[Dermatoses in pregnancy].

A number of immunological, endocrine, metabolic and vascular changes with various influences on the skin occur in the organism during pregnancy. The specific dermatoses of pregnancy are a heterogenous group of diseases with a different impact on both mother and child. Their clinical characteristics, diagnostic criteria and therapeutic options are discussed in the present review.

Beneficial effects of spin-labelled nitrosourea on CCNU-induced oxidative stress in rat blood compared with vitamin E.

This study was carried out to determine the effects of a recently synthesized 1-ethyl-3-[4-(2,2,6,6-tetra-methylpiperidine-1-oxyl)]-1-nitrosourea (SLENU), compared with vitamin E as a positive control, on 1-(2-chloroethyl)-3-cyclohexyl-1-nitrosourea (CCNU)-induced oxidative stress in rats. We determined plasma malonyl dialdehyde (MDA) levels and the activities of erythrocyte superoxide dismutase (SOD) and catalase (CAT). Forty two white albino healthy rats were treated once daily for 30 days with oral preparations of CCNU (12.5 mg/kg and 25 mg/kg), SLENU (25-200 mg/kg), and combinations of these. The CCNU-induced increase in plasma MDA level and the usual decrease in erythrocyte SOD and CAT activities were reversed by SLENU, but not by vitamin E. We have previously demonstrated that SLENU is a superoxide scavenger. A combination of our present findings with previous results thus leads us to proposing a new chemotherapeutic combination of CCNU and SLENU that is devoid of high toxicity.

[Treatment of life-threatening bleeding in obstetrics and gynaecology --NovoSeven (recombinant factor VIIa)].

The study offers the results from the application of recombinant factor VIIa (NovoSeven) in 19 patients with obstetric or gynaecological problems showing severe haemorrhage of non-surgical origin, which could not be controlled by standard therapy (surgery and resuscitation). Haematological and haemostaseological values as well as final outcome are being monitored. The role of the drug and its special indications in complex urgent treatment of life-threatening bleeding are being discussed. Conclusions. The use of NovoSeven (recombinant factor Vlla) on time and when indicated guarantees rapid and effective haemostasis. Thus laparotomy or relaparotomy can be avoided. If no effect is observed after the application of NovoSeven indicates bleeding from a major blood vessel, demanding laparotomy and surgery. The use of the drug leads to reducing the incidence of blood derivatives transfusions and (being a recombinant product)--no risk of transmissive infections.

[Herpes simplex virus infection in the newborn].

Herpes simplex virus (HSV) infection is a rare disease, but with serious sequelae in the newborn. Despite active treatment with high dose Acyclovir, prognosis on psychomotor development remains unfavorable. A short review of the recent literature about clinical forms, diagnosis, treatment, and disease prophylaxis is presented.

[Use of recombinant factor VIIa for the control of massive bleeding caused by uterine hypotonia in post-placental period].

We report our clinical opinion for recombinant activated factor VII (NovoSeven, Novo Nordisk, Copenhagen, Denmark) administration in puerperae with massive haemorrhage caused by uterine hypotonia. Four women with severe bleeding in post-placental period received NovoSeven in bolus IV. The blood loss and laboratory changes in hematology and haemostasis parameters are monitored. The right time of application and the mean effective dose of the medication are discussed. The bleeding was ceased in all cases. Decrease in values of Hb, Er and PTT was noted. The mean administered dose of 72 micrograms/kg BW was effective. The laboratory values showed tendency for improvement on the 24 hour after administration and normal levels on discharge. The use of recombinant factor VIIA in puerperae with severe bleeding in the postplacental period is effective and safe enough and could be an alternative to the extreme surgical procedures.

[Genital infections in girls with type 1 diabetes].

The aim of the study was to investigate the relation between genital infections (frequency, etiology, clinical presentation) among girls with type 1 diabetes and some factors as age of the girls, duration and metabolic control of diabetes. Forty-three girls with type 1 diabetes and twelve healthy girls for controls were involved in the study. Methods used are: genital tract inspection, direct microscopy and cultures from genital discharge and urine. Glycaemic control in the diabetic girls was assessed by measuring total glycosylated haemoglobin. The results show vulvovaginal candidiasis in 27 (62.8%) in diabetic girls versus 2 (16.6%) in controls (p < 0.01). There was clear prevalence of non-albicans Candida species. Concomitant infection with bacterial pathogens were proved in only 6 (13.9%) of diabetic girls and 1 (8.5%) of the controls. The clinical presentation of genital candidiasis was vulvovaginitis and most of the affected diabetic girls (88.9%) were in puberty period. There was no relation between duration and metabolic control of diabetes and genital infections.

[Lansinoh in the treatment of sore nipples in breastfeeding women].

UNLABELLED: The aim of the study was to assess the efficacy and side effects of purified lanolin (Lansinoh) in the prophylactics and treatment of sore nipples. Fifty-seven women aged 20-35 years have been treated - 41 (72%) with phototype III-IV and 16 (28%) with phototype I-II. Thirty-seven (65%) of them were primiparous and 20 (35%) were multiparous. Pre-delivery preparation of the breast has been performed in 16 (28%) of the women. Two groups of patients were formed: I gr. - 10 women (17,5%) with no complaints applying the preparation with preventive aim and II gr. - 47 (82,5%) women with sore nipples of different severity. Therapeutic efficacy was assessed according to the nipple attribute score. In the first group of patients due to the application of Lansinoh the skin remained smooth and elastic and breast-feeding was not disturbed. In the II group Lansinoh lead to a statistically significant difference in the nipple attribute score before and after treatment. The improvement was most pronounced in patients with light and moderate forms of inflammation, in women with phototype III-IV, in multipara and in patients with pre-delivery preparation of the breast. CONCLUSION: Lansinoh is purified lanolin suitable for prophylactics and treatment of sore nipples.

[Emergency contraception with levonorgestrel in adolescents].

OBJECTIVE: The aim of the study was to asses effectiveness and adverse reactions in adolescent girls received Postinor (0.75 mg Levonorgestrel) for emergency contraception. METHOD: Forty-two teenage girls with established regular menstrual cycles for 24 months period and 128 intercourses are included. Excluding criteria are: clinical data for gynecological endocrine diseases, other contraceptive methods used and contraindication for using gestagens. The girls received tablets Postinor containing 0.75 mg levonorgestrel in dosage 1 or 2 pills after non-protected sexual contact. Before the study the girls are examined for their knowledge about fertile period of the menstrual cycle, opportunities of hormonal contraception and about their ability to make adequate decisions in unpredicted situations and complications of the unprotected or inadequate protected intercourse. RESULTS: No one pregnancy occurred during the study period. Main first adverse reactions after receiving Postinor were nausea (12.4%) and intermenstrual bleeding (30.9%), persisting up to three days. Systemic adverse reactions reported from the women were breast tenderness (11.2%), dizziness (6.5%) and headache (5.6%). These side effects were mild and did not require treatment. CONCLUSION: Emergency contraception with Levonorgestrel is effective, with less contraindications compared with other emergency contraception methods. Postinor is available and reliable contraceptive medication.

Influence of antigen stimulation on the oxidative stress parameters in outbred and inbred rabbits.

The influence of antigen stimulation on the oxidative stress parameters in two groups of rabbits-inbred and outbred were explored by evaluation of the level of lipid peroxidation products (MDA) in the plasma membrane, and the activity of erythrocyte antioxidant defense enzymes superoxide dismutase (SOD) and catalase (CAT). There was not a significant difference between levels of MDA in inbred and outbred rabbits before immunization. However, SOD activity in inbred rabbits was significantly increased in comparison with that of outbred (p = 0.006). Significantly higher plasma levels of lipid peroxidation products were detected in both inbred and outbred rabbits during immune response in comparison to the corresponding groups before immunization (p = 0.008 and p = 0.002). SOD and CAT activities in erythrocytes of rabbits during immune response were also significantly increased compared to that before immunization. In addition, during immune response SOD and CAT activities were found to be positively correlated to each other in both inbred and outbred rabbits (r = 0.727 and r = 0.916). In conclusion, our results suggest the presence of an increased oxidative stress during the antigen stimulation accompanied by an adaptive increase of SOD and CAT activities. 30 days after immunization, the plasma levels of MDA and the activities of SOD and CAT in erythrocytes decreased and reached values close to the controls.

[A clinical and microbiological study of bacterial vaginosis and vaginitis in pregnant women].

Infectious complications in pregnancy and delivery are still y serious problem in obstetrical, gynecological and neonatological practice. The main part of them are caused from the anaerobic organisms associated with bacterial vaginosis as well from sexually transmitted pathogens. In this study we present data about the frequency of spreading of bacterial vaginosis and vaginitis in pregnant women at different stages of pregnancy. We held y study of 106 pregnant women aged between 15 and 35, registrated at the Pregnancy Consultation Office in eleven. About half of the women did not have data for genital discharge and the microbiological analysis showed normal vaginal flora. Two thirds of the rest investigated women had data for candidiasis, bacterial vaginosis, or both. Only one case of Trichomonas vaginalis-vaginitis was detected. The results from the microscopic examination of the specimens from 10 (9.4%) of the women were suspected for N. gonorrhoeae and/or Chlamydia trachomatis-cervicitis. Group B streptococci in significant quantity was found in five cases. These results showed high frequency of spreading of bacterial vaginosis and cervico-vaginitis in pregnant women.

[A clinical study of the efficacy of and tolerance for the combined transdermal preparation Estracomb TTS (CIBA)].

Result from the first study, carried out in our country an the therapeutic effects of Estracomb TTS in women with peri- and postmenopausal symptoms are reported. Seven women with postcastration syndrome and eight women with an intact uterus and no vaginal bleeding for a least 6 months were treated. In all women enrolled in the study the application of the preparation Estracomb TTS led to a significant improvement of peri- and postmenopausal symptoms without serious side effects.
