Intracervical administration of prostaglandin E2 prior to vacuum aspiration. A prospective double-blind randomized study.

Two-hundred and ninety-three patients without a previous vaginal delivery were randomized to intracervical/extra-amniotic application of 0.5 mg prostaglandin E2 (PGE2) or to gel only. Of the patients who received PGE2, 18.7% were admitted before the next morning due to spontaneous abortion, bleeding or pains. No other side-effect was observed. A statistically significant dilatation of the cervical canal was found in the prostaglandin group. Thirty percent of the treated patients did not need further dilatation of the cervix 25.4% were non-responders to PGE2 and 7.7% were hyper-responders. The number of uterine perforations, pelvic inflammatory disease (PID) or retained pregnancy products were not influenced by the pretreatment with PGE2.

Definition and treatment of microcarcinoma of the cervix uteri.

Patients with tumors without vessel invasion have a better prognosis than patients with tumors of the same size with vessel invasion. The depth of stromal infiltration should be measured from the epithelial surface. In patients with an infiltration less than 1 mm, conization will be sufficient treatment, but hysterectomy is necessary when the disease is more advanced. When the tumor has infiltrated into vessels, additional treatment including pelvic lymphadenectomy or external irradiation should be performed.

Carcinoma↗

Carcinoma of the cervix stage IV treatment problems.

Sixty-seven patients with cervical cancer stage IV were treated in 1967-72. Only 21 received a complete course of radiotherapy and none received surgery or chemotherapy. Two patients had mobile tumors with extension to the bladder. Both died from distant metastases without local recurrences. Two patients with metastases to the lymph nodes in the groin, died within 3 and 4 years, respectively. All the other patients with distant metastases survived less than 2 years.

Adenocarcinoma↗

Primary radical surgery in carcinoma of the cervix uteri.

Fifty patients with cervical carcinoma were treated by primary radical hysterectomy and transperitoneal pelvic lymphadenectomy by one surgeon and followed 10 to 20 years. Of 15 patients with pelvic lymph node metastases 10 received postoperative irradiation of whom 7 developed recurrence and died from cancer. Of 5 patients with metastases who did not receive radiotherapy, 3 developed recurrence: 1 with distant metastases died. The other 2 with localized recurrences were treated with radiotherapy and survived more than 9 years. The 5- and 10-year observed survival rates for stage I b patients were 85% and 76% respectively.

Outpatient cervical conization with the CO2 laser.

Ninety-six patients with grade 3 cervical intraepithelial neoplasia (CIN) were treated with CO2 laser conization, and 45 were treated with cold-knife conization. Local anesthesia was used in 83% of the patients in the laser group; the rest received general anesthesia. In the laser group seven patients developed peri- and postoperative hemorrhages. Hemorrhages occurred among four of those treated with cold-knife conization. Laser conization with local anesthesia was well tolerated and can be recommended as an outpatient procedure for patients with endocervical grade 3 CIN or lesions suggestive of microinvasive cancer.

Synthesis of the trisaccharide moiety of gangliotriosylceramide (asialo GM2).

The synthesis of the trisaccharide methyl glycoside beta-D-GalNAc-(1----4)-beta-D-Gal-(1----4)-beta-D-Glc-OMe, which corresponds to the carbohydrate portion of gangliotriosylceramide (asialo GM2), was accomplished by the reaction of 4-O-acetyl-3,6-di-O-benzoyl-2-deoxy-2-phthalimido-D-galactopyranosyl bromide (18) with a benzylated derivative of methyl 4-O-beta-D-galactopyranosyl-beta-D-glucopyranoside. Comparative studies with a 6'-benzyl ether and a 6'-benzoate revealed that the substituent at O-6' is crucial to the outcome of glycosylations at O-4', the ether derivative being much the more reactive. tert-Butyl 4-O-acetyl-3,6-di-O-benzoyl-2-deoxy-2-phthalimido-D-galactopyranoside, which was readily converted into the corresponding bromide 18, was obtained from the gluco derivative via a single-step, crown ether-assisted epimerization.

The conformation of Salmonella O-antigenic polysaccharide chains of serogroups A, B, and D1 predicted by semi-empirical, Hard-Sphere (HSEA) calculations.

The preferred conformation of the tetrasaccharide repeating units of Salmonella Serogroups A, B, and D1 have been calculated. The semiempirical calculations used the Hard-Sphere, Exo-Anomeric (HSEA) approach to derive a conformational model, which could be used to assist the interpretation of conformations significantly populated in aqueous solution. The calculated model was extended to include a pentasaccharide repeating unit bearing an alpha-D-glucopyranose-branch point. The 3,6-dideoxyhexose to D-mannose linkage was shown to possess a steep energy surface with a minimum, which results in good exposure of the dideoxyhexose O-2 and O-4 atoms. Stereochemical changes involving the equatorial or axial disposition of these atoms are the distinguishing structural features of the A, B, and D1 serogroups. A lipophilic surface involving the 6-deoxy groups of the dideoxy-D-hexose and L-rhamnose residues was identified, and the possible implications of these features in antigenic determinants is discussed. The preferred conformation predicted by the HSEA method correlates with the known antigenic specificities of polysaccharides belonging to these Salmonella serogroups.

The conformation of Salmonella O-antigenic oligosaccharides of serogroups A, B, and D1 inferred from 1H- and 13C-nuclear magnetic resonance spectroscopy.

The conformational model derived by the HSEA calculation method was used to interpret the n.m.r. data for solutions of oligosaccharides corresponding to the Salmonella serogroups A, B, and D1 antigenic determinants. The favored conformer, derived by calculation, accounted for the observed, chemical-shift changes and accurately predicted the existence and magnitude of inter-ring proton n.O.e.'s. Extensive proton-density and compression of proton, Van der Waals radii were correlated with deshielding of specific proton-resonances. The model of lipopolysaccharide conformation accounts for the known antigenic properties of Salmonella O-antigens.

Lymph drainage from the vulva.

Fifty-four patients with cervical cancer stage Ib were injected preoperatively with 99mTc-colloid in different areas of the vulva and three controls in the axilla. The radioactivity in the groins and the pelvis was measured with a scintillation camera and in the removed pelvic lymph nodes with a well counter. A low activity could be measured in the removed pelvic nodes of the controls indicating the radioactivity which had reached the pelvic nodes via the bloodstream. Clitoris and perineum had a bilateral lymph flow. In all the other patients the vast majority of the radioactivity was found in the ipsilateral lymph nodes. A significant, but small amount of activity was recorded in the contralateral nodes in 67% of the patients and in all patients injected in the anterior part of the labium minus. The medial external iliac lymph chain had an uptake of 73% of the total activity measured in the removed pelvic nodes. Only in 13% of the patients was the activity higher in the lateral than in the medial external iliac chain. Direct lymphatic pathway from the clitoris to the pelvic lymph nodes was not found. Anastomoses exist probably in 13 out of 42 patients between the presacral and/or common iliac nodes on both sides.

Stage IB carcinoma of the cervix, the Norwegian Radium Hospital: results and complications. III. Urinary and gastrointestinal complications.

The combination of surgery and radiotherapy for early cases of cancer of the cervix has been accused of producing prohibitive complication rates. In a series of 612 patients with cancer of the cervix, Stage IB, the frequency of major complications from the urinary and gastrointestinal tract has been studied, and an attempt has been made in determining the most probable etiology of these complications. No significant increase in complications can be attributed to the use of preoperative intracavitary irradiation. In patients with pelvic lymph node metastases the combination of radical surgery and a full course of radiotherapy is associated with a high complication risk, as 12% of these patients developed major complications. Their 5-year survival, however, was high: 54%.

Antigenic determinants of Salmonella serogroups A and D1. Synthesis of trisaccharide glycosides for use as artificial antigens.

The disaccharide glycoside 8-methoxycarbonyloctyl 4,6-O-cyclohexylidine-2-O-(tetra-O-benzyl-alpha-D-galactopyranosyl)-alpha-D-manno pyranoside (7) was used as a common intermediate to the trisaccharide determinants of both Salmonella serogroups A and D1. Acetalation of 8-methoxycarbonyloctyl alpha-D-mannopyranoside provided the 4,6-acetal derivative, which was selectively benzoylated to give the partially protected mannoside 4. Reaction of 4 with tetra-O-benzyl-alpha-D-galacto-pyranosyl chloride afforded the fully protected disaccharide, which, after transesterification, gave the selectively blocked, disaccharide glycoside (7). Addition of tyvelose by way of its 2,4-di-O-benzoyl-3,6-dideoxy-alpha-D-arabino-hexopyranosyl chloride derivative gave the blocked trisacchride determinant of Salmonella serogroup D1. 2,4,-Di-O-benzyl-3,6-dideoxy-alpha-D-ribo-hexopyranosyl chloride reacted with 7 to provide, after removal of blocking groups, the paratose-containing determinant of serogroup A.
