Monoarthritis with heterophil-negative infectious mononucleosis. Case of an older patient.

A patient aged 57 years with infectious mononucleosis and monoarthritis of the knee joint is described. To our knowledge, this association in elderly patients has not been reported heretofore, nor has the pathogenesis of arthritis in infectious mononucleosis been established. Because of the clinical picture and the synovial fluid findings, we suggest that the main mechanism of this arthritis is viral replication in the synovia.

Low molecular weight nonheme iron and a highly labeled heme pool in the reticulocyte.

The ligands that bind iron and heme in reticulocyte and may regulate their distribution are not known. Guinea pig reticulocyte hemolysates were labeled with 59Fe and filtered through molecular weight sieving columns. 59Fe-containing peaks of ferritin, transferrin, and hemoglobin were identified, as well as a low molecular weight peak containing predominantly nonheme iron, and a 17,000 mol wt heme containing peak with a specific activity (59Fe/heme) 20-fold greater than hemoglobin. We note that the low molecular weight peak, the existence of which has been postulated, has been identified for the first time without the addition of a chelating agent. We speculate that the 17,000 mol wt peak is an alpha-chain pool.

Echocardiography in infective endocarditis. Lack of specificity in patients with valvular pathology.

59 patients with suspected infective endocarditis on a natural valve were studied by M-Mode echocardiography to determine the specificity of the ultrasonic technique in detecting valvular vegetations. All echocardiograms were read independently by two observers who were unaware of the final diagnosis. Among 40 patients who later proved not to have infective endocarditis, two (5%) were diagnosed by echocardiography as having either possible or probably vegetation by at least one observer. Both patients with a false positive diagnosis of vegetation had pre-existing valvular pathology, the presence of which greatly complicated the interpretation of the echocardiogram. Inter-observer disagreement occurred in 5 of the 59 studies (8.5%). The results of this study suggest that caution should be exerted in the echocardiographic diagnosis of vegetation in patients with pre-existing valvular pathology.

Iron removal from transferrin. An experimental study.

We have studied the facilitation of iron transfer from transferrin to desferrioxamine by various anions. Most of the anions which can substitute for HCO-3 in the ternary complex of transferrin - Fe - HCO3 do not facilitate iron transfer; anions which do facilitate iron transfer do not necessarily form stable ternary complexes. Combinations of anions effective in transfer have a less-than-additive effect, suggesting a common reaction pathway. We suggest that the transfer of iron from transferrin to desferrioxamine involves a substitution step and a subsequent chelation step, and that the efficiency of the overall reaction is a function of both these attributes of the anion.

Splenic echinococcal cyst burrowing into left pleural space.

We describe a rare case of hydatid cyst in the spleen which communicated to the left pleural cavity. The patient presented with respiratory distress, characterized by nonproductive cough and dyspnea. The difficulties in diagnosis, using standard laboratory and radiologic techniques, were overcome by the use of ultrasound examination diagnosing both the cysts and the supra-diaphragmatic extension.

Rectus sheath hematoma: ultrasonographic diagnosis.

Five cases of rectus sheath hematoma were correctly diagnosed by ultrasound. The main feature in each case was a well defined superficial abdominal transsonic mass; it appeared spindle shaped on longitudinal sections and ovoid on transverse sections. Ultrasonography is suggested as the procedure of choice for the diagnosis of this condition.

Red cells in sickle cell crisis: observations on the pathophysiology of crisis.

The pathophysiology of the occurrence and resolution of sickle cell crisis is unknown. The molecular abnormality is constant, while crisis is episodic. In the present study, red cell filterability and sickling with deoxygenation have been measured during sickle cell crises. Recovery from sickle crisis is associated with an increased filterability of the circulating red cell and a decreased susceptibility of the red cell to sickle with deoxygenation (p less than 0.05). The possibility that these changes are responsible for the resolution of crisis is suggested.
