Serum lipid and lipoprotein levels in long-term insulin-dependent diabetes mellitus. Relation to residual insulin secretion, microvascular lesions and environmental factors.

The aim of the present study concerning patients with long-term insulin-dependent diabetes mellitus was to determine whether the serum lipid and lipoprotein concentrations differ in subjects with and without residual insulin secretion. We also investigated whether factors such as sex, smoking, physical activity and microvascular lesions were associated with particular lipoprotein profiles. C-peptide excretion (greater than or equal to 0.2 nmol) in 24-hour urine samples was used as an indicator of residual insulin secretion. Twenty-two pairs of patients with and without residual insulin secretion matched for age at onset and disease duration were participating in the investigations of glycaemic control and microvascular lesions. The HbA1c was significantly lower in C-peptide excretors than in the non-excretors (6.9 +/- 0.3 vs. 7.9 +/- 0.3%, p less than 0.025). The lipids and lipoprotein fractions were all within normal limits. The HDL2/3 ratio was significantly higher in C-peptide excretors than in non-excretors (1.72 +/- 0.28 vs. 1.10 +/- 0.09, p less than 0.05). Multiple regression analysis showed that factors, such as physical activity, body mass index and glycaemic control could explain more of the variation in the different lipid and lipoprotein fractions than residual C-peptide excretion alone. The only fraction correlating with C-peptide excretion was HDL3 cholesterol. It is concluded that minute residual insulin secretion per se is of minor importance for the regulation of lipids and lipoproteins. Glucose control and residual insulin secretion together with environmental factors seem to be of great importance for the regulation of the lipid and lipoprotein levels in insulin-dependent diabetes mellitus.

Fatty acid composition in serum lipids and adipose tissue in patients with morbus Crohn after ileal resection.

In 36 patients with Crohn's disease who had been operated with ileal resection but were in a stable clinical condition, the fatty acid composition in the circulation and in adipose tissue was correlated to resection length. Fatty acid composition was also compared to data from a group of randomly selected healthy men and women of similar age. In plasma Crohn patients had reduced circulating levels of unsaturated fatty acids. On the other hand, increased proportions of these fatty acids were found in adipose tissue. It is unlikely that in this group of patients these data are unspecific reflections of disease activity. A more reasonable explanation seems to be an increased demand for precursors of local prostaglandin synthesis in the remaining bowel.

Weight reduction with dietary fibre supplements. Results of two double-blind randomized studies.

We report two studies, in which fibre/placebo tablets were added to a weight reduction regimen in the treatment of moderately obese women. In Study I, 60 females were treated for a two-month period with general dietary advice, providing a mean daily energy intake of 1,400 kcal. In addition, the fibre group received a 5 g dietary fibre supplement. In Study II, 45 females were treated for a three-month period with a similar programme, in which the recommended daily energy intake was 1,600 kcal and the fibre supplement 7 g/day. In both groups weight changes, hunger ratings, blood pressure, defecation pattern and possible side-effects were recorded every second week. Before treatment mean body weight was 95.4 kg (Study I) and 99.3 kg (Study II). Six patients dropped out of Study I, and four out of Study II. In Study I mean weight loss, 7.0 kg, in the fibre group was significantly higher (p less than 0.05) than 6.0 kg in the placebo group. In Study II mean weight loss in the fibre group of 6.2 kg was significantly higher than the 4.1 kg in the placebo group (p less than 0.05). No significant difference in hunger feeling between the groups was found. Systolic blood pressure was reduced in all four groups at the end of the treatment, whereas diastolic blood pressure was reduced only in the fibre group in Study II. The results suggest that dietary fibre is of additive value in the treatment of moderately obese patients. The fibre supplement, however, needs to be comparatively high.

Long-term effects of commonly available weight reducing programmes in Sweden.

A majority of people attempting to lose weight will use methods that do not involve medical expertise or professional organizations. Consumer organizations continually have problems with so-called slimming products which claim results that are scientifically unfounded and impossible. In this study four methods commonly used in Sweden were studied with emphasis on long-term effects. One-hundred and twenty moderately overweight women were allocated to one of five groups and repeatedly interviewed and monitored over a 2-year period. The long-term results with a protein powder preparation and a kelp-lecithin-vitamin capsule were poor. However, results were also poor with the nutritionally adequate recommendations issued by the Stockholm Home Economics consultation service but lacking programme support or follow-up. The weight losing programme of a 'slim club' was found to result in the greatest overall sustained weight loss (2.8 +/- 4.7 kg, mean +/- s.d.). An attempt to define 'compliance' with each programme was made, which showed that the majority of overweight women were not able to adhere to these given programmes for a 2-year period, probably because of unsatisfactory results. We consider it essential to document the long-term outcome of these methods in order to facilitate understanding of the situation of these overweight subjects and to assist consumer organizations in their efforts to prevent the spread of scientifically unsound weight reduction methods.

Long-term physical training in female type 1 (insulin-dependent) diabetic patients: absence of significant effect on glycaemic control and lipoprotein levels.

No objective evidence has been presented to support the beneficial effect of physical training on glycaemic control in Type 1 (insulin-dependent) diabetic patients trained two to three times a week for several months. In the present study we examined the possibility that a daily exercise programme would be more suitable for improving glycaemic control. Thirteen patients completed a 5-month study; 6 were randomized to exercise training (20 min daily bicycle exercise) and 7 served as non-exercising controls. The training resulted in an 8% increase in maximal oxygen uptake (p less than 0.05). No change in glycaemic control occurred during the study period in either group. In addition, serum lipid and lipoprotein levels were followed. Total cholesterol decreased during the study period irrespective of training. No effect was noted on the levels of LDL, VLDL, HDL and HDL2 cholesterol. A significant training effect was obtained in the HDL3 subfraction (-10%, p less than 0.05). Total triglycerides were unchanged, but a decrease in the level of LDL triglycerides was observed with training (-12%, p less than 0.01). It is concluded that, in female Type 1 diabetic patients, daily physical training for several months does not improve glycaemic control and results only in minor changes in serum lipoprotein profiles.

Intralipid disappearance in critically ill patients.

Intralipid elimination patterns were compared in 25 healthy controls, 12 patients recovering from uncomplicated cholecystectomy, and 25 critically ill patients. The intravenous fat tolerance test revealed a similar fractional removal rate (k2) in healthy controls and critically ill patients, but k2 was increased in cholecystectomy patients. The concentration of cross-reactive protein (CRP) correlated positively to the concentration of total triglyceride and low-density lipoprotein-triglyceride, and negatively to low-density lipoprotein-cholesterol and high-density lipoprotein-cholesterol. The extrapolated zero-time concentration of Intralipid in the critically ill patients was only one-third of the value in healthy controls. After this initial loss, however, Intralipid was removed from the circulation after first-order kinetics. These low concentrations of Intralipid were not correlated with concentrations of CRP. Possible explanations for this phenomenon include a change in the configuration of the lipid particles, the so-called creaming phenomenon, and/or immediate and substantial uptake of the emulsion by certain organs.

Long-term effects of pindolol on serum lipoproteins in hypertensive patients.

Serum lipoproteins were analysed in 11 hypertensive patients before and after 2 and 16 months on pindolol therapy. In all patients significant blood pressure reductions were achieved with a mean pindolol dose of 13 mg (range 5-30) per day at 16 months' follow-up. After 2 months a significant increase by 35% was found in VLDL TG and cholesterol concentrations, but after 16 months all lipoprotein values had reverted to pretreatment levels. HDL did not change during pindolol therapy. It is not established whether the lack of long-term effects of pindolol on serum lipoprotein is specifically due to its high degree of intrinsic sympatomimetic activity.

Personality traits predicting weight loss outcome in obese patients.

A personality inventory, the Karolinska Scales of Personality (KSP), was administered to a group of 28 severely obese patients before treatment with jaw fixation. Several items significantly predicted incapacity to maintain the achieved weight loss 1 year after treatment: anxiety, monotony avoidance, suspiciousness and a low degree of socialization. These personality traits constitute a syndrome of ego weakness or impulsivity which warrants further study.

Psychiatric determinants of weight changes in jaw-fixed obese patients.

Forty-five severely obese patients were treated with jaw fixation as a means of initiating weight loss. Patients who reported eating for consolation showed significantly greater difficulties in maintaining the achieved weight loss than the others. This finding constitutes further evidence of a relationship between obesity and depression. Low capacity to experience feelings of aggressiveness also seems to be linked to poor treatment results.

Long term treatment of severe obesity: four year follow up of results of combined behavioural modification programme.

A four year programme of treatment for severe obesity combining standard techniques such as behavioural modification, exercise, nutritional advice, and, in addition, readmission of patients who relapse has been developed. One hundred and seven subjects of both sexes were treated. Thirty nine had their jaws fixed from the start. After four years 104 out of 107 subjects were traced; 33 (31%) had left the programme. The mean loss of weight in the remaining 74 subjects was 11.7 kg (range -20 to 55.5): 14 had lost more than 20 kg, 35 had lost 5-20 kg, 17 had lost 0-5 kg, and eight were above their weight before treatment. The rate of dropping out in this study was lower than that generally reported. Our data suggest that combined behavioural modification used as a programme for reducing weight may result in a substantial loss of weight for several years even for severely obese subjects.

Treatment of prostatic cancer: effects on serum lipoproteins and the cardiovascular system.

We studied 32 patients with prostatic cancer before, and after 1 and 6 months of treatment with orchiectomy, estramustine phosphate or conventional estrogens (polyestradiol phosphate plus ethinyl estradiol). Lipid metabolism was evaluated by lipoprotein analysis and the intravenous fat tolerance test. Effects on the cardiovascular system were studied by exercise electrocardiography, blood volume estimation and thoracic electrical impedance measurement, a sensitive method to detect early signs of fluid retention. Present treatment programs for prostatic cancer seem to result in effects on lipoprotein metabolism that probably are of minor importance for the development of atherosclerotic manifestations. Measurement of thoracic impedance may be of value to detect fluid retention in individual patients.

Intramuscular ceruletide does not affect food intake in obese and non-obese human subjects.

Intramuscular ceruletide or placebo was given in a randomized double-blind crossover design to 12 non-obese and 12 obese individuals, 30 min before a palatable lunch meal. No significant effects were found on the amount of food intake or the hunger ratings in any group. Although rapid CCK or ceruletide infusions have been found in some studies to reduce food intake in man and animals, slow infusions have increased food intake. Under the present study conditions, the moderate rate of release of ceruletide from intramuscular depots did not affect the food intake.

Agglutinate formation in serum samples mixed with intravenous fat emulsions.

The degree of fat agglutination (the so-called "creaming phenomenon") was measured in sera from 51 critically ill patients, 200 ambulatory patients and 24 healthy volunteers. A 400-microliter serum sample was mixed with 10 microliter of a fat emulsion (Intralipid), incubated at 37 degrees C, and examined after 2, 6, and 24 h. Almost all samples from critically ill patients exhibited creaming, as did 31% of the samples from ambulatory patients. Serum samples from healthy volunteers were incubated with purified C-reactive protein (CRP). At a CRP concentration of 40 mg/L, creaming was always observed. Creaming also occurred at normal serum levels of CRP, but at increased concentrations of other serum proteins. Finally, the creaming test was performed with 12 different fat emulsions, mixed with sera from critically ill and healthy subjects. Healthy sera produced no creaming and sera from critically ill patients invariably showed creaming with every emulsion.

Long-term beta 1-selective adrenergic blockade and adrenergic receptors in human subcutaneous adipocytes.

The influence of beta-adrenergic blockade with metoprolol, a beta 1-selective agent, on the adrenergic regulation of lipid mobilization was explored in subcutaneous adipocytes removed from 13 patients with essential hypertension. Treatment with metoprolol, which was associated with adequate beta-adrenergic blockade and an antihypertensive effect, resulted in a significant increase (p less than 0.05) in the binding of the beta-adrenergic antagonist (-)-(3H)-dihydroalprenolol and a 50% increase (p less than 0.01) in the maximum lipolytic response to the beta-adrenergic agonist isopropylnoradrenaline. In 7 patients with normotriglyceridaemia the total plasma triglyceride level increased significantly (p less than 0.025) during metoprolol treatment, a change that was due to an increase in the very low density lipoprotein triglycerides. The findings suggest that chronic treatment with the beta 1-selective adrenergic blocker metoprolol leads to a significant increase in beta-adrenoceptor density and an increase in the lipolytic response to beta-adrenergic agonists. This latter finding may, in some measure, account for the increased plasma triglyceride level observed.

Effects of long-term therapy with labetalol on lipoprotein metabolism in patients with mild hypertension.

The effects of labetalol on serum lipoproteins, the intravenous fat tolerance test (IVFTT) and lipoprotein lipase (LPL) and hepatic lipase (HL) activities were studied in 16 patients with mild hypertension before and after 6 months of therapy. Most patients were found to be normotensive on 200 mg labetalol/day. Before therapy the mean concentration of serum TG was 0.75 +/- 0.21 (SD) mmol/l, of total cholesterol 5.41 +/- 1.25 mmol/l and of HDL cholesterol 1.67 +/- 0.61 mmol/l. After labetalol no significant changes were found in the concentrations of TG and cholesterol in the VLDL, LDL and HDL fractions. The mean values for the IVFTT and for LPL and HL activities were in the normal range and remained unchanged during therapy.
