure, the correlation between the two determinations was highly significant. The difference in readings between direct and digital arterial pressure was not significant even when the pressure was fluctuated with administration of vasoactive drugs and stress tests. In 2 of 6 volunteers the photosignals of the digital arterial pulses became too small to measure pressure 30 min after they entered in a cold room. However, the digital/brachial blood pressure difference did not alter even when the signals became small during the cold exposure in the remaining 4 subjects. Afterward, the subjects visited our outpatient clinic, where their digital blood pressure was taken. The value was compared with the indirect, contralateral brachial blood pressure. The absolute difference between the readings increased significantly when hypertension advanced. Among the patients (WHO, stage I) treated with antihypertensive drugs, the digital/arm pressure gap was the greatest in patients treated with beta-adrenergic blockers, diuretics and calcium channel blockers in decreasing order. These results indicate that digital blood pressure is influenced by the severity of hypertension and drugs; i.e., hypertensive vascular injury and/or atherosclerosis lowers the peripheral blood pressure leading to impaired peripheral tissue perfusion in the hypertensive patient, and beta-adrenergic blockers may deteriorate peripheral circulation in spite of their hypotensive effects.

[Ultrasonographic evaluation of advancement of uterine endometrial carcinoma; comparison with postoperative histological findings].

Preoperative ultrasonographic examinations of endometrial carcinoma have been performed in 47 patients in Tokushima University Hospital since 1976. The size of intrauterine echo masses, cervical involvement and thickness of uninvolved myometrium were measured by longitudinal and transverse scan. The results were compared with postoperative histological findings. The results were as follows; 1) Endometrial echoes were identified in 94% (44/47). Echogenic masses which were characteristic of endometrial carcinoma were observed in 79% (37/47). 2) The average size of echogenic masses in pT1 carcinoma was significantly smaller than in more advanced cancers (pT2 or more). Especially when the area (longitudinal x anteroposterior diameter) of a mass was under 500 mm2 and the volume (longitudinal x anteroposterior x transverse diameter) was under 10,000 mm3, the probability of pT1 carcinoma was significantly high. 3) Eighty-seven percent (13/15) of cases with and 81% (22/27) of cases without cervical involvement of endometrial carcinoma were correctly diagnosed by ultrasonography. 4) Extrauterine extension of the tumor was found in 63% (5/8) when the thickness of the uninvolved myometrium measured by ultrasonography was under 3mm, whereas when the thickness was over 3mm, extrauterine extension of the tumor was found in only 19% (5/27). From these data, ultrasonography was effective for the evaluation of the advancement of endometrial carcinoma, especially cervical involvement, myometrial invasion and extrauterine extension.

[Clinical investigation on the involvement of an endogenous digitalis-like substance in blood pressure regulation].

We assessed the role of circulating digitalis-like substance(s) on the blood pressure regulation in patients with essential hypertension, cardiac diseases, diabetes mellitus and renal diseases by measuring digoxin-like immunoreactivity (DLI). Plasma DLI concentrations tended to correlate with blood pressure in all patient groups. Plasma DLI correlated to plasma aldosterone concentration in patients with essential hypertension, which suggested close interrelationship between DLI and electrolytes metabolism with adrenal steroids. Serum immunoreactive insulin (IRI) levels significantly correlated with blood pressure. Because plasma DLI levels correlated with serum IRI, increased levels of insulin could have induced sodium retention leading to increased DLI levels. Digitalis-like substance, but not insulin, would have directly increased blood pressure in patients with abnormal glucose tolerance. Plasma DLI levels significantly correlated with the severity of renal insufficiency in patients with renal diseases. Plasma DLI highly correlated with amounts of plasma proteins, particularly with albumin, which would be due to the binding of DLI with albumin in plasma. Because the level of non-binding DLI is extremely low when assayed with a digoxin-radioimmunoassay, it was impossible to assess the level of a free-form of DLI, i.e., active DLI. That could be a reason why the correlation between the DLI and the other parameters was not highly significant. Collectively, these findings suggest that the DLI is one of the major determinants of blood pressure rises, regardless of any cause.

[Usefulness of radial artery graft bypass: two cases of internal carotid artery aneurysms].

Trapping and EC-IC bypass surgery was performed for two cases of internal carotid aneurysm using the radial artery as bypass material. Neither case has any permanent neurological deficit because of an adequate blood supply via the radial artery graft bypass. Although the radial artery graft has been in common use among cardiac surgeons, it is still rare in the neurosurgical field. The advantage of the radial artery is discussed with comparison to other graft materials such as saphenous vein and superficial temporal artery. In properly selected cases, the radial artery graft is efficient in preventing the ischemic damage caused by trapping of aneurysm at the main arterial trunk.

Thickness of uninvolved fibromuscular stroma and extrauterine spread of carcinoma of the uterine cervix.

The minimum thickness of cervical fibromuscular stroma remaining uninvolved with invasive cervical carcinoma was examined in relation to pelvic node metastases and 5-year cancer death rate, using specimens from Stage IB, IIA, and IIB patients who underwent radical hysterectomy and pelvic lymphadenectomy. The nodal metastasis and 5-year cancer death rates were 7% and 8%, respectively, in patients with the uninvolved fibromuscular stroma thickness above 3 mm, and 37% and 26%, respectively, in patients with the thickness below 3 mm. The thickness of cancer-unaffected cervical fibromuscular stroma seemed to be closely related to and to be a more useful parameter of the biological behavior of invasive cervical carcinoma than the depth of the cancer invasion. A threshold value of the minimum thickness of the tissue as a barrier against extrauterine spread of cervical cancer could not be identified in this study.

Colposcopic findings of gland openings in cervical carcinoma: their histological backgrounds.

During the past 8 years 1501 colposcopies were performed paying a special attention to cervical glands for diagnosis of cervical intraepithelial neoplasia (CIN) and carcinoma. Normal openings could exclude these diseases at the error rate of only 5.1%, while atypical openings could identify them at the high confidence rate of 96.3%. False positive evaluations were mostly attributable to a marked degree of squamous metaplasia in the glands. The absence of gland openings in the main portion of the lesions amounted to 88.6% in invasive carcinomas but to only 12.4% in CIN. Adenoma malignum was a highly characteristic exception.

Molecular structure of human lymphocyte receptor for immunoglobulin E.

We have isolated and sequenced a cDNA clone encoding the human lymphocyte receptor for IgE (Fc epsilon R). The deduced protein sequence reveals that Fc epsilon R consists of 321 amino acids, without any signal sequence, and is oriented with its N-terminus on the cytoplasmic side and its C-terminus on the outside of the cell. This molecule shows striking sequence homology with chicken asialoglycoprotein receptor (hepatic lectin), suggesting a possible role for Fc epsilon R in endocytosis. Fc epsilon R mRNA is expressed in B cells, B cell lines, and macrophage cell lines. It is not expressed in T cells or T cell lines, with the exception of an HTLV-transformed T cell line. mRNAs expressed in a macrophage line and in the latter T cell line differ in size from mRNA expressed in B cells. Human BSF-1 (or IL-4) induces the expression of Fc epsilon R mRNA in B cells, but not in T cells.

Relationship between exposure and environmental concentrations in organic solvent workplaces.

The correlation between the exposure concentration (ExpC) of workers and the environmental concentration (EnvC) in their workplaces were examined in 143 printing, painting, gluing, degreasing and other solvent workplaces manned by 535 workers of both sexes. ExpC was measured with carbon felt dosimeters, and EnvC was with grab sampling of air into poly-tetrafluoroethylene bags; both were followed by GC analyses. When ExpC and EnvC were compared on both group and individual basis, the correlation was found to be positive but rather weak, and individual difference in ExpC within the workers of the same workplace was not negligible. Workplaces were classified after regulatory practice into three categories, and attempts were made to find out the conditions so that none among the 174 workers in the 43 Category 1 (i.e., "clean" from regulatory view point) workplaces would be exposed to solvent vapors over the current occupational exposure limit (OEL). Accordingly, it was found that the requirement will be met in case half the OEL is taken as the E value (the EnvC limit set by the regulation). Thus, the same conclusion was reached through the analyses of two separate batches of experiences, i.e., one in the present study and the other in the previous report (Ikeda and Ohtsuki 1985).

A clinico-pathological evaluation of parametrial involvement of deep invasive carcinoma of the uterine cervix.

Whole parametrial tissues from 235 patients with deep invasive carcinoma (greater than 5 mm in depth) of the cervix were systematically examined for involvement of cancer. Significantly higher incidences were observed in patients over 60 years of age than those of other age groups, and in patients with either adenocarcinoma or adenosquamous carcinoma than those with squamous cell carcinoma. The parametria were considered not to be the base camps but the carcinoma. The parametria were considered not to be the base camps but the pathways for pelvic node metastasis because of extranodal situation of most of small cancer nests in parametria, similar extent of incidence of metastases between parametrial and other pelvic nodes, and discrepancy in the incidence between parametrial involvement and ipsilateral nodal metastases. The relative 5-year survival rate was no less than 67% even in patients with parametrial involvement and/or pelvic node metastases. This rate seems to suggest some roles of these tissues as a barrier against extrapelvic expansion of cancer.

Preoperative evaluation of endometrial carcinoma by selective hysterography: a preliminary report.

Using a new instrument designed by the Authors having two cannulas to allow filling of a contrast medium exclusively in the cervical canal and uterine cavity, selective hysterography was performed for 22 cases of endometrial carcinoma. This method was useful not only for evaluation of the size, location and growth patterns but also for estimation of the cervical involvement of endometrial carcinoma.

Experimental subcutaneous granulomas in sheep with Dermatophilus-like microorganisms from porcine tonsil.

The pathogenicity of the Dermatophilus-like microorganisms from porcine tonsil and the light and electron microscopic findings were studied with adult ewes. The early lesions were abscessess and advanced ones were granulomas after the subcutaneous inoculation. The granulomas were composed of the central bacterial colonies and the layers of the neutrophils, epithelioid cells and giant cells, and peripheral connective tissues. Epithelioid cells and giant cells were identified by the large euchromatic nuclei, abundant organelles and interdigitation of the blunt pseudopods in the ultrastructure. The lesions were very similar to subcutaneous granulomas in sheep and cattle due to Dermatophilus congolensis (atypical dermatophilosis), actinomycosis and nocardiosis.
