A case of multiple sclerosis with an onset mimicking the Kleine-Levin syndrome.

A 20 years old patient is described, who presented two episodes of day-time hypersomnia, orthostatic hypotension and psychotic behavior. Compulsive masturbation and abnormalities of the eating habits were also present. Both episodes cleared up spontaneously in about one week. From the clinical point of view of Kleine-Levin syndrome, was suspected. However a CSF examination showed IgG oligoclonal bands and an increased IgG index. A NMR showed multiple area of increased signal intensity, suggestive of a demyelinating disease. On these elements a final diagnosis of MS was made, in spite of a normal neurological examination. This case was compared to other cases of MS in which the disease begins with acute remitting psychiatric symptoms. However cases of MS with complex episodes as observed in our patient were not found in the Literature.

The role of the right hemisphere in processing negative sentences in context.

Given a pattern consisting of x1, x2,..., xn similar elements and y1, which is perceived as different from the former, it is more plausible to assert that y is not x rather than to assert that x is not y (Wason, J. verb. Learn verb. Behav. 4, 7-11, 1965). In order to appreciate such a difference, the entire set has to be considered. Right-hemisphere brain-damaged patients were submitted to a series of visually presented patterns, each pattern consisting of seven similar items and one dissimilar item. Their task was to complete a statement referring to a single element of each pattern. Statements were either simple affirmative or negative sentences. Errors and reaction times were recorded. Patients with a right-hemisphere injury were found to be insensitive to the plausible-implausible dimension in completing negative sentences. It is hypothesized that right-hemisphere brain-damaged patients are less adequate in this task because they are less capable in putting each element into the visual context.

Gall bladder sludge formation during prolonged fasting after gastrointestinal tract surgery.

In this study we have ultrasonographically assessed the prevalence of sludge in a group of 48 fasting patients after gastrointestinal tract surgery. Ultrasound examinations were carried out daily in each patient, beginning on the day before surgery. The period of fasting lasted from seven to 10 days. The presence of sludge was demonstrated within the seventh day in seven out of the 48 patients. In 38 cases fast lasted for a further three days. The total number of sludge-positive patients after 10 days was 12 out of 38. Ultrasound controls were performed after six and 12-24 month interval and showed the presence of gall stones with different ultrasonographic patterns in three sludge positive patients. We conclude that in the early postoperative period there is a high risk for sludge development and that in some cases sludge may subsequently evolve into gall stones.

Postextrasystolic potentiation in isolated rat myocardium: dependence on resting muscle length.

The degree and decay of postextrasystolic potentiation were studied in isolated rat papillary muscle at Lmax, the length at which developed force was maximum, and at shorter lengths. Potentiation curves were constructed relating the degree of potentiation with the time interval between any given extrasystole and the last steady-state beat. At all stimulation rates tested (ranging from 3 to 60/min), potentiation curves were found to be dependent on muscle length: at any interval the degree of potentiation increased progressively with decreasing initial muscle length. At all lengths and stimulation frequencies tested, the decay of potentiation was a function of the number of beats following the extrasystole. This relationship could be fitted by a single exponential equation. The exponential constant of the decay was significantly smaller at Lmax than at shorter lengths. These results suggest that muscle length affects more than one step of the E-C coupling processes.

[Effects of parenteral nutrition on gastric secretion in patients in cerebral coma].

Total parenteral feeding reduced gastric secretions, but patients sometimes complain of pyrosis and acidity. The gastric secretion, gastrinaemia and insulinaemia of 96 patients subjected to TPN were measured. Results showed a nonpathological increase in gastrinaemia and acidity which might be related to the high levels of insulinaemia.

[Diabetes mellitus and acute myocardial infarct].

A brief overview of the relevant literature is followed by the presentation of data obtained in a series of 389 males and 127 females admitted for acute myocardial infarct between 1-1-1974 and 31-12-1978. Diabetes mellitus was more common in these patients (13.9% in males and 12.5% in females) than in the general population (5%).
