Purification and characterization of a liposomal-forming tetraether lipid fraction.

Polar lipid E, PLE, a native tetraether lipid mixture from Sulfolobus acidocaldarius is shown to spontaneously form multilamellar liposomes in aqueous media. PLE lipids were isolated as a single fraction from the crude lipid extract of S. acidocaldarius on a reverse-phase column followed by TLC and methanol precipitation. The methanol-precipitated mixture was able to form liposomes, whereas the non-precipitated material did not. It has thus been demonstrated, for the first time, that tetraether lipids from S. acidocaldarius can form liposomes, per se, in aqueous media.

Purification of glycerol dialkyl nonitol tetraether from Sulfolobus acidocaldarius.

A modified procedure for extraction and purification of hydrolyzed archaebacterial lipids is described. Lipids were extracted from Sulfolobus acidocaldarius using a Soxhlet extraction procedure followed by trichloroacetic acid solvent-extraction of the residue. The yield of total extractable material by this protocol was 14% which, after a two-phase wash, yielded 10% lipid. Modifications to the published steps for purifying the subsequently hydrolyzed lipids were developed to purify glycerol dialkyl nonitol tetraether (GDNT). The nearly colorless final macrocyclic product was characterized by TLC, IR, NMR, and mass spectrometry.

Giant retroperitoneal pleomorphic lipoma.

A 65-year-old man developed a progressively enlarging abdominal mass that ultimately resulted in a weight gain of 25 kg., leg edema and an inability to walk. His condition was initially considered to be inoperable but exploration resulted in the removal of a 19.5 kg., 50 cm. intraabdominal pleomorphic lipoma. The patient remains well 4 years post-operatively.

Separation of phospholipids using a diethylaminoethyl-silica gel column and thin-layer chromatography.

Phospholipids derived from egg yolk are readily separated by DEAE-silica gel column chromatography using stepwise gradient elution. The overall recovery of phospholipids from the column is 85-95% at a loading capacity of 120 mg of lipids per 10 g of DEAE-silica gel. The complete separation of eight phospholipids (phosphatidylcholine, sphingomyelin, lysophosphatidylcholine, phosphatidylethanolamine, phosphatidylglycerol, phosphatidylinositol, phosphatidylserine and lysophosphatidylserine; 5 micrograms each) is also achieved by one-dimensional DEAE-silica gel thin-layer chromatography with the solvent system chloroform-methanol-water-pyridine-58% ammonia solution (130:55:8:4:4, v/v).

Balancing infection control practices and family-centred care in a cohort of paediatric suspected severe acute respiratory syndrome patients in Hong Kong.

OBJECTIVE: To identify key nursing issues for paediatric patients suspected of severe acute respiratory syndrome (SARS) in relation to the family-centred model of nursing care and to develop a data-based model of paediatric nursing care to be better applied in situations of suspected SARS or where outbreaks of other infectious diseases occur. METHODS: This is a retrospective descriptive case series, which analysed the medical and nursing records of all highly-suspected/suspected SARS patients admitted to a major acute hospital in Hong Kong. Key nursing personnel were also interviewed. RESULTS: The study included a total of 17 highly-suspected and 49 suspected SARS paediatric patients (age: 1-16). None of the paediatric patients was eventually diagnosed of SARS. Most cases presented fever (highly-suspected: 76.5%/ suspected: 100%), cough (64.7%/71.4%), nausea and vomiting (35.3%/28.6%). Nursing care was provided to the patients as necessary. Both the children and parents experienced fear of SARS, as well as separation anxiety arisen from hospitalization in a strict isolation setting. CONCLUSIONS: Infection control overshadowed the family-centred nursing practices in the management of SARS paediatric patients. A major nursing care issue for SARS paediatric patients was to achieve a careful balance between attending to patients' physical and psychological needs and adhering to the infection control guidelines, while at the same time offering psychological support to family members. The current philosophy and practice of family-centred nursing model neglect the special needs of children with infectious diseases and predominantly apply to children with non-infectious diseases that family visits are allowed and nursing care by parents are encouraged.
