Histological, electron-microscopic and immunomorphologic studies were made in 30 cases of basal cell carcinoma (BCC) of the skin. In the immunomorphological study the antibodies to pankeratin, to keratins of N8, 17 (K N8, K N17) and to laminin were used. Two microscopical types of BCC growth distinguished by their clinical manifestations were revealed: compact-nodulous and diffuse-infiltrative. In BCC with diffuse-infiltrative type of growth the basement membrane was markedly fragmentated or completely absent in some regions. This indicates aggressive character of these tumors. This type of BCC growth was characterized by pronounced expression of K N8. The expression of K N17 was revealed in all BCCs.
Distribution of cell-cell adhesion molecules of E-cadherin (EC) and beta-catenin (BC) infiltrating breast carcinoma (BC) and markers of epithelium, basal membrane has been studied by immunofluorescence in 50 infiltrating BC tissue samples including 17 ductal carcinomas, 23 lobular carcinomas and 10 combined carcinomas. Two types of EC-BC structure alterations were found: homogeneity disorder accompanied by appearance of local thickening and gaps or complete lack of these structures. Alterations of type 1 were found in all the analyzed samples of ductal and combined carcinomas and partially in lobular carcinomas, in in situ structures and in invasion components as well. Alterations of type 2 were found in 16 cases of lobular carcinomas of 23 examined. Our data suggest the existence of two groups of lobular BC: carcinomas with partial depletion of EC-BC structures and carcinomas which lost these structures. Further investigations are needed to evaluate clinical importance of the difference found.
An immunomorphological study of 52 samples of normal thyroid tissue, 10 adenomas, 42 well differentiated carcinomas and 2 metastases has been performed using markers of the epithelium, extracellular matrix and angiogenesis. Basal keratin N 17 of the compound epithelium occurred only in malignant tumors. The highest number of the keratin N 17-positive cells was registered in sclerosing A-cell cancer, medullary and mixed C-cell cancer which is very aggressive. These cells locate in the foci of proliferation of tumor structures and in the regions of infiltrative growth. Multiple defects of basal membranes and prevalence of capillaries marked by antibodies to CD-31 over those identified by the antibodies to factor VIII were found in the areas of strong expression of keratin 17. Thus, the above markers detect foci of aggressive growth even in well differentiated thyroid carcinomas. This fact may be of prognostic value in oncomorphological practice.
An immunomorphological study of 30 breast carcinomas was conducted. In accordance with the histological characteristics they had been previously diagnosed as lobular infiltrative carcinomas (LIC). We have studied E-cadherin cell-cell adhesion molecules and also epithelial markers and basement membranes. We have analyzed the cryostate and paraffin sections of in situ structures, invasion components and metastases. E-cadherin staining was negative, positive or mixed. The E-cadherin negative group consisted of 17 carcinomas of classic LIC accompanied with dyshesive invasion. The E-cadherin positive group consisted of 6 carcinomas. According to our results this diagnosis was changed to ductal infiltrative carcinoma. The E-cadherin mixed group consisted of 7 special type carcinomas with double lobular-ductal differentiation, which had in situ structures of negative or positive cells. In these cases in situ structures consisting of both E-cadherin negative and positive parts were also observed. It is shown that E-cadherin expression can be used in doubtful cases in order to differentiate between the lobular and ductal forms of breast carcinoma.
Monoclonals against three individual proteins of the epithelial intermediate filaments, prekeratins (PK40, PK49, PK55), were used for immunofluorescence studies of the cryostat sections of the rat embryonic liver. The dynamics of expression and intracellular distribution of prekeratins reflects that of morphological rearrangement of the liver. The development of the system of liver beams was accompanied by changes in the expression and intracellular distribution of PK49 and PK55 and the development of the system of bile ducts by changes in all three PKs. From day 20-21 of embryogenesis all three PKs are expressed in cholangiocytes, while PK49 and PK55 in hepatocytes only.
Monoclonal antibodies to keratin 8 which is characteristic for glandular epithelium and those to keratin 17 characteristic of basal cells of complex epithelium were used. Material from 14 females after surgery because of ovarian tumours and uterine carcinoma and from 10 females with dysplasias of various degree and uterine carcinoma was investigated. In addition, ectocervix of vaginal portion at a distance from the point of two epithelia junction was studied in 42 samples. It is shown that keratins 8 and 17 are expressed in the ectocervix sporadically and have a basal location. Keratin 8 is expressed in column epithelium of the areas having normal structure of endocervix. Proliferation of subcolumn reserve cells is followed by an active expression of keratin 17. Proliferative multilayer areas with foci of the immature metaplasia, dysplasias and carcinoma in situ are characterized by cells with coexpression of both keratins, i.e. cells with double differentiation (glandular and squamous).
A rare relapsing mammary tumor is described. The tumor consisted of a major mesenchymal component and a minor tubular one, there were many mitoses. Myofibrils, intercellular desmosomal contacts, basal membranes and secretory granules were found electron-microscopically. Coexpression of all intermediate filament (IF) proteins was found: total keratin, keratins of the liminal and myoepithelial types; IF specific for mesenchymal cells - vimentin, that for glial cells - glial acidic fibrillar protein and nerve cells - neurofilaments. Thus, as distinct from adenomyoepithelioma, the tumor is composed of pluripotential elements which are the exception from the rule of the IF proteins tissue specificity. The term "pluricelluloma" is suggested for this tumor.
