Erythema multiforme: As a complication of allergen-specific immunotherapy.

Here we present a 25-year-old female patient admitted with a complaint of blistering lesions on her face, neck, chest and extremities appearing after the first dosis of specific immunotherapy and diagnosed as erythema multiforme. To our knowledge, there are no papers in the literature reporting erythema multiforme due to specific immunotherapy.

Cell adhesion molecules in subretinal fluid: soluble forms of VCAM-1 (vascular cell adhesion molecule-1) and L-selectin.

PURPOSE: In this study we investigated the presence of soluble VCAM-1 and soluble L-selectin-1 in subretinal fluids (SRF) of patients suffering from rhegmatogenous retinal detachment . METHOD: Subretinal fluids were collected from drainage sclerotomies during surgery from 27 patients with rhegmatogenous retinal detachment complicated by proliferative vitreoretinopathy (PVR) or uncomplicated retinal detachment. Levels of sVCAM-1 and sL-selectin-1 were quantified with ELISA. RESULTS: The mean +/- SEM values of sVCAM-1 and sL-selectin-1 were 222.2 +/- 81 ng/ml and 171.7 +/- 42.1 ng/ml, respectively. The concentrations of sVCAM-1 in patients with Grade C PVR (498.2 +/- 1703 ng/ml) were significantly different from those with Grade B PVR (45.6 +/- 16.5 ng/ml) and uncomplicated retinal detachments (19.4 +/- 12.3 ng/ml). SVCAM-1 concentration in detachments which had been present for more than 8 weeks was 738.8 +/- 431 ng/ml, significantly higher than the levels in detachments of shorter duration (132.4 +/- 47.7 ng/ml). sL-selectin-1 level in Grade C PVR (291.6 +/- 92.8 ng/ml) was higher than in uncomplicated retinal detachments (72.8 +/- 13.5 ng/ml). Significantly elevated levels of sL-selectin-1 were observed in detachments lasting more than 8 weeks (605 +/- 151.1 ng/ml) compared to those of shorter duration (96.3 +/- 13.1 ng/ml). CONCLUSION: The present study supports growing evidence that these cell adhesion molecules are involved in the inflammatory process during the development and progression of PVR.

Accessory scrotum: an unusual localization of scrotal skin.

Accessory scrotum is an extremely rare congenital scrotal anomaly defined as the occurrence of scrotal skin outside of its proper location with no testicular tissue. We report a preterm twin male infant who had an accessory scrotum located on the right inguinal and proximal femoral region.

Bilateral abdominal aplasia cutis congenita associated with atrial septal defect: a case report.

A full-term infant girl who had bilateral, symmetrical, sharply defined, triangular-shaped ulcers on both sides of the umbilicus is described. A punch biopsy specimen showed histologic findings of aplasia cutis. Echocardiographic examination revealed primum-type atrial septal defect. To our knowledge, this association is the first of its kind to be reported in the literature.

Human erythrocytes are not suitable for determination of intravascular volume of perfused rat liver.

Homologous or heterologous erythrocytes have been widely used for the estimation of intravascular volume of the liver. However, cross-species blood mediates immune response in the organ, and foreign cells are rapidly cleared from the plasma, indicating that heterologous erythrocytes may not be a suitable marker for determination of vascular space. This aspect was investigated in the perfused rat liver preparation following bolus administration of human (heterologous) erythrocytes into the portal vein. To compare the extent of its distribution within the liver, rat (homologous) erythrocytes and Evans blue were chosen as the reference vascular and extracellular markers, respectively. Hepatic distribution of human erythrocytes was influenced by the perfusion medium (with and without protein) and injection number (first and second injections). Mean transit time and hence volume of distribution decreased in the presence of protein and repetition of the injection. Even in the presence of protein, the volume of distribution obtained for human erythrocytes was larger than that of the extracellular volume of the liver obtained with Evans blue (0.22 +/- 0.01 vs. 0.20 +/- 0.02 ml/g), indicating that they are not suitable for determination of intravascular volume of the perfused rat liver preparation.

Fabrication of a dual-purpose surgical template for correct labiopalatal positioning of dental implants.

An implant-supported prosthesis can potentially provide a functional and esthetic reconstruction, provided that the implants are precisely placed in the predetermined location and angulation. Since the accurate placement of osseointegrated implants in the anterior maxilla with questionable bone support is often a clinical challenge, the use of computed tomography and surgical guides is crucial. This article describes a technique to fabricate a template that served as a guide for radiographic evaluation and was then modified for use in the surgical phase of the treatment.

QT dispersion and signal-averaged electrocardiogram in hemodialysis and CAPD patients.

OBJECTIVE: The aim of this study was to compare QT dispersion (QTd) and signal-averaged electrocardiogram (SA-ECG) parameters that may predict risk of malignant arrhythmias in patients on hemodialysis (HD), on continuous ambulatory peritoneal dialysis (CAPD), and in controls. SETTING: Controlled cross-sectional study in a tertiary-care setting. PATIENTS: 28 HD (M/F 18/10; mean age 32 +/- 9 years), 29 CAPD (M/F 17/12; mean age 34 +/- 10 years), and 29 healthy controls (M/F 17/12; mean age 32 +/- 8 years) were included. INTERVENTIONS: On ECG, minimum (QTmin) and maximum (QTmax) QT duration and their difference (QTd) were measured. In SA-ECG, duration of filtered QRS, HFLA signals less than 40 microV, and RMS voltage (40 ms) were also measured. RESULTS: Higher serum Ca2+ and lower K+ levels were found in CAPD compared to HD. All QT parameters were increased in HD and CAPD compared to controls. QT dispersion was significantly prolonged in HD compared to CAPD. In HD, QTd was correlated with left ventricular (LV) mass index (r = 0.53, p = 0.004), but not in CAPD (r = -0.09, p = 0.63). QT dispersion was significantly prolonged in patients with LV hypertrophy compared to patients without hypertrophy on HD (68 +/- 18 ms vs 49 +/- 18 ms, p = 0.008). In the analysis of SA-ECG, 3 of the 28 (11%) HD and 2 of the 29 (7%) CAPD patients had abnormal late potentials. Patients on HD and CAPD had significantly higher filtered-QRS duration compared to controls (105 +/- 15 ms and 104 +/- 12 ms vs 95 +/- 5 ms, respectively, p = 0.04). Patients with LV hypertrophy had higher filtered-QRS duration compared to patients without hypertrophy (109 +/- 12 ms vs 95 +/- 8 ms, p < 0.001). CONCLUSION: Dialysis patients had prolonged QTd and increased filtered-QRS duration in SA-ECG compared to controls. Patients on HD had longer QTd than patients on CAPD. QTd has been correlated to LV mass index in HD, but not in CAPD. This difference might be due to the effect of different dialysis modalities on electrolytes, especially the higher serum Ca2+ levels.

Formulation and in vitro/in vivo evaluation of terbutaline sulphate incorporated in PLGA (25/75) and L-PLA microspheres.

Terbutaline sulphate (TBS) is widely used in the treatment of bronchial asthma, chronic bronchitis and emphysema. Because of its short biological half life and dosing schedule, a long acting TBS formulation is required to improve patient compliance. The objective of this study was to develop a TBS containing biodegradable microsphere formulation. Poly(D,L-lactide-co-glycolide) (PLGA) and poly(L-lactic acid) (L-PLA) were chosen as matrix materials. A solvent evaporation method was used for preparation of microspheres. Surface morphology, particle size distribution and encapsulation efficiency were investigated. In vitro release studies were performed in pH 7.4 phosphate buffer. In vitro distribution of microspheres were studied in the Swiss albino male mice. All microspheres were spherical in shape and had a porous surface with mean diameters of 9-21 microm. The encapsulation efficiency was influenced by the polymer type, but not the molecular weight. About 90% of the initial amount was trapped in PLGA microspheres, and the remainder was on the surface. In the case of L-PLA, 50% of the total drug was associated with the surface of microspheres. The In vitro release pattern was biphasic characterized by an initial burst phase followed by a slower phase. The L-PLA microspheres released approximately 92% of the initial payload in 72 h. On the other hand, TBS release was increased with an increase in the molecular weight of PLGA. Biodistribution of L-PLA microspheres was characterized by an initially high uptake (35%) by the lungs. All these results suggest that L-PLA and PLGA microspheres have the potential to be used for passive lung targeting.
