[Clinical experience with alpha and beta interferon in childhood cancer].

Alpha and beta interferon were tested for antitumor activity and clinical toxicity in 15 children suffering from cancer. The drug was administered IV, IM, IT or intralesionally daily in the majority of cases in total doses of 18 X 10(6) to 9,634 X 10(5) IU. Major toxicities were a flulike syndrome, elevation of transaminase activity and leukopenia. A minor response (less than 50%) was observed in one patient with glioblastoma, treated by intrathecal administration, and an objective local response was noted in one rhabdomyosarcoma patient with multiple subcutaneous metastases, who was treated by intralesional administration. CNS leukemia in two patients improved without hematological response. Further trials are warranted.

Cloning and characterization of the umu operon responsible for inducible mutagenesis in Escherichia coli.

In Escherichia coli, radiation and chemically inducible mutagenesis requires a functional umuC gene product. The umuC mutants are defective in mutagenesis and slightly sensitive to DNA damaging agents. A chromosomal fragment that complemented the umuC mutations for UV mutability and UV resistance was cloned into miniF vector plasmid pMF3 by a shotgun method. A restriction map of the hybrid plasmid was constructed. Further subcloning, Tn1000 insertion inactivation, and complementation tests revealed that there are two genes, umuD and umuC in the former umuC region. The gene products of umuD and umuC were identified by the maxicell method to be proteins with Mr of 18 000 and 46 000, respectively. The two genes comprise an operon, and the transcriptional direction is from umuD to umuC. A plasmid carrying an umuC'-lac'Z gene fusion was constructed in vitro to study the regulation of the umu operon. It was shown that the umu operon is inducible by UV and chemical mutagens, and is regulated by the recA and lexA genes.

[Antitumor activity of human interferons: their direct cytotoxic activity and clinical effects].

It was found that both interferon-beta and interferon-alpha had a direct cytotoxic action against a few sensitive cultured cell lines. The action was not concentration dependent, but time dependent. Interferons expressed their maximum effect when they were given daily for long time to nude mice bearing transplantable ascitic human cancer cell lines, indicating that interferons had schedule dependency. In preliminary phase II study of interferons (3-9 X 10(6) U daily im, iv or local use) to 40 patients with various malignancies, only one patient with B-cell lymphoma responded to systemic use of both interferons and two out of five patients with pleural malignant effusion of breast cancer also responded to intrapleural use of interferons. Side effects were usually mild, but included fever, weakness, gastrointestinal, hematopoietic, liver and kidney function disturbances.

[The clinical effects of a new antiviral 9-(2-hydroxyethoxymethyl) guanine (aciclovir) against herpes virus infections].

The clinical effects of a new anti-viral 9-(2-hydroxymethoxymethyl) guanine (Aciclovir) against Herpes virus infections have been investigated. The patients had malignant tumours or auto-immune disease complicated by shingles and chicken pox due to Vaicella zoster virus (VZV) (43 cases), Herpes simplex virus (HSV) (10 cases) and 9 cases which were clinically diagnosed as Herpes, though the virus was not confirmed as the causative agent. As a general principle the dosage of Aciclovir was 5 mg/kg, t. i. d. for 5 days by slow intravenous infusion. The clinically effective rate against VZV was 93%, being excellent in 42% and against HSV it was 80%, being excellent in 40% and when the results of the cases of unknown origin were included it was excellent in 40% and the cumulative effective rate was 88%. Concerning the efficacy in reduction of pain, swelling, disappearance of vesicles and new scab formation, the effect was most noticeable after the third day of treatment. Treatment given early in the disease is likely to provide better results. Concerning side effects, one of 62 patients had proteinuria and the other had a drug rash and an abnormal liver function test. It is likely that the combination of treatment and the primary disease had some influence, but the cause/effect relationship to Aciclovir treatment is not clear.

[Autologous bone marrow transplantation in solid tumors in children].

Seven children with advanced solid tumors (2 rhabdomyosarcomas, 2 Ewing's sarcomas, 1 astrocytoma, 1 T cell lymphoma and 1 neuroblastoma) received high dose chemotherapy and/or radiotherapy followed by autologous bone marrow transplantation. Four patients achieved complete remissions, two had partial remissions, and one was no response. Side effect of autologous bone marrow transplantation was few compared with that of allogeneic bone marrow transplantation in which graft versus host reaction, profound posttransplantation immunodeficiency and interstitial pneumonitis were unavoidable. In this report methods of bone marrow cryopreservation and elimination of tumor cells from harvested bone marrow were also discussed.

A novel Japanese Burkitt's lymphoma cell line, P32/Ishida, with a new variant chromosomal translocation (2;14).

A novel cultured cell line, P32/Ishida, was established from a Japanese 8-year-old boy with abdominal Burkitt's lymphoma. The P32/Ishida cells were proved to have immature B-cell phenotypes on the basis of immunological surface marker analysis: surface immunoglobulins (gamma, mu, kappa), Ia-like antigen, B1 antigen, and common acute lymphoblastic leukemia antigen were positive. A small amount of IgM, but no IgG, could be detected in both cell extract and culture supernate of P32/Ishida cells. Cytogenetic studies revealed that the P32/Ishida cells consisted of cells having three derivative karyotypes characterized by common marker chromosomes, dup(1), and 14q + due to a new variant chromosomal translocation (2;14), which has not been identified in usual Burkitt's cell lines. Epstein-Barr virus nuclear antigen and terminal deoxynucleotidyl transferase activity were confirmed to be negative.

Comparative study of protected environment for children with malignant disease.

Thirty-three children with malignancy were investigated to evaluate the effect of reverse isolation with or without decontamination on prevention of infection. They were allocated to three groups: Group A, isolation in a laminar air flow room plus oral nonabsorbable antibiotics and sterile food; group B, isolation in a vertical air flow room without decontamination; group C, isolation in a conventional private room. Infectious episodes were the fewest and the rate of response to cancer chemotherapy was the highest in group A. Pyrexial days (greater than 38 degrees C) and days on systemic antibiotics were significantly fewer in groups A and B. Episodes of infection were also significantly fewer in group A but not in group B, as compared with group C. The usefulness and the necessity of a laminar air flow room as well as a vertical air flow room for management of childhood cancer were demonstrated.

[Clinical evaluation of antiemetics for vomiting due to cancer chemotherapy in children].

Comparative study of antiemetic effect on vomiting due to cancer chemotherapy was performed in 62 children with various malignant diseases. Twenty-one children were treated with metoclopramide, 23 children with domperidone and remaining 18 children received methylprednisolone. Each drug was administered intravenously after administration of anticancer agents, and repeated if necessary. The most effective antiemetics was methyl-prednisolone with effective rate of 89% in comparison with 51% of domperidone and 17% of metoclopromide treated group, respectively. Methyl-prednisolone may be useful for severe vomiting due to anticancer drugs. Safer and significantly better therapeutic efficacy was observed in the group treated with domperidone than that with metoclopromide.

A novel pre-T cell line derived from acute lymphoblastic leukemia.

A cultured cell line, P30/Ohkubo with pre-T cell phenotype, was established from an 11-year-old girl with acute lymphoblastic leukemia (ALL). According to the morphological, cytochemical and immunological analyses, P30/Ohkubo was found to have common-ALL antigen, Ia-like antigen, pan-T cell antigen, and L2 morphology with localized acid phosphatase staining, but not rosette-forming capacity with sheep erythrocytes, B1 antigen or surface immunoglobulins. Chromosome analysis revealed that P30/Ohkubo had two constant marker chromosomes [11q +, del (9)], but lacked 14q +. These findings indicate that P30/Ohkubo is a novel cultured cell line of pre-T cell phenotype. These characteristics have remained unchanged for more than 20 months since establishment of the cell line.

Management of malignant testicular tumors in children.

The result of therapy of 67 cases of children with malignant testicular tumors is presented. The majority of these cases were embryonal carcinoma in which metastases had occurred in the retroperitoneal lymph nodes, lungs, and bones. Long-term survival was obtained in 89% of the cases treated by simple orchiectomy followed by retroperitoneal irradiation. Two cases with metastases benefitted from chemotherapy. The prognosis of embryonal carcinoma of the testis is correlated with the age of the patient and the histologic pattern of the carcinoma; early detection and appropriate treatment are most important for its cure.

Leukocyte adenosine triphosphatase activity in human bronchial asthma.

Changes in adenosine triphosphatase (ATPase) activity of the peripheral blood leukocytes were investigated in patients with bronchial asthma. Estimation of the leukocyte Mg++- and Ca++- dependent ATPases was carried out according to Hadden's method, incubating ATP with the membrane fraction of the leukocyte. The leukocyte ATPase activity was significantly elevated among asthmatic patients compared with control subjects. This elevated ATPase was seen in all asthmatics irrespective of acute attacks or the drug treatment. There was no clear correlation between the activity of ATPase and the percentage of leukocytes, neutrophils and eosinophils. There was no relationship between ATPase activity and adenyl cyclase activity of the same leukocytes from asthmatic patients.

Leukocyte cyclic-3', 5'-nucleotide phosphodiesterase activity in human bronchial asthma.

Changes in cyclic-3', 5'-nucleotide phosphodiesterase activity of the peripheral blood leukocytes were investigated in patients with bronchial asthma. Leukocyte phosphodiesterase activity was significantly elevated during asthmatic attacks. Elevated activity was seen in most active asthmatics irrespective of the drug treatment. The ratio of the adenyl cyclase activity to the phosphodiesterase activity of the same leukocyte decreased to less than 1.0 during asthmatic attacks.
