[Comparison of latex agglutination and coagglutination methods for the determination of bacterial antigens in cerebrospinal fluid].

The latex agglutination and coagglutination tests were used to demonstrate specific bacterial antigens in cerebrospinal fluid of patients with bacterial meningitis. Latex agglutination was more sensitive than coagglutination. It detected 90% (18/20) of culture positive CSF specimens, whereas coagglutination detected 60% (12/20). Antigens were also detected by latex agglutination in seven of seven additional cerebrospinal fluid specimens after 32-48 hours of antimicrobial therapy, but four of seven by coagglutination.

[Determination of bacterial antigens by latex agglutination tests in cultured cerebrospinal fluid specimens from bacterial meningitis].

Between December 1988 and February 1991, the latex agglutination test was evaluated for its ability to detect the antigens of N. meningitidis, S. pneumoniae and H. influenzae in the samples of cerebrospinal fluid (CSF) of 57 patients with bacterial meningitis. Totally antigens were detected in 34 of 42 (80.9%) patients with bacterial meningitis proven by culture. Latex agglutination was positive in 60% of CSFs for N. meningitidis, 88.2% of CSFs for S. pneumoniae and 100% of CSFs for H. influenzae. Antigens were detected in 7 of 9 (77.8%) patients with no bacterial organisms grown on CSF culture but seen on microscopy after Gram staining. No antigens were identified in CSFs of 4 patients with culture and gram stain negative. 32-48 hours after the treatment started, the CSF samples were obtained again from 11 patients and antigens were detected in 9 of them.

Treatment of testicular hydrocele with tetracycline sclerotherapy.

Fifteen patients with primary hydrocele of the testis were treated by aspiration and injection of tetracycline. Only 5 patients (33.3%) were cured by a single injection of tetracycline. Furthermore, 46.6% of our patients experienced severe scrotal pain, three of whom required open surgery. We do not recommend single injection tetracycline therapy, and further injections were not performed due to severe side effects.

Gamma-glutamyl transpeptidase alterations in liver and serum induced by haloperidol.

Changes in the activity of the microsomal enzyme gamma-glutamyl transpeptidase (GGT) were examined in liver and serum following subacute and chronic haloperidol administration to rats. Haloperidol increased both serum and liver GGT levels. The enzymatic activity returned to baseline values on the sixth day after withdrawal of the drug. It is concluded that alterations in serum GGT levels may be expected during prolonged treatment of haloperidol, and that these alterations reflect enzyme induction in liver microsomes where the drug is entirely metabolized.

Perianal necrotizing fasciitis in a neonate.

A two-week-old term male infant, weighing 1,600 grams was transferred to the neonatology unit of Doküz Eylul University hospital with sharply demarcated cutaneous gangrene surrounding the perianal region. He did well at birth. In his history, on the 10th postnatal day, a red, painful, warm cutaneous lesion was observed which was thought to be secondary to repeated and inappropriate rectal temperature measurements. Besides an ill-appearing child, a nontender frank cutaneous gangrene developed within several days. Klebsiella pneumoniae was cultured from the involved area. Blood cultures were negative. A frozen section of soft tissue biopsy could not be performed because of the localization of the lesion. The patient was successfully treated by surgical debridement and high doses of parenteral antibiotics.

Acrodermatitis enteropathica with Pseudomonas aeruginosa sepsis.

Acrodermatitis enteropathica is characterized by eczematous and scaly plaques on the face, scalp, acral, and anogenital regions. In addition to typical lesions, unusual prominent vesiculobullous lesions are also described. We report a full-term, 9-month-old boy who has acrodermatitis enteropathica and Pseudomonas sepsis. In this patient there were clinical findings of sepsis and eczematous vesiculobullous lesions on the periorificial and acral areas. Serum zinc level was extremely low. Pseudomonas aeruginosa was identified in cultures of blood and fluid which was aspirated from the bullous lesions. After oral zinc sulfate and intravenous antibiotic treatment his condition improved within 2 weeks.

Indocyanine green angiographic findings in young patients with Grönblad-Strandberg syndrome.

PURPOSE: The objective of the present investigation was to define the indocyanine green (ICG) angiographic features of angioid streaks (AS) in young patients with Grönblad-Strandberg syndrome and to compare them with findings on fluorescein angiography (FA) and red-free photographs. METHODS: Complete ophthalmological examination, red-free photography, FA and ICG angiography were performed on 6 patients, 4 women and 2 men, ranging in age from 21 to 33 years, with Grönblad-Strandberg syndrome. RESULTS: ICG angiography showed AS in the form of hypofluorescent lines with numerous associated hyperfluorescent foci. The AS were more clearly visualized and were seen to be more numerous and larger by ICG angiography. Choroidal neovascularization (CNV) and macular involvement had become bilateral in all of the cases. ICG angiography allowed precise localization of CNV in some cases, but it was not much superior to FA in the determination of CNV and the visualization of 'peau d'orange'. CONCLUSIONS: ICG angiography provides some information different from FA in the evaluation of AS, but usually neovascular complications and peau d'orange appearance could be seen more clearly in the red-free photographs and FA. The hypofluorescent AS pattern was significantly observed in young patients with Grönblad-Strandberg syndrome.

Treatment of trichiasis with argon laser.

PURPOSE: In this prospective study, argon laser photocoagulation was used to eliminate misdirected cilia. The cases were followed up to see the results and recurrences with this treatment. METHODS: 60 eyelids of 45 patients with trichiasis were treated with the argon laser. After topical anesthesia a blue-green argon laser was used with 1 watt power, for 0.20 seconds, with 100 micrometer beam diameter. The beam was directed coaxially to the lash follicle to create a 2-3 mm crater and vaporization was observed. After crater formation, the laser parameters were changed to 1.2 watt power, for 0.20 seconds, with 200 micrometer beam diameter, to destroy residual follicular tissue. Up to five lashes were treated in one session. At the end of each session an antibiotic ointment was used t.i.d. for a week. RESULTS: Patients have been followed for 4-12 months (mean 6 months). Recurrences have been seen in 15 of 60 eyelids (25%). Laser treatment has been used with the same protocol for the cases with recurrence and results have been successful in eight of them. Postoperatively severe pain, hemorrhage, scar formation, neovascularization of lid margin, or infection were not seen in any patient. In three cases mild hypopigmentation and in three other cases mild lid notching were observed. CONCLUSIONS: Argon laser lash ablation can be done as an effective office procedure with topical anesthesia. The advantages include low recurrence and complication rates in carefully selected patients and minimal postoperative discomfort.

Flap complications in our learning curve of laser in situ keratomileusis using the Hansatome microkeratome.

PURPOSE: To determine the incidence and the type of flap complications in our initial series of eyes undergoing laser in situ keratomileusis and the impact of microkeratome-related complications on best spectacle-corrected visual acuity. METHODS: We reviewed the charts of the 630 consecutive eyes operated in the Refractive Surgery Department of Cerrahpasa Medical School, University of Istanbul, Turkey using the Hansatome microkeratome and the Summit, SVS Apex Plus excimer laser between March 1998 and November 1999. The mean follow-up was 12.6 months. RESULTS: The incidence of total flap complications was 19.8%. Flap folds (5%) were the most common complication. The incidence of epithelial ingrowth was 3.3%. Diffuse lamellar keratitis was encountered in 20 cases (3.17%). Displaced flaps were seen shortly after the procedure in 8 eyes (1.26%) and repositioned immediately. Improper keratectomy occurred in 14 eyes (2.2%) and incomplete keratectomy in four procedures (0.63%). The presence of interface debris and hemorrhage was each 1.9%. We observed one interface abscess, which was culture-negative and cured with fortified antibiotics, and one free flap. CONCLUSIONS: Although the LASIK procedure with the Hansatome had an easy learning curve without any of the serious complications that frequently occur in this phase, we still observed flap related complications that affected visual outcome.

In vitro effect of silicone oil and liquid perfluorocarbons on platelet aggregation.

PURPOSE: To study the in vitro effect of silicone oil of different viscosities and liquid perfluorocarbons on platelet aggregation. METHODS: Silicone oil with a viscosity of 5700 cs and 1000 cs and liquid perfluorocarbons of perfluoroperhydrophenantren and perfluorodecaline were studied to investigate the effect of these agents on platelet aggregation under in vitro conditions. The experiments were performed by using platelet rich plasma with an aggregometer. Aggregation was induced with three different agonists, including 5"-adenosine diphosphate (ADP), epinephrine (EPI), and collagen (Col). The results were obtained as a percentage of maximal aggregation and compared with controls using Kruskal-Wallis one way analysis of variance test. RESULTS: The tests with ADP as aggregating agent revealed that the percentage of maximal aggregation was a mean of 72.66+/-3.51% for ADP only, 58.66+/-3.05% for silicone oil of 1000 cs, 62.66+/-2.08% for silicone oil of 5700 cs, 56.00+/-7.00% for perfluoroperhydrophenantren, and 52.3+/-3.1 % for perfluorodecaline. With EPI, aggregation was induced in all control samples with a mean of 76+/-9.54%. The mean percentage of maximal aggregation was 66.7+/-3.06 for silicone oil of 1000 cs, 72.33+/-5.5% for silicone oil of 5700 cs, 71.67+/-3.79% for perfluoroperhydrophenantren, and 70.33+/-2.52% for perfluorodecaline. With collagen, it was 86.67+/-1.53% for controls, 83.67+/-3.51% for silicone oil of 1000 cs, 85.33+/-4.51% for silicone oil of 5700 cs, 83.33+/-4.93% for perfluoroperhydrophenantren, and 81.33+/-4.16% for perfluorodecaline. Statistical analysis revealed no significant change in the percentage of maximal aggregation for all tested substances in the experiments. CONCLUSIONS: Silicone oil of different viscosities and perfluoroperhydrophenantren and perfluorodecaline have minimal antiaggregating effect on platelets. The level of effect is not statistically significant.

Efficacy of topically applied glutamate-aspartate and pentoxifylline solutions in decreasing myocardial damage during open-heart surgery in rats.

During open-heart surgery, the period between cross-clamping and maintenance of homogeneous diastolic arrest is often accompanied by significant ischaemic-hypoxic injury. The topical application of glutamate-aspartate or pentoxifylline may reduce energy demands during this period and thus prevent myocardial damage. Fifty rats were divided into five groups. In group A (control) the pericardial cavity was opened, all inlet and outlet vasculature cross-clamped, and the heart excised after 60 s. In groups B-E, the pericardial cavity was opened, all inlet and outlet vasculature cross-clamped for 60 s (groups B and D) or 90 s (groups C and E), and the pericardial cavity filled with glutamate-aspartate solution (groups B and C) or pentoxifylline solution (groups D and E) for 2 min. Following clamping, blood was withdrawn from the right atrium for biochemical analysis, and the heart excised for histological analysis. Histopathological and biochemical analysis showed a significant reduction in ischaemic-hypoxic cardiac injury in rats treated with topically applied glutamate-aspartate or pentoxifylline.

Comparison of the effects of hypertonic saline and crystalloid infusions on haemodynamic parameters during haemorrhagic shock in dogs.

Haemorrhagic shock, a leading cause of mortality, frequently accompanies cardiac interventions. The administration of small volumes of hypertonic saline solutions and crystalloid solutions may reduce the severity and duration of shock, and result in a decrease in adverse outcomes. Under general anaesthesia, hypotension was induced in 12 healthy dogs. After 30 min, dogs received either hypertonic saline solution or lactated Ringer's solution for fluid, resuscitation. In both groups, decreases in haemodynamic variables were observed during the shock period. Following resuscitation, increases in haemodynamic variables were observed. This increase lasted longer in dogs receiving hypertonic saline. In both groups, haematocrit and oxygen content values decreased after resuscitation and lactate levels increased. During resuscitation, lactate levels decreased. Sodium and osmolarity increased with resuscitation, then subsequently decreased. In conclusion, in a model of haemorrhagic shock, the administration of hypertonic saline solutions provides haemodynamic stability and tissue oxygenation more rapidly.

Serum interleukin 2 levels in patients with rheumatoid arthritis and correlation with insulin sensitivity.

Interleukin 2 (IL-2), a Th1 lymphocyte-derived cytokine, is thought to play an important role in the pathogenesis of type 2 diabetes and rheumatoid arthritis (RA). The aim of our study was to evaluate changes in serum IL-2 levels and their correlation with glucose metabolism abnormalities, such as insulin resistance, in patients with RA. Thirty-six subjects with varying degrees of disease activity and 20 healthy age-, sex- and body mass index-matched control individuals were evaluated. Patients with any causes of peripheral insulin resistance were excluded. After a 12-h overnight fast, fasting insulin levels, homeostatic model assessment-insulin resistance (HOMA-IR) estimated insulin sensitivity, and serum IL-2 levels were significantly higher in all patients with RA than in the control individuals. Fasting insulin, HOMA-IR scores and IL-2 levels were correlated in the RA group. This study showed that patients with RA have altered IL-2 regulation, and that there was a significant correlation between serum IL-2 levels and insulin sensitivity.

Histological assessment of the effect of alpha-tocopherol on fracture healing in rabbits.

To investigate the effect of alpha-tocopherol (vitamin E) on fracture healing in rabbits, two groups of 10 rabbits were either injected with alpha-tocopherol (treated) or untreated (controls). The right femurs of both groups were fractured, and the treated group were injected intramuscularly with 20 mg/kg alpha-tocopherol daily for 5 days starting on the day of fracture. After 21 days, histological sections of the fractured region were examined and scored. Fracture healing had progressed further in the alpha-tocopherol group than in the control group. A statistically significant difference between the histological grading of fracture healing in the two groups was found. This difference may result from an antioxidant (alpha-tocopherol) effect on free oxygen radicals in the fracture area. We conclude that alpha-tocopherol may affect fracture healing favourably and might be useful as a therapeutic agent in clinical fracture management.

The effect of caudal bupivacaine versus tramadol in post-operative analgesia for paediatric patients.

In this study 20 paediatric surgical patients were randomized to two groups after induction of general anaesthesia. Group 1 received 0.25% bupivacaine (2 mg/kg) and group 2 received 5% tramadol (2 mg/kg) both as a caudal block. Heart rate, mean arterial pressure, arterial oxygen saturation, respiratory rate and pain and sedation scores were recorded at 1, 2, 4, 6, 12 and 24 h post-operatively. Acetaminophen was administered rectally (20 mg/kg) if the pain score was > 3/10. The pain and sedation scores in group 2 were significantly lower compared with group 1. There were no significant differences in heart rate, mean arterial pressure, arterial oxygen saturation and respiratory rate between the two groups. In conclusion, caudal tramadol was superior to bupivacaine in analgesic efficacy and in reducing the need for additional analgesia during the post-operative period in paediatric patients.
