[Localization of the placenta by means of nuclear medical methods].

Accurate localization of the placenta in the cavity of the uterus is indicated prior to transabdominal amniocentesis and in cases where hemorrhages occur during pregnancy. Several nucleo-medical techniques are available for this purpose. Earlier a so-called placentography was performed, a method later replaced by placental scintigraphy. Today the gamma scintillation camera is ideal for localizing the seat of the placenta. In case of a normally embedded placenta, the profile view makes it possible to determine whether the placenta is attached to the anterior or posterior wall. In case of an anterior wall placenta it is possible with a radioactively labelled marker to draw the outline of the placenta on the abdomen of the patient so that the examiner is certain to puncture in an area that does not contain the placenta. Diagnosis of placenta praevia is assured in all cases by use of the gamma scintillation camera. Short-term introduction of an emitter into the posterior fornix vaginae makes it possible to determine the position of the internal orifice of the cervix uteri. If the caudal part of the placenta extends up to this mark, a placenta praevia is likely and all obstetrical precautions indicated have to be taken.

[Collagenolytic activity of cervix carcinoma].

We estimated collagenolytic activity of 10 invasive carcinomas of the cervix by means of a biological assay. Collagenolysis was found in 7 specimens. In order to detect the origin of the collagenolytic enzyme additional inhibition studies with ethylendiaminetetraacetate and normal human serum were performed. Normal human serum inhibited the enzymatic process by a mean percentage of 14.9%, EDTA up to 9.1%. D-penicillamine and 1.10-o-phenanthroline which inhibit collagenase specifically stopped the enzymatic activity of the tumor completely. According to our results it can be suggested that the collagenolytic activity of carcinoma of the cervix uteri is not derived from serum or from polymorphonuclear cells but is created by the tumor itself.

The 'LH/HCG test', a valuable aid in the diagnosis of tubal pregnancy.

174 patients with suspected tubal pregnancy were thoroughly examined. The diagnosis was confirmed in 102 women. As the usual noninvasive and invasive examination methods proved to be rather inaccurate, a new procedure, the 'LH/HCG test', was evaluated. Results were positive in all our cases. It was therefore concluded that this assay should receive primary consideration when tubal pregnancy is in question.

[Differential diagnosis of tumors in the pelvis of gynecologic patients by angiography (author's transl)].

The value of angiography in differential diagnosis of tumours of the pelvis is discussed based on 15 cases and the relevant literature. Inflammatory infiltrates and neoplasms arising from the sigmoid colon can be separated from gynaecologic tumours by angiography. In 4 cases with pelvic masses in which differentiation between a myomatous uterus and an ovarian tumour was not possible clinically, unequivocal evidence was cleaned from a selective injection into the internal iliac artery. Angiography is particularly well suited for the delineation of parietal pelvic tumours--especially of vascular malformation.

[Comments on the diagnosis of tubal pregnancy (author's transl)].

51 women with suspected tubal pregnancy were admitted to our hospital during the past two years. Tubal pregnancy was present in 31 women. Diagnosis is rarely accurate with the conventional examination methods. The "LH/HCG" test was positive in all of our patients. Hence it appears resonable, whenever tubal pregnancy is suspected, to combine several noninvasive examination methods, special preference being given to the LH/HCG test.

[Pregnancy-connected hypotension - the influence of mineralo-corticoids on fetal nutrition (author's transl)].

A considerable number of nutritionally deficient newborns are to be expected when over an extended period of time during pregnancy, the mother's bloodpressure readings are 110/65 or below and when no therapy to increase this bloodpressure was administered. We therefore instituted blood pressure raising therapy on a total of 40 pregnant patients with bloodpressure values under 115/70 (25 mg Desoxycorticosteron-trimethylacetate i.m.) even though these patients did not voice any subjective complaints. This treatment resulted in an increased utero-placental perfusion which lead to a significant improvement of the placental perfusion values. Compared with the "fetal outcome" of untreated controls, the infants delivered by patients who had received therapy were considerably heavier at birth and showed almost no signs of prenatal malnutrition. Based on this study we feel that patients although completely free of pregnancy-connected hypotonic complaints (RR 110/65 or below) should be placed on a bloodpressure-raising VASODILATINg therapy for the improvement of fetal nutrition.

[The influence of the continuous epidural anaesthesia on uteroplacental blood flow (author's transl)].

Continuous lumbar epidural anaesthesia causes a drop in maternal blood pressure by decreasing flow resistance in the peripheral vascular bed. By infusion of 1000 ml NaCl blood pressure can be kept constant. Our work shows, that dilatation of the peripheral vessels has a positive effect in patients with reduced uteroplacental flow, as it results in an augmentation of placental flow. In women with normal placental function such an effect could not be seen. The practical use of these results is illustraded by a case report.

[New aspects in diagnosis and therapy of placental insufficiency. Placental perfusion measurements; placental perfusion test (PPT) and betamimetic long term treatment (clinical and experimental data (authors transl)].

The rate of utero-placental blood flow depends on functional components (perfusion pressure and flow resistance within the area of the vascular bed of the placenta), as well as on morphological factors (regressive changes in the placenta). Different primary maternal conditions and diseases may lower the rate of placental flow, leading to placental insufficiency; the highest percentage, by far, of placental dysfunction is found in patients suffering from gestosis. Hypocirculation initially present in cases of EPH gestosis and caused by arteriolar spasms triggers off a vicious circle involving placental infarction and severe reduction in the utero-placental perfusion rate. This in turn leads to fetal hypotrophy, a high rate of premature births and perinatal mortality. Verification of HPL, HCG, alpha-Fetoprotein or E3 in maternal serum and amniotic fluid or urine greatly improved the recording of partial placental functions. Along with ultrasonic biometry, cardiotocography and amnioscopy, these hormonal parameters allow only indirect assessment of the placental function. On the other hand, measurements of the utero-placental flow offers a direct approach. In order to evaluate the placental flow measurements it is imperative to obtain a curve indicating the course over the last third of the pregnancy-in addition to establishing a general normal range. In case of placental insufficiency, it is necessary to determine whether this is due to functional disorders alone, or to more extenisve morphological changes. A placental perfusion test (PPT) was developed in order to make this distinction. Beta2-mimetic treatment is indicated if functional factors predominate, whereby it appears essential to obtain the requisite experimental data for precise quantification of beta-mimetic action.

[Asteroid plastic surgery as optimum management of complete and partial vaginal occlusion (author's transl)].

Opinions differ about the type of surgical procedure to be applied in cases of complete or partial vaginal occlusion. The large percentage of postoperative vaginal stenoses and functional disorders necessitated a search for a new technique. The mucous membrane of the introitus vaginae is incised in the shape of a lying cross and the four flaps thus made are detached. Then an exactly lateral incision is made on each side of the crossing point described above resulting in a broad access to the vagina. Once the opening is wide enough, a second group of triangular flaps is formed vertically to this horizontal incision; the dimensions of the triangular flaps in the inner and outer group must be identical. Then the outer four flaps are turned inward and the four inner flaps are turned outward and interlinked so that a zig-zag line is formed which encircles the vagina in the area of the former stenosis. Thus far this surgical technique has been applied five times and yielded satisfactory results in every instance.

On radioactive labelling of the lymph drainage regions of the pelvis.

The introduction of radioisotopes in the surgical management of cervical carcinoma at our clinic in 1970 constituted a major advance in this respect. Prior to surgery the lymphatic tissue of the pelvic region is labelled by subcutaneous applications of radioisotopes. Our experience shows that the body's kinetics encourage the deposition of the radionucleide and thus visualization even of the groups of lymph nodes located in the deeper layers of the pelvic region. This method allows virtually complete lymphonodectomy with resultant improvement of cure rates.

[Unviable micromelic dwarfism: a syndrome with dystrophy of the thorax and polydaktyly type Saldino-Noonan (author's transl)].

Besides classical achondroplasia various severe osteochondrodysplastic disorders have been differentiated in recent years. Their most important features include obligatory perinatal mortality, severe dystrophy of the thorax and shortening of tubular bones. Additionally some of these syndromes present visceral malformations, cleft-lip, and malformations of the genitals. A detailed case report of a patient is given. Various features: dystrophy of the thorax, micromelia, postaxial polydactyly, metaphyseal dysplasia, malformations of the great vessels, and polcystic kidney suggest the classification as short rib-polydactyly syndrome, type Saldino-Noonan.

[Influence of allylestrenol on hpl serum levels in high-risk pregnancies (author's transl)].

32 patients with high-risk pregnancies were treated with 3 X 5 mg of 17alpha-ally1-17beta-hydroxy-estr-4-ene (allylestrenol, Gestanon) daily. Only those patients entered this study, who had pathological HPL serum levels during two successive control investigations. In addition, cytological analyses of vaginal smears were performed before treatment was started. The patients were treated with allylestrenol until normal HPL levels were recorded, but at least during 14 days. HPL serum levels were estimated every second day through 14 days. Thereafter HPL assays were performed in weekly intervals. Determinations of HPL were done by radioimmunoassay. Analyses of vaginal smears were done weekly. Statistical analysis of HPL serum levels were performed by Student's-t-test. A statistical significant increase of HPL serum levels were registered during allylestrenol therapy. These results suggest that the pregnancy maintaining effect of allylestrenol is at least partly due to a stimulation of placental function. Analyses of vaginal smears indicate a progestational effect of the norsteroid.

[Placental function and foetal lung maturity (author's transl)].

The placental function and foetal lung maturity were simultaneously assessed before delivery in 48 high-risk pregnancies, the actual time varying from case to case. A statistically significant correlation was found between placental function and foetal lung maturity. Since the actual time of investigation often lay back several weeks before delivery, no significant connection was detected between the state of foetal lung maturity at the time of amniocentesis and the clinical picture in respect to lung maturity of the infant at birth. The discrepancy is tabulated and possible causative factors are discussed.

[Optimal monitoring in EPH gestosis and idiopathic placentar insufficiency (author's transl)].

EPH gestosis and placental insufficiency sui generis are cause of fetal hazards. Clinical symptomatology and anamnesis indicate different tests to detect pathological conditions for the fetus. HPL, HCG, urinary estriol, measuring the placental flow-rate, ultrasonic diagnosis as all other usual examinations during pregnancy were carried out. In EPH gestosis a reduction of all placental functions was found like as for thrombocytes and fibrinogen. Placental insufficiency showed an even greater reduction of placental function, only thrombocytes and fibrinogen remained constant. Weight and length of the newborn were according the pathological placental function reduced. Morphology of the placenta could proof the results of the previous done tests.

[Some considerations on forceps delivery with excenter handle (author's transl)].

The introduction of electronic monitoring techniques in delivery increasingly has lead to the use of vaginal surgery in delivery. Vacuum and forceps extraction, in our view, do not compete with each other. Years ago forceps extraction was decisively improved by introducing the axial traction forceps. However, we believe that this technique has not been given due attention and is not applied widely enough. In vacuum extraction and in forceps deliveries without axial traction considerable part of the force is exerted in the direction of the symphysis. When using the excentric handle forceps, it is necessary in addition to the traction and pressure components to include the lever action. This is a so-called couple of forces. The additional torque involved in the use of an axial traction forceps shifts the fetal head during extraction in the direction of the hollow of the sacrum. The force needed for this shift and the progression of the head is many times less than that required for conventional forceps extraction. This is due to the absence of the component directed at the symphysis which impedes delivery. The axial traction forceps may also be used in case of the infant's abnormal positioning of the head. In case of a transverse position of the fetal head, a special fastener on the forceps makes it possible to use an excentric handle on the traction hook of the Kielland forceps and thus render possible rotation of the fetal head from the transverse position. The extentric handle in forceps extraction is optimal in order to overcome increased mechanical resistance to delivery. Adjustment of the handles to all types of forceps being used today is possible. From the results mentioned it is clear that the use of the axial traction forceps constitutes a valuable contribution toward declining perinatal mortality rates.
