[Clinical study of centrifugal pump bypass as an auxiliary technique for surgical treatment of descending thoracic aorta].

Left heart bypass or arterial bypass using a centrifugal pump (Bio-pump bypass) with a H-PSD shunt tube was performed as an auxiliary technique for the treatment of descending thoracic aorta in 13 cases. Hemodynamic differences during aortic clamp were compared between cases using Bio-pump bypass and 4 cases of axillo-femoral temporary bypass which were carried out in the first term. Furthermore, in Bio-pump bypass cases, the bypass route was investigated from the point of view of operative complications in relation to bypass technique. Peripheral blood pressure and urinary output during aortic clamp were significantly increased in the Bio-pump bypass group. Those results indicated that the Bio-pump bypass was useful as an auxiliary technique especially for high risk patients with low renal function. In particular, left heart bypass was a beneficial technique which was not affected by modality or region of disorder in the descending thoracic aorta. However, this technique should be selected carefully for patients with cardiac disease, for instance severe left ventricular hypertrophy, because two cases of operative complications consisting of pericardial effusion related to the bypass technique were experienced in this study.

Giant congenital nevus and malignant melanoma.

Frequency of malignant transformation arising in giant congenital nevi is considered to be 4%-5%. More than a half of the patients in which malignant melanoma developed in giant congenital nevi were under the age of 10. It may be hypothesized that dermabrasion of giant congenital nevus may provoke malignant transformation. Some of the cell groups in giant congenital nevus are potentially malignant. Some groups of nevus cells were larger in size than those of other portions of nevus. Electron microscopic observation revealed that nuclei of these larger nevus cells were significantly indented, and melanization of melanosomes was irregular. Coexistence of alpha-like actin with beta- and gamma-actins in giant congenital nevus cells and disappearance of alpha-like actin in malignant melanoma cells were noted.

[Dermatological findings in the annual examination of the patients with yusho in 1987-1988--with special reference to the alteration of the skin symptoms in the patients followed up for 20 years].

We reported on the skin symptoms of the patients with yusho in the 1987 and 1988 annual examinations. We also studied the alteration of the skin symptoms in the patients followed up for 20 years. These patients showed obvious improvement in their skin symptoms during these 20 years. However, about a half of the patients still had comedones and/or acneiform eruptions, although the number of eruptions had decreased. The cases with blood PCB pattern A (typical yusho pattern) demonstrated higher skin severity grading points than those of the other groups (patterns B and C), but even in these cases the skin eruptions had improved.

[Stages of information processing and cerebral hemispheric differences in random shape recognition].

The purpose of this experiment was to investigate the differences of cerebral hemisphere function with respect to the way visual pattern information is stored in the long-term memory. The shape recognition in the left and right visual fields was measured after the subjects were trained to associate the shapes with relevant verbal labels, irrelevant verbal labels, or no verbal labels. Subjects were given a shape recognition test by the visual half-field presentation and both free- and aided-recall tests for retention of associated verbal labels. Results indicated that accuracy of recognition of shapes in the left visual field was greater than in the right visual field. Pretraining condition produced a significant main effect but no differential field effect. While irrelevant verbal labeling and observation conditions recognized the stimulus with equal reaction time in either visual field, the latency of relevant verbal labeling condition was longer in right visual field than in left visual field. These results were discussed in terms of the differences of cerebral hemisphere function in each stage of information processing.

Analysis of tissue lymphocytes by double fluorescent staining--gastric cancer tissue and regional lymph nodes.

An immunohistochemical study was performed on human lymphocytes in the tissue of gastric cancer, and also in the regional lymph nodes, by double fluorescent staining, using monoclonal antibodies. Leu3a+8+ cells (inducer T cells) which consist about 30 per cent of Leu 3a+ cells were seen in the tissue surrounding the gastric cancer. The other 70 per cent of Leu 3a+ cells were Leu3a+8- cells (helper T cells). In the lymph nodes they were noted in T cell areas in almost the same proportions, while in germinal centers, only Leu3a+8- cells were found. On the other hand, OKT8+Leu15- cells (cytotoxic T cells) were noted in a large number, while OKT8+Leu15+ cells (suppressor T cells) were few. Further, an increase of OKT8+Leu15- cells was seen around gastric cancer or metastatic cancer in lymph nodes. These immunohistochemical findings suggest that cytotoxic T cells are the main component in the tissue of gastric cancers and the regional lymph nodes. Increases in inducer T cells and helper T cells are probably required to induce cytotoxic T cells around the cancer tissue.

Effect of retinoic acid on cornified envelope formation: difference between spontaneous envelope formation in vivo or in vitro and expression of envelope competence.

A large number of cross-linked envelopes form spontaneously when cell lines derived from chemically induced mouse skin papillomas are cultured in medium containing 1.2 mM calcium. This phenomenon is associated with high activity of the cross-linking enzyme, epidermal transglutaminase (TGase). The influence of retinoic acid (RA) on envelope formation was studied in detail in a papilloma cell line, PE. Retinoic acid (3 microM) completely blocked cornified envelope (CE) production but reduced TGase activity only 50%. A rabbit antiserum was produced against sonicated CEs isolated from newborn mouse skin. On Western blots of epidermal extracts, diffuse staining was observed for particulate proteins of suprabasal, but not basal, cells and similar immunoreactive material was absent from the cytosolic fraction of both cell layers. The antibody also recognized particulate proteins from PE cells induced to differentiate by calcium, but not from cells grown in the presence of high calcium and RA. The antiserum appears to recognize partially cross-linked CE precursor proteins judging by the diffuse staining, the molecular weight range of the proteins stained, and their origin in the particulate cellular fraction. Cross-linked envelopes could be induced in RA-treated PE cells by permeabilization with 0.75 M NaCl or 50 micrograms/ml A23187. However, this treatment failed to cause the appearance of proteins recognized by the antiserum. Preincubation of the antiserum with purified fragments of CEs from newborn mouse epidermis, but not with cross-linked envelopes from permeabilized, RA-treated PE cells, removed immunoreactivity. These results indicate that the cross-linked envelopes formed in RA-treated cells after permeabilization lack a set of proteins contained in CEs from stratum corneum and may even be composed of different proteins. Retinoic acid appears to prevent CE formation in part by inhibiting activation of epidermal TGase but in addition by influencing the synthesis of precursor proteins.

[Recency test as an indicator of frontal lobe function and the verbal regulation of motor behavior].

The development of relationships between the temporal ordering of verbal or non-verbal events and the verbal regulation of motor behavior was examined with a total of 142 4- and 5-years old. Performances on a two-choice button-pushing task were compared between high or low groups determined by either verbal or non-verbal recency task. Children conducted the button-pushing task with their dominant hand under following two conditions: verbalization and non-verbalization of their button-pushing activity. Results showed that the low group in verbal recency task manifested significantly poorer performances than the high group, whereas there was no difference in performance between the high and the low groups in non-verbal recency task. These findings were interpreted as supporting the view that the verbal regulation of motor behavior are related to the function of the left-frontal lobe.

[Anxiety symptoms and their association with work environment and other factors].

Anxiety symptoms and their association with work, work environment, family environment and physical health were studied by mailed questionnaire in 1379 male and 1427 female employees working in a Tokyo-based company and a Kyushu branch of a government-subsidized corporation. Subjective anxiety symptoms were found to increase with poor physical health, poor marital relationship and physical work overload and when the employee felt his superior(s) difficult to deal with. They were little influenced by whether the employee felt his work environment dangerous or not.

[Counseling in industries: its current practices and its future].

Counseling in industries, along with mental health in industries is becoming a major concern among industrial nations in recent years. However, large scale studies on this subject has been few and limited in scope, due partly to the fact that no governmental regulation or accreditation for counseling in industries or for counselors exist. Nevertheless, some enterprises have initiated counseling services. To investigate the actualities of counseling in industries, the authors send questionnaires to 1385 enterprises with an employment of over 500. 52.8% of the questionnaires were collected. Among them 25.8% replied that they already had counseling services. The questionnaire asked counselors at these enterprises about the system of counseling, counselors' duties, counselors' techniques, counselors' training, counselors' educational background and about clients. Since the practice of counseling is determined, to an extent, by the background of the counselors, the authors separated counselors into four groups: medical counselors who graduated from medical schools; psychological counselors who graduated from colleges or graduate schools with a major in psychology; nurse counselors who graduated from schools of nursing; and lay counselors who did not meet the criteria for the above three groups. The results showed that the four groups tend to respond differently to the questionnaire. Based on these results, the authors discussed four different types of counseling in industries. It is hoped that this study, along with future studies, will pave the way for the establishment of coherent systems of counseling in industries, as well as for the accreditation of several types of counselors.

[Gastric and duodenal ulcers as occupational psychosomatic diseases].

Various stresses are considered to be involved in the development of gastric and duodenal ulcers. We interviewed patients of either gastric ulcer group (GU) or duodenal ulcer group (DU) with regular occupation who were admitted to our Psychosomatic Illness Department in order to examine the differences in the characteristics of their stress and developmental factors of their diseases between the two groups. Developmental factors were scored for comparison. The following results were obtained. Though group DU was generally younger than group GU, it tended to have a greater number of episodes. No significant difference was found between the two groups in living habits, life history, or familial, social and physical environments. However, group DU had greater stress than group GU in the working environment. A significantly greater number of patients in group DU smoked and had problems in personal relations as compared with GU. There was a significant correlation between smoking habit and stressfulness of the working environment in both groups. In group DU, there was a significant correlation between the father-child relationship and personal relations. In group DU, a significant number of those who had problems in the relation with their superiors at work had problems in the father-child relation. These findings show that patients with duodenal ulcer had more problems in the working environment, particularly in personal relations, than those with gastric ulcer, and smoking habit appears to accelerate the development of the diseases. Also, a possible association was suggested between problems of personal relations at work and the history of growth.

Human epidermal growth factor in gastric carcinoma as a biologic marker of high malignancy.

The presence of human epidermal growth factor (hEGF) was studied in a total of 210 gastric carcinomas comprising 52 early carcinomas, 113 advanced carcinomas and 45 scirrhous carcinomas. An immunohistochemical study revealed no hEGF-immunoreactivity in early gastric carcinomas, while hEGF-positive tumor cells were detected in 24 (21.2%) of the 113 advanced carcinomas and in 15 (33.3%) of the 45 scirrhous carcinomas. The incidence of hEGF-immunoreactivity in well-differentiated adenocarcinomas was significantly higher than that in poorly differentiated adenocarcinomas (P less than 0.05). Moreover, hEGF-immunoreactive tumor cells were observed in 13 (30.4%) of the 42 scirrhous poorly differentiated adenocarcinomas, the incidence being significantly higher than that in non-scirrhous poorly differentiated adenocarcinomas (P less than 0.05). The average hEGF content in the tumor tissue estimated by radioimmunoassay was 3.77 +/- 0.61 (mean +/- SE) ng/g wet weight in immunohistochemical hEGF-positive tumors and 2.19 +/- 0.18 ng/g wet weight in hEGF-negative tumors, the difference being significant (P less than 0.05). Patients with hEGF-positive carcinomas (excluding scirrhous carcinomas) had much worse prognosis than those with hEGF-negative carcinomas. These results suggest that EGF produced by tumor cells plays an important role in the invasive growth and productive fibrosis of gastric carcinoma and also serves as a biologic marker of high malignancy in patients with gastric cancers.

[Employment conditions and a system of occupational health administration for disabled workers].

A survey using a questionaire was conducted to elucidate the employment conditions and a system of occupational health administration status of the disabled workers in private enterprises. The average employment rate of disabled workers was found to be higher in small-scale enterprises, than in large-scale enterprises. Also, the low employment rate was found among cases of spinal cord injuries and hemipregia. The reasons for workers becoming disabled were "private accident and illness" (59.2%) "congenital" (30.6%) and "labor accident" (10.2%). The main physical factor influencing work efficiency of disabled workers was "degree of disability" while "work morale" was found to be the most influential psychological factor. Also it was found that about 50% of private enterprises will accept the reinstatement and employment of spinal cord injury or hemipregia workers. But it will be necessary that the disabled workers are able to do the requested job. However, it is necessary for employers of disabled workers to make various efforts in overcoming situations such as, changes in organization, improvement of occupational health administration staffs and improvements in the working environment and equipment.

Handedness and sex differences in selective interference of verbal and spatial information.

The purpose of this experiment was to investigate handedness and sex differences with regard to the way verbal and spatial information is coded in selective-interference tasks. Left- and right-handed males and females were required to recall both letters and their positions in a matrix under three different kinds of interpolated conditions: visual, auditory, and noninterpolated activities. The results indicated that the selective-interference effect differed in right- and left-handers, whereas there were no differences by sex. Identity information had a dual encoding for both handedness groups, but it was relatively more biased toward visual encoding in left-handers than in right-handers; position information, however, was predominantly visually encoded--and to the same extent--in both handedness groups. These findings were interpreted as supporting the view that the differences in coding manner of right- and left-handers are related to differences in their hemispheric specialization.

Handedness and sex differences in the processing manner of verbal and spatial information.

Left- and right-handed males and females were given the task of viewing sets of six or eight letters in a 5 X 5 matrix and recalling either (a) the letters, (b) the positions of the letters, or (c) both the letters and the positions. Recall of positions by left-handers was inferior to their recall of letters, and it was inferior to recall of both letters and positions by right-handers. There was no differential recall of letters and positions according to sex. Handedness appears to be a better predictor of cognitive abilities than does sex. The results are discussed in terms of hemispheric specialization referring to handedness and sex.

[Clinical and immunological study on the high incidence of malignant neoplasms in patients with chronic renal failure].

With the advancement of dialysis techniques, rehabilitation and high morbidity are becoming serious problems for patients with chronic renal failure. The authors, with the purpose of investigating the carcinogenic state of such patients, studied the incidence of malignant neoplasms and their immunity both clinically and fundamentally. Out of 499 patients with chronic renal failure, malignant neoplasms were found in 17 cases (3.40%), which is obviously a high rate with a statistically significant difference from that of the ordinary healthy population. In the fundamental immunity study, OKM1+ cells of the patients were significantly lower than those of healthy people, and a decrease in NK and K cells was inferred. Incidentally, the lymphocytes of patients strongly inhibited the mitogen response of healthy subjects. This decline in cellular immunity is considered to be closely related to the carcinogenicity of patients with chronic renal failure.
