[A comparative study of apical hypertrophy with giant negative T waves and non-obstructive hypertrophic cardiomyopathy: evaluation by computed tomography, echocardiography and left ventriculography].

To assess the relationships between patterns of left ventricular hypertrophy and giant negative T waves (greater than 15 mm, GNT), 24 patients with GNT were categorized in two groups: Group A consisted of 12 patients with GNP but without left ventricular hypertrophy (greater than 13 mm at the chorda level of the interventricular septum and/or left ventricular posterior wall by UCG, LVH); and group-B consisted of 12 patients with GNT and LVH. Twelve patients with LVH but no GNT compromised as group-C. Left ventriculography (LVG) was performed, and left ventricular mass (LVM) and apical hypertrophy were assessed by CT. The configurations of the end-diastolic left ventricle by LVG (RAO 30 degrees) were as follows, by group: A: spade in eight patients and papillary muscle hypertrophy (PMH) shape in four. B: spade in five, PMH in four, banana in one and oval in two. C: banana in nine and oval in three. The LVM (g/BSAm2) were as follows; A: 121.5 +/- 14.9, B: 130.5 +/- 14.8, C: 190.6 +/- 42.1 (C greater than A, B, p less than 0.001). The apex/base ratios calculated by CT were as follows; A: 1.43 +/- 0.31, B: 1.21 +/- 0.19, C: 1.09 +/- 0.20 (C greater than A, p less than 0.05). The left ventricular mass in patients with GNT was less than that of those without GNT. Giant negative T waves were most often associated with apical hypertrophy.

[Effects of the intravenous injection of an ionic contrast medium on the circulating plasma volume: an assessment with plasma colloid oncotic pressure].

UNLABELLED: We investigated effects of intravenous injection of ionic contrast medium (ICM) on circulating plasma volume (CPV) by measurements of changes in plasma colloid oncotic pressure (COP). METHOD: ICM (60% meglumine iothalamate 30 ml) was injected intravenously as a bolus into 7 healthy male volunteers. The venous blood samples were drawn before, and 1, 2, 3, 5 and 10 minutes after the injection. Relatively massive doses of the ICM (150-180 ml) were infused intravenously into 6 male cardiac patients with normal cardiac and renal function during ECG gated cardiac X-ray computed tomography. The blood samples were drawn before, soon after and an hour after the infusion. The plasma COP values of the blood samples were measured with a needle-type osmometer and the sequences of CPV were estimated from the changes of COP values. RESULTS: The bolus injection of the ICM increased the CPV by 10%, 8-9 ml of the CPV per 1 ml of the ICM, and the increase continued during the study (at least for 10 minutes). The massive infusion of the ICM resulted in about 20% increase of the CVP soon after the infusion, and then induced a diuletic effect. The CVP returned to the basal level in one hour, but some patients became rather dehydrated. CONCLUSION: These findings suggested that an intravenous injection of ICM produced considerable expansion of CPV and subsequent dehydration. Careful considerations would be necessary when patients with impaired cardiac or renal function are treated with intravenous ICM during X-ray computed tomography or digital subtraction angiography.

Transient neurologic disturbances induced by high-dose methotrexate treatment.

Temporary neurologic abnormalities were detected in 9 of 60 patients undergoing treatment with high-dose methotrexate and citrovorum factor rescue (MTX-CF) for osteosarcoma. The incidence of abnormalities and abnormalities themselves were more severe than previously reported. This was attributed to an increased dose and more frequent administration of MTX-CF. In view of the transient nature of the abnormalities, a biochemical cause is implicated, and the mechanisms by which it may occur are discussed.

Alpha-lactalbumin as a lysosomal enzyme-releasing factor.

In the early stage of mammary gland involution, biochemically detectable lysosomal damage occurs. The mechanism(s) underlying this damage is not well understood. We found that alpha-lactalbumin from mouse milk induced the release of enzymes from the lysosomes of mouse mammary epithelial cells in vitro, and this induction also occurred with bovine alpha-lactalbumin. This enzyme release was accelerated by the addition of whey proteins with a molecular weight of 50 000 to 60 000. We also found that the lysosomal membrane of mammary epithelial cells had a strong affinity for alpha-lactalbumin.

Process of the combination of alpha-lactalbumin with lysosomes in mammary epithelial cells.

We previously reported that alpha-lactalbumin combines with the lysosomal membrane of mammary epithelial cells and that it acts to release lysosomal enzymes. However, the details of this combination within the cells remained undetermined. We now report that 125I-bovine-alpha-lactalbumin in the medium entered mouse mammary epithelial cells, and about 13 per cent of the alpha-lactalbumin that entered the cell bound to lysosomes. About 75 per cent of the alpha-lactalbumin that reached the lysosome was tightly bound to the lysosomal membrane. It appears that alpha-lactalbumin in the secretory vesicles does not migrate out, because murine and bovine whey did not induce the release of Golgi enzymes in vitro.

Successful treatment of pulmonary and abdominal metastatic osteosarcoma.

A patient with telangiectatic osteosarcoma of the proximal fibula while receiving adjuvant chemotherapy, developed pulmonary and, later, mesenteric metastases. The pulmonary metastases were resected and the mesenteric metastasis was eradicated with surgery and intra-arterial cis-diamminedichloroplatinum II. This represents an additional example of an alteration in the clinical pattern of metastases induced by chemotherapy and the ability to achieve cure with a multidisciplinary approach.

Protective effect of fosfomycin against aminoglycoside ototoxicity.

The protective effect of fosfomycin against aminoglycoside (dibekacin)-induced ototoxicity was studied in rats. Rats were injected with 100 or 50 mg/kg of dibekacin with or without 500 mg/kg of fosfomycin for 60 or 120 consecutive days. Inner ear damage appeared to be more reduced histopathologically in animals given both dibekacin and fosfomycin than in animals given dibekacin alone. Similarly, renal damage appeared to be reduced histopathologically and functionally by the combined administration of dibekacin and fosfomycin. The mechanism of reduced ototoxicity may be as follows: fosfomycin inhibits the accumulation of dibekacin in the kidney, and reduces its concentration in the kidney and serum. Consequently, the amounts of dibekacin reaching the inner ear are decreased, and ototoxicity is reduced.

Low ototoxicity and its mechanism of netilmicin.

The ototoxicity and nephrotoxicity of netilmicin were compared with those of dibekacin, kanamycin and amikacin using rabbits. Groups of 5 rabbits each were given doses of 50 or 100 mg/kg of either one of the four drugs for 30 days, and 10 days after the last injection, all animals were prepared for histopathological studies. Results show the least ototoxicity of netilmicin in comparison to the other three antibiotics. It has been concluded that the low ototoxicity of netilmicin is due not to its lack of accumulation in the perilymph but to its low toxicity to the hair cells.

Reduction of cisplatin ototoxicity by fosfomycin in animal model.

The protective effect of fosfomycin against cisplatin-induced ototoxicity was studied in rats. Sixty-four Fischer rats were injected intravenously with daily doses of 1, 2, 5, and 10 mg/kg of cisplatin with or without 300 mg/kg of fosfomycin for a varying period from 1 to 10 days. The total dose of 10 mg/kg of cisplatin was given equally in all animals. Inner ear damage appeared to be more reduced histopathologically in animals given both cisplatin and fosfomycin than in animals given cisplatin alone. Similarly, renal damage appeared to be reduced histopathologically and functionally by the combined administration of cisplatin and fosfomycin.

Comparison of intra-arterial cis-diamminedichloroplatinum II with high-dose methotrexate and citrovorum factor rescue in the treatment of primary osteosarcoma.

A randomized two-arm study was undertaken to determine relative tumoricidal effects of intra-arterial cis-diamminedichloroplatinum II (I/A-CDP) and high-dose methotrexate with citrovorum factor rescue (MTX-CF) in the treatment of the primary tumor in patients with osteosarcoma. Responses were evaluated by clinical, radiographic, angiographic, and pathologic parameters. Fifteen patients were randomized to receive MTX-CF and 15 to I/A-CDP. In the MTX-CF arm there were four responses (three complete responses, one partial response) whereas in the I/A CDP arm there were nine responses (seven complete responses, two partial responses). Two patients who failed MTX-CF and requested alternative treatment with I/A-CDP also responded. The total I/A-CDP response was 11/17.

Masticatory rhythm analyzing method and clinical application.

The rhythm of mastication is one of the important factors in evaluating the masticatory function. This masticatory rhythm should be observed from both the mandibular movements and masticatory muscular activities. In addition, the changes in the rhythm from the beginning to the end of the mastication process should be reflected by the chosen parameters. In order to achieve this, a new analyzing method was devised in which the electromyographic burst duration time was divided into three divisions based upon the mandibular movement phase, and two parameters were chosen to represent the rhythm. In this paper, the new method was used on the patients with a temporomandibular joint dysfunction and patients who had a mandibular segmental resection in order to evaluate its validity and clinical applicability. By the comparison of the masticatory rhythms before and after treatment, the effectiveness of this analyzing method in evaluating the masticatory functions was shown.
