[123I-IMP single photon emission computed tomography (SPECT) study in childhood epilepsy].

N-isopropyl-p[123I]-iodoamphetamine (IMP) single photon emission computed tomography (SPECT), X-ray computed tomography (X-CT) and magnetic resonance imaging (MRI) were performed in 18 children with idiopathic seizures. In children with idiopathic seizures SPECT identified abnormal lesions in the highest rate (50%) compared with X-CT (11%) and MRI (13%), but the findings of SPECT poorly correlated with the foci on electroencephalography (EEG). Idiopathic epilepsy with abnormal uptake on SPECT was refractory to medical treatments and frequently associated with mental and/or developmental retardation. Perfusion defects identified on SPECT probably influenced the development of the brains in children. IMP SPECT is useful in the diagnosis and medical treatment in children with seizures.

[Evaluation of hepatic function by hepatobiliary scintigraphy using factor analysis].

Factor analysis of hepatobiliary scintigraphy using 99mTc-N-pyridoxyl-5-methyltryptophan (99mTc-PMT) was performed, and functional factor of liver parenchyma (FA-hepatogram) was obtained. Two parameters (peak time, T 1/2) were calculated from this hepatogram. A good correlation was obtained between these parameters and ICG-R15; furthermore, these parameters were prolonged in patients with normal volunteers, chronic hepatitis, compensated and decompensated liver cirrhosis in this order. Conventional hepatogram by ROI method (ROI-hepatogram) was not suitable for the evaluation of global liver function in condition such as a large intrahepatic mass, intrahepatic gall bladder, dilatation of biliary tree, and severe liver cirrhosis. But, even in such cases, FA-hepatogram was useful for the evaluation of global liver function.

Usefulness of turbo-FLASH dynamic MR imaging of dissecting aneurysms of the thoracic aorta.

PURPOSE: We attempted to determine whether Turbo-FLASH dynamic magnetic resonance imaging is clinically useful for the diagnosis of dissecting aneurysm of the thoracic aorta by differentiating slow flow from thrombus in the false lumen. METHODS: Magnetic resonance imaging (MRI) was performed in ten patients using three different pulse sequences (spin echo, cine and turbo-FLASH dynamic MRI). The three imaging techniques were compared for their ability to demonstrate anatomical and hemodynamic abnormalities by visual assessment. RESULTS: In all patients with thrombus in the false lumen, the thrombus was accurately differentiated from slow flow on contrast enhanced turbo-FLASH dynamic MRI, but only with difficulty on spin echo and cine MRI. Dynamic MRI was also useful for the evaluation of flow dynamics in the false lumen. CONCLUSION: Turbo-FLASH dynamic MRI is recommended for patients with dissecting aneurysm of the thoracic aorta, not definitely diagnosed by spin echo and cine MRI.

Feasibility of ventricular function analysis by digital subtraction angiography.

Continuous or cine mode of digital subtraction angiography (DSA) following intravenous injection of contrast media was evaluated in regard to feasibility of obtaining physiologic data in cardiac abnormalities. Computer programs for cardiac function analysis including stroke volume, ejection fraction and display of left ventricular wall motion in end-diastole and end-systole have been developed with use of automatic contour plotting and densitometry for depth information. Experiments with contrast-filled balloons demonstrated good correlation between the true volumes and the calculated volumes. Clinical study also demonstrated good correlation between the values obtained from conventional left ventriculography and DSA.

Determination of left ventricular volumes with use of DSA density values.

A computer program was developed to obtain left ventricular volumes from the density values of DSA. The highest density value along the horizontal pixel line over the left ventricle in 30 degrees RAO was considered to correspond to the greatest width on the same level of 60 degrees LAO. The left ventricular volume was obtained by converting the density values into the width and integrating these values. This method estimated experimental balloon volumes with considerable accuracy. Clinical evaluation revealed good correlation between DSA and conventional cine left ventriculography.

Value of ultrasound-guided core-needle biopsy for peripheral intrathoracic and mediastinal lesions.

To evaluate the value of ultrasound (US)-guided core-needle biopsy (CNB) for peripheral intrathoracic and mediastinal lesions. Fourteen patients who had intrathoracic or mediastinal lesions underwent US-guided CNB with 17- or 18-gauge needles. The lesions in this study were seven cases of lung carcinomas including four cases of adenocarcinomas and three cases of squamous cell carcinomas, two cases of thymomas, and one case each of aspergillosis, sarcoidosis, MFH, lung abscess, and lung fibrosis. Diagnosis was possible in 13 of 14 (92.8%) cases histologically. There were three cases of complication (21.4%); one case each of pneumothorax, pleuritis, and hemosputum. They were all cured with medication. We concluded that real-time US-guided CNB is a useful and safe method for the examination of peripheral intrathoracic and mediastinal lesions.

Bis(hydroxamamide)-based bifunctional chelating agent for 99mTc labeling of polypeptides.

To develop chelating molecules that provide 99mTc-labeled polypeptides of high in vivo stability and high specific activities under mild reaction conditions, an asymmetrical bis(benzohydroxamamide) compound with an amine group, 4'-aminomethyl-N,N'-trimethylenedibenzohydroxamamide [NH2-C3(BHam)2], was designed and synthesized. The amine residue of NH2-C3(BHam)2 was converted to a maleimide group by reaction with N-succinimidyl-6-maleimidohexanoate, and the conjugation product was coupled to thiol groups of a monoclonal antibody against osteogenic sarcoma (OST7, IgG1) pretreated with 2-iminothiolane to prepare C3(BHam)2-OST7. 99mTc radiolabeling of C3(BHam)2-OST7 was performed by the exchange reaction with [99mTc]glucoheptonate. [99mTc]C3(BHam)2-OST7 was further characterized using directly radioiodinated OST7 ([125I]OST7) and [111In]labeled OST7 with 1-[4-[(5-maleimidopentyl)amidobenzyl]ethylenediamine-N,N, N'N'-tetraacetic acid (EMCS-Bz-EDTA) as references. [99mTc]C3(BHam)2-OST7 was obtained with radiochemical yields of over 94% at protein concentrations as low as 0.2 mg/mL at room temperature for 1 h. [99mTc]C3(BHam)2-OST7 remained stable after incubation in freshly prepared murine plasma and in the presence of cysteine. Similar binding affinities to tumor cells were observed between [99mTc]C3(BHam)2-OST7 and [125I]OST7. When injected into normal mice, [99mTc]C3(BHam)2-OST7 exhibited radioactivity levels in the blood similar to [111In]-EMCS-Bz-EDTA-OST7 up to 24 h postinjection with significantly faster elimination rate of the radioactivity from the liver. In nude mice bearing osteogenic sarcoma, no significant differences were observed in the radioactivity levels in the blood and the tumor between [99mTc]C3(BHam)2-OST7 and [125I]OST7 at 24 h postinjection. These findings indicated that C3(BHam)2 provided 99mTc chelate of high stability at low concentrations even when conjugated to an intact antibody. Such characteristics render bis(hydroxamamide) compounds useful as chelating molecules for preparation of 99mTc-labeled polypeptides.

A preliminary phantom study on a proposed model for quantification of renal planar scintigraphy.

A method for estimation of the absolute renal activity within background activity in renal planar scintigraphy will be presented. This method corrects for oversubtraction of background activity with consideration of background activity of the kidney volume, and employs the depth-independent buildup factor (DIBF) method for the correction of attenuation and scatter of photons. This method requires the depth and thickness of the organ and the thickness of background for a background activity correction. The transmission factor (TF) for a volume source is derived from integrating TF for a thin source over the thickness of the organ for the DIBF method. To validate this method, phantom studies with various uniform background activity concentrations were performed and the data were compared with conventional background subtraction that do not consider the organ's volume. The results showed that with the conventional background subtraction method at all depth activity was underestimated with errors of 5%-30% for organ/background concentration ratios of 5-40, while this method estimated the true count rate with errors of less than 5%. More accurate quantifications of renal functions such as renal uptake, glomerular filtration rate (GFR) and renogram may be obtained by this proposed method on the planar images.

Correlation of 99mTc-GSA hepatic scintigraphy with liver biopsies in patients with chronic active hepatitis type C.

99mTc-DTPA-galactosyl human serum albumin (99mTc-GSA) liver scintigraphy was performed in 230 patients with chronic active hepatitis type C, and its quantitative indices were compared with histological findings. 99mTc-GSA findings correlated well with four indices of the histology activity index (HAI), especially with the fibrosis score. Ninety patients were given interferon treatments, and 99mTc-GSA findings were compared with the results of the treatments. We classified the effects of interferon treatment into three groups according to clinical outcome: group 1: good effect (HCV-RNA negative, n = 34), group 2: moderate effect (HCV-RNA positive, but the value of GPT was normal for six months after the end of treatment, n = 19) and group 3: no effect (n = 37). Quantitative indices of 99mTc-GSA showed significant differences between groups. Follow-up study with 99mTc-GSA scintigrams was obtained in eight patients. The results of 99mTc-GSA improved in three patients in group 1 and deteriorated in five patients in group 3. There is a possibility that 99mTc-GSA scintigraphy can be used to predict the clinical outcome of chronic active hepatitis type C after interferon treatment.

Phantom evaluation of scatter and attenuation correction in thallium-201/technetium-99m acquisition in myocardial perfusion single-photon emission computed tomography.

PURPOSE: This phantom study was carried out to evaluate the usefulness of scatter correction combined with transmission-based attenuation correction in separate and simultaneous 201Tl/99mTc myocardial SPECT. METHODS: An anthropomorphic torso phantom was used in this study. We used the triple-energy-window (TEW) method for scatter correction and transmission computed tomography (TCT) images for attenuation correction. Images without corrections (UC) and images with corrections (SAC) for scatter and attenuation were reconstructed for the evaluation. RESULTS: The differences in defect size between 99mTc and 201Tl UC images led to interpretation errors in separate (separate protocol) and simultaneous dual-isotope studies (simultaneous protocol). These errors were more prominent in the infero-posterior wall in the simultaneous protocol. Improvement for overestimation in object size and underestimation in defect contrast was visually obtained, and increased contrast was also shown by the myocardium-to-defect count (MD) ratios on SAC images in the separate and simultaneous protocols. However, 201Tl SAC images in the simultaneous protocol still had less defect contrast than the corresponding 201Tl SAC images in the separate protocol. CONCLUSIONS: From the results of our phantom experiment, separate rest 201Tl/stress 99mTc-sestamibi acquisitions may be recommended in clinical practice. Further clinical and phantom studies will be needed to validate the method using scatter correction combined with transmission-based attenuation correction.

Hyperoxia-induced acute lung injury using a pig model: correlation between MR imaging and histologic results.

PURPOSE: To correlate MR imaging findings with pathology in experimental hyperoxia-induced acute lung injury, which has similar pathology to diffuse alveolar damage (DAD). MATERIALS AND METHODS: Seventeen Yorkshire pigs were studied. These animals were exposed to more than 80% oxygen for 24, 48, 72, and 96 hours in a sealed cage. The lungs were removed and inflated with air infused through the trachea, and then examined by both MRI and high-resolution CT (HRCT). T1-weighted spin-echo (T1WSE), T2-weighted fast (T2WFSE), and half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequences were performed. RESULTS: Severity in MR findings and signal-to-noise ratios (SNR) on MR images were well correlated with pathological scores. CT values were also correlated well with pathologic scores. Abnormal SNR values were obtained from a pathological score of 5, whereas abnormal CT values were obtained from a pathological score of 15. Furthermore, significant differences in SNR were observed in each histopathological phase. CONCLUSION: SNRs on MR images were superior to CT values in detecting early pathologic changes in DAD. MR study also is potentially useful for evaluation of the histopathological phases in acute lung injury.

Single photon emission computed tomography in children with idiopathic seizures.

Single photon emission computed tomography (SPECT) with N-isopropyl-p[123I]-iodoamphetamine (IMP), X-ray computed tomography (X-CT), and magnetic resonance imaging (MRI) were performed in 20 children with idiopathic seizures. In children with idiopathic seizures, SPECT could detect the abnormal sites at the highest rate (45%) compared with CT (10%) and MRI (12%), but the abnormal sites on SPECT correlated poorly with the foci on electroencephalograph (EEG). Idiopathic epilepsy with hypoperfusion on SPECT was refractory to treatment and was frequently associated with mental and/or developmental retardation. Perfusion defects on SPECT scans probably affect the development and maturation of the brain in children.

SPECT evaluation of simulated white matter lesions: experimental study using a brain phantom.

An experimental study using a brain phantom was performed to evaluate the detectability of simulated white matter lesions on two types of single photon emission computed tomography (SPECT) systems. A ring-type SPECT system with high spatial resolution was able to demonstrate these lesions in the white matter, while a widely used general-purpose single-head rotating gamma camera failed to show them. The detectability of white matter lesions was decreased by photons scattered from the highly radioactive gray matter and the poor spatial resolution of the SPECT system. In order to improve the detectability of white matter lesions it is important to apply scatter correction and improve the spatial resolution of the SPECT system. Brain phantom studies made it easier to interpret white matter lesions that were difficult to interpret on the basis of clinical images alone, and the knowledge acquired from the brain phantom study will contribute to the interpretation of clinical images.

Measurement of effective renal plasma flow (ERPF) with 123I-orthoiodohippurate (I-123-OIH).

We evaluated the necessity of attenuation correction for calculating effective renal plasma flow (ERPF) using I-123-orthoiodohippurate (OIH), an agent introduced recently to replace I-131-OIH. From a kidney phantom experiment, the attenuation coefficient (mu) for I-123-OIH in the human body was determined as 0.1262 cm-1. I-123-OIH renal scintigraphy was performed in 23 patients with urinary diseases with blood sampling for calculation of ERPF by Tauxe's method. Calculated renal uptake, with or without attenuation correction, was compared with ERPF determined by Tauxe's method. There was good correlation between renal uptake without attenuation correction and ERPF obtained by Tauxe's method (r = 0.737), while much better correlation was found between attenuation-corrected renal uptake and ERPF calculated by Tauxe's method (r = 0.816). In conclusion, attenuation correction for renal uptake is an important technique for estimating ERPF using I-123-OIH renal scintigraphy.

Functional evaluation of transplanted kidneys by Gd-DTPA enhanced turbo FLASH MR imaging.

We evaluated the usefulness of dynamic turbo FLASH MR imaging in the differential diagnosis of complications after renal transplantation in 17 patients (10 from living relatives and 7 from cadavers). Coronal turbo FLASH dynamic images were obtained every 5 sec for 5 min after an intravenous bolus injection of Gd-DTPA. Corticomedullary differentiation (CMD) on spin echo coronal T1-weighted images and MR renogram patterns of the renal cortex and medulla were obtained for quantitative analysis of the Gd-DTPA-enhanced dynamic turbo-FLASH images. The signal intensity ratio of the medulla to cortex after Gd-DTPA enhancement was compared among four groups: normal (n = 9), acute tubular necrosis (ATN) or cyclosporine A (CyA) tubulopathy (n = 6), acute rejection (AR) in the living related donor kidney (n = 4), and AR in the cadaveric kidney (n = 5). Although loss of CMD was seen in severe renal dysfunction in the transplanted kidneys, there was considerable overlap among the four groups. On dynamic study, there was significant differences in the signal intensity ratio of the medulla to cortex between normally functioning kidneys or ATN/CyA tubulopathy and AR (p < 0.01). In patients with severe renal dysfunction, the arterial cortical peak was indistinct. In conclusion, MR renograms obtained from dynamic turbo FLASH MR imaging played a significant role in evaluating dysfunction of the renal transplant.
