The optic disk in anterior ischemic optic neuropathy associated with retinal artery occlusion.

Two patients who had retinal artery occlusion associated with impaired peripapillary choroidal circulation were followed up ophthalmoscopically, to evaluate the appearance of the optic disk. The first patient showed central retinal artery occlusion, and the second patient demonstrated branch retinal artery occlusion. A peripapillary choroidal filling defect was noted in both cases by fluorescein angiography, suggesting an association with anterior ischemic optic neuropathy. The first patient exhibited optic atrophy without demonstrating pale disk edema. The second patient showed pale disk edema, but a sectorial defect of disk swelling was noted in the quadrant that corresponded to the area affected by branch retinal artery occlusion.

Influence of vitamin E on platelet survival.

Vitamin E plays an important role in protecting fat tissue from peroxidation. Platelet survival was compared among vitamin E deficient, control and alpha-tocopheryl nicotinate supplemented rats. The mean tocopherol level in the sera of the rats fed vitamin E deficient diet for two months was 21.5% of control, and the lipoperoxide level in the sera of these animals had not significantly (p greater than 0.05) increased. Platelet aggregation was not accelerated significantly (p greater than 0.05) after exposure to a concentration of 10 microM of ADP used as an aggregating agent. The rate of platelet survival was determined by the 51Cr-labeling technique. The survival rates were all normal in vitamin E deficient, control and alpha-tocopheryl nicotinate supplemented rats, with no significant difference (p greater than 0.1) among the groups. These results suggest that depletion of vitamin E per se does not shorten the periods of platelet survival.

Quantitative estimation of the right ventricular overloading by thallium-201 myocardial scintigraphy.

Thallium-201 myocardial scintigraphy was performed on 55 patients with various types of right ventricular overloading. The right ventricular (RV) free wall was visualized in 39 out of the 55 patients (71%). The mean values of right ventricular systolic pressure (RVSP) and pulmonary artery mean pressure (PAMP) in the visualized cases (uptakers) were 54.6 +/- 24.1 and 30.5 +/- 15.3 mmHg, respectively. These values were significantly higher than those of the non-visualized cases (non-uptakers). There were 12 RVSP-"normotensive" uptakers and 15 PAMP-"normotensive" uptakers. The RV free wall images were classified into three types according to their morphological features. Type I was predominantly seen in cases of RV pressure overloading, type II in RV volume overloading and type III in combined ventricular overloading. RVSP in the type III group was significantly higher than that in other two groups. The radioactivity ratio in RV free well and interventricular septum (IVS), the RV/IVS uptake ratio was calculated using left anterior oblique (LAO) view images. The RV/IVS uptake ratio closely correlated with RVSP and PAMP (r = 0.88 and 0.82, respectively). In each group of RV free wall image, there were also close correlations between the RV/IVS uptake ratio and both RVSP and PAMP. Our results indicate that the RV/IVS uptake ratio can be used as a parameter for the semi-quantitative estimation of right ventricular overloading.

[Clinical evaluation of cefotiam in internal medicine, with special reference to infectious disease associated with hematological disorders (author's transl)].

Clinical evaluation of cefotiam (panspolin), a new cephem antibiotics, was performed in the infectious disease associated with hematological disorders and in the respiratory system. In hematological dis orders, 40% of good and 25% of fair results were obtained in clinical effects. In respiratory infections, however, 92% of good results were obtained. Opportunistic infection due to Gram-negative bacilli are often experienced in patients with leukemia. Since cefotiam has sufficient bacteriocidal effects in broad spectrum, it would be a good therapeutic agent against infectious diseases associated with hematological disorders.

Cycloscopy of pseudoexfoliation.

We examined patient's eyes with pseudoexfoliation gonioscopically and cycloscopically. In almost all eyes with pseudoexfoliation, accumulation was found in the zonules and ciliary processes in addition to the lens surface and iris border. In the fellow eyes of unilaterally affected eyes, pseudoexfoliative deposits were found in the zonules and ciliary processes in more than one half of the cases, leading us to conclude that pseudoexfoliative deposits in the zonule and ciliary process may be an early and consistent characteristic of pseudoexfoliation and may be an integral part of the clinical picture of pseudoexfoliation.
