[Application of the nucleolar organizer regions to urinary cytology and its computer-assisted image analysis].

Quantification of argyrophilic nucleolar organizer regions (AgNORs) stained by the silver colloid method in urinary exfoliative cells from 36 benign urological diseases and 33 transitional cell carcinomas (TCC) of the urinary bladder was carried out. Also, AgNORs of these cells were measured by means of the computer-assisted image analysis system. There was a significant correlation between AgNORs numbers of cells from voided or wash-out smears and those from touch smears (p less than 0.01). The mean AgNOR numbers per nucleus showed a stepwise increase from non-infected benign urological diseases (mean +/- SD: 3.33 +/- 0.60) through infected urological diseases (3.88 +/- 0.58), grade 1 of TCC (5.23 +/- 1.39) and grade 2 of TCC (6.34 +/- 0.86), to grade 3 of TCC (8.09 +/- 1.19). The maximum number of AgNORs in each group indicated almost the same results. The estimation of the distribution of the maximum AgNOR number might be of great value for rapid and reliable detection of bladder cancers. Moreover, the computer-assisted image analysis of AgNORs could offer an objective index for the cytological assessment of urinary bladder diseases including TCC.

[Endourological re-establishment by transluminal puncture for a complete obliteration of traumatic urethral disruption].

We performed transluminal re-canalization by needle puncture under fluoro-endoscopic control on a total of ten patients (membranous urethra, 8; bulbous urethra, 2) with complete obliteration after traumatic urethral disruption. The re-canalized tract was created by balloon dilation, internal urethrotomy or endoscopic resection of the scar tissue. The patient age ranged from 10 to 57 years (mean age; 37.9 +/- 15.3 years). A transluminal puncture was attempted in three patients by transperineal-transurethral approach and in seven by transurethral approach. In one of them, who sustained a complete membranous disruption 5 weeks ago, transluminal puncture failed because of the movable proximal urethra. The remaining nine patients voided without difficulty for the mean follow-up period of 18.1 +/- 14.3 months (range: 2-46). Three patients had mild stress urinary incontinence. A transluminal puncture is less invasive, safe and reliable for re-establishment of the true proximal part of the disrupted urethra. It can be recommended as a useful technique for endo-urological re-establishment for a completely obliterated urethra. Although this endoscopic re-establishment requires urethral sounding or internal urethrotomy for 6 months postoperatively, it may be an alternative to conventional surgical repair.

Pharmacokinetics of an etoposide infused over three days: concomitant infusion with cisplatin.

The pharmacokinetics of a 72-hour infusion of 240 mg/m2 etoposide administered concurrently with 90 mg/m2 cisplatin was studied in 12 lung cancer patients. The area under the curve (AUC), elimination half-life, steady state concentration, systemic clearance, renal clearance of etoposide and distribution volume at steady state were 225.4 +/- 39.2 micrograms x h/ml, 8.1 +/- 3.4 h, 3.1 +/- 0.6 micrograms/ml, 18.8 +/- 3.1 ml/min/m2, 3.1 +/- 1.4 ml/min/m2, 9.6 +/- 3.8 l/m2, respectively, which were in accordance with those reported previously in patients treated with etoposide alone. Although concentration at 24 hours, total bilirubin level and total protein level were correlated with the AUC which in turn correlated with hematologic toxicity, the variables were not predictive of hematologic toxicity. We conclude that the concomitant administration of cisplatin at a dose level of 30 mg/m2/day might not affect the pharmacokinetics of prolonged etoposide infusion.

[A comparative clinical study of adjuvant chemotherapy of tumors in the head and neck areas by means of HCFU].

In order to evaluate the efficacy of HCFU as an adjuvant chemotherapy agent for squamous cell carcinoma of the head and neck areas, a randomized comparative study was performed with cooperative facilities. 191 patients who had undergone radical therapy were randomly assigned to the following two groups: Group A was the control group, which underwent radical therapy only, and Group B was a group which received long-term HCFU administration (300-600 mg/day x 12 weeks or more). In the comparison of non-recurrence rate in both groups, Group B had better results among patients who were at the advanced clinical stages such as N1 through N3, Stage II (p = 0.0583) and Stage III (p = 0.0970). The adverse reactions to HCFU included pollakiuria, feverishness, and digestive symptoms, but these were mild and the recovery was possible.

[Downhill esophageal varices due to superior vena cava syndrome].

This study examined the incidence of downhill esophageal varices due to SVC syndrome. The presence of downhill esophageal varices was examined by esophagoscopy or autopsy. Downhill esophageal varices were detected in two of nine (22%) patients with SVC syndrome. One case was a 66-year-old male case who and underwent upper lobectomy for lung cancer in September 1985. He developed SVC syndrome due to recurrence in 1986 and died in 1987. Downhill esophageal varices wer in the upper esophagus by esophagoscopy and confirmed by autopsy. The another case was a 61-year-old female who developed carcinoma of the tongue in 1986 and lymph node metastasis to the mediastinum in 1987. Autopsy revealed downhill esophageal varices in the upper and middle esophagus. Eleven cases of downhill esophageal varices due to SVC syndrome reported in Japan were reviewed and discussed.

[Changes in oxygen uptake and carbon dioxide elimination during induced hypotension in humans].

Changes in oxygen uptake and carbon dioxide elimination during induced hypotension were examined in eight anesthetized patients undergoing radical mastectomy. 1) The induced hypotension to 75% of control values had no obvious effect on oxygen uptake and carbon dioxide elimination. 2) The results suggest that it is important to avoid the decrease in oxygen transport by low blood perfusion and to avoid the increase in oxygen debt during induced hypotension. 3) Non-invasive metabolic monitoring of gas phase will be more necessary in future to monitor the anesthetic depth and to manage the nutrition.

[A case of intrathoracic perineurial cyst].

A 67-year-old man was referred to our surgical clinic for evaluation of an abnormal shadow on the chest X-ray examination. CT scan and MRI revealed a tumor in the posterior mediastinum and destruction of the pedicle and intraspinal invagination by the tumor. The tumor in a paravertebral area are linked with a vertebral canal through a destructed intervertebral foramen. MRI showed that the tumor was a cyst which contained fluid. On thoracotomy a hen's egg sized cyst was found in the posterior mediastinum, the cyst was excised. Pathological diagnosis was a perineurial cyst. This case was clearly differentiated from an intrathoracic meningocele and a neurilemmoma microscopically. This case is the first intrathoracic perineurial cyst in Japan.

Metabolism and pulmonary toxicity of intratracheally instilled cupric sulfate in rats.

Though copper (Cu)-handling workers are known to be at risk to develop metal fume fever and acute respiratory failure, very little is known about the metabolic fate and pulmonary toxicity of Cu compounds deposited in the lung. We have investigated time-course and dose-related changes in Cu contents, the production of metallothionein (MT) and inflammatory responses in the rat lung following intratracheal instillation of cupric sulfate (CuSO4). Intratracheally instilled Cu was cleared rapidly from the lung with a half-time of 7.5 h. Copper-binding MT was induced in the lung tissue following Cu instillation and the amount of MT increased with the dose of CuSO4. However, the production of MT contributed little to the accumulation of Cu in the lung. The pulmonary toxicity of CuSO4 was evaluated by examining time-course and dose-effect profiles of cytological and biochemical inflammatory indices (enzymes, protein and elements) retrieved in bronchoalveolar lavage fluids. These results revealed that the inflammatory indices reached maximum values in their activities and contents at 12 h to 3 days after instillation and a dose of 5 micrograms Cu/rat was sufficient to produce acute inflammatory responses in the rat lung.

Thermodynamic analysis of the interaction of prolactin with its receptor in the rabbit mammary-gland microsomes.

The interaction of prolactin (PRL) with its membrane receptor depends markedly on temperature. Thermodynamic parameters for this reaction have been evaluated from data for time-course kinetics and equilibrium binding at multiple temperatures between 19 and 31 degrees C. The free-energy change with temperature and the van't Hoff plot were found to be linear. These suggest that there are minimal structure changes at the PRL-receptor contact site over this temperature range. The positive signs of the entropy and enthalpy of reaction, and of the entropy of activation (delta S++) for association, indicate that the hydrophobic bonding is the most significant force involved in PRL-receptor formation. The delta S++ for dissociation was negative, and the enthalpy of activation for dissociation was about 20.3 kJ.mol-1 larger than that for association, indicating that the PRL-receptor complex is further stabilized by contributions of hydrogen bonds and van der Waals contacts after the initial interaction. The free energy of activation for dissociation, at 25 degrees C was about 2.5-fold larger than that for association. This would cause slow dissociation of PRL from its receptor.

[Treatment results of maxillary sinus carcinoma. A retrospective study].

The treatment results of a total of 617 cases with maxillary sinus carcinoma mainly treated by radiotherapy at the Department of Radiology, Osaka University Hospital between 1967 and 1983 were reviewed. The overall actuarial 5-year survival rate was 36.2%. The cases were divided into 3 groups by period of the treatment modalities corresponding each 5-year survival rate of 24.1% on 217 patients for 1967 to 1971, 40.0% on 155 patients for 1972 to 1975, and 44.6% on 245 patients for 1976 to 1983. The treatment results have improved in 5-year survival rate as modifying treatment modality. The actuarial 5-year local control rate was 60.0% for T1 on 9 patients, 43.1% for T2 on 181 patients, 31.7% for T3 on 246 patients, and 19.2% for T4 on 181 patients, respectively. For more detailed analysis of the results, the cases were divided into 4 groups according to the initial treatment; the group 1 was treated by radiotherapy alone (RT), the group 2 by radiotherapy combined with intra-arterial infusion of 5-fluorouracil (RT + 5-FU), the group 3 by radiotherapy combined with surgical removal of the tumor mass through curettage under the general anesthesia (RT + OP) and the group 4 by multidisciplinary procedure combined with radiotherapy, 5-FU regional infusion and curettage (RT + 5-FU + OP). In the T2 cases, the 5-year actuarial local control rate was 25.5% for RT on 51 patients, 45.0% for RT + 5-FU on 55 patients, 58.2% for RT + OP on 23 patients and 51.5% for RT + 5-FU + OP on 52 patients, respectively. In the T3 cases, the rate was 13.2% for RT on 85 patients, 33.6% for RT + 5-FU on 78 patients, 52.3% for RT + OP on 31 patients and 45.9% for RT + 5-FU + OP on 52 patients, respectively. In the T4 cases, the rate was 7.6% for RT on 77 patients, 27.2% for RT + 5-FU on 52 patients, 17.8% for RT + OP on 20 patients and 31.5% for RT + 5-FU + OP on 32 patients, respectively. The combination therapy with 5-FU regional infusion and/or curettage had successfully improved the local control rate. On the T2 and T3 cases of RT + OP, an additive effect of 5-FU infusion was not significant, and also on the T4 cases of RT + 5-FU, any usefulness of curettage could not be found.(ABSTRACT TRUNCATED AT 400 WORDS)

Immune reactivity in bronchogenic carcinoma and its relation to 5-year survival rate.

We performed a prospective study on the correlation of various parameters of the immune response with the 5-year survival rate in patients with bronchogenic carcinoma. Parameters were initially examined before starting treatment. Delayed hypersensitivity skin tests, lymphoblastogenesis, natural killer (NK) cell activity, and interleukin-2 (IL-2) production were employed to assess immune competence. Each reaction was classified into four or five grades in accordance with intensity; the 5-year survival rate of the patients showing each grade of the immune response was calculated. A correlation between response before treatment and the survival rate was most clearly noted for lymphoblastogenesis. The skin tests and the NK cell activity showed poorer correlations, and no exact correlation was noted between the IL-2 production and the immune response.

The difference in endocochlear and endolymphatic sac d.c. potentials in response to furosemide and canrenoate as diuretics.

We investigated the effects of furosemide, a loop diuretic, and canrenoate, an aldosterone antagonist, on the endocochlear potential (EP) and the endolymphatic sac potential (ESP) in the guinea pig. Furosemide produced no significant change in the ESP at a dose of 100 mg/kg after an intravenous infusion for 20 min. However, this dose decreased the EP to a negative level. Canrenoate produced no significant change in the EP at an intravenous dose of 300 mg/kg for 20 min, but it did decrease the ESP. The differences in the EP and ESP in the response to the diuretics indicate a dissimilarity of the origin of both d.c. potentials in the endolymphatic space.

Na/K-ATPase in rabbit paranasal sinus mucosa during induced sinusitis.

Sinusitis was produced in rabbits, after which animals were separated into three groups: allergic sinusitis, induced purulent sinusitis, and spontaneous purulent sinusitis. Mucosal specimens were taken from these animals and normal controls. Na/K-ATPase was localized cytochemically and its activity studied in order to define the energy metabolism of secretion. The Na/K-ATPase reaction was unable to be clearly distinguished in either the allergic sinusitis specimens or the normal mucosa. In both purulent sinusitis groups, an intensive reaction was observed in the subepithelial glands and a weak reaction was found in the goblet cells. The Na/K-ATPase activity in the purulent sinusitis groups was significantly higher than that in the normal control group. The increased Na/K-ATPase activity may be an affect of hyperactivity of the secretory cells.

Ifosfamide, cisplatin, vinblastine combination chemotherapy in the treatment of advanced non-small-cell lung cancer.

A total of 42 evaluable patients with previously untreated advanced non-small-cell lung cancer were treated with a combination of cisplatin (80 mg/m2, day 1), vinblastine (5 mg/m2, days 1 and 15), and ifosfamide (1.2 g/m2, days 1-3). In all, 1 complete response and 15 partial responses were obtained, for an overall response rate of 38% (95% confidence limits, 23.6%-54.4%). The median duration of response was 15 weeks, and the median overall survival was 56 weeks. Toxicity mainly consisted of moderate to severe alopecia in 28 patients (67%), moderate to severe nausea and vomiting in 27 subjects (64%), and leukopenia comprising less than 1,000 leukocytes/mm3 in 6 cases (14%). In all, 16 patients (38%) had microscopic hematuria (WHO grade 1), but no hemorrhagic cystitis was documented. Although this three-drug combination appears to have moderate antitumor activity against non-small-cell lung cancer, the addition of ifosfamide to the combination of cisplatin and vinblastine did not seem to improve the response rate.

Sodium dependency of GABA uptake into glial cells in bullfrog sympathetic ganglia.

The kinetics of sodium dependency of GABA uptake by satellite glial cells was studied in bullfrog sympathetic ganglia. GABA uptake followed simple Michaelis-Menten kinetics at all sodium concentrations tested. Increasing external sodium concentration increased both Km and Vmax for GABA uptake, with an increase in the Vmax/Km ratio. The initial rate of uptake as a function of the sodium concentration exhibited sigmoid shape at 100 microM GABA. Hill number was estimated to be 2.0. Removal of external potassium ion or 10 microM ouabain reduced GABA uptake time-dependently. The effect of ouabain was potentiated by 100 microM veratrine. These results suggest that at least two sodium ions are involved with the transport of one GABA molecule and that sodium concentration gradient across the plasma membrane is the main driving force for the transport of GABA. The essential sodium gradient may be maintained by Na+, K(+)-ATPase acting as an ion pump.

Antimetastatic effect of defibrinogenation with batroxobin depends on the natural killer activity of host in mice.

Using batroxobin, a thrombin-like enzyme found in snake venom, the effects of defibrinogenation on artificial lung metastasis in mice were studied. The role of natural killer (NK) cells in the inhibitory effects of defibrinogenation on metastasis was also investigated. Artificial lung metastasis experiments were performed by inoculating either B16-F10 cells or B16-BL/6 cells, highly metastatic strains of B16 melanoma cells, into C57BL/6 mice via the tail vein. The administration of batroxobin significantly inhibited lung metastasis, as did NK activity augmented by poly (I).poly (C) were administered, lung metastasis was more markedly inhibited. When NK activity was suppressed by administration of anti-(asialo GM1) antibody, lung metastasis was markedly increased. When batroxobin was administered with anti-(asialo GM1) antibody, no inhibitory effects on lung metastasis, such as those seen with batroxobin alone, were observed. The administration of batroxobin had no effect at all on spleen lymphocyte NK activity. These results indicated that defibrinogenation due to batroxobin inhibits lung metastasis, and these effects depend on NK activity of the host.

Depressed adenoma in the large intestine. Endoscopic features.

To clarify the presence of depressed adenomas in the human large intestine, a prospective study was performed from January 1986 to December 1987. During these two years, 997 colonoscopies were conducted in patients, excluding cases of familial adenomatosis coli. Of 32 small, depressed lesions biopsied, seven were depressed adenomas, demonstrating that depressed adenomas do exist in the colon and rectum, and can be detected endoscopically. Resembling a sucker, they are easily detected through inflation and deflation.
