Predicting ten-year survival of patients with primary cutaneous melanoma: corroboration of a prognostic model.

BACKGROUND: Recently, the Pigmented Lesion Group at the University of Pennsylvania described a 4-variable model for predicting 10-year survival for patients with primary cutaneous melanoma. The variables are tumor thickness, anatomic site of the lesion, age, and gender. The objective of the current study was to test the validity of this model, employing the large data base of the New York University Melanoma Cooperative Group. METHODS: The predicted probabilities of 10-year survival for 780 patients with primary cutaneous melanoma were determined by multivariate logistic regression, using the 4 variables. RESULTS: The overall 10-year survival rate of the current study group was 78.4%. Of the four variables, tumor thickness, anatomic site of the lesion, and age were found to be independent predictors of survival. Although survival was better for women, gender was not a statistically significant factor in predicting 10-year survival when entered into the multivariate logistic regression model. In the current study, the probability of 10-year survival of patients with melanomas < 0.76 mm ranged from 93-99%, depending on the age and primary site. Age and site had more impact on the prognosis of intermediate and thick melanomas than on thin melanomas. Thus, for melanomas 0.76-1.69 mm, 1.70-3.60 mm, and thicker than 3.60 mm, the probabilities of survival ranged from 70-94%, 39-82%, and 23-68%, respectively. CONCLUSIONS: The wider ranges in survival rates for thicker melanomas, depending on the other variables, emphasize the importance of including variables in addition to tumor thickness in a prognostic model. Using a large data base from a medical center, the current study supports the prognostic multivariate model of the Pigmented Lesions Group of the University of Pennsylvania; however, the authors of the current study did not find gender to be statistically significant in this multivariate model.

Midazolam in treatment of various types of seizures in children.

Midazolam is a recently developed water-soluble benzodiazepine that shares anxiolytic, muscle relaxant, hypnotic and anticonvulsant actions with other members of this class. There are limited studies that midazolam can be used successfully to treat seizures in adults and children. In this study, 0.2 mg/kg intramuscular (i.m.) midazolam was administered to 11 children (eight boys and three girls), aged 3 days to 4 years (mean age 1.8 +/- 1.4 years), with seizures of various types. In all but one child, seizures stopped in 15 s-5 min after injection. No side effects were observed. These results suggest that i.m. administration of midazolam may be useful in a variety of seizures during childhood, especially in case of intravenous (i.v.) line problem.

Kaposi's sarcoma: the efficacy of a single fraction of 800 cGy.

BACKGROUND: Although Kaposi's sarcoma is the most common neoplasm in association with AIDS, classic Kaposi's sarcoma is rare. The best treatment modality is still unclear. METHODS: Between December 1993 and December 1995, 42 fields in 12 patients with Kaposi's sarcoma were treated at our department. Only 3 patients had a history of renal transplantation and immunosuppressive treatment. A single dose of 800 cGy with 4-6 MeV electron beams was delivered to all patients. RESULTS: Complete response rates at the 1st, 6th and 12th months after treatment were 45.2, 62 and 89.7%, respectively. Side effects included various degrees of hyperpigmentation and mild edema and fibrosis in 1 patient. CONCLUSION: Our results suggest that a single application of 800 cGy is as effective as a more protracted course of radiotherapy with a comparable complication rate.

The three-dimensional bone interface of an osseointegrated implant. II: A morphometric evaluation after three months of loading.

To clarify the three-dimensional bone structure around two plasma-sprayed hydroxyapatite-coated titanium implants loaded for 3 months in a monkey, computer graphic evaluation was performed after computer-assisted integration of 70 serial buccolingual sections at 75 microm intervals. Quantitative analysis of the bone contact ratio (BCR) was also conducted with programmed software. The graphics revealed the macroscopic bone structure. The BCR values varied in portions from the top to the bottom of the implant and in the buccolingual or mesiodistal directions. Because these two implants had more cortical bone at the top portion in the lingual direction, the BCR was greatest in the lingual direction. The BCR values at the bottom portion of the two implants were greater than those at the top portions along the horizontal plane, which suggested the effect of loading. The lowest BCR value was observed at the top portion of one implant at a site where moderate periimplant gingivitis was observed. The total surface BCR value for the implants was 69.3% and 64.5%. These results provide additional detailed information on the bone structure around the hydroxyapatite-coated implant after short-term loading.

Monocyte activity in Behçet's disease.

Monocytes obtained from patients with Behçet's disease (BD) were examined for differentiation markers (expression of CD14 and antigens reacting with monoclonal antibodies 25F9 and G16/1) and for expression of selected adhesion molecules. There was significantly raised expression of the CD14 molecule, and increased staining with 25F9 and G16/1 antibodies in monocytes obtained from patients with BD. A monocyte activation marker, soluble CD14, was also found to be raised in patients' serum compared with normal serum (8.1 +/- 9.2 vs 1.4 +/- 0.7 micrograms/ml). Furthermore, BD patients' monocyte culture supernatants caused significantly increased adhesion of normal neutrophils to endothelial cell monolayers in vitro. All these findings show that BD patients' monocytes are active in vivo and produce a number of pro-inflammatory cytokines which may play a role in the chronic inflammation found in these patients.

Neutrophil adhesion to endothelial cells and factors affecting adhesion in patients with Behçet's disease.

OBJECTIVES: To study the in vitro adhesion of polymorphonuclear leucocytes (PMNLs) to endothelial cells in patients with Behçet's disease (BD), and the humoral and cellular factors which may contribute to adhesion. METHODS: A total of 118 patients with BD and 60 healthy controls were studied. In vitro adhesion of chromium-51 labelled normal neutrophils to human umbilical vascular endothelial cell (HUVEC) monolayers were studied in the presence of normal serum or serum obtained from patients with BD. Adhesion of neutrophils from patients with BD to HUVEC stimulated with tumour necrosis factor (TNF), interleukin-1 (IL-1), and lipopolysaccharide (LPS) and adhesion molecule (CD11a, CD11b, CD18 and L-selectin) expression on the patient's neutrophils and lymphocytes were determined, and the serum concentration of IL-8 was measured. RESULTS: Sera from patients with BD were found to enhance the adherence of normal PMNLs to HUVEC monolayers in vitro. Patients' sera induced an increase in surface expression of CD11a and CD18 on normal neutrophils and intercellular adhesion molecule-1 (ICAM-1) expression on HUVECs. The number of CD11a positive neutrophils was greater in the blood of patients with BD than in that of healthy controls (89.4% v 71%; p < 0.001). Pretreatment of HUVECs with IL-1 alpha, TNF alpha or LPS resulted in an increased adhesion of patients' PMNLs greater than that observed for normal PMNLs. Monoclonal antibodies to CD11a, CD11b, CD18, and ICAM-1 caused varying degrees of inhibition of neutrophil adhesion. The concentration of IL-8 was also found to be significantly increased in sera of patients with BD (490 (SD 470) pg/ml) compared with normal controls (97.5 (56.3) pg/ml). CONCLUSION: Abnormalities of neutrophils, endothelial cells, or both, have been suggested to be responsible for many of the clinical manifestations of BD. Our findings may explain the underlying mechanism of neutrophil accumulation in Behçet's lesions.

Hemodynamic profiles in severe preeclamptic and superimposed preeclamptic pregnancies.

In this study, we investigated the hemodynamic profiles of pregnancy-induced hypertension cases. Hemodynamic measurements were performed in five severe preeclamptic and three superimposed preeclamptic cases. We applied Swan-Ganz catheters and measured hemodynamic parameters. According to the results, we suggest that pulmonary edema occurs more easily in severe preeclampsia than in superimposed preeclampsia.

A comparison of stress and strain distribution characteristics of two different rigid implant designs for distal-extension fixed prostheses.

Stress and strain patterns around two rigid implant designs used as an abutment were compared and the displacement of natural teeth was investigated. The finite-element method was utilized to determine the stress and strain distributions. The results were evaluated in terms of the maximal tensile and compressive stress and strain in the bone around the abutment tooth and implants. The results indicated that the ITI 1 (hollow screw) implant led to high stress concentrations particularly in the apical region. The stress-transferring characteristics of ITI 2 (solid screw) implant were found to be more suitable than were those of ITI 1.

Re-evaluation of skin-explant model in graft-versus-host disease prediction.

An in vitro skin explant model has been proposed for prediction of graft versus host disease and reported to be highly sensitive and specific for this purpose. In this study we have re-evaluated this model in both HLA full-matched BMT recipient-donor sibling pairs and also in HLA one haplotype-matched parent-children pairs. All assessments were made blindly by 3 independent observers. The predictive value of the test for the occurrence of clinical GvHD in 14 BMT patients was found to be less sensitive than reported previously (correlation coefficients were +0.019, +0.067 and -0.061 between clinical GvHD and in vitro primed allogeneic, primed mixed and unprimed allogeneic settings, indicating "poor" correlation). False positive and false negative results were high and there were also significant discrepancies between three blind observations in the grading of skin changes. Weighted kappa analysis revealed that there were "fair" correlations between the 3 observers (K = 0.25). These results indicated that the skin explant model is an unpredictable test system and there are great problems in standardization of the method.

Comparative bioavailability of three batches of four commercial acetaminophen tablets.

The aim of this investigation is to evaluate four brands of acetaminophen tablets and three batches of each brand for their in vitro properties and in vivo bioavailabilities using urinary excretion data. All batches dissolved at least 80% of their acetaminophen content within 30 minutes. The differences between the dissolution rates of the batches were not found statistically significant except the batches of brand C. With respect to their dissolution rates, the brands have the following order: A less than B less than C less than D. Statistical analysis of the percent bioavailabilities and the elimination rate constants showed that there was no significant difference between brands and or between the batches. The percent excreted in the first three hours was found to be significantly different between the solution and tablet A, between tablet A and tablet C, and between Tablet A and Tablet D.

Effects of different calcineurin inhibitors on sex hormone levels in transplanted male patients.

Hormonal abnormalities in male patients with end-stage renal diseases are primarily organic and related to uremia as well as the other comorbid factors that frequently contribute to chronic renal failure and concomitant drug administration. The restoration of hormonal profiles after successful renal transplantation is still controversial. Immunosuppressive drugs may influence hormonal profiles. Our cross-sectional study of 37 male kidney transplant recipients investigated two groups according to their calcineurin inhibitor therapy, namely 21 cyclosporine versus 16 tacrolimus patients. The two groups were matched for age, graft function, mean duration of dialysis before transplantation, and duration of follow-up after transplantation. There was no statistical significant difference in baseline circulating levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone (TTE), and prolactin (PRL) between the two groups. We found that calcineurin inhibitors have favorable effects on sexual hormone levels of male renal transplant patients and that there is no difference in baseline hormone levels between cyclosporine- and tacrolimus-treated male patients.
