Isolation of Clostridium perfringens from foals.

Clostridium perfingens was isolated from four of 29 healthy foals and from all twelve foals with gastrointestinal diseases. The range of viable counts of C. perfringens in the faeces was 10(1)-10(5)/g and in the intestinal specimens 10(1)-10(7)/g. Of 41 isolates of C. perfringens, 37 were considered to be type A. Enterotoxin of the organism was demonstrated in the intestinal contents of five of eight foals with enteric diseases. These findings suggested that C. perfringens is a likely pathogen of foal intestinal diseases.

[A case of thoracoabdominal aortic aneurysm ruptured into the right extrapleural cavity].

A 63-year-old man of thoracoabdominal aortic aneurysm was transferred to our department. Thoracic and abdominal enhanced CT scan revealed a Crawford's type I A thoracoabdominal aortic aneurysm ruptured into the right extrapleural and retroperitoneal spaces. Without any more additional examination, graft replacement and reconstruction of a lower intercostal artery were performed with an aid of femoro-femoral bypass. Although the postoperative course was complicated by hypertension, hypoventilation and liver dysfunction, the patient recovered from the operation and 10 months later he is leading an almost normal life. Since emergency operation of thoracoabdominal aneurysm is the most courageous challenge because of the difficulties of exposure and visceral organ protection against ischemic, there have been only nine cases with successful surgery in Japan. Now we actively reconstruct lower intercostal and lumbar arteries to prevent spinal cord ischemia without ESCP monitoring in emergency cases.

[A study on the pattern of myocardial distribution and its relationship to the genesis of giant negative T wave in patients with hypertrophic cardiomyopathy].

Quantitative measurements of cardiac mass were performed in patients (pts) with hypertrophic cardiomyopathy (HCM) to assess the relationship between the pattern of myocardial distribution and the degree of T wave inversion on the electrocardiogram. Thirty-three pts with HCM (10: apical hypertrophy, 18: non-obstructive, 5: obstructive) and 10 age-matched normal subjects were studied. Electrocardiographically triggered X-ray computed tomographic scannings were obtained from apex to base of the left ventricle. Left ventricular apex/septum wall thickness ratio (Ta/Ts), left ventricular apex/base mass ratio (Ma/Mb) and total left ventricular mass (LVM) were measured at end-diastole. In 61% of the pts the electrocardiogram showed giant negative T wave (GNT: defined as greater than 1mV). All pts with apical hypertrophy had GNT, however GNT was observed only in 44% of non-obstructive and in 40% of obstructive pts. Both Ta/Ts and Ma/Mb were significantly greater in pts with GNT than those without GNT (1.34 +/- 0.40 vs 0.66 +/- 0.21, p less than 0.001, 1.65 +/- 0.28 vs 1.28 +/- 0.17, p less than 0.001) and normal controls (1.34 +/- 0.40 vs 0.99 +/- 0.17, p less than 0.01, 1.65 +/- 0.28 vs 1.28 +/- 0.11, p less than 0.001), while LVM was significantly increased in pts without GNT than those with GNT (191.4 +/- 57.0 vs 139.4 +/- 39. 2, p less than 0.01) and normal controls (191.4 +/- 57.0 vs 97.2 +/- 13.9, p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

Detection of dengue 4 virus core protein in the nucleus. II. Antibody against dengue 4 core protein produced by a recombinant baculovirus reacts with the antigen in the nucleus.

The dengue 4 virus (DEN-4) core gene and part of the PreM genes were inserted into the baculovirus polyhedrin gene region. The recombinant baculovirus directed the synthesis of the DEN-4 core protein fused to a part of the polyhedrin protein (Mr 25K), as determined by Western blot analysis using DEN-4 core monoclonal antibody. A mouse polyclonal antibody prepared against the DEN-4 core fusion protein showed antigenic reactivity with the authentic DEN-4 core protein (Mr 15.5K) present in the nucleus as well as in the cytoplasm of DEN-4-infected Vero cells as demonstrated by the peroxidase-antiperoxidase staining method. This antibody did not react with cells infected with DEN-1, -2, -3 or Japanese encephalitis virus, or mock-infected cells.

[Hemodynamic changes on the course of septic multiorgan failure patients].

The hemodynamic changes on the course of septic multiorgan failure (s-MOF), and the effects of norepinephrine administrated at septic shock state were evaluated in thirteen patients who died of s-MOF in ICU. The course in ICU was divided into following three stages. Stage I was for a few days after admission in ICU. Stage II was severe infected state. Stage III was the terminal state, i.e., a few days before death. Norepinephrine was administrated when conventional catecholamines (dopamine and/or dobutamine) could not maintain the blood pressure level in the shift from stage II to III, and the hemodynamic changes were evaluated before and after the administration. The following results were obtained: 1) Hemodynamic changes: The stage I was characterized as a moderately hyperdynamic state. The stage II exhibited a typical hyperdynamic state distinguished by a decrease in systemic vascular resistance (SVR). The stage III was distinguished as a normodynamic shock state, because of fall of cardiac output within normal control level. 2) Norepinephrine increased cardiac index, heart rate, and right ventricular stroke work index, but mean arterial pressure, stroke index and SVR did not exhibit any change. However, norepinephrine was useful aid to maintain the circulation in hypotensive state, when conventional catecholamines could not maintain blood pressure any more.

[Changes of plasma amino acid on the course of septic multiple organ failure].

The changes of plasma amino acid (AA) pattern on the course of septic multiple organ failure (SMOF), and the effect of branched chain amino acid enriched solution (BCAA-S) on the AA pattern were evaluated in thirteen patients who died of SMOF in ICU. The course of ICU was divided into the following three stages. Stage I was a few days after the admission in ICU. Stage II was severe infected state. Stage III was terminal state. Following results were obtained: 1) Total AA value (TAA) of each stage was 2,693 n.mol/ml, 6,666 n.mol/ml and 19,480 n.mol/ml. 2) BCAA/aromatic AA (B/A) of each stage was 1.4, 1.03 and 1.05. Above values were lower than those of normal one (2.44). 3) Most kinds of AA in stage III were exhibited significantly higher than those of stage I as evaluated in absolute value. 4) In the BCAA-S administered group, B/A exhibited higher values than those of control, but, there was no sign of amelioration for SMOF condition in the former group. In severe infected or septic shock state, AA metabolism is deeply deteriorated. So, we can not decide whether BCAA-S given in this condition plays beneficial effect on the course of SMOF or not.

Segmented heterochromia in black scalp hair associated with iron-deficiency anemia. Canities segmentata sideropaenica.

A newly recognized disorder of black scalp hair is characterized by the irregularly alternating segmentation of hair into dark and light bands. A 15-year-old girl had segmented heterochromic scalp hair in association with iron-deficiency anemia. The clinical and laboratory investigations support the view that low serum iron levels play a critical role in reducing eumelanogenesis and in the possible failure of melanin transfer. The segmented heterochromic hair recovered completely after iron supplementation, which coincided with increased eumelanogenesis in the recovered hair. This clinical experience indicated participation of iron in the kinetics of melanogenesis within the follicular melanocytes.

[A case of primary malignant fibrous histiocytoma of the right lung].

A malignant fibrous histiocytoma (MFH) is the most common soft tissue sarcoma found in adults, but a primary MFH of the lung is very rare. A chest X-ray taken of a 73-year-old man revealed a massive lesion in the right upper lobe, and a subsequent right upper lobe lobectomy uncovered a soft, light yellow tumor in the right S3 area, measuring 2.8 x 2.6 x 1.7 cm. Histologically, the tumor manifested a typical storiform pattern of spindle-sharp cells with no sign of a myosarcoma. As no abnormal lesion was detected in the abdomen and extremities, the tumor was diagnosed as a primary MFH of the lung.

[A clinical experience of upper urinary tract disease using an endourological technic].

Fifty-three patients with upper urinary tract disease were treated by using endourological technique between July, 1986 and July, 1987; percutaneous nephrolithotripsy (PNL) in 26 cases, transurethral ureterolithotripsy (TUL) in 11 cases, endopyelotomy for ureteropelvic obstruction in 3 cases, dilation of ureteral stenosis in 3 cases and percutaneous nephrostomy in 10 cases. The stones could be successfully removed in 24 cases (92%) of PNL and 10 cases (91%) of TUL. No severe complications were seen in this series. The many reports of PNL and TUL were investigated in detail, but other endourological approach will need further long-term followup.

Eccrine spiradenoma. An electron microscopic study.

Electron-microscopic studies of two typical lesions of eccrine spiradenoma were performed to correlate ultrastructure and histopathology. The salient ultramorphological features of the parenchyma were an adenoid cystic organization composed of epithelial, myoepithelial, and nonepithelial cell types, and the presence of intracytoplasmic luminae within the epithelial cells. There were no indications that the parenchyma was secretorily active. The stroma ramified through the parenchyma, occupying extensive areas and forming tenuous septa of the loose connective tissue in which blood vessels and nerve fibers were embedded. Profiles of cystoid spaces resulting from invagination of stroma into the parenchyma were frequently encountered. Based on our observations, eccrine spiradenoma appears to differentiate toward ductal structures of eccrine sweat apparatus.
