Genital prolapse: a legacy of the West?

A retrospective study was undertaken to determine if there has been a change in the incidence of genital prolapse and urinary stress incontinence in Chinese women in Hong Kong. The medical records of 2,312 consecutive patients admitted for major gynaecological surgery for benign conditions in 5 out of 7 major public teaching hospitals in Hong Kong for the calendar year of 1993 were reviewed by way of an audit of all gynaecological operations performed in these hospitals. A total of 578 cases (25%) were identified as having genital prolapse and or urinary problems.

Free perforation of gastric carcinoma. Results of surgical treatment.

OBJECTIVE: To define by actual standards the results of treatment of free perforation of gastric carcinoma in a consecutive number of patients treated at one institution. DESIGN: Case series of patients with perforated adenocarcinoma of the stomach treated in Hong Kong between 1984 and 1992. SETTING: Urban academic medical center. PATIENTS: Thirty-four Chinese patients who were operated on for perforated gastric carcinoma. A risk scoring system was used to predict postoperative mortality. Factors with a possible influence on postoperative mortality and long-term survival were studied using univariate and multivariate analysis. INTERVENTION: All patients underwent laparotomy, which was performed for closure of the perforation in four patients and for gastrectomy in 30. MAIN OUTCOME MEASURES: Thirty-day mortality and survival times. RESULTS: The 30-day mortality rate was 20%, and the median survival time was 10 months (range, 2 to 92 months). The risk score was the only significant predictor of 30-day mortality, and the pathologic TNM staging, of long-term survival. CONCLUSIONS: A significant proportion of patients can be saved and offered good palliation with emergency gastrectomy; those likely to die can be identified before surgery.

Case report: spontaneous pregnancy following thymectomy for myasthenia gravis associated with premature ovarian failure.

We report the case of a 29-year-old woman who was diagnosed to have premature ovarian failure (POF) and myasthenia gravis at the age of 27 years. She was treated by thymectomy and pyridostigmine. Hormone replacement was started after the operation and she subsequently conceived spontaneously. We believe this is the first report of a spontaneous pregnancy after the diagnosis of POF associated with myasthenia gravis.

Plasma progesterone in women with a history of recurrent early abortions.

Basal body temperature, vaginal hormonocytology, and plasma progesterone concentrations were studied serially in consecutive menstrual cycles of 8 normal subjects and of 10 patients with a history of recurrent early abortions. The menstrual patterns and characteristics of basal body temperature of the patients did not differ significantly from those of control subjects. The mean plasma progesterone concentrations at the midluteal phase of the study cycles of three patients were below those of the lower limit of the control group (4 ng/ml). The results of this study support the conviction that an inadequate luteal phase is more likely to occur in women with a history of recurrent early abortions.

Induction of labor by acupuncture electro-stimulation.

Induction of labor by acupuncture electro-stimulation was carried out in 31 patients. This was successful in 21 cases. The pattern of uterine contractions induced in these patients was similar to that in normal labor. No serious fetal or maternal complications occurred. The delay between the commencement of stimulation and the onset of contractions suggests that a humoral mechanism may be involved.
