[Value of the electrophoresis mobility test in the differential diagnosis of obscure kidney findings].

The differential diagnosis of unclear carcinoma-suspicious renal findings often finishes with the test-exposition and nephrectomy. Also when all modern diagnostic possibilities are used not always a clear establishment of the diagnosis may be achieved. The cell electrophoresis mobility test after Field and Caspary may contribute to the differential-diagnostic clarification of the findings, when adequate antigens are used. For this purpose 10 own cases are discussed and these results are compared with suitable comparative groups. Summarizingly by the present study can be shown that the immunologic diagnostics of tumours in the field of unclear renal tumours may be a very good help in the diagnostics. When adequate antigens are used in the cell electrophoresis mobility test a coordination concerning the histospecificity can be achieved.

[Organ preservation in intra- and extracorporeal kidney surgery].

With the help of cases of own patients is reported on the possibility of organ preservation in intra- and extracorporal renal surgery. In 5 patients with tumours, 3 patients with coral calculi and one patient with an extended disease of the ureter five times the extracorporal renal surgery with following autotransplantation and initial pulsatile fall stream perfusion was performed. Four times the in-situ-perfusion with hypothermic Collins solution and in-situ-operation was performed. Indication and advantages for the different methods are demarcated.

[Possibilities and limits of kidney transplantation].

Chronic haemodialysis and renal transplantation are mutually supplementing methods for the treatment of patients with terminal renal lesion. The two methods have proved their worth in clinical practice. The expectance of life of patients with chronic renal insufficiency could essentially be improved during the last years. In last consequence the successes of the transplantation of kidneys depend on the solution of immunobiological problems, which are not yet cleared up nowadays. 1. In the determination of genotypical determinants possibly not all are known or recognizable. 2. The at present clinically usable examination methods do not yet allow to recognize rejections so early that by an aimed immunosuppressive treatment irreversible damages on the graft may be prevented. After a transplantation of kidneys of relatives a long survival time of transplanted patients is better than after a transplantation of kidneys taken from dead bodies. The rejection is still the main factor of the failure of the graft, the sepsis is the most frequent cause of death. It is neccessary, to develop less toxical remedies for the adaptation of the graft. Nevertheless, thousands of optimally transplanted patients prove the usefulness of the allogenic transplantation of the kidney.

A contribution to immunological tumour diagnostics in urology.

Fifty-four patients with malignant and other diseases of the urinary bladder and the prostate were examined for lymphocyte sensitization by means of electrophoretic mobility test (EM test). In this antigens from brain tissue (EF) and from malignomas of kidney, urinary bladder and prostate were used. Positive HEP values were obtained in 40 patients out of 42 with malignant tumours and in 5 patients out of 12 with other kidney diseases. With the corresponding tumour-associated antigens (TAA) in the EM-test, the histological tumour diagnosis could be confirmed in every case. Positive findings after using EF as well as the corresponding TAA are with great probability an indication of a malignant disease.

[Enzymatic and morphologic studies on long-term perfused dog kidneys].

21 canine and 12 human kidneys were submitted to long-term perfusion for 24 hours. During that time LDH-activity, histologic, enzyme-histochemic, and ultrastructural examinations were carried out. The findings proved that kidneys with lesser damage from ischemia showed lower activities during the first 4 to 6 hours than kidneys which were more injured before. The latter showed the same ultrastructural alterations as kidneys after perfusion for 24 hours, which suggests an irreversible partial damage of the organ.

[Immunologic course of kidney transplants studied by means of cellular and humoral immunity tests].

In 10 human-allo-transplanted persons and evaluation of the postoperative course was done with the help of specific and unspecific, cellular and humoral immune tests which were compared with clinical parameters. While the E-rosette-test did not show any correlation to the typical courses in the immunoglobulins postoperatively after a clear decrease at the time of the clinically diagnosed first set reaction an increase above all of the IgM could be observed. Unspecific tests, such as CRP and FSP may, concerning the rejection diagnostics, confirm the suspicion in as far as other factors, such as infection of the urinary tract were excluded. By choice of a suitable relation system the evidence of the immunoglobulins could possibly be proved, whereas for the cellular demonstration methods the E-rosette test does not seem to be recommendable in the performed way.

[Perfusion technic during clinical intra- and extracorporeal tumor removal from a solitary kidney].

On the basis of own experiences in operations of tumour-carrying kidneys two perfusion techniques for the intra- or extracorporal renal surgery are exolained and weighed one to another. In the in-situ-perfusion the pulsatile mechanical perfusion with supervisable flow, perfusate pressure and under direct measuring of temperature on organs has proved. The disadvantage of the in-situ-perfusion in contrast to the extra-corporal surgery consists in the impossibility of the local after irradiation.

[Course of cellular immunity in urologic neoplasms].

Apart from a purely qualitative evidence concerning the presence of a specific sensibilisation it is possible to estimate also its strength or the blocking factors appearing in the serum of tumour carriers by means of the electrophoresis mobility test. By this it seems to be possible to make an evidence concerning the immunological reaction position of the tumour patient with the possibility to make conclusions as to the prognosis of the patient.

[A contribution to the early diagnosis and differential diagnosis of bladder neoplasms by means of immunological methods].

According to the present examinations it is pretty probable that already in early histological forms of the carcinoma of the urinary bladder the EM-test is able to indicate immunologically malignity, i.e. that the papillary non-infiltrating carcinoma of the transitional cell as well as the carcinoma in situ of the urinary bladder (23) also immunologically behave like tumors of the bladder. In differential-diagnostic questions concerning the organ localisation of the primary tumor the EM-test seems to give valuable references. Histologically not unequivocally malignant tumors of the bladder with positive results in the EM-test should be submitted to an exacter examination and control.

[Immunodiagnosis of urologic tumors].

In 42 patients with malignant and benign diseases of the kidney, the urinary bladder and the prostate the sensitization of the lymphocytes was investigated in the cell electrophoresis mobility test. In these cases human encephalitogenic protein and tumour-associated antigens from malignomas of kidney, urinary bladder and prostate were used as antigen. When using human encephalitogenic protein 3 "falsely positive" and 2 "falsely negative" results were got, whereas, when using the corresponding tumour-associated antigens in the cell electrophoresis mobility test in every case the histologically ascertained tumour diagnosis could be confirmed.
