Transient pseudohypoparathyroidism and neonatal seizure.

The case of a neonate is presented who had late onset seizure associated with hypocalcemia, hyperphosphatemia, and raised parathyroid hormone. The infant did not have any stigmata of pseudohypoparathyroidism. The hypocalcemia was initially resistant to calcium therapy, but responded to vitamin D analog therapy. The diagnosis of 'transient neonatal pseudohypoparathyroidism' was entertained, as the infant remained stable and seizure-free with normal serum biochemistry during 8 months of follow-up.

Brain abscess following mastitis in a 3-month-old infant.

Neonatal mastitis is rare and generally carries a good prognosis, once treated appropriately, but systemic spread could lead to complications. A case of a 3-month-old infant is presented who developed a brain abscess associated with neonatal mastitis.

Maternal sickle cell trait and fetal hypoxia.

Patients with sickle cell trait (SCT) usually run a benign course. But they may develop vaso-occlusive crisis, which may lead to hypoxia. During these episodes, pregnant women with SCT may effect the developing fetus. This report describes an interesting finding of subtle degree of fetal hypoxia associated with maternal SCT. Twenty mothers with SCT were compared with 20 controls for the amount of circulating nucleated red blood cells (NRBC) and marker of fetal hypoxia at birth. Elevated number of circulating NRBC were noted in the cord blood of neonates born to mother with SCT as compared with controls, suggesting evidence of intrauterine fetal hypoxia. A larger prospective study is needed to elaborate further on this association.

The importance of simple microscopy.

A case of severe neonatal anaemia, the cause of which was found to be severe fetomaternal haemorrhage is presented. The diagnosis was confirmed by simple microscopic examination of the maternal blood using the technique of acid elution, the Kleihauer-Betke test. In the differential diagnosis of anaemia of a newborn, the diagnosis of fetomaternal haemorrhage must be considered and the simple Kleihauer-Betke test should be performed on the maternal blood as soon as possible.
