Colposcopy for postmenopausal women.

The number of postmenopausal women colposcopically diagnosed has markedly increased during the last 15 years. In some of them, a clear colposcopic impression, required for planning an appropriate therapy, was disturbed by severe senile cervicocolpitis, when examined by the conventional technique using acetic acid. With a 5-year experience of 79 paired colposcopies, it was concluded that estrogen administration is necessary as a routine precolposcopic procedure in such cases and oral administration of conjugated estrogens at 1.25 mg/day for 14 days is adequate for this purpose.

Lesions of articular, sternal and growth plate cartilage in rats.

Degenerative osteoarthritis was observed in the femur and sternum in specific pathogen free Fisher 344 rats at 13 and 19 months of age. Histological changes consisted of degeneration of the matrix and erosion in the articular cartilage of the femur. In the sternal cartilage, degeneration and necrosis of the matrix, formation of cysts, and cleft-like fractures were observed. In addition, degeneration of the matrix, appearance of eosinophilic streaking, and necrosis and fissure between the growth plate and epiphyseal trabeculae were seen in the femur. The changes in the sternum were essentially the same as the femur.

[Occupational stress of assembly line female workers in confectionery work].

Concomitant with frequent occurrence of disorders of the neck, arm, hand and low back among assembly-line female workers in confectionery work, there is an increased number of patients with occupational cervicobrachial disorders and/or low back pain. In suspicion of the close correlation between the working conditions and development of these local disorders, a field study was undertaken. The following are the results obtained. More than 90% of these assembly-line female workers consisted of inexperienced part-time employees, mostly of middle to old age. They were engaged in decorating conveyor-carried cakes with cream and chocolate. The work necessitated repetitive movements of the upper limbs and concentration of visuosensory and nervous attention in a half-sitting slouching posture. This was considered to exert excess load on the local muscles and nervous and sensory systems. The causes that intensified the local symptoms as pointed out by the workers consisted of (1) repetitive use of the arms and hands, (2) static posture during the work and (3) sustained standing position. Complaints of low back pain were conspicuous from the unsuitable height of a conveyor-belt. Thirty five percent of the female workers needed medical treatment for cervicobrachial and/or low back pain. The forced adaptation to the belt height, sustained unnatural working posture and the imposition of forced movements seemed to be the main factors in the onset of cervicobrachial and/or low back pain in interrelation with working hours. On the basis of these results, the work load on a machine-paced assembly-line was analyzed and the necessity of improvement of working conditions was discussed.

Induction of IgG secretion in a human B cell clone with recombinant IL 2.

A human B cell clone (SGB3) responsive to IL 2 was established. Both recombinant IL 2 and B cell differentiation factor (BCDF) induced IgG secretion in SGB3 cells in a dose-dependent manner, but IL 2 did not affect the proliferation of SGB3 cells. FACS analysis showed that SGB3 cells expressed Tac antigen and anti-Tac antibody inhibited IL 2-induced IgG secretion without any inhibitory effect on BCDF-induced IgG secretion. These results showed that IL 2 could directly act on B cells and provide a differentiation signal through IL 2 receptors distinct from BCDF receptors. Physiologic relevance of IL 2 in the antibody response was discussed.

Undenuded invasive cervical carcinomas erroneously treated by cauterization.

Two cases of occult invasive carcinoma of the cervix have been reported with emphasis on diagnostic failure and subsequent inappropriate treatments. A prompt replacement with malignant cells simulating dysplasia or carcinoma in situ was observed at the sites of cauterization preoperatively. However, histology of postoperative specimen showed apparent invasiveness in both cases and lymphatic metastases in one of them. These findings have led us to a concept that the present cases may represent a variety of occult carcinomas, which is in between the common and verrucous types of squamous cell carcinomas.

Granulomatous sub-dermal lesions in sheep inoculated with Dermatophilus congolensis.

Dermatophilus congolensis was inoculated subcutaneously into 6 ewes and the resulting lesions were examined by light and electron microscopy. The organism was recovered from the subcutaneous lesions. The lesions were suppurative in the early stages and granulomatous in the advanced stage, but abscesses were the usual lesion observed. The granulomas were composed of several layers; central bacterial colony, neutrophil layer, layer of macrophages, epithelioid cells and Langhans and foreign body type giant cells with a periphery of connective tissue. Multiloculated granulomas were observed. Electron microscopic examination confirmed the cell types of the lesions. Epithelioid and giant cells were characterized by the shape and number of the nuclei and complex and abundant organelles. Interdigitations of the blunt pseudopodia of adjacent cells were also found. It is concluded that the organism can produce both granulomatous lesions and abscesses which differ from those of orthodox dermatophilosis but are similar to those described in "atypical dermatophilosis".

Tonsillar changes in pigs given pseudorabies (Aujeszky's disease) virus.

All of the eight 5-day-old pigs orally given pseudorabies (Aujeszky's disease) virus developed tonsillitis. The initial changes occurred in the subepithelial area between the lymphoid nodule and the crypt epithelium, showing a characteristic pattern of necrosis. The necrosis became more severe and gained access into the lymphoid nodule and crypt epithelium. Coincident with the histopathologic changes, numerous specific immunofluorescences were detected, first in the nucleus and in some parts of the cytoplasm of cells distributed in the subepithelial area. The fluorescence subsequently spread into adjacent lymphoid nodules and crypt epithelial cells. Ultrastructurally, many enveloped virus particles were detected in the center of the necrosis. Thereafter, the crypt epithelial cells also underwent degeneration, and a small number of virus particles were detected in the nucleus of the degenerating epithelial cells. In the more advanced stage, the enveloped virus particles were discharged into the crypt lumen.

Changes in urinary delta-aminolevulinic acid at low lead exposure level with special reference to production activity.

A 7-year follow-up survey on 53 workers was carried out in a lead storage battery factory to evaluate the significance of urinary delta-aminolevulinic acid (ALA) and blood lead in a work environment where lead in the air was considered to be about or less than the current occupational exposure limit. While lead in the blood and ALA in the urine had a good correlation to each other cross-sectionally as well as longitudinally, geometric means of lead concentrations in the workroom air samples which were collected following grid sampling strategy, did not correlate with ALA in urine significantly. On the contrary, the semiannual production of batteries significantly correlated with changes in mean ALA in urine. The questionnaire survey proved that the prolongation in work hours, lead to an increase in the mean ALA in urine as well as a higher incidence of higher-than-normal urinary ALA. The results clearly demonstrated the importance of the biological indicators, such as lead in blood and urinary ALA, as well as the necessity of paying attention to non-industrial hygiene factors, such as the production rate of batteries and the length of the daily shift, for the protection of the workers' health when lead in the air is moderate.

Demonstration of Aujeszky's disease virus antigen in trypsin-digested paraffin-embedded tissue sections by immunofluorescence.

After trypsin digestion of buffered formalin-fixed, paraffin-embedded brain section, immunofluorescent indication of Aujeszky's disease virus antigen was successful in experimentally infected cow and goat. This result was comparable to those obtained with light and electron microscopic examinations. This method will be diagnostically useful where fresh-frozen tissue is not available.
