Hemangioblastomas of the medulla oblongata were successfully excised in two separate cases. The uncertainties of this procedure involve the value of preoperative radiation therapy, the relationship between the tumor and the medulla oblongata, and the surgical techniques themselves. Cardiovascular and respiratory disorders often complicate this type of surgery, and postoperative dysphagia is a frequent sequela.
The computed tomographic (CT) and other roentgenological characteristics of a skull diploic epidermoid cyst in a 14-year-old boy with Ito syndrome, a neurocutaneous syndrome, are presented. When this boy was 3 years 10 months of age, a plain roentgenogram of the skull showed an area of decreased density in the left parietal bone. The area has grown for the subsequent 10 years plus and a characteristic sclerotic margin has become evident. CT examination showed a cystic lesion in the diploe, the outer third of high density and the inner third low density. CT allowed prediction of the operative and histological findings. It was useful not only for the diagnosis but also in the pathogenesis.

The clinical effectiveness of KL 007 (oxybutynin hydrochloride) was studied on patients with urinary disturbance, mostly complaints of urinary frequency, who were suffering from nervous pollakisuria, irritable bladder or neurogenic bladder. Fifty two patients were administered KL 007 three times daily at the dose of 6 mg, 9 mg, or 12 mg per day for one or two weeks. An overall clinical effectiveness of KL 007 was seen in the cases of hypertonic bladder. As to the clinical effectiveness of subjective symptoms, KL 007 improved the symptoms of urinary frequency, urinary incontinence, discomfort upon micturition, urinary urgency and sense of residual urine. Transient side effects were observed in 23 cases (41.4%), and dry mouth was most observed. No severe side effects were noticed.
E-643, a newly developed alpha 1-adrenergic blocker with a postsynaptic action, was tried on 17 subjects, 10 patients with neurogenic bladder and 7 patients with lower urethral obstruction. The results of treatment were excellent in 5, good in 7, and poor in 5 of them, the rate of effectiveness therefore being 70.6%. The inhibitory effect of E-643 on the contractility of the detrusor was no stronger than that of other alpha-blockers. The rate of effectiveness for neurogenic bladders was as high as 80% whereas that for lower urethral obstruction was much lower: 57%. Concerning benign prostatic hyperplasia (B.P.H.) alone, this agent was markedly beneficial. In the case of B.P.H the urethra pressure in the prostatic urethra tends to drop and urination is facilitated even without a change in maximum urethral closure pressure, which reduces the amount of residual urine, this sequence of events taking place on condition that the contractility of the detrusor is adequate. According to urodynamic criteria, the underlying causes of the so-called bladder neck contracture can be classified into organic and functional obstruction, but it is mistaken to use this term for functional disturbance of the bladder neck. It seems worthwhile to determine the clinical effect of alpha-blockers on functional urethral obstruction. E-643 produced few if any appreciable signs or symptoms of side effects either subjectively or objectively.
The mechanism of impact head injuries has generally been discussed on the basis of the cranium is a rigid box completely filled with incompressible contents. In this paper, two cases of head injuries in patients with severe craniectomized skull defects are reported and their conditions are discussed. The 1st case was a 46 year-old female with a large frontoparietal skull defect following surgery on a brain tumor. While hospitalized, she fell down from her bed (50 cm in height) and suffered strong impact on the craniectomized area. From immediately after the injury, there was flaccid hemiparesis, but CT scans showed no morphological changes. The 2nd case was a 12 year-old female with a major frontal craniectomized skull defect following surgery. She fell down in a corridor at school and suffered an impact injury in the defect area. For several days after the fall, she had a headache and nausea, but there were no other neurological deficits. In CT scans, there were findings of a cortical contusional hemorrhage. Wounds of the skull with a partial defect, i.e. "a semiclosed box" in the narrow sense, cause conditions which are difficult to explain by conventional dynamic traumatology. In this paper, these rare head injuries are discussed on the basis of these two cases experienced by the authors and the literature.

Klippel-Trenaunay Syndrome with hemicranial hypertrophy and macrodactyly is reported. The patient was a 3-year-old Japanese woman with an extensive nevus flammeus which extended over the right side of the neck, axilla, upper arm, and leg. Her right extremities were longer and thicker than left, and showed apparent macrodactyly in the right hand. Her skin of right side body was irregular, rough surface and was pigmented prominently. Hyperthermia was present in the hypertrophic limbs, but hyperhidrosis was none. She had moderate mental retardation, delayed speech. Her hemicranial hypertrophy (same side as hypertrophic limbs) was examined by CT scan. Her right lateral ventricle was dilatated and cortical sulci was well demonstrated. Her right hemicranial volume was enlarged but rt-cerebral hemisphere was atrophic, and her infratentorial structures were symmetrical. Presented case, Klippel-Trenaunay Syndrome with hemicranial hypertrophy, suggested the embryological theory affecting on trilaminar disc in embryonal stage as the genesis of this rare syndrome.
A new nonsteroidal anti-inflammatory agent, fentiazac, was used for analgesia after tooth extractions and minor oral surgery in two Japanese dental hospitals. The drug was administered as a single oral dose of either 50 mg or 100 mg. The 50-mg dose provided rapid analgesic effect, but its effect lasted only two to three hours in a number of patients. At a dose of 100 mg, fentiazac proved effective for 85% of 53 patients, usually providing marked reduction of disappearance of pain within one hour or less. Among patients in whom pain reappeared, the mean time for recurrence was four hours, indicating a satisfactory duration of analgesic effect. One side effect--loss of appetite--was reported by one patient in the entire series of 71 subjects. It is concluded that fentiazac is a highly effective analgesic agent with a wide margin of safety for use after dental procedures that produce pain.

We report on three cases in which shunt devices (Sophy programmable pressure valve) broke while implanted in the hypochondriac region. None of these patients had an apparent history of trauma related to the shunt valve. The outlet connector was broken at the same site in each case. The dissociation between the movement of the thorax and that of the abdomen may have caused the formation of a crack in the outlet connector.
OBJECTIVE: The purpose of this study was to determine the accuracy of high resolution CT (HRCT) in the detection of facial canal dehiscence and semicircular canal fistula, the preoperative evaluation of both of which is clinically very important for ear surgery. MATERIALS AND METHODS: We retrospectively reviewed the HRCT findings in 61 patients who underwent mastoidectomy at Yamagata University between 1989 and 1993. The HRCT images were obtained in the axial and semicoronal planes using 1 mm slice thickness and 1 mm intersection gap. RESULTS: In 46 (75%) of the 61 patients, the HRCT image-based assessment of the facial canal dehiscence coincided with the surgical findings. The data for the facial canal revealed sensitivity of 66% and specificity of 84%. For semicircular canal fistula, in 59 (97%) of the 61 patients, the HRCT image-based assessment and the surgical findings coincided. The image-based assessment in the remaining two patients, who both had massive cholesteatoma, was false-positive. CONCLUSION: HRCT is useful in the diagnosis of facial canal dehiscence and labyrinthine fistula, but its limitations should also be recognized.
