By means of a new stimulation procedure--double and multiple atrial stimulation--the so-called sinoatrial conduction time (SACT) was calculated in 62 patients. The results were compared to the SACT calculation after premature single atrial stimulation (correlation coefficient 0.94). By means of the multiple atrial stimulation we were able to measure the "sinoatrial conduction time" in patients with SA-block of type II (missing reset phase) if the cases with SA entry block were excluded. The modified stimulation procedure helps to improve the assessment of functional disturbances of the sinoatrial node. This method for calculation of SACT is an advantage especially in patients with abnormally delayed SA conduction and in those with very short duration of spontaneous cycle length.
Among 412 female patients with fibroadenoma 30 (7.1%) were shown to have considerable epithelial proliferations, 7 (1.7%) carcinoma in situ (n = 4) or carcinoma (n = 3). All patients with carcinoma or carcinoma in situ in the fibroadenoma were older than 40 years of age. Differentiation of coarse or fine granulation of microcalcifications did not improve the accuracy of mammography. In 32% of fibroadenomas with epithelial proliferations similar changes were also found in the adjacent area of the breast whereas only in 8.8% of fibroadenomas without proliferative changes could epithelial proliferation be observed in adjoining areas. With increasing age the risk of carcinomatous degeneration in fibroadenomas rises to 17%. In multiple fibroadenomas only one of them may show carcinomatous changes whereas the others may appear unremarkable. This necessitates excision of all such tumours. In the same way as carcinomas fibroadenomas are most frequently found in the upper outer quadrant of the breast. Total resection including a surrounding layer of healthy tissue remains the treatment of choice.

In a survey the frequent possibilities of the diagnostics of rheumatic fever are described. The rheumatic fever is a general disease on immunological basis caused by an infection with beta-haemolysing streptococci of group A. The disease appears as visceral form (carditis rheumatica, chorea minor) or as peripheral manifestation (acute polyarthritis, erythema anulare, subcutaneous nodules, purpura rheumatica). The diagnostics is essentially based on clinical, physical, biochemical and immunoserological findings on the basis of modified criteria after Jones. Immunological methods for the demonstration of circulating antibodies gain an increasing diagnostic significance.

In 152 patients with diabetes mellitus, chronic ischaemic heart diseases and hypertension the so-called ptf-product resulting from the height of the amplitude of P and its time of spreading) measured in the lead V1 (PV1-index), was determined and compared with the values of 93 persons with healthy heart. Patients of the groups of diseases mentioned showed significantly higher pathological values than persons with healthy heart (limiting value -0.02 mms). A comparison between patients with chromic ischaemic heart disease at the age of 36-55 years and a group of diabetics of the same age without the clinical signs of an ischaemic heart disease resulted in no statistically ascertained differences. But compared with a group of persons with healthy heart of the same age the groups clearly differed: 63.9% of pathological values in the total group of disease compared with 18.5% of pathological values in persons with healthy heart. In 137 patients with latent diabetes (protodiabetes) in 54.7% also pathological ptf-values were found. It is possible that the determination of the ptf-product is able to ascertain already early forms of a chronic ischaemic heart disease still before its clinical manifestation. The large number of pathological values in patients with protodiabetes could be a first reference to early beginning disturbances of the left-sided atrial activity in the sense of a microangiopathy.
