New enzymatic determination of sialic acid in serum.

This enzymatic method for determination of sialic acid involves use of neuraminidase (EC 3.2.1.18), N-acetylneuraminate lyase (EC 4.1.3.3), acylglucosamine 2-epimerase (EC 5.1.3.8), N-acetylhexosamine oxidase (from Pseudomonas sp.), and peroxidase (EC 1.11.1.7). Because the method does not require pyruvic acid in the assay medium, interference by pyruvic acid in serum can be avoided. This simple, accurate assay is little affected by other substances in serum.

Treatment of a case of asynchronous bilateral renal cell carcinomas with inferior vena caval thrombus and hepatic metastases.

A 56-year-old man underwent a right radical nephrectomy with removal of a vena caval thrombus and resection of hepatic metastases for right renal cell carcinoma. Twenty-one months after the first operation, a left renal cell carcinoma was detected which was treated with left radical nephrectomy and followed by hemodialysis. The patient then enjoyed an active life for 14 months but died 18 months after his second operation due to lung metastasis. Bilateral radical nephrectomy and hepatic resection may be justified since there is no strongly effective adjuvant therapy for such advanced cases.

Determination of acid phosphatase in biological fluids using a new substrate, 2,6-dichloro-4-nitrophenyl phosphate.

A new substrate, 2,6-dichloro-4-nitrophenyl phosphate (DCNP-P), is used for the determination of acid phosphatase (EC 3.1.3.2) in serum and urine. It was hydrolyzed by acid phosphatase to 2,6-dichloro-4-nitrophenol (DCNP) and phosphoric acid. At a pH of 4.5-6.0, the absorption of DCNP liberated by acid phosphatase was much higher than that of p-nitrophenol, which is commonly used as an aglycone in the acid phosphatase assay. By using DCNP-P as a substrate for acid phosphatase activity, determinations can be made without the colour reaction which requires the addition of an alkaline solution, and can be determined by the rate assay that does not require measurement of sample blanks in serum or urine. This method using DCNP-P is highly sensitive and is the most suitable for the rate assay of acid phosphatase activity in biological fluids.

Resistance of human bronchial epithelial cells to teleocidin in a two-stage carcinogenesis experiment.

The tumor-promoting activity of teleocidin in two-stage carcinogenesis was tested using human bronchi grafted into athymic nude mice. On treatment with 7,12-dimethylbenz(a) anthracene (DMBA) plus teleocidin, no bronchial carcinomas were produced in 44 grafted human bronchi, but 8 spindle cell sarcomas developed in mouse tissues. Application of DMBA to grafted human bronchi produced only 3 spindle cell sarcomas in the tissues of 30 mice. Application of teleocidin did not produce any tumors in 24 grafts of bronchi obtained from heavy smokers. Thus, teleocidin had an effect on mouse tissues, but seemed to have no effect on human bronchial cells, although it is uncertain whether the grafted human bronchi were initiated by DMBA.

Coexistence of papillary and nodular carcinomas in the urinary bladder.

Of 203 patients with transitional cell carcinoma of the urinary bladder which had been cystectomized and examined by step-sectioning during the period between 1969 and 1986, 33 had coexistent papillary, papillonodular and/or nodular carcinomas in the bladder. Of them, 12 (36%) had a history of papillary carcinomas, 26 (79%) had multiple tumors and 22 (67%) had a concomitant carcinoma in situ, and/or mucosal dysplasia. Moreover, microscopic papillary structures were identified in 32 out of the 33 (97%) cases with papillonodular and/or nodular carcinomas, indicating some of the nodular invasive carcinomas in the bladder to have developed from papillary carcinomas.

5-Hydroxytryptamine and (+)-lysergic acid diethylamide displace [3H]thyrotropin-releasing hormone at its binding sites in the rat limbic forebrain.

Binding of [3H]thyrotropin-releasing hormone (TRH) to its receptors in the rat limbic forebrain was partially displaced by 5-hydroxytryptamine (5-HT, ligand for 5-HT1 receptors) and (+)-lysergic acid diethylamide ((+)-LSD, ligand for 5-HT1 and 5-HT2 receptors) at nanomolar concentrations. Spiperone (ligand for 5-HT2 receptors) displaced [3H]TRH in a dose-dependent manner at micromolar concentrations. These results suggest that some TRH receptors are related to 5-HT1 receptors, probably adjoining them on the membrane. This type of TRH receptor is shown to be among the high-affinity receptors which we reported previously. The significance of the receptor-coexistence is such that TRH facilitates serotonergic transmission by increasing the density of 5-HT1 receptors. This finding seems to support a pharmacological observation of other investigators that TRH potentiates 5-HT-induced hyperactivity in mice, probably by affecting postsynaptic 5-HT receptors.

Histopathological study of human cisplatin nephropathy.

The present study was conducted to obtain detailed information on the morphological changes in the human kidney as related to cisplatin therapy. The kidneys of ten autopsy cases showing impaired renal function following cisplatin therapy with hydration were examined histologically. Sporadic lesions of degeneration, necrosis, and regenerative changes were observed in the pars convoluta and pars recta of the proximal tubule, distal tubule, and collecting duct. Enlarged and pleomorphic nuclei in regenerated epithelial cells and hyperplasia of the collecting duct lining cells were considered to be specific histological findings in renal injury related to cisplatin therapy. Electron microscopic examination of a case receiving a large dose of cisplatin without signs of renal insufficiency showed an increased number of cytosomes containing electron dense granules in all tubular portions and conspicuous nuclear indentation in the thin limb of the loop of Henle and collecting duct. The platinum concentration in autopsy organs and biopsy cases was analyzed, but no correlation could be found between the concentration level of platinum in the kidney and impairment of renal function.

Two cases of bladder carcinoma with similar initial features that exhibited completely different clinical courses.

Two cases of bladder carcinoma are described. The patients were of similar age, were both smokers and were treated for the same period, but exhibited completely different later clinical courses. Initially, both had a single, papillary, pedunculate tumor, identified as a transitional cell carcinoma, grade 2, pTa. One patient, six years later, had multiple papillary tumors covering almost all the mucosal surface and underwent simple cystectomy. Histologically the tumors were identified as transitional cell carcinomas, grade 2, pT1. The other patient, nine years later, had a single nodular invasive tumor with a concomitant, very tiny papillary tumor and underwent radical cystectomy. Histologically the tumor was transitional cell carcinoma, grade 2 greater than 3, pT4. Many of the questions raised by these cases are unanswered, but comparison of such cases should provide some clues to the natural history of bladder carcinoma.

The frequency of papillomavirus infection in cervical precancerous lesions in Japan: an immunoperoxidase study.

In order to establish the frequency of human papillomavirus (HPV) infection in cervical precancerous lesions of Japanese patients, cervical materials routinely biopsied in the past year were examined immunohistochemically for the papillomavirus genus-specific antigen. Of a total of 832 cervical biopsy specimens, 46 (5.5%) were immunohistochemically positive for HPV. In this study, 206 patients were diagnosed as having dysplasia or carcinoma in situ (CIS), and HPV antigen was found in 15% of these patients. It was found in 13% of patients with mild dysplasia, 28% of those with moderate dysplasia, 17% of those with severe dysplasia and 4.5% of those with CIS. However, HPV antigen was detected only in the epithelium with dysplastic change, not in cancerous areas.

Testosterone in infant testis yolk sac tumor indicating a mixture of germ cell and sex cord/stromal elements: a new concept.

The presence of testosterone in yolk sac tumor of the infant testis was investigated immunohistochemically. All 16 cases tested were positive for the hormone. The antigen-positive tumor cells were primitive mesenchymal cells and columnar cells forming small ducts, which resembled Leydig cells and Sertoli cells, respectively. These cells accounted for about 1 to 20% of tumor cells and were intermingled with typical yolk sac structures. In contrast, no yolk sac tumor of adult testis was definitely positive for the antigen. In view of its distinctive histological and immunohistochemical features and clinical behavior, yolk sac tumor in the infant testis is most likely a separate and specific entity, composed of a mixture of germ cell and sex cord stroma derivatives.

Primary neoplasm in a vesical diverticulum: analysis by step-sectioning.

A primary neoplasm arising in a vesical diverticulum was cystectomized and analyzed by step-sectioning of the whole bladder in an attempt to determine the carcinogenic process. Serial sections of the cystectomy specimen revealed a small area of mild dysplasia in the mucosa of the bladder and the diverticulum without a tumor. Most of the mucosal cells in the tumor-bearing diverticulum were desquamated, but a small area of dysplasia in the remaining cells was also observed.

Determination of alpha-amylase in biological fluids using a new substrate (beta-2-chloro-4-nitrophenyl-maltopentaoside).

A novel substrate, beta-2-chloro-4-nitrophenylmaltopentaoside (beta CNPG5), was used for the enzyme-coupled determination of alpha-amylase in biological fluids. It was hydrolyzed specifically by alpha-amylase to about 90% producing beta-2-chloro-4-nitrophenylmaltoside (beta CNPG2) and maltotriose. Under the assay conditions, the absorption of 2-chloro-4-nitrophenol (CNP) generated by the secondary reaction of alpha-glucosidase and beta-glucosidase as auxiliary enzymes is about twice the absorption of 4-nitrophenol (PNP), which is the end product currently measured in some alpha-amylase assay methods. The sensitivity of the assay using beta CNPG5 was thus much higher than that using 4-nitrophenyl-maltopentaoside (PNPG5) as substrate. The absorption of CNP did not fluctuate with temperature or with pH between 6.8 and 7.2, which are the conditions normally used for determination of amylase activity in biological fluids.

Early stage adenocarcinoma of the uterine cervix. Histopathologic analysis with consideration of histogenesis.

Thirty cases of early stage adenocarcinoma, 5 mm or less in depth, were selected from 1942 primary carcinomas of the uterine cervix for histologic analysis to clarify their histogenesis. There were 16 carcinomas of endocervical type, 12 of endometrioid type, and 2 clear cell carcinomas. There were 22 early invasive adenocarcinomas and 8 adenocarcinomas in situ. In 27 cases the carcinoma was adjacent to the transformation zone and in 3 it was separate from it. In 10 cases adenocarcinoma coexisted with in situ squamous cell carcinoma. Only one patient whose tumor was 3 mm in depth developed a pelvic recurrence after radical hysterectomy. All other patients remained disease-free after treatment by hysterectomy. It is suggested that most adenocarcinomas of the uterine cervix originate from endocervical glands adjoining the transformation zone and that they may develop directly from normal-appearing epithelium without passing through adenomatous or dysplastic changes.

Various types of thyrotropin-releasing hormone receptors in discrete brain regions and the pituitary of the rat.

Receptors for thyrotropin-releasing hormone (TRH) in the rat brain and the pituitary are heterogenous. The receptors were classified into four types according to the dissociation constant (KD). High-affinity receptors (KD less than 3 nM) are present in the pituitary, hypothalamus, amygdala, and limbic forebrain which contains the nucleus accumbens and the septum. Intermediate-affinity receptors (KD, 5-16 nM) are evidently present in the frontal cortex, hippocampus, striatum, thalamus, and the brainstem, but may also be present in other regions. Low-affinity TRH receptors (KD, 50-80 nM) are seen in the limbic forebrain, amygdala, and the hypothalamus. Very-low-affinity receptors (KD, 215 nM) exist in the pituitary. Experiments using DN-1417 (gamma-butyrolactone-gamma-carbonyl-histidyl-prolinamide citrate), a synthetic TRH analogue with a more potent central activity, indicated the presence of TRH receptors having a high affinity to DN-1417 at least in the limbic forebrain but not in the pituitary. This type of receptor is not labeled by [3H](3-methyl-histidine2)-TRH. Density of the TRH receptor is the highest in the pituitary and next highest in the amygdala.

Large cell carcinoma of the lung--ultrastructural and immunohistochemical studies.

Twenty-seven cases of surgically resected large cell carcinoma of the lung including nine cases of giant cell carcinoma were examined ultrastructurally and immunohistochemically. Ultrastructurally, of 18 large cell carcinomas other than giant cell carcinoma eight showed characteristic differentiation toward adenocarcinoma, four toward adenosquamous carcinoma, and one each toward squamous cell carcinoma and neuroendocrine cell carcinoma, but the remaining four were undifferentiated. Six of the nine giant cell carcinomas also showed features of adenocarcinoma, two showed features of squamous cell carcinoma, and one was undifferentiated carcinoma. Immunohistochemically, secretory component (SC) was observed in seven of 14 cases with features of adenocarcinoma and two of four cases with features of adenosquamous carcinoma. Carcinomas with only squamous cell differentiation did not stain for SC. Keratin staining was positive in five of the 14 with features of adenocarcinoma, three of the four cases with features of adenosquamous carcinoma and two of the three cases with features of squamous cell carcinoma. The numbers of tumor cells positive for keratin and/or SC were small. One carcinoma with neurosecretory type granules was stained positively for calcitonin. These findings indicate that many large cell carcinomas showed differentiation toward glandular cells and/or squamous cells, and some did not show any differentiation ultrastructurally or immunohistochemically, indicating that the majority of large cell carcinomas are poorly differentiated form of either adenocarcinomas or squamous cell carcinomas.

Carbohydrate antigen defined by a monoclonal antibody raised against a gastric cancer xenograft.

A monoclonal antibody, ST-4-39, was obtained by using a human gastric cancer xenograft, St-4, as an immunogen. Immunization was achieved by transferring immunocompetent mouse spleen cells into a nude mouse bearing St-4. Hybridomas were produced with the spleen cells of the mouse after rejection of the tumor and screened for immunohistochemical reactivity with cancers and normal tissues on formalin-fixed paraffin sections. ST-4-39 immunohistochemically reacted with various cancers including gastric, colorectal and pancreatic cancers as well as some normal tissues. ST-4-39 and NS 19-9 differed in immunohistochemical reactivity, although they reacted with some cancers and a few normal tissues in common. PBS extracts of normal and cancer tissues were examined for antigen reactive with ST-4-39 by sandwich enzyme immunoassay. Extractable antigen was detected in adenocarcinomas of colon, stomach and lung, while it was detected only in salivary gland and trachea among normal tissues examined. Gel filtration analysis of the antigen indicated a molecular weight of greater than or equal to 1 X 10(6), and the antigenic determinant was suggested to be a carbohydrate chain with terminal sialic acid by studies using periodic acid, neuraminidase and pronase treatments. Furthermore, the ST-4-39 antigen affinity-purified from two gastric cancer strains was shown to contain multiple carbohydrate determinants including sialyl-Lewisa and sialyl-Lewisx, suggesting the antigen to be a mucin.
