There is a significant association with HL-A3 type and pernicious anaemia. In addition there is a negative association with HL-A2. There is no association between HL-A type and the presence or absence of immunity to intrinsic factor.
Immunosupprissive therapy was carried out using steroids, azathiprine and antilymphocyte globulin in one patient with systemic lupus erythematosus (SLE) and four with dermatomyositis. Response to treatment was based upon clinical evaluation. The patients with SLE and two of the four with dermatomyositis responded favourably to the regime. It is suggested that this preliminary study is sufficiently promising to warrant the creation of a rigidly controlled investigation.
A four-day residential workshop was conducted for students beginning their clinical studies at the St George Hospital in 1974. The aim of the workshop was that the students should understand the functioning of the hospital and feel comfortable as students in the hospital, so that they would learn their clinical skills more effectively in their subsequent years at the hospital. Each student lived in a hospital bed with an "illness" for two days, and spent another two days helping care for these "student patients" and exploring the hospital. Evaluation at the end of the workshop suggested that the objectives had been achieved.

Sites of synthesis of hemopexin and transferrin were determined by culturing various tissues of rabbits and monkeys in the presence of labeled amino acids. Labeling of the serum proteins was examined by means of autoradiographs of immunoelectrophoretic patterns as well as by precipitation in the test tubes employing immunospecific antisera. Good correlation was seen between the results obtained by the two different methods. The liver was found to be the only site of many tissues studied that synthesized hemopexin. Transferrin production was observed in the liver, submaxillary gland, lactating mammary gland, testis, and ovary.

The H2S water screening test and the membrane filtration faecal coliform count were compared with Escherichia coli counts for water samples collected from household water sources and domestic drinking water in rural Malaysia. Water samples were taken from 151 wells, 44 taps supplying water from the treated municipal supply and 192 domestic stored water supplies. E. coli were detected in 20% of the samples (42% of wells, 7% of tap water and 6% of drinking water). Excellent correlation (Spearman's rank correlation rs = 0.93) was found between the faecal coliform and E. coli counts for all sample types. The H2S method was poorly correlated whether read at 18 or 30 h. False positive rates were highest for well water, and false negative rates were highest for both well and drinking water samples, with low E. coli counts. The faecal coliform test was an excellent predictor of the presence of E. coli in these water samples, while the H2S test was very inadequate.
PURPOSE: This study was performed to test the efficacy of a supervised, hospital-based exercise program compared with a home-based exercise program involving minimal supervision, for both walking ability and quality of life measures in patients with exercise-limiting intermittent leg claudication. METHODS: Twenty-one patients were assigned randomly to 12 weeks of supervised exercise or to a home-based exercise group. After 12 weeks the participants in the supervised group transitioned to a home-based program. Both groups were then reevaluated at the end of 24 weeks. The initial claudication distance (ICD) and absolute claudication distance (ACD) on progressive treadmill exercise was measured at baseline, 12 weeks, and 24 weeks. Additionally, self-reported quality of life status was evaluated using the MOS SF-36 questionnaire. RESULTS: Each group improved (P < 0.01) ACD from baseline to 12 weeks, which was sustained at the 24-week follow-up. Both groups experienced similar long-term improvements (P < 0.05) in ACD (521.5 +/- 253.4 meters to 741.9 +/- 365.6 meters for the supervised group, 532.2 +/- 263.5 meters to 715.0 +/- 394.4 meters in the home group, P not significant, between groups). The supervised group experienced a greater improvement (P < 0.01) in the ICD after 12 weeks than the home group but not at 24 weeks. The on-site group also experienced significant improvements in ICD after 24 weeks (P < 0.05). Neither group manifested an improvement in self-reported physical function or mental health as assessed by the MOS SF-36. CONCLUSION: A structured exercise program was more effective in improving the ICD over a 24-week period than a less formal, home-based program. However, if patients are screened properly and receive adequate instruction, a home-based program can be a safe, low-cost alternative providing similar long-term (24 weeks) exercise benefits in ACD.
BACKGROUND: High-density lipoprotein cholesterol (HDL-C) is a powerful independent risk factor for coronary heart disease (CHD) among the elderly. Regular vigorous physical activity has been found to raise the concentration of HDL-C and thus reduce the risk of CHD. There is little data on the effect of non-vigorous activity on HDL-C in the elderly. OBJECTIVE: The aim of this study was to compare CHD risk factors, especially HDL-C, in a group of elderly persons who engage in regular non-vigorous physical activity with a group of frail elderly examined in a previous study. METHODS: Each subject (51 women and 19 men) had anthropometric measures taken and completed a questionnaire on lifestyle and medical history. Total cholesterol (TC), HDL-C and lipoprotein (a) were analysed. Results were compared with those of a frail group examined previously using similar methodology. RESULTS: HDL-C, adjusted for age, body mass index (BMI) and waist-to-hip ratio (WHR), was greater among women (p < 0.01) and men (p < 0.05) who were engaged in a regular physical activity at least once a week. TC was higher among active women (p < 0.001), but there was also a trend towards a lower TC/HDL ratio. Therefore, although TC is higher in active women, this could be due to a higher proportion of the cholesterol fraction consisting of HDL-C. WHR was negatively associated with HDL-C in frail men (p < 0. 05), active men (p < 0.01) and active women (p < 0.05). BMI was negatively associated with HDL-C in frail women (p < 0.05). CONCLUSIONS: This sample of elderly people who participate in regular weekly non-vigorous physical activity have a higher HDL-C than frail individuals who do little or no exercise. Since HDL-C is consistently reported to be inversely associated with CHD in the elderly, an elevation in HDL-C concentration may provide some protection to elderly persons who participate in regular nonvigorous physical activity compared to frail elderly individuals who are largely sedentary. Caution should be exercised in the interpretation of a TC only reading in active elderly women without an accompanying measure of HDL.
