Mucinous carcinoma of the breast: further characterization of its three subtypes.

Nineteen cases of mucinous carcinoma of the breast were studied. Twelve tumours were of the pure type, and seven were mixed. All had abundant neutral and acidic mucin, and stained strongly with CAM 5.2. Of the 12 pure mucinous tumours, six were devoid of argyrophilic granules and were S-100 negative, and only one was CEA positive. All six patients are alive with no evidence of recurrence (mean follow-up 42 months). The other six pure mucinous tumours were rich in argyrophilic granules. Five of these showed S-100 positivity and all were CEA positive. One patient developed local recurrence and one died of myocardial infarction with no evidence of tumour recurrence (mean follow-up 80 months). Of the seven mixed tumours, only one contained an occasional cell with argyrophilic granules and four had variable degrees of CEA positivity. Two patients died and one developed bony metastasis (mean follow-up 40 months). Our findings emphasise the microscopic and prognostic differences between the three subtypes of mucinous carcinoma of the breast, and support the concept of dividing pure mucinous tumours into two distinct subtypes. We suggest that the latter subtyping can be qualitatively made on the basis of the presence or absence of argyrophilic granules in the tumour cells.

Gastric metaplasia and Campylobacter pylori infection of duodenum in patients with chronic renal failure.

Duodenal biopsy specimens from 80 patients with chronic renal failure, who were undergoing haemodialysis, were examined by light microscopy for evidence of inflammation, gastric metaplasia, and Campylobacter pylori infection. Chronic duodenitis was present in 47 (59%) of patients, of whom only seven (9%) showed evidence of active inflammation. Gastric metaplasia was present in 50 (62.5%) of patients, yet Campylobacter pylori was identified in only two patients (2.5%). It is suggested that the duodenal environment of patients with chronic renal failure remains hostile to the growth of these organisms in spite of the presence of gastric metaplasia.

Oestrogen receptors in mucinous carcinoma of the breast: an immunohistological study using paraffin wax sections.

An immunohistological method (Shintaku-Said method) for the demonstration of oestrogen receptors in routinely processed paraffin wax embedded tissue was applied to 19 cases of mucinous carcinoma of the breast. Seventeen (89%) tumours showed variable degrees of positivity and two were negative. In eight cases the receptors were also assayed biochemically using a dextran-coated charcoal method, and the results of the two methods showed good correlation. No difference in the distribution of positive and negative cases was noted between pure and mixed mucinous tumours, and in the latter group the pattern of staining of the mucinous elements was similar to that seen in the solid elements. It is concluded that the major advantage of this method is its ability to offer for study the distribution of the receptors in individual cells and specific histological structures. The results also indicate that most mucinous carcinomas of the breast are oestrogen receptor positive, irrespective of whether they are pure or mixed type.

Mammary duct ectasia and pituitary adenomas.

Mammary duct ectasia developed in three postmenopausal patients who had had pituitary chromophobe adenomas. The first patient had bilateral duct ectasia that developed 8 and 11 years after hypophysectomy. The second patient, who also had bilateral ectasia, had a prolactin-producing pituitary adenoma for which bromocriptine was prescribed. The ectasia developed in one breast before commencing bromocriptine therapy, and in the other breast 2 years later. The third patient also had a prolactin-producing pituitary adenoma. Unilateral duct ectasia developed while bromocriptine was taken. The ectasia in all patients was very marked and affected all excised ducts. Cholesterol granulomas were sometimes very extensive. These cases suggest a relationship between certain hypothalamic/pituitary disorders, possibly related to prolactin secretion and the development of mammary duct ectasia in postmenopausal patients.

Chronic intestinal pseudo-obstruction in two patients. Overlap of features of systemic sclerosis and visceral myopathy.

Two patients with chronic intestinal pseudo-obstruction are reported, one of whom had definite systemic sclerosis while the other had certain manifestations of the disease. Biopsies of the small intestine revealed increased collagen and elastic fibres in the muscularis propria and subserosa in each case, while ganglion cells were normal. In addition, one patient had severe atrophy of the inner muscle layer and the other obvious vacuolation of the muscularis propria. Ultrastructural examination confirmed degeneration of the smooth muscle fibres. These latter features have been considered the hallmark of visceral myopathy and their finding in this patient therefore suggests that the different causes of pseudo-obstruction may not be completely demarcated and cases in which the features overlap may occur.

Liver cell carcinoma with orcein-positive granules and no identifiable copper.

A liver cell carcinoma removed surgically by partial hepatectomy from a 79-year-old woman contained abundant orcein-positive granules in many tumour cells. The granules were similar to those usually identified as representing copper-associated protein, yet they lacked copper, as demonstrated by negative rhodanine and rubeanic acid stains and energy dispersive X-ray microanalysis.

Carcinoma of the breast in women aged 35 or less.

Breast cancer in young women is an unusual problem which has acquired a poor prognostic reputation in the eyes of many clinicians. We have reviewed all 59 patients with malignant disease of the breast aged 35 years or less managed in the Breast Unit at Charing Cross Hospital between 1974 and 1984. After exclusion of 5 patients (2 with lymphomas, 1 with fibrosarcoma, and 2 with inadequate available data), 54 patients were available for analysis. More than 3 months delay from first symptoms to presentation was seen in 45 per cent and 36 per cent were considered 'late' at initial clinical staging (T3-4, N2-3 or M1). Nevertheless, a 5-year survival of 66 per cent was seen among the 29 patients with sufficient length of follow-up. Distribution among the main histological types was very similar to that found in a consecutive series of 264 breast cancers of all age groups in our unit. We conclude that breast carcinomas in young patients do not significantly differ in type or behaviour from other age groups and that with treatment similar to other premenopausal patients, a prognosis comparable to older patients can be expected.

Apocrine carcinoma of the breast containing foam cells. An electron microscopic and immunohistological study.

The detailed light and electron microscopic and immunohistological features of an invasive apocrine carcinoma of the breast developing in a 78-year-old woman are presented. The stroma of the tumour contained non-neoplastic lipid-filled (foam) cells. To our knowledge, these cells have not been described before in invasive breast carcinoma. Their electron microscopic and immunohistological features confirm their histiocytic nature.

Distribution of carbonic anhydrase I in gastric and duodenal tissue sections.

Forty-four sections of normal and abnormal gastric and duodenal tissue were examined by an immunoperoxidase technique for the demonstration of carbonic anhydrase I. The presence of the enzyme in normal gastric parietal and surface epithelial cells was confirmed. The enzyme was also demonstrated in normal and metaplastic intestinal absorptive cells, and in gastric adenocarcinoma of the usual and superficial types. The enzyme was normally absent in antral and Brunner's glands, normal and metaplastic goblet cells, and signet ring tumor cells. Gastritis and duodenitis were associated with a decrease in the staining intensity of the enzyme in surface epithelial cells, suggesting a possible decreased enzyme activity, which may enhance the risk for peptic ulceration in susceptible persons.

Multicentric early adenocarcinoma involving stomach and duodenum.

Six separate foci of superficial adenocarcinoma were seen in the first part of the duodenum of a 53-year-old man with a multicentric superficial adenocarcinoma of the stomach. To our knowledge, duodenal involvement in such a tumor has not been reported before. The duodenal foci were partly covered by gastric-type surface epithelium, suggesting that they might have arisen in areas of gastric metaplasia or heterotopia.

The incidence of amyloidosis complicating inflammatory bowel disease. A prospective survey of 177 patients.

Three cases of secondary amyloidosis are reported, two in patients who had unequivocal Crohn's disease and one in whom the clinical course was that of Crohn's disease, but with histologic findings that were more suggestive of ulcerative colitis. All had evidence of renal failure. A prospective study of 177 patients with inflammatory bowel disease of greater than five years' duration was carried out in an attempt to establish the incidence of secondary amyloidosis, using rectal biopsy and simple renal function tests. No new cases were found. Neither was there evidence of renal failure due to other conditions. In the absence of renal dysfunction, a search for secondary amyloidosis probably is not justified.

Human chorionic gonadotrophin expression in colorectal adenocarcinoma.

The presence of human chorionic gonadotrophin (HCG) in colorectal adenocarcinoma was studied histologically in 45 tumors using immunoperoxidase technique. Ten neoplasms (22.2 percent) contained HCG-positive tumor cells. These cells were present mostly at the periphery of the tumors. Many formed parts of glands, while some were arranged in syncytial clumps or columns, or singularly. Thus, these tumor cells resembled trophoblastic tissue not only in being HCG-positive but also in their peripheral distribution and occasionally in morphologic appearance. HCG-positive tumors were seen more commonly in the rectosigmoid region (90 percent) and were more aggressive than HCG-negative tumors. In this study, lymph node or liver metastases were present in 70 percent of HCG-positive tumors compared with 29 percent of negative tumors--a difference which is statistically significant.

Metastatic spindle-cell renal carcinoma presenting as multiple intestinal polyps.

A 75-year-old woman presented with rapidly progressive intestinal and general symptoms including diarrhea, melena, weight loss, back pain, and lassitude. Endoscopy revealed multiple intestinal polyps which, microscopically, consisted of metastatic tumor deposits composed predominantly of spindle-shaped tumor cells. The primary tumor, a mixed spindle-cell and clear-cell carcinoma, was later identified in the kidney at autopsy. The patient had a cholecystectomy 13 years previously.

Alveolar variant of invasive lobular carcinoma of the breast. A tumor rich in estrogen receptors.

During a study of the correlation between the histologic types of breast carcinoma and their hormone receptor contents, eight cases of the alveolar variant of invasive lobular carcinoma were identified, and all had estrogen receptor protein concentrations higher than 400 fmol/mg cytosol protein. Four more cases of the same variant were later prospectively diagnosed and their high estrogen receptor concentrations correctly predicted. All 12 tumors occurred in postmenopausal women. Most (92%) presented in Stage I or II of the disease. None of the patients died of the tumors during the short period of follow-up (median two years). It is concluded that the alveolar variant of lobular carcinoma of the breast is a tumor with consistently high estrogen receptor concentration and probably a low-grade malignancy. It is also suggested that the tumor may be the lobular equivalent of the ductal infiltrating Comedo carcinoma.
