Reconstruction of the thoracic oesophagus using autotransplanted small intestine. An experimental study in the piglet.

A technique for overbridging thoracic oesophageal defects with revascularized intestinal autotransplants was evaluated in 32 piglets. A microvascular end-to-side anastomosis was performed between the 1.5 mm transplant artery and the aorta. Specially constructed vascular scissors were used to cut a suitable opening in the aorta. The venous anastomosis was performed between the transplant vein and the hemiazygous vein. After partial oesophageal resection, the revascularized autotransplant was interposed between the oesophageal ends and anastomosed. The series carried a high rate of complications, but the study showed, however, that the technical prerequisites exist to perform this kind of operation.

Ultrasonography of the gallbladder. Investigation of the nonvisualized gallbladder at oral cholecystography.

One hundred and one patients with nonvisualized gallbladder at double dose oral cholecystography were examined with ultrasonography. In 41 of 44 operated patients, ultrasonography suggested gallbladder disease. Operation in all of these cases confirmed the diagnosis. In three of the patients with cholelithiasis operation revealed in addition gallbladder carcinoma. In three patients, in which ultrasonography demonstrated normal appearance of the gallbladder surgery revealed chronic cholecystitis and stone impacted in the cystic duct. The diagnostic value of ultrasonography as complementary method for evaluation of gallbladder disease, particularly in cases of nonvisualization of the gallbladder at ultrasonography is stressed. The high incidence of gallbladder carcinoma in the present material in patients with cholelithiasis point to the fact that this group of patients should be the subject for careful investigation.

Experimental porta-caval anastomosis and motor nerve conduction velocity in the rat.

The influence of portal-systemic shunting on maximal motor nerve conduction velocity (MCV) was analyzed in rats with portacaval shunts. At 3 and 8 weeks after the shunting there was a marked but transient fall in MCV. This was not prevented by colectomy. At 16 weeks the MCV had almost normalized, despite sustained portal-systemic shunting. When examined at 3 weeks postoperatively, MCV was less affected by ischaemia than in control animals. The results favour hepatocellular failure as the more important pathophysiological mechanism in hepatic neuropathy.

Quantitative Doppler and ultrasound measurements in surgically performed arteriovenous fistulas of the arm.

Quantitative Doppler flow velocity determinations were made combined with ultrasound estimation of the arterial diameter of the feeding vessel in arteriovenous fistulas of the arm used both for dialysis administration of cytostatics and parenteral nutrition. In 83 patients flow velocity alone was determined while the volume flow was calculated in 63. The effect of the length of the dialysis period and age of the fistula on the different parameters is discussed.

[Clinical aspects and diagnosis of gallbladder cancer].

17 patients with primary carcinoma of the gallbladder were studied. In 7 cases a palliative operation for biliary or gastrointestinal obstruction was performed, in 6 patients a biopsy only. None of the 4 patients undergoing a more "radical" procedure survived 2 years. The main symptoms were pain, jaundice and weight loss. Other manifestations were acute cholecystitis and pyloric obstruction. A correct preoperative diagnosis was established only in 3 cases. Most of the small tumors are discovered by the surgeon who performs a cholecystectomy for stones. Simple cholecystectomy, however, is a palliative procedure in most cases with carcinoma involving all layers of the wall.

Infusion tomography and ultrasonography of the gallbladder in the diagnosis of acute cholecystitis.

Sixty-three patients with the clinical suspicion of acute cholecystitis were examined with infusion tomography of the gallbladder. Ultrasonography was performed in 51 of these cases. The technique and diagnostic principles of both methods when used in combination is stressed. Thus in a case of gangrenous cholecystitis when opacification of the gallbladder wall may not appear at infusion tomography, ultrasonography may demonstrate signs of gallbladder disease. Infusion tomography, on the other hand, may be of great value if ultrasonography is not informative.

Brain proteins in experimental portal-systemic shunting.

Experimental portal-systemic shunting is accompanied by a reduction of the soluble brain proteins. Isoelectric focusing does not indicate any selective decrease of one particular protein or group of proteins. The reduction persists with sustained shunting. The interference with brain protein metabolism is compatible with a grossly normal behavior, alterness and locomotion.

[Morphological liver changes after portacaval and splenocaval anastomosis in rats].

Body weight, liver weight and liver histology in rats 3 weeks after portacaval and distal splenocaval anastomosis were analyzed. Rats with portacaval shunt showed a reduction of body and liver weight and histological signs of liver atrophy. There was no difference between the results in rats with splenocaval shunt and controls.

[Gastric secretion and gastrin values following portocaval and splenocaval shunts in the rat].

The basal and gastrin-stimulated gastric acid output was measured in rats with a distal splenocaval shunt. In addition, serum gastrin levels were determined in rats with splenocaval and portacaval shunts. The rise in gastric acid output with increasing gastrin dosage was the same in rats with splenocaval shunt as in the controls. Serum gastrin levels were not influenced by portocaval or splenocaval shunting.

Obstructive nephropathy with infection. A diagnostic pitfall.

Four patients with indistinct abdominal symptoms associated with fever are presented. In all cases the diagnosis proved to be obstructive nephropathy with infection. The proper treatment of the patients was delayed, essentially because of a non-investigative diagnostic attitude. The necessity of making a correct diagnosis in the early course of an acute abdominal disease with fever is emphasised and some investigations that may be of great value for reaching a precise diagnosis are indicated.

Effect of hypercapnia and hypocapnia on tryptophan and tyrosine hydroxylation in rat brain.

The effects of induced hypo- and hypercapnia upon the rate of hydroxylation of tryptophan and tyrosine in the rat brain were studied by measuring the accumulation of 5-HTP and DOPA following administration of the aromatic L-aminoacid decarboxylase inhibitor 3-hydroxybenzylhydrazine HCl (NSD 1015). The results suggest that the hydroxylation of tryptophan varies directly with the tissue Po2. On the other hand, the hydroxylation of tyrosine did not show a simple relationship to Po2 but appeared to be influenced by pH changes.

The effect of hepatectomy on the energy state and on acid-base variables of the rat brain.

The influence of hepatectomy on cerebral energy metabolism was studied in rats in which an end-to-side portacaval anastomosis had been performed one week earlier. The hepatectomy gave rise to an exaggeration of the increase in tissue ammonia content resulting from the shunt. Three hours after the hepatectomy there was a fall in tissue glutamate content but no change in alpha-ketoglutarate. The energy state of the tissue, as evaluated from the concentrations of ATP, ADP, and AMP, was unchanged. There was a significant increase in cerebrospinal fluid pH but no change in intracellular pH.
