Human parvovirus B19-induced transient pancytopenia in a child with hereditary spherocytosis.

We report a child with hereditary spherocytosis who developed human parvovirus B19 (HPV B19)-induced transient pancytopenia. Symptoms of acute-phase illness subsided promptly after infusion of gammaglobulin containing HPV B19 antibody. The HPV B19-containing serum on admission inhibited not only erythroid colony growth but also myeloid and megakaryocyte colony growths. The inhibitory effect was abrogated by convalescent-phase serum or gammaglobulin solution containing HPV B19 antibody. These findings suggest that HPV B19-induced inhibition of haematopoiesis may be pathogenetic for transient bone marrow failure of the patient. The passive immunization may be effective in shortening the duration of acute-phase illness of HPV B19 infection.

[A phase II study of Futraful suppository in head and neck cancer].

A phase II study of FT-207 suppository (Futraful Suppository) for head and neck cancer was conducted at the Kagoshima University Hospital Dept. of Otorhinolaryngology and its 6 affiliated hospitals. Forty cases with head and neck cancer were entered in the trial, of which 37 were perfectly evaluable. Partial response (PR) (16.7%) was achieved in one of 6 patients with maxillary cancer, in 2 of 9 cases with oral cancer (22.2%), in one of 7 with laryngeal cancer (14.3%), in 3 of 13 with pharyngeal cancer, while 2 of the same 13 achieved a complete response (CR) (38.5%), for a total response rate of 24.3%. FT-207 suppository was continuously administered as standard, at a dose of 750 mg each twice daily, or 1,500 mg/day. The response rate was dose-dependent; 2 CR cases with a total dose of 105 g for administration over a total period of more than 150 days. Adverse effects appearing in 35.0% of the overall were mainly anorexia, nausea and vomiting; none of them, however, were serious.

No evidence for gamma-interferon mediated haematopoietic inhibition by T cells in aplastic anaemia: an observation in the course of immunosuppressive therapy.

Two patients with aplastic anaemia were treated with immunosuppressive agents and peripheral blood T cells were cryopreserved serially. Inhibitory activity of T cells to autologous CFU-E, gamma-IFN production by T cells and T cell subpopulations were assayed after remission. Inhibitory activity to autologous CFU-E was not correlated with the numbers or ratios of T cell subpopulations. gamma-IFN production by T cells were within the normal range when inhibitory activity was found. In addition, gamma-IFN production increased after haematopoietic recovery. These findings suggest that gamma-IFN is not a soluble mediator of T cell-mediated haematopoietic inhibition in aplastic anaemia.

Simultaneous transient erythroblastopenia and agranulocytosis: IgG-mediated inhibition of erythrogranulopoiesis.

We report a case of simultaneous transient erythroblastopenia and agranulocytosis recovering spontaneously. In vitro study using autologous bone marrow cells after recovery demonstrated IgG-mediated inhibition of both erythropoiesis and granulopoiesis but not megakaryopoiesis. The inhibitory activity disappeared shortly after remission. These findings suggest that IgG-mediated inhibition of hematopoiesis may be pathogenetic for transient bone marrow failure of the patient.

Intravenous gammaglobulin therapy for thromboneutropenic neonates of mothers with systemic lupus erythematosus.

i.v. IgG therapy was used to treat thromboneutropenia in 2 male neonates of mothers with SLE. The mothers of both neonates were thrombocytopenic but not neutropenic during the last 3 months of their pregnancies. Anti-platelet and anti-neutrophil IgG directed against platelets and neutrophils from the neonates were detected in the sera of both mothers. The platelet and neutrophil counts increased significantly with i.v. IgG therapy in both neonates. i.v. IgG therapy is a safe and effective regimen for passive immune thromboneutropenia.

[Air-borne dust and its protein in laboratory animal facilities].

Atopic individuals who were exposed to dust in animal rooms for a long time were more likely than non-atopics to have animal-related symptoms. Standard skin prick tests were performed using crude antigens from dander and urine, and 75 to 100% of atopic persons had positive skin reactions. It seems that allergens in animal rooms result from air-borne dust and the dust originates in animal urine and dander. The protein content occurs as dust particles both below and above 10 micron in diameter. The dust particles above 10 micron cannot pass through face masks, so half of the dust protein is arrested under the mask, but smaller particles pass through. In order to minimize the symptoms, we need to improve the design of cages, animal rooms, experimental rooms, ventilation systems and other protective equipment.
