Ventricular performance in relation to heart rate and AV delay at rest.

The influence of heart rate (HR) and AV delay (AVD) on left ventricular haemodynamics was studied in 12 patients classified as having coronary heart disease (CHD), hypertensive heart disease (HHD), dilated cardiomyopathy (DCM) or who served as controls. Using the conductance catheter technique, haemodynamics were measured during pacing rates of 80 to 180 beat.min-1 at AV delays of 0 to 240 ms. A 3-D linear regression analysis of the data quantified the influence of HR and AVD in principle for each group. An increase in HR resulted in a rise in the cardiac index without changing ejection fraction in the control group only, but led to a decrease in these parameters in HHD and DCM; cardiac index remained constant in CHD. CHD patients frequently had a more pronounced left ventricular end-diastolic pressure (LVEDP) elevation with higher HR, whereas left ventricular end-diastolic volume (LVEDV) and stroke volume decreased. In patients with HHD, lengthening of the AVD resulted in an increase in LVEDV and a decrease in LVEDP and left ventricular end-systolic volume (LVESV) leading to a higher ratio of stroke volume to LVEDP than in the other subsets. In DCM, longer AVD also resulted in a higher SV/LVEDP ratio, but in contrast to HHD the influence of AVD variation on LVEDP and therefore on the LVEDV/LVEDP ratio was missing.

Haemodynamic analysis of atrioventricular tachycardia.

The aim of the study was to delineate the influence of the ventriculo-atrial interval (VAI) in tachycardia with regard to the underlying heart disease. Haemodynamic studies were performed by the conductance catheter technique during paced tachycardia with a HR of 140, 160 and 180 beat.min-1 at various VAI in 10 patients; three with coronary heart disease (CHD), three with hypertensive heart disease (HHD) and four serving as controls. The influence of the VAI accounted for an overall change in cardiac index (CI) of 30 +/- 14%. Alterations in left ventricular peak systolic pressure (LVPSP) depending on VAI were significantly higher (P < 0.01) in CHD patients (32 +/- 9%) than in other groups (14 +/- 9% in the controls and 17 +/- 8% in HHD patients). The influence of VAI on left ventricular end-diastolic pressure (LVEDP) did not differ between the subgroups and accounted for a mean overall change of 32 +/- 14%. Atrial activation during the last third of the cardiac cycle led to the highest values of CI, LVEDP and LVPSP in the control group, whereas in HHD and CHD groups minimal values of CI were correlated with maxima of LVEDP and LVPSP. Conversely, with atrial activation during the medium third of the cardiac cycle minima of CI and LVEDP were observed in the controls, whereas in HHD and CHD patients the highest cardiac index coincided with the lowest LVEDP. Thus tachycardias have different haemodynamic effects depending on the nature of myocardial impairment and the timing of AV coupling.

Eumycetoma caused by Curvularia lunata in a dog.

Curvularia lunata was cultured from black granules found in granulomatous tumefactions excised from the subcutis of a three year old Medium Schnauzer dog. Draining sinuses were present in some of the tumefactions. Accordingly the diagnosis of eumycotic mycetoma was made. This diagnosis was confirmed by histopathological examination. During the four years following the first surgical intervention, several more similar tumefactions were excised on three different occasions. The dog died of chronic renal failure at the age of 8 years. There was no bone involvement or visceral diffusion of the fungus. The granules were examined by scanning electron microscopy. Immunoglobulins in the dog's serum, assessed by a qualitative test, proved to be equal to immunoglobulins in the serum of a control dog. Precipitating antibodies against C. lunata were not found. The dog was treated for 150 days with itraconazole. In spite of good initial results, recurrence of the fungal lesions were observed after the treatment's interruption. Further treatment with itraconazole for 45 days proved ineffective. No side effects of the drug were observed. This is, to the best of our knowledge, the first case in which C. lunata is identified as the causative agent of an animal eumycetoma.

Aspergillus clavatus as the probable cause of a lethal mass neurotoxicosis in sheep.

Sprouted barley grains, the waste product of malt extract production, were incriminated as the cause of a lethal (96% mortality) neurotoxic syndrome in sheep fed the grains. Clinical manifestations, comprising tremors, lameness, abnormal gait, paralysis and death indicated a tremorgenic mycotoxicosis. Whilst 50% of the flock died within 17 days, mortality continued over more than 5 months. Pathological findings were limited to neuronal degeneration and necrosis in the midbrain. Germinating grains were shown to be contaminated with growth of Aspergillus clavatus.

Enzootic nasal tumour of sheep.

This is the first report of Enzootic Nasal Tumours (ENT) of sheep in Israel. The report includes a detailed clinical, pathological and histopathological description of the syndrome. All 7 affected sheep suffered clinically from serous to muco-purulent nasal discharge and respiratory distress due to obstruction of the upper airway by a neoplastic process. Three of the tumours were histopathologically identified as tubular adenomas, 3 were mucoid adenomas and one was a papillary adenoma. The tumours caused destruction of the turbinate bones and nasal septum.

Platelet-associated DNA and anti-DNA antibody in systemic lupus erythematosus with nephritis.

We report DNA and specific anti-dsDNA antibody (dsDNAb) in excess of controls, associated with platelets from 35 patients with systemic lupus erythematosus (SLE). Deoxyribonuclease (DNase) treatment of intact platelets resulted not only in a drop in platelet-associated DNA, but also a dramatic fall in platelet-associated dsDNAb. This suggests that at least some DNA and virtually all the dsDNAb is bound to the platelet surface, probably in the form of an immune complex. The quantity of platelet-associated DNA did not correlate with amounts of platelet-associated anti-dsDNA antibody (PAdsDNAb); perhaps because platelets also have receptors for DNA itself. Total platelet-associated IgG was elevated in only six patients with normal PAdsDNab.

The chronic course of a probable monensin toxicosis in cattle.

Feedlot calves given monensin in their feed developed an excessive mortality which peaked 3-4 mo after monensin was withdrawn. Typically the calves died suddenly and necropsy revealed areas of paleness in the skeletal muscles (early cases) and in the myocardium (more chronic cases). Histology (in 19 calves) showed swollen myocytes with loss of striation and sarcoplasmic vacuolization, followed by the appearance of necrotic fibres and infiltration with macrophages and neutrophils, and finally local and generalized fibrosis. Calcification was not evident. A differential diagnosis ruled out an etiology of nutritional and toxicological origin, thus leaving the probability of atypical (chronic) monensin toxicosis.

Lymphosarcoma in a spotted hyena, Crocuta crocuta.

Lymphosarcoma of the mesenteric and mediastinal lymph nodes with peritoneal and mediastinal implantations, as well as widespread intravascular metastases and thrombosis with haemorrhages was diagnosed in a spotted hyena.

Hepatic cirrhosis suggesting aflatoxicosis in a flock of geese.

An outbreak of hepatic cirrhosis in a flock of young geese was investigated. The affected geese were cyanotic with purple beaks, shanks and footwebs. On post mortem examination severe atrophy of the liver was seen and, histologically, extensive areas of necrosis, cirrhosis and bile duct proliferation were prominent. Liver function tests and serum enzyme levels confirmed that the liver damage was extensive. The findings are compared with those found in aflatoxicosis of other avian species.
