[Multi-modal treatment of patients with multiple liver metastases caused by sigmoid cancer].

A case of sigmoid cancer with multiple liver metastases (S2PON3 + H3) who was treated by multi-modal treatment was reported. The multi-modal treatment is including intra-arterial administration of anti-cancer drugs as a pre-surgery treatment, intra-arterial infusion chemotherapy lasting for three to five weeks (three times), hyperthermia treatment combined with intra-arterial administration of anti-cancer drugs and intra-arterial expandable metalic stent. The patients lived for 2 years and 4 months in good condition.

Long-term follow-up of patients with sick sinus syndrome: a comparison of clinical aspects among unpaced, ventricular inhibited paced, and physiologically paced groups.

To analyze the prognosis of the sick sinus syndrome (SSS), we compared the clinical aspects among unpaced, ventricular paced, and physiologically paced patients who were followed over a long period. Unpaced intrinsic SSS was not always progressive and patients did not necessarily require permanent pacing. The incidence of concomitant AV conduction disturbance was 65.6% before pharmacologic autonomic block, (PAB), but this was significantly reduced to 31.7% after PAB. Follow-up study of the physiologically paced groups revealed no development of either new or more than second degree AVB. The VVI group had significantly more complications (68%) than the physiologically paced groups, mainly chronic atrial fibrillation (36%) and thromboembolism (20%). In addition, cardiothoracic ratio (CTR) in the VVI group was significantly greater compared with that in the physiologic groups. Nine deaths occurred during the follow-up period in the pacing groups, including six with VVI and three with physiologic pacing. In the VVI pacing group, heart failure and thromboembolism were most commonly the causes of death, while in the physiologic pacing groups, the causes of death were noncardiac. Although the survival rate in the ventricular paced group was not significantly different from that in the physiologic pacing groups, cardiac deaths were fewer in the latter group. Considering our clinical data, the decision to use ventricular pacing needs to be carefully weighed in patients with sick sinus syndrome, and physiologic pacing is more highly recommended.

Left atrial myxoma with an unusual tumor vascularity demonstrated by angiography--report of two cases.

We describe two cases of left atrial myxoma, demonstrating an unusual tumor vascularity as revealed by coronary angiograms. Both cases had suffered typical episodes of transient ischemic attack (TIA). Coronary angiography revealed tumor blood supply from coronary arteries in both cases, and also a leakage of contrast medium from the surface of the tumor blood vessels to the left atrium in one case. In the medical literature available in English reviewed so far, no report of such a leakage has been reported. Selective coronary arteriography can be a useful diagnostic method of delineating left atrial myxoma and its blood supply.

Familial dilated cardiomyopathy and human leucocyte antigen. A report of two family cases.

Two familial cases of dilated cardiomyopathy were evaluated by HL-A typing. In the case of the first family, the mode of inheritance is likely to be an autosomal dominant trait. Only the affected individuals carried the identical HL-A haplotype (A2, Bw54, Cw1, DR4, DQw3), while the unaffected members do not share this pattern. In the second family case, the disease is probably inherited by autosomal recessive traits. All of the family members examined shared the identical HL-A haplotype (A24, Bw52, DR2, DQw1), but only the affected individuals were homozygous for this haplotype.

Loss of Lewis antigen expression on erythrocytes in some cancer patients with high serum CA19-9 levels.

The Lewis (Le) phenotype of both erythrocytes and sera and serum CA19-9 levels were studied in 49 patients with pancreatic carcinoma, in 37 with gastric cancer, in 22 with colorectal cancer, in 21 with bile duct carcinoma, and in 19 with hepatocellular carcinoma. The Le phenotype was determined in sera with the use of the dot-immunobinding assay and on erythrocytes. The localizations of the Le antigen and CA19-9 were studied in pancreatic tissues from 22 patients with pancreatic carcinoma. The prevalence of Le(a-,b-) on erythrocytes was significantly higher in patients with pancreatic carcinoma than in normal controls. Nineteen of 21 patients with pancreatic carcinoma, whose Le phenotype on erythrocytes was Le(a-,b-), had Le antigen in tissues and sera, and they had a raised serum CA19-9 level. The remaining 2 patients were of the Le(a-,b-) phenotype for both erythrocytes and sera, and their serum CA19-9 levels were below 6 U/ml. Neither Le antigen nor CA19-9 could be localized in tissues of these 2 patients. Two patients with gastric cancer, 6 with colorectal cancer, and 6 with bile duct carcinoma had Le antigen in sera in spite of having Le(a-,b-) on erythrocytes. These results indicate that the Le phenotype on erythrocytes can undergo a change not infrequently in patients with pancreatic carcinoma as well as in patients with other gastrointestinal cancers, but patients with the Le(a-,b-) phenotype in sera cannot synthesize CA19-19.

[Enhancement of anti-tumor effects by arterial embolization combined with hyperthermia in the treatment of hepatic tumors].

The effectiveness of arterial embolization using DSM (Degradable Starch Microspheres, Spherex, Sweden) combined with hyperthermia were investigated in the treatment of VX2 carcinoma in rabbits and also in human hepatic tumors. These microspheres temporarily obstructed the blood flow at the precapillary level. The blood flow in the tumor was inhibited to a greater extent and for a longer period than in normal muscle. The rise in temperature at the tumor site during heating was shown to be significantly higher than in muscle. The pH in the tumor clearly showed a stronger degree of inhibition than that achieved with hyperthermia alone. Histological examination demonstrated no marked damage to the muscle following DSM + hyperthermia. In the patients with hepatic tumor, hyperthermia was performed using an RF Thermotron 8 (8 MHz) externally, followed by injection of 15 ml of DSM combined with anti-cancer drugs via a catheter. The rise of temperature in DSM + chemo-hyperthermia during heating showed a higher value than that with hyperthermia alone and effective cases (PR) totalled 5 from among 8 cases treated. It was concluded that the heating efficiency may be improved by arterial chemo-embolization in the treatment of hepatic tumors.

Study of chronic alcoholic pancreatitis by means of serial pancreozymin-secretin tests.

Fourteen patients with suspected chronic alcoholic pancreatitis (CAP) and 21 patients who had been shown to have the disease were followed up by the pancreozymin-secretin test in order to clarify the serial changes in exocrine pancreatic function in alcoholic pancreatitis. The initial and final test data for secretory volume, maximal bicarbonate concentration, bicarbonate output (BO), and amylase output (AmO) of exocrine secretion were compared in these two groups. Patients with suspected CAP showed a significant serial decrease only in AmO; definite CAP developed in 3 of them during the follow-up period. In definite CAP, a significantly progressive decrease in BO as well as AMO was observed. It is suggested that in the earlier stage of CAP, AMO is initially affected, whereas decreased secretion of both bicarbonate and enzyme becomes apparent in the later stage.

Immunoperoxidase detection of pancreatic oncofetal antigen in human tissues.

Normal human adult tissues (from the pancreas, stomach, small intestine, colon and rectum), human fetal pancreas and various human tumor tissues were investigated for the expression of pancreatic oncofetal antigen (POA) by the peroxidase-antiperoxidase complex method. The antibody used in this study was raised by immunization of rabbit with POA purified from ascites of a patient with pancreatic cancer; it was the same as in our previous report on enzyme immunoassay for serum POA. POA was localized in the ductal cells of human fetal pancreas and ductal cell type pancreatic cancer tissue. It was hardly seen in acinar cells of human fetal or adult pancreas, or in acinar cell type pancreatic cancer tissue. Several cancer tissues other than the pancreas revealed positive POA-staining but less frequently. POA was also detected in small amounts in the small duct of normal adult pancreas. In other normal human tissues, POA was recognized in deep crypts of the small intestine and colon. These results indicate that POA is present abundantly in immature ductal cells of the pancreas such as in fetal pancreas and pancreatic cancer, and that high serum levels of POA seen in patients with pancreatic cancer and other malignant tumors may originate from these malignant tissues.
