Diisopropanolnitrosamine (DIPN) induced rat thyroid lesions. I. A histological classification.

DIPN induced localized lesions of the rat thyroid gland are described histologically and classified into three major types: type 1, foci of cellular alteration; type 2, proliferative nodules; and type 3, overt carcinomas. Follicular, papillary and mixed papillofollicular subtypes are recognized in the type 2 lesions, and follicular, papillary, anaplastic and mixed subtypes in the type 3 lesions. The nature of the individual lesions is discussed.

Diisopropanolnitrosamine (DIPN) induced rat thyroid lesions. II. An electron microscopic study.

An ultrastructural study was carried out on DIPN induced lesions of rat thyroid glands, in order to characterize in detail each type or subtype as classified by light microscopy in our previous report. Organelles closely related to the endocrine function of the thyroid epithelium such as apical or subapical vesicles, lysosomal granules, colloid droplets, Golgi apparatus, rER, and microvilli on the apical surface, were found to decrease in parallel with structural and cellular atypism of the thyroid lesion under examination, suggesting impairment of their function in the malignant lesions. On the other hand, observations of transitional forms among the lesions in term of the subcellular structure were considered to support the possibility that transformation from one type to another could occur.

Diisopropanolnitrosamine (DIPN) induced rat thyroid lesions. III. An immunohistochemical study on their functional aspects.

The functional aspects of DIPN induced rat thyroid lesions classified as in our previous studies were analyzed by immunohistochemical detection of Tg, T4, and T3 in serial or semi-serial paraffin sections. The type 1 lesions, i.e., foci of cellular alteration of individual follicles, showed the same pattern of immunostaining as seen in the surrounding hyperplastic follicles and in normal controls. On the other hand, the type 3 lesions, i.e., carcinomas, were mostly negative for T4 and T3 with only partial staining for Tg. The type 2 lesions, i.e. proliferative nodules, which might include a state of nodular hyperplasia as well as benign neoplastic nodules, demonstrated intermediate stainability between the type 1 and type 3 lesions. These results suggest that the histological structures of focal lesions of the thyroid gland may be closely related to their functional activities. Also, the importance of follicular structure formation in thyroid hormone synthesis was demonstrated in each type of focal lesion.

[Squamous metaplasia of the gastric mucosa associated with an aberrant pancreas].

A squamous metaplasia was found in the superficial regenerative epithelium on the aberrant pancreas tissue (Heinrich type II), situated in the posterior wall of the antrum. The squamous epithelium was positively stained by keratin, according to the PAP method, and intestinal metaplasia also was found surrounding the squamous metaplasia. In this case, it was decided that the squamous metaplasia development was associated with the regenerative change of the superficial ulceration. Squamous metaplasia is extremely rare in the stomach, and thus it was difficult to consider that the adenosquamous cell carcinoma of the stomach had developed from the antecedent squamous metaplasia.

Isolation and characterization of chum salmon growth hormone.

Two molecular forms of salmon growth hormone (sGH), sGH I and II, have been isolated from the pituitary glands of the chum salmon (Oncorhynchus keta); a two-step extraction procedure, under alkaline (pH 10) conditions, subsequent to acid-acetone extraction was employed for extraction of the sGHs. They were then purified by iso-electric precipitation at pH 5.6, gel filtration on Sephadex G-100, and high-performance liquid chromatography on ODS. Intraperitoneal injection of sGH I and a combination of sGH I and II at doses of 0.01 microgram/g body wt at different intervals resulted in a significant increase in body weight and length of juvenile rainbow trout. The GH producing cells in the pituitary of mature chum salmon were identified in the proximal pars distalis immunocytochemically with a specific antiserum; no cross-reactivity was seen in the prolactin cells in the rostral pars distalis. A molecular weight of 22,000 was estimated for both sGHs by gel electrophoresis in sodium dodecyl sulfate. Isoelectric points, by gel electrofocusing, of 5.6 and 6.0 were estimated for sGH I and II, respectively, with differences present in the amino acid composition and the N-terminal residue, suggesting that they may be genetic variants coded on two separate genes. The partial amino acid sequences of sGH I at both terminal regions have been determined.

Histopathological changes, mucin production and cellular DNA content in rectal mucosa of patients with ulcerative colitis: a comparison between findings in adults and adolescents.

Histopathological examination, specific mucin staining and analysis of cellular DNA content of rectal mucosa were carried out in twenty-eight tissue specimens biopsied from ten adult and eight adolescent patients with ulcerative colitis. There was an increase in proliferative activity in the rectal mucosa in both groups with active colitis, but, there were no significant differences in the potential for malignant transformation of rectal mucosa in these groups.

[Treatment of cancer pain by continuous subcutaneous infusion of buprenorphine].

The efficacy of continuous subcutaneous infusion of buprenorphine for the treatment of terminal cancer pain was studied. Continuous subcutaneous administration of 4-8 micrograms/kg/day of buprenorphine, examined by the visual analogue scale was revealed to have satisfactory analgesic potency for control of every type of terminal cancer. This therapy can be undertaken by any member of the medical staff because of its safety and simplicity. The indications for this method are almost unlimited when the subcutaneous tissue can absorb the drug at a constant rate. Continuous subcutaneous buprenorphine administration via a portable infusion pump allows patients with severe pain from cancer the opportunity to move about freely.

Cytophotometric DNA analysis of rectal mucosa of patients with ulcerative colitis.

Cytophotometric DNA analysis of rectal mucosa was carried out in 11 patients with ulcerative colitis, in attempt to determine the proliferative activity of rectal mucosa. There was no significant difference in the mean DNA value, modal DNA value, total variation of DNA content and population of over 3c cells of rectal mucosa between controls and the patients. However, the differential determination of the proliferative activity in the upper, middle and lower thirds of the crypt disclosed that a population of over 3c cells in the upper and middle thirds of the crypt was significantly larger in the patients with longstanding disease than in the controls.

The transport of 125I-labeled human high molecular weight urokinase across the intestinal tract in a dog model with stimulation of synthesis and/or release of plasminogen activators.

In an attempt to elucidate the mechanism of fibrinolytic enhancement by orally administered urokinase, studies on the intestinal transport of urokinase were carried out, using 125I-labeled human high mol wt urokinase, administered intraduodenally in the experimental dog model with a saphenous vein thrombus. Using the plasma sample obtained from blood 45 minutes after intraduodenal administration of the urokinase, protein fractions were isolated by a sequential two-step affinity chromatography method, first with [N alpha-(epsilon-aminocaproyl)-DL-homoarginine hexylester]-Sepharose followed by a specific anti-human low mol wt urokinase rabbit IgG-Sepharose (adsorbed-eluted and unadsorbed). Each of the isolated protein fractions was further purified by gel filtration on Sephacryl S-300. The proteins isolated by the two-step affinity chromatography method were transported human urokinase with radioactivity in the adsorbed-eluted fraction, and newly synthesized and/or released dog plasminogen activators, probably urokinase-type and tissue-activator type, without radioactivity. In an antibody quenching assay, dog urokinase and the immuno-affinity unadsorbed fraction were not neutralized, but the immuno-affinity adsorbed-eluted fraction was completely neutralized by the specific anti-urokinase IgG antibody. Proteins isolated from control plasma (after administration of saline) by the two-step affinity chromatography method in the unadsorbed fraction had negligible amounts of activator activity. In these studies, we were able to show that synthesis of plasminogen activators was stimulated, with the activators being released, from either the liver or the vascular endothelium. Also we showed that urokinase is transported across the intestinal tract in the dog model.

[Fundamental and clinical studies on sulbactam/cefoperazone in the field of obstetrics and gynecology].

A combination of sulbactam, a beta-lactamase inhibitor, plus cefoperazone (SBT/CPZ = 1/1) was studied in the field of obstetrics and gynecology, and the results were following: Absorption and transfer into genital organ tissues were good. With the 1g intravenous injection the maximum serum concentrations in the uterine artery were 88.6 micrograms/ml for CPZ and 50.0 micrograms/ml for SBT, and the maximum tissue concentrations were 12.2-17.4 micrograms/g for CPZ and 9.8-21.4 micrograms/g for SBT. When the 2 g was administered, the maximum tissue concentrations were 11.9-26.7 micrograms/g for CPZ and 6.0-8.0 micrograms/g for SBT. These elimination showed the similar trend as their serum levels, and their tissue levels were higher than MIC80 for the main organisms. Their penetration into the intrapelvic dead space exudate was also good and showed that the peak levels were 30.1 micrograms/ml for CPZ and 17.4 micrograms/ml for SBT at 2 hours after the 2 g intravenous injection. The peak levels of 34.4 micrograms/ml for CPZ and 8.8 micrograms/ml for SBT at 6 hours after 2 g intravenous drip infusion were obtained. Their elimination was slow, and the concentration higher than MIC80 for main organisms was maintained for a long period of time. For gyneco-obstetrical infections such as adnexitis, intrauterine, intrapelvic and external genital organ infections, a daily dose of 2-4 g of SBT/CPZ produced a 100% clinical efficacy in 10 patients and a 88.9% bacteriological effect. The eradication ratio was more than 80% against beta-lactamase producing organisms. Side effects were few. The above results indicated that SBT/CPZ was useful in the field of obstetrics and gynecology.

[Pharmacokinetics and clinical studies on ceftriaxone in the field of obstetrics and gynecology].

Ceftriaxone (Ro 13-9904, CTRX), a new cephem antibiotic, was studied in the field of obstetrics and gynecology, and the following results were obtained. The absorption and tissue penetration of CTRX into intrapelvic genital organs were good. The peak serum level in the uterine artery after a single intravenous injection and that after an intravenous drip infusion for 30 approximately 60 minutes, both with 1 g, were 162.5 micrograms/ml and 84.4-93.8 micrograms/ml, respectively. High concentrations were obtained also in genital organ tissues; the maximum concentration was 93.8 micrograms/g by intravenous injection and 56.3-59.4 micrograms/g by intravenous drip infusion. Changes in the tissue concentration were similar to those in the serum, the level over MIC80 against main pathogenic organisms being maintained for a long time. The penetration of CTRX into intrapelvic dead space exudate was good. The level reached a peak of 18.8 micrograms/ml 2 hours after an intravenous injection with 1 g and 13.3 micrograms/ml after 12 hours, while the level over MIC80 against main pathogenic organisms was maintained for a long time. CTRX was effective in 15 out of 16 cases (93.8%) with gyneco-obstetric infections such as intrauterine, intrapelvic, adnexal infections, and postoperative would infections, administered with 1 g twice a day. No side effects were observed.

A family of hereditary stomatocytosis associated with normal level of Na+-K+-ATPase activity of red blood cells.

A rare familial case of hereditary stomatocytosis with hemolytic anemia, increased autohemolysis, increased osmotic fragility, and shortened erythrocyte survival is described. The erythrocytes were abnormally permeable to sodium and potassium. In addition, "Na-K pump" rate of the red blood cells was increased, while Na+-K+-ATPase, Mg2+-ATPase and Mg2+-Ca2+-ATPase activities were within normal limits. Splenectomy induced marked improvement of anemia.
