[Treatment of postoperative mediastinitis using an omental pedicle flap].

Four patients with postoperative mediastinitis who were treated by omentopexy at the Fukuoka University Hospital between 1989 and 1990. Three of the 4 patients healed successfully, another one died of multiple organ failure 83 days after surgery. All patients were received coronary artery bypass surgery harvesting a left internal thoracic artery for ischemic heart disease. Three patients had diabetes mellitus, one patient had renal failure preoperatively. Recognition of mediastinitis was made by sternal wound purulent discharge and sternal dehiscence. Culture of the discharge fluid yielded methicillin-resistant Staphylococcus aureus in three, and Enterococcus cloacae in one. Irrigation with popidone-iodine or blonopol were ineffective. Thus, the wound was treated with debridement and omentopexy with an omental pedicle flap, respectively. Postoperative course after omentopexy were excellent, had no complications. We conclude that the omentopexy is useful in the treatment of postoperative refractory anterior mediastinitis.

[A case of drug-induced esophageal ulcer developed at the esophageal constriction due to the right aortic arch].

A 26-year-old male has a tooth extracted at a dental surgery, and was given 40 mg Froben, as an anti-inflammatory analgesic agent, and ingested one tablet without water. After 2 days, anterior chest pain occurred. Esophagography and endoscopy were performed and multiple esophageal ulcers were confirmed. Esophagography revealed an esophageal stenosis which was considered to be caused by the extramural compression on the oral side of the ulcers. In the chest roentogenography and CT scanning, the right aortic arch was observed. This case was diagnosed as having drug-induced esophageal ulcer developed at the esophageal constriction due to the vascular ring.

Nucleotide sequence and transcriptional mapping of the major capsid protein gene of pseudorabies virus.

The gene encoding the 142-kDa major capsid protein (MCP142) of pseudorabies virus (PrV) was isolated and sequenced. Nucleotide sequence analysis revealed that the MCP142 gene has a single open reading frame of 3993 nucleotides (nt) encoding 1330 amino acids. The 4400-nt major RNA from the MCP142 gene was detected in PrV-infected cells. The 5' end of the transcript was located 60 nt upstream of the initiation codon. The 3' end of the transcript was located 18 nt downstream of a putative poly(A) signal sequence TATAAA and 133 nt downstream of the termination codon. In comparing amino acid sequence homology between MCP142 of PrV and other available herpesviruses MCP was shown to have 58% homology with herpes simplex virus type 1 and varicella-zoster virus, 27% with Epstein-Barr virus, and 24% with human herpesvirus 6 and human cytomegalovirus. It has greater homology with those of the alpha-herpesviruses than with those of the beta-herpesviruses and the gamma-herpesviruses.

Immunohistological demonstration of spread of Aujeszky's disease virus via the olfactory pathway in HPCD pigs.

The spread of Aujeszky's disease virus (ADV) from nasal mucosa via the olfactory pathway was studied in HPCD pigs. ADV antigen was detected in the epithelial cells, nasal gland cells, olfactory nerve cells and peripheral nerve fibres in the nasal cavity and in neuroglial cells in the olfactory bulb. Results indicate that the olfactory pathway is one of the most important neuronal pathways of ADV infection in pigs.

Pathogenesis of Aujeszky's disease virus infection in swine tracheal organ culture.

Two different strains of Aujeszky's disease virus (ADV) were inoculated into swine tracheal organ culture. Both viruses replicated in and destroyed the tracheal epithelium and epithelial cells. ADV antigen was first localized in ciliated epithelial cells by fluorescent antibody and immunoperoxidase examinations. Corresponding to the distribution of ADV antigen, many ADV particles were observed in ciliated epithelial cells. Results demonstrated that the tracheal epithelium infected with ADV is reduced in its ciliary activity.

Renal sympathetic nerves modulate glomerular ANP receptors and filtration.

We examined characteristics of atrial natriuretic peptide (ANP) receptors in glomeruli isolated from subacutely (3-5 days) denervated (DNX) and contralateral nondenervated (non-DNX) kidneys of normal rats (NL) and rats subjected to water deprivation for 48 h (WD). Total ANP receptor density in DNX kidneys of WD rats, measured by competitive inhibition binding between 125I-labeled ANP and ANP, was twofold higher than non-DNX kidneys (726 +/- 96 vs. 384 +/- 32 fmol/mg protein, P less than 0.05). Equilibrium association constant (Ka) was not significantly different (2.33 +/- 0.43 vs. 3.34 +/- 0.78 x 10(9) M-1). In NL rats, there was no difference in ANP receptor density between DNX and non-DNX kidneys (244 +/- 20 and 264 +/- 16 fmol/mg protein). Production of guanosine 3',5'-cyclic monophosphate (cGMP), a putative second messenger of ANP, in response to ANP (10(-7) M) in glomeruli isolated from DNX was significantly larger than non-DNX kidneys of WD rats. To determine whether these changes in ANP receptors have functional consequences in vivo, glomerular capillary ultrafiltration coefficient (Kf) was assessed by micropuncture technique in WD Munich-Wistar rats. In DNX kidneys, ANP infusion (4 micrograms.kg-1.h-1) significantly increased whole kidney glomerular filtration rate (GFR) and single-nephron (SN) GFR (0.64 +/- 0.06 to 0.89 +/- 0.17 ml/min and 25 +/- 2 to 33 +/- 2 nl/min, respectively; n = 7) and Kf (1.26 +/- 0.29 to 2.18 +/- 0.41 nl.min-1.mmHg-1).(ABSTRACT TRUNCATED AT 250 WORDS)

Studies on macrocyclic lactone antibiotics. XIII. Anti-tubulin activity and cytotoxicity of rhizoxin derivatives: synthesis of a photoaffinity derivative.

Chemical modification of the side chain in rhizoxin, a potent antimitotic agent, was attempted in order to study structure-activity relationships and also to devise a probe for photoaffinity labeling of tubulin. An OsO4/NaIO4 oxidation gave a nor-rhizoxin 20-al (5) which was converted to 20-ol (6) by a NaBH3CN reduction. Starting from these two compounds as key intermediates, a series of Wittig reaction products 7-2, and of 20-O-acylates 13-21 were prepared and their anti-tubulin activity and cytotoxicity were determined. An aryl azide derivative 23 was synthesized as a photoaffinity analogue.

[A case of bilateral phrenic nerve paralysis following open heart surgery].

Coronary artery bypass surgery was performed on a 58-year-old female under cold cardioplegia with topical ice slush cooling. Bilateral phrenic paralysis was observed postoperatively, in spite of prevention with a mat during aortic cross clamp, cold injury owing to ice slush was thought to be causative. Mechanical ventilatory support continued for more than two months until her complete recovery of diaphragmatic function. She complained of sleeplessness besides respiratory symptoms, and felt uneasy. Although cold injury is mostly reversible, it is stressed that we must deliberately wean from mechanical ventilatory support, turning our attention to symptoms and blood gas analysis with as much mental assistance as possible.

[Enhanced effect of intraperitoneal administration of CDDP combined with caffeine against peritoneal metastasis].

The enhanced effect of CDDP combined with caffeine against P-388 leukemia was investigated. No synergistic effect was shown after one hour simultaneous treatment with CDDP and caffeine. But the growth inhibition was enhanced by the addition of caffeine after one hour treatment with CDDP. It is suggested that caffeine inhibits DNA repair by the reduction of CDDP-induced elongation of G2 + M phase. The increase in life span of mice after ip transplantation was observed by frequent ip injections of caffeine after CDDP injection. It is suggested that the effect of intraperitoneal administration of CDDP against peritoneal metastasis is enhanced by combination with caffeine.

[Significance of blood vessel invasion in gastric carcinoma--prediction of liver metastases from the blood vessel invasion in the primary tumors].

This study was conducted to elucidate the factors related with liver metastases, to clarify the significance of blood vessel invasion and then to predict liver metastases from these findings. Patients examined were 102 cases which underwent gastrectomy and were followed-up for more than 5 years or until death at our department. Two new staining methods were applied in this study; Victoria blue + hematoxylin eosin double staining for elastic fiber and Factor VIII related antigen for vascular endothelium. Significant differences in qualitative frequency of blood vessel invasion, the number of lymph node metastases, and the depth of invasion were found in those patients with liver metastases, as compared with 5 year survivors. Quantitative analysis of blood vessel invasion revealed significant importance of blood vessel invasions in the submucosa, in the forms of complete thrombus, wall invasion and partial thrombus, in the diameters of 0.01-0.1 mm and 0.1-1.0 mm. Applying discrimination coefficients of linear discrimination analysis, prediction of liver metastases was possible with 81.8% sensitivity, 85.3% specificity, and 83.6% accuracy. Liver metastases can be predicted from the qualitative and quantitative analyses of blood vessel invasion in the primary tumors by elastic fiber staining.

Comparative pathology of HPCD pigs infected with wild-type and ara-T-resistant strains of Aujeszky's disease virus.

Fourteen 1- to 4-week-old hysterectomy-produced and colostrum-deprived (HPCD) pigs were inoculated intranasally with wild-type and ara-T-resistant strains of Aujeszky's disease virus (ADV), and the pathological lesion induced by the two strains was compared. The wild-type strain (YS-81) led to high mortality, and the pigs developed multifocal necrosis throughout the body and encephalitis. In comparison, the ara-T-resistant strain (YS-81TR) of the virus killed only 1-week-old HPCD pigs inoculated with 10(6.0) PFU per ml of the virus and did not kill pigs more than 2-weeks of age. The latter revealed consistently severe pneumonitis on post-inoculation day (PID) 14. Results of the present study indicated that the ara-T-resistant strain of ADV was less virulent for HPCD pigs than the parental wild-type strain of ADV and that it was able to grow better in the lung than in any other tissue.

[Surgical results of high grade constriction of tricuspid annulus in DeVega technique for secondary tricuspid regurgitation].

Sixty-six patients were operated on by our high degree constriction technique using DeVega tricuspid annuloplasty from 1985 to 1989. They were 20 men and 46 women, age distributed from 28 to 71 (mean 54.5). Preoperative tricuspid annular diameter ranged from 29 mm to 45 mm, average 36.3 mm. Those were constricted to 27 mm in 38 patients, and to 25 mm in 28 patients. Postoperative residual tricuspid regurgitation was observed in only 8.5% of total cases followed up 14 to 55 months. This results were comparable with other reports. We concluded that our tight constriction DeVega method is considered better to prevent postoperative tricuspid regurgitation than ordinary methods.

Radical surgery for gastric cancer. A review of the Japanese experience.

Overall results after operations for gastric cancer in Japan are far superior to results obtained in the US and Europe. We have reviewed the Japanese literature in an effort to determine what factors explain this difference. It appears that the survival differences are due mainly to a greater frequency of early gastric cancer in Japan; meticulous histopathologic evaluation of the surgical specimens, resulting in more accurate pathologic staging; and the presumed benefit of extended nodal dissection when it extends outside of the level of node-positive disease. Although patients with both apparent and confirmed direct adjacent organ invasion can be helped by resection of those organs, extended resections of uninvolved pancreas and spleen do not improve rate of survival beyond the benefit of improved nodal dissection. Overall, there would appear to be justification for reexamining extended nodal dissection for gastric cancer in the US. Opportunities for a meaningful national study are significant.

Discovery of atrial natriuretic factor in the brain: its characterization and cardiovascular implication.

1. We have devised a radioimmunoassay for atrial natriueretic factor (ANF). Its application to rat brain extract led to the discovery of ANF in the brain. In addition to the hypothalamus and the pontine medullary region, it was widely distributed. 2. ANF in the brain is stored in a low molecular weight form, in contrast to pro-ANF in the atria. Thus, the processing of pro-ANF in the bran neuronal cells is different from that in the atria. 3. ANF was found in the anterior and posterior lobes of the pituitary, the peripheral ganglia, adrenergic neurons, and the adrenal medulla. 4. Brain ANF suppressed stimulated dipsogenesis, basal and stimulated vasopressin release, and angiotensin II-stimulated pressor effects. 5. ANF in the peripheral neuronal system inhibits catecholamine synthesis and release. Thus, central ANF functions to reduce the peripheral fluid volume and vascular tone in concert with the peripheral ANF.

Carcinoma at the esophago-gastric junction--its biological behaviour toward the squamous epithelium.

The purpose of this paper is to assess the clinical and histopathological significance of tumors at the esophago-gastric junction (EGJ). The biological behaviour of these tumors was studied, with regard to the infiltrative pattern and mitotic index, in 15 patients with tumors at the EGJ. Twenty-seven tumors at the cardia, with or without esophageal invasion, were chosen for DNA analysis at their most proximal and deepest apexes. The esophageal and gastric apexes of the tumors had the same infiltrative pattern in 14 out of 15 cases and this tendency was not altered with further stratification of the anatomical layers. In comparing the deviations of the stem lines, the relative frequency of nuclei with DNA over 4n and 6n and aneuploid from the DNA histograms between the esophageal and the deepest apexes of the tumor revealed no statistically significant differences, depending on esophageal invasion. From these results, the specificities of EGJ toward tumor invasion or activity were not regarded as consequential, and the EGJ couldn't be regarded as a barrier against tumor invasion.
