[Current immunologic findings in bladder cancer].

As a result of the growing significance of immunological findings in malignant growths, more attention is being paid to these questions in the diagnosis and treatment of carcinomas of the urinary bladder in humans. In this particular field the findings are not very numerous. In the field of immunodiagnosis demonstrable immunoreactions caused by cells seem to possess greater significance. Skin tests have only had prognostic significance to date. In the field of immunotherapy a non-specific BCG treatment was carried out on a small number of patients with various forms of application. These results do not give any certain information. The author refers to studies on chemically induced carcinomas of the urinary bladder in animals.

[The immunologic behavior of carcinoma in situ of the bladder].

A report is given on 13 patients with c. i. s. of the bladder, which are examined in vivo and in vitro with respect to their immune reactivity. In skin tests with 5 recall antigens and the primary antigen DNFB there is no difference as compared with a control group of 29 patients. In an EM test 11 of the 13 patients with c. i. s. reacted with histospecific lymphocyte sensitisation, 10 out of 13 positive results were obtained with the antigen HEP. These findings support the current view of c. i. s. of the bladder as the first stage of a malign degeneration.

[The electrophoresis mobility test (EMT) in the immunologic after care of patients with kidney transplantations].

Of 80 recipients of kidney transplants 46 patients could after operation immunologically be controlled by means of the electrophoresis mobility test. Here a good correlation to the clinically observed or not established immune responses was shown. The method seems to be suitable for a postoperative immunological control, needs, however, a testing in a great number of patients.

Recovery of free androgens in the rat prostate in vivo and in vitro after treatment with orally active testosterone undecanoate (TU).

Short term castrated rats were sacrificed at different time intervals after oral administration of tritium labeled TU. Blood and organs were collected and measured for total radioactivity uptake. At the time of peak values of radioactivity the in vivo metabolite pattern was investigated by thin layer chromatography. In vitro studies were performed in plasma, muscle and prostate cytosol. The radioactivity uptake into plasma and prostate in vivo reached a first maximum 2 1/2 h after oral administration of TU. A second peak appeared after 5 h in the prostate. While only 16% of the total plasma radioactivity appeared in the non esterified fraction, the tissues contained 69-71%. In plasma, the bulbocavernosus/levator ani muscle and in the skeletal muscle, free testosterone was the main unesterified androgen (14-28%). Prostate and seminal vesicles contained primarily 5 alpha-DHT (47 and 34%, respectively). Further metabolites were found in the fractions of 17 beta-hydroxy-5 alpha-androstanediols, epiandrosterone, androsterone, 4-androstenedione and 5 alpha-androstanedione. After incubation in vitro with TU, prostate and muscle cytosol contained significant amounts (39% and 17%, respectively) of free androgens. It was concluded that peripheral organs as well as the prostate itself are capable of cleaving the undecanoic ester and that the biological action of TU is exerted by unesterified androgens which occur in a pattern similar to that obtained after i.v. injection of free testosterone.

[Determination of alanine aminopeptidase and lactate dehydrogenase in the urine as a control over the patient's course after kidney transplantation].

In 42 patients the determination of alanine aminopeptidase and lactate dehydrogenase in the urine was performed after transplantation of a kidney. In the evaluation of the enzyme activities it was referred to the clinically made rejection diagnosis as well as to the increase of the serum creatinine. We found out that with regard to the clinically made rejection diagnosis the alanine aminopeptidase in 56% and the lactate dehydrogenase in 55% of the cases showed an increase before the beginning of the rejection therapy. Before the corresponding increase of the serum creatinine we could establish an increase of the enzyme activity in the alanine aminopeptidase in 45% and in the lactate dehydrogenase in 65% of the cases. For a better interpretation of the urinary enzyme activities during the rejection crises the average activities of the enzymes immediately after the dismissal of the patients as well as three months, six months and one year after the transplantation are cited. From our investigation results that the daily determination of alanine aminopeptidase and lactate dehydrogenase in the urine is suited, taking into consideration other possible influence factors on the enzyme activity as well as other parameters, to support the diagnostics in finding the rejection after the transplantation of a kidney.

[Therapeutic procedure in acute ulcer hemorrhage after cadaver donor kidney transplants].

In 62 cadaver-donor-allograft recipients we observed 11 times gastrointestinal complications 7 times (11%) an ulcer or an erosion of the mucous membrane could be secured gastroscopically. The therapeutic remedy of choice seems to be the well-timed 2/3-resection after Billroth II, since a too long hesitation increases the number of complications, has a negative influence on the choice of the intervention and increases the lethality.

[Advanced malignant kidney tumors with the clinical picture of septic kidney].

At the instance of 4 patients the differential-diagnostic difficulties between advanced malignant renal tumours and the so-called septic kidney are demonstrated. In fever conditions in connection with unclear urographic findings, in particular with the so-called dumb kidney, it is right primarily to search for a septic renal disease. In spite of urgency of diagnosis and above all of the therapeutic consequences resulting from this the presence of an advanced malignant renal tumour must be included in the differential diagnosis also when there are no leading tumour symptoms. This demands the rapid use of a highly specialised radiologico-urologic diagnostics. Many clinical and paraclinical findings are differential-diagnostically without importance, since they may be typical for the two pictures of the disease. A certain preoperative diagnosis should be achieved above all on account of the differentiated operative approach and the operative way of access.

[Use of the dura in the surgical treatment of bladder cancer].

It is reported on 9 patients in whom a dura graft was performed on account of a carcinoma of the urinary bladder. Apart from one patient who before underwent radiotherapy the dura healed. As to complications three times formations of calculi and twice rejection of larger parts of the dura were found. Indications to dura graft as well as its advantages and disadvantages are discussed and it is referred to immunological problems which are still to be clarified.

[Compliance and active cooperation of patients in preventing recurrence of urinary calculi].

An anonymous questioning of 200 patients with urinary calculi concerning the observation of the general and special measures of the metaphylaxis of urinary calculi resulted in the fact that in particular the measures of the general metaphylaxis (bodily movement, diet, dilution of the urine) are not or only parlty obeyed by a large part of the patients. The examinations show that for the improvement of the understanding of the disease and the behaviour of the patients concerning their health further on a great attension must be paid to the quality of the information by the physician and to the communication between physician and patient. An insufficient cooperation of the patients in the metaphylaxis of the urinary calculi would call in question the effectivity of all efforts and measures concerning the prevention of relapses.

[Critical considerations on the production of a pulsating fluid stream].

Two types of gears are used for perfusion of cadaveric kidneys. Electric models are presented for both types and the optimal devices are discussed. Based upon in vivo conditions, the relevant parameters are transferred to the model and an according equipment for pulsatile fall-stream perfusion is presented, which is compared with another gear. The clinical criterion for good initial perfusion is the erythrocyte count in the initial perfusate. Judged from that, the chosen device is significantly better than its predecessor.
