Electron-microscopic cytochemistry of the catecholaminergic innervation of TRH neurons in the rat hypothalamus.

The catecholaminergic innervation of thyrotropin-releasing hormone (TRH) neurons was examined by use of a combined method of 5-hydroxydopamine (5-OHDA) uptake or autoradiography after intraventricular injection of 3H-noradrenaline (3H-NA) and immunocytochemistry for TRH in the same tissue sections at the electron-microscopic level. TRH-like immunoreactive nerve cell bodies were distributed abundantly in the parvocellular part of the paraventricular nucleus (PVN), in the suprachiasmatic preoptic nucleus and in the dorsomedial nucleus of the rat hypothalamus. In the PVN, a large number of immunonegative axon terminals were found to make synaptic contact with TRH-like immunoreactive cell bodies and fibers. In the combined autoradiography or 5-OHDA labeling with immunocytochemistry, axon terminals labeled with 3H-NA or 5-OHDA were found to form synaptic contacts with the TRH immunoreactive nerve cell bodies and fibers. These findings suggest that catecholamine-containing neurons, probably noradrenergic, may innervate TRH neurons to regulate TRH secretion via synapses with other unknown neurons in the rat PVN.

[Carcinoma of the oral cavity].

The diagnosis and treatment of carcinoma of the oral cavity have improved with the recent progress of medical science, further enhancing the recovery of oral function and therapeutic results. These processes are described with special reference to primary cases of carcinoma of the oral cavity (squamous cell carcinoma) examined in our department during the past eight-year period.

A study of the clinical characteristics and treatment of oral carcinoma in situ.

Twelve patients with oral carcinoma in situ were examined to clarify the clinical features and the most suitable therapy for this lesion. Seven cases were classified clinically as the erythroplakic type, two as the leukoplakic type, and three as the mixed type. The cumulative survival rate of ten cases, excluding two cases with lesions of the soft palate, was 100% in 10 years. Of 12 lesions of oral carcinoma in situ 50% progressed into invasive carcinoma. Erythroplakic lesions of oral carcinoma in situ were more aggressive than the other two types. The most suitable therapy for this lesion is prompt surgical treatment.

Immunocytochemical observations of corticotropin-releasing-factor-containing neurons in the rat hypothalamus with special reference to neuronal communication.

Synapses between neurons with corticotropin-releasing-factor-(CRF)-like immunoreactivities and other immunonegative neurons in the hypothalamus of colchicine-treated rats, especially in the paraventricular nucleus (PVN) and the supraoptic nucleus (SON) were observed by immunocytochemistry using CRF antiserum. The immunoreactive nerve cell bodies and fibers were numerous in both the PVN and the SON. The CRF-containing neurons had synaptic contacts with immunonegative axon terminals containing a large number of clear synaptic vesicles alone or combined with a few dense-cored vesicles. We also found CRF-like immunoreactive axon terminals making synaptic contacts with other immunonegative neuronal cell bodies and fibers. And since some postsynaptic immunonegative neurons contained many large neurosecretory granules, they are considered to be magnocellular neurosecretory cells. These findings suggest that CRF functions as a neurotransmitter and/or modulator in addition to its function as a hormone.

[Long-term prognosis of oral squamous cell carcinoma histologically judged to have been eliminated by the treatment with bleomycin alone].

This study was intended to reveal the long-term prognosis in 21 patients with oral squamous cell carcinoma evaluated histologically as having disappeared by bleomycin treatment. Of 16 cases treated with bleomycin alone, recurrence was observed in 4, and cervical metastases in 2, with survival achieved for 5 years or more in 7 (more than 10 years in 3). No recurrence was observed in 5 patients in whom surgery was additionally performed after the bleomycin treatment, but cervical metastasis was found in one of them. In the cases with epithelial dysplasia after bleomycin therapy, recurrence and metastasis were frequent compared to those with normal epithelium, and this discrepancy was of statistical significance. It has been proved that there are long-term survivors among patients with oral carcinoma treated with bleomycin alone.

Surface roughness of squamous cell carcinoma of the oral mucosa.

The surface roughness value of 39 oral squamous cell carcinomas was measured by the Rmax method. The means of the eight measurement values of each carcinoma ranged from 87 micron to 1176 micron. The average value of all 39 lesions was 341 micron. The average value of the lesions of the papillomatous and granulomatous types was larger than that of the leukoplakic and erosive types. The lesions with the size of more than 2 cm in diameter were rougher than those of less than 2 cm. The lesions of the gingiva were rougher than those on the tongue. There was an inclination that the surface of the verrucous carcinoma was rougher than that of the invasive squamous cell carcinoma and that the surface of early carcinoma was smoother than that of the invasive squamous cell carcinoma.

Maxillary sinus carcinoma: result of radiation therapy.

Four hundred and sixteen patients with carcinoma of the maxillary sinus received primary therapy consisting of external beam irradiation alone or in combination with surgery and/or chemotherapy at the Department of Radiology, Tokyo Medical and Dental University Hospital, between 1953 and 1982. In our institution, methods of treating cancer of the maxillary sinus have been changed from time to time and showed different control rates and clinical courses. An actuarial 10-year survival rate of 21% has been obtained by megavoltage irradiation alone as well as 34% actuarial 10-year survival rate by megavoltage irradiation with surgery. After the introduction of conservative surgery followed by conventional trimodal combination therapy (antrotomy: intraoral partial maxillectomy + radiation + intra-arterial infusion), the local control rate has been improved. The amount of functional, cosmetic, and brain damages have been remarkably decreased by this mode of therapy. The actuarial five year survival rate was 67%. In addition, along with the improvement of the local control rate, the control of nodal and distant organ metastases have been emerging as one of the important contributions to the prognosis of this disease.

Immunocytochemical localization of serotonin-like immunoreactivities in the ciliated epithelium of the frog palatine mucosa.

Immunocytochemical localization of serotonin (5-HT)-like immunoreactivities was studies in the ciliated epithelium of the frog palatine mucosa by the peroxidase anti-peroxidase (PAP) method. 5-HT-like immunoreactivity was found only in the small granular vesicles (100-150 nm in diameter) and not in any mature large secretory granules or in other cell organellae in the goblet cells. No 5-HT-like immunoreactivities were found in any other epithelial and secretory cells in the palatine epithelium. It appears therefore that 5-HT-like immunoreactive granular vesicles have certain physiological effects on the ciliary movement of the ciliated cells or in the goblet cells.

Immunocytochemical localization of TRH and autoradiographic determination of 3H-TRH-binding sites in the arcuate nucleus-median eminence of the rat.

Immunocytochemical and autoradiographic localization of thyrotropin-releasing hormone (TRH)- and 3H-TRH-binding sites was studied in the arcuate nucleus-median eminence region of the rat. TRH-like immunoreactivity was found in dense granular vesicles (90-140 nm in diameter) in TRH-like immunoreactive nerve fibers and terminals. In the median eminence, the immunoreactive terminals were observed to be in direct contact with the perivascular basal lamina of the portal vessel and to form synaptoid contacts with tanycytes. In the arcuate nucleus, the immunoreactive terminals were often found to form axosomatic and axo-axonic, and/or axo-dendritic synapses. The uptake of tritiated TRH into the nerve fibers and terminals of the median eminence was also observed by autoradiography and the distribution and localization of silver grains in them were analyzed quantitatively by circle analysis. Thirty minutes after intraventricular infusion of 3H-TRH, radioactive labeling occurred in type-2 and 3-nerve fibers and terminals containing dense granular vesicles in the median eminence. It is therefore suggested that the neurons labeled after 3H-TRH infusion possess certain functions as physiological recognition sites or receptors for TRH.
