Severe hindlimb ischemia causes periosteal proliferation in the rat tibia.

In this rat study, we found tibial periosteal hyperplasia and hypertrophy, and appositional new bone formation 3 days after transient hindlimb ischemia. This response was positively correlated to the extent of muscle necrosis, which was increased either by raising the environmental temperature during ischemia or by prolonging the period of ischemia. By changing the temperature from 21 degrees C to 34 degrees C, the area periost in percent of the total tibial area was increased from 5 to 17, and by changing the duration of ischemia from 3 to 5 hours, it increased from 8 to 18.

[A case of water intoxication presenting as urinary retention in a psychotic patient].

A 43-year-old man, who had been treated for chronic schizophrenia, was admitted to our hospital with the chief complaint of urinary retention. His serum levels of sodium and osmotic value were low and serum creatine phosphokinase (CPK) was elevated to 2,624 mg/dl. He had been consuming about 9 liters of water a day for ten years. A diagnosis of water intoxication was made and treated successfully with water restriction and drip infusion. To our knowledge, this is the 23rd case of water intoxication with serum creatine phosphokinase elevation reported in the Japanese literature and this is the first case with the chief complaint of urinary retention.

Recombinant bovine conglutinin, lacking the N-terminal and collagenous domains, has less conglutination activity but is able to inhibit haemagglutination by influenza A virus.

Conglutinin is a bovine serum protein which was first described as a vertebrate lectin. This protein belongs to the family of C-type lectins. These lectins are composed of four characteristic domains: (1) an N-terminal cysteine-rich domain, (2) a collagen-like domain, (3) a neck domain and (4) a carbohydrate recognition domain (CRD). Recently lectins have been shown to function as immunoglobulin-independent defence molecules due to a complement-mediated mechanism or opsonization. Our previous study showed that bovine conglutinin can inhibit haemagglutination by influenza A viruses and act by directly neutralizing them due to its lectin properties. In order to elucidate the biological role of the collagen-like domain, a recombinant partial conglutinin lacking this collagen-like domain was produced in an Escherichia coli system and its biological activities were examined. A 497 bp sequence, consisting of a short collagen region (two repeats of G-X-Y amino acid sequences), the neck domain, and the CRD of conglutinin cDNA, was amplified by the reverse-transcriptase PCR technique. The cDNA was transferred to a bacterial expression vector system (pRSET-A) and stable transfectants with a high level of conglutinin production were obtained. SDS/PAGE and Western blotting analyses showed a recombinant fusion protein of 27 kDa. Results of a cross-linking study and gel-filtration assay indicated that the recombinant conglutinin can form a trimeric structure and that it has sugar binding activity and specificity similar to that of native conglutinin. The recombinant conglutinin was also found to inhibit haemagglutination caused by influenza A virus as well as to possess less conglutination activity. These results suggest that in order for conglutinin to inhibit haemagglutination caused by the influenza virus, as well as to have sugar binding activity or to form trimers, it does not require the N-terminal and collagenous domains; however, they are essential for full conglutination activity.

Clinical characteristics of acyclovir-resistant herpetic keratitis and experimental studies of isolates.

BACKGROUND: We treated two patients with dendritic keratitis that did not respond to acyclovir (ACV) ointment therapy. Their systemic immune status was normal: however, one patient had a long history of atopic disease and the other had previously undergone topical corticosteroid treatment. HSV-1 was isolated from the patients and inoculated into animals to investigate its viral pathogenicity and latent infection. METHODS: HSV-1 isolates from the patients were tested for drug sensitivity to acyclovir, ganciclovir, idoxuridine, trifluridine, foscarnet and interferon-beta in vitro. In in vivo studies, bilateral corneas of two New Zealand white rabbits and 10 BALB/c mice in each of four groups were infected by the respective viral isolates. The extent of corneal epithelial and/or stromal lesions produced by the viruses was evaluated. The trigeminal ganglial tissues of the mice were examined for viral latent infection by co-culture with Vero cells. RESULTS: Herpetic keratitis in both patients was characterized by prolonged clinical course, succeeded by various types of corneal lesions and ocular complications. In in vitro studies, the two HSV-1 isolates demonstrated cross-resistance to ACV, ganciclovir and/or idoxuridine. Both strains demonstrated weakly virulent corneal epithelial and/or stromal lesions in rabbits and mice. One isolate displayed delayed advent but prolonged course of epithelial lesions in rabbits. The latent infection incidences of the isolates in mice trigeminal ganglia were 6.25% (1/16) and 0% (0/18) respectively. CONCLUSION: Topical immune depression may induce ACV-resistant HSV-1 infection in the cornea, with a prolonged course in association with ocular complications. The prolonged infectious course of the viral isolates in the animal study partially supported the clinical demonstrations in the patient. The existence of latent infection by one ACV-resistant HSV-1 in its animals may indicate the possibility of its recurrence. Trifluridine may be an alternative choice for treating corneal epithelial lesions caused by ACV-resistant HSV-1.

[Prostatic carcinoma presenting as an abdominal mass: a case report].

Prostatic carcinoma presenting as an abdominal mass is a very rare disorder. A 64-year-old man was referred to our hospital with the chief complaint of a right lower abdominal mass on March 6, 1984. Physical examination revealed a firm, smooth, fixed, non-tender, pulseless fist size mass in the right lower abdomen. Rectal examination revealed an apple size, smooth and elastic firm prostate which had an induration. Needle biopsy of the prostate showed well-differentiated adenocarcinoma. At first we regarded the abdominal mass as retroperitoneal tumor unassociated with prostatic carcinoma. After preoperative irradiation (20 Gy) to the pelvis, the abdominal mass was resected and bilateral orchiectomy was performed on April 11, 1984. The mass was histologically diagnosed as a metastatic lesion from prostatic carcinoma. On the basis of these findings, we considered the mass to be due to pelvic lymph node metastasis from prostatic carcinoma. His postoperative course was uneventful. In April 1991, he is still alive without evidence of recurrence or bone metastasis.

[A case of ureteral endometriosis; the usefulness of MRI for preoperative diagnosis].

A rare case of ureteral endometriosis is reported. A 46-year-old nulliparous woman was referred to our hospital because of right hydronephrosis which was detected by ultrasonography; Drip infusion pyelography showed right hydroureteronephrosis with the distal obstruction. MRI demonstrated right ovarian endometriosis and resultant ureteral stricture. Based on these findings, we made a preoperative diagnosis of right ureteral endometriosis. Right partial ureterectomy and ureteroureterostomy were performed. Pathological examination of the resected segment showed endometrial tissue around the ureteral lesion. She was administered LH-RH agonist (buserelin); her postoperative course was uneventful. IP showed no abnormalities 6 weeks after the operation. To our knowledge, this case is only the 55th case reported in the literature in Japan.

Combined cisplatin and radiation therapy for advanced bladder cancer.

Since September 1985, 44 patients with advanced urinary bladder cancer have been treated by combined cisplatin and full-dose radiotherapy. The patients were 32 males and 12 females, and their ages ranged from 33 to 83 years, with a median of 67.4 years. Radiotherapy consisting of a tumor dose of 50-60 Gy was administered with cobalt-60. Cisplatin was infused 5 days at a daily dose of 20 mg on the 1st and 4th weeks of treatment. Of the 39 evaluable patients 27 (69.2%) achieved a complete response. Toxicity was also evaluated for those 44 patients. Mainly gastrointestinal toxicity was noted: loss of appetite in 28 (64%), nausea and/or vomiting in 21 (48%), and diarrhea in 8 (18%). Leukocytopenia was noted in 16 (33%) and mild thrombocytopenia in 5 (11%). Mild dermatitis was noted in 8 (18%).

[An attempt to classify vascular impotence using penile erection curve patterns].

Papaverine hydrochloride was administered to the cavernous body of the penis in a total of 262 patients who were diagnosed as impotent at our Reproduction Center, Toho University, during the 5 years' period from 1984 to 1988. The condition of erection was accurately recorded by the use of both a mercury strain gauge and rubber strain gauge to classify the erection curve, and papaverine hydrochloride outflow into the peripheral blood was concurrently measured in some of the cases in order to assess its relation with the erection curve pattern. As for the concentration of papaverine hydrochloride in the peripheral blood, it was found that the better the state of erection, the smaller the outflow of peripheral blood. An assessment of the relationship of the concentration of papaverine hydrochloride with the classification of erection pattern showed that the amount of outflow was smaller in the N type (normal) pattern and that the amount of outflow was increasingly larger in the following order: A type (arterial), N' type (arterial and venous) and V type (venous). The distribution of the erection curve patterns, in the 262 cases was: N type, 45%; A type, 10%; N' type, 22.5%; and V type, 22.5%. From the above findings, it is presumed that classification of erection curve pattern under papaverine hydrochloride loading is useful for the differentiation of vascular impotence, particularly for the diagnosis of disorders in the outflow system.

[A case of prostatic hyperplasia in a young male].

Prostatic hyperplasia in young males is a very rare disease. A 28-year-old man was admitted to our hospital on June 24, 1989, with the chief complaints of intermittent macrohematuria and dysuria. Endoscopic examination revealed two papillary tumors in the prostatic urethra; no urinary bladder tumor was found. A transurethral resection was done for the urethral tumors. Pathological examination revealed prostatic hyperplasia. These lesions were considered ectopic prostatic tissue at first. However, imaging showed that this may in fact be prostatic hyperplasia of the apical region near the capsule. The post-operative course was uneventful and he is presently observed as an outpatient. This patient represents the seventh case reported in the literature.

Survival and disinfection of adenovirus type 19 and enterovirus 70 in ophthalmic practice.

In an experimental model designed to simulate actual clinical situations, the risk period of adenovirus type 19 (AdV-19) and enterovirus 70 (EV-70) transmission by contaminated instruments has been investigated, as well as the effective methods of disinfection against these viruses. No EV-70 could be detected 6 hours after inoculation on applanation tips contaminated with viruses and naturally dried, indicating that drying the instruments and keeping them in a moisture-free environment is a convenient disinfection procedure against EV-70. In contrast, AdV-19 maintained its initial infectious level for 11 days on contaminated and subsequently dried tips. Both viruses when placed in eyedrop solutions at room temperature sustained their infectivity for more than 20 days. It was found that heating at 90 degrees C for 5 seconds is the most simple and effective disinfection against AdV-19 and EV-70. AdV-19 was about 60 times more resistant to ultraviolet irradiation than EV-70.

[Treatment using CDDP and radiation therapy in treating bladder tumor].

Combination therapy consisting of cis-diamine-dichloroplatinum (CDDP) and radiotherapy was used to treat bladder cancer. The subjects were 20 patients with bladder cancer we experienced since September, 1986. They were between 33 and 83 years old and consisted of 13 males and 7 females. Among the histotypes and grades, there were 3 cases of G2, 7 of G2-G3 and 10 of G3 metastatic epithelial carcinoma. The classification of primary tumors was T2 in six cases, T3a in 12 cases and T4 in two cases. In 12 cases where the bladder was preserved, this therapy was performed after TUR biopsy or TUR, and the effects were evaluated after observation periods of 5-17 months. Side effects caused by CDDP were anorexia in 15 cases, nausea and vomiting in 10 cases, leukopenia in 10 cases and thrombocytopenia in four cases. Those caused by the radiotherapy were diarrhea in seven cases and skin disorders in six cases. All of these side effects were mild and the patients recovered spontaneously.

[Clinical results of endourologic technique for upper urinary calculi].

Between May, 1985 and September, 1987, percutaneous nephrolithotripsy (PNL) and transurethral ureterolithotripsy (TUL) have been performed at our hospital and five affiliated hospitals. We report on the therapeutic results of 216 cases treated during this period. PNL and TUL were performed 168 times on 120 patients (119 males and 41 females) and 57 times on 56 patients (35 males and 21 females), respectively. Rate of success with PNL was 80.7% (103/130) for renal calculus, 85.7% (18/21) for upper ureteral calculus, and 47.1% (8/17) for middle-lower ureteral calculus. Rate of success with TUL was 57.9% (33/57). Of these, cases of ureteral lower edge calculus showed the lowest rate of success, being 33.3% (2/6). Complications of the cases undergoing PNL and TUL were: ureteral damage in 13; hemorrhage in 6; renal pelvic damage in 4; ureteral lower edge stenosis in 3; pyrexia in 3; cardiac insufficiency in 2 and retention of perfusate in posterior peritoneal cavity, pneumonia and renal insufficiency in 1 case each.

Multiple classes of binding sites for palmitic acid on the fatty acid-binding protein molecule.

Binding characteristics of fatty acid-binding protein (FABP) toward palmitic acid were studied. On the analysis of the interaction between FABP and [3H]palmitic acid over a wide range of concentrations of the fatty acid, at least three saturation plateaux were observed. By Scatchard-plot analysis, it appeared that FABP possesses three classes of binding sites for palmitic acid with different affinities [Kd1 = 1 x 10(-6) M (N = 1), Kd2 = 4 x 10(-6) M (N = 2), Kd3 = 2 x 10(-5) M (N = 10)]. Results of both sedimentation analyses and chromatofocusing of FABPs labeled with various concentrations of [3H]palmitic acid suggested that the FABP used was homogeneous. These results indicate that several classes of binding sites for palmitic acid with different affinities are present on the FABP molecule.

[Cystitis in 8 patients treated with tranilast].

We encountered 8 cases of cystitis probably caused by Tranilast. Bladder biopsy performed on 6 of the 8 cases revealed eosinophilic cystitis in 3 cases. In the lymphocyte stimulation test using Tranilast as an antigen, a positive and false positive reaction was seen in one case each. This disease seemed to occur as a result of allergy of the bladder specific to Tranilast.
