[A case of adrenal myelolipoma].

A case of adrenal myelolipoma is reported. The patient was a 63-year-old female on whom liver dysfunction was diagnosed and a tumor was detected accidentally in the right adrenal region by ultrasound abdominal test. This patient was the 26th patient from whom this tumor was extirpated in Japan. Reports on similar cases are on the increase owing to the advances in diagnostic imaging. A half of such cases were asymptomatic, and 85% of them were right unilateral cases.

A novel role of fatty acid-binding protein as a vehicle of retinoids.

Intracellular transport and storage of retinoids were shown to be conducted by fatty acid-binding protein (FABP). When rat liver cytosol was gel filtrated, retinyl palmitate-binding activity was mainly eluted in the fraction with a Mr. of around 14,000, in which both FABP and cellular retinol-binding protein (CRBP) co-existed. From the binding analysis of purified FABP and CRBP to retinyl palmitate, FABP was found to have a relatively high affinity (Kd = 1.4 X 10(-6) M) to retinyl palmitate, while binding of retinyl palmitate to CRBP was scarcely detectable. By using anti-FABP serum, it was shown that FABP was distributed in organs relating to absorption and storage of retinoids, such as jejunum, ileum, and liver. In liver, the protein was localized in the parenchymal cells and with particularly high concentration in the perisinusoidal cells, probably fat-storing cells.

[Topical use of thrombin in prostatic surgery].

We describe the usefulness of topical application of thrombin as a hemostatic aid in prostatic surgery. We used a specially designed 3-way bag catheter, one way of which opened to the prostatic fossa. After enucleation of the prostate suprapubically, we inserted the bag catheter into the bladder before any hemostatic procedure was performed on the prostatic fossa, inflated the bag, gently pulled down the catheter against the prostatic fossa, and then injected the thrombin solution (5,000 units/5 cc. X 2) into the fossa. Fifty patients were randomized into two groups; the "thrombin" method group and the hemostatic "ligature" group, and compared. The "thombin" method group showed statistically significant superiority to the "ligature" method group in reduced operation time and operative bloodloss. On the contrary, the duration of postoperative hematuria was longer with the "thrombin" method but not significantly. Even the prolonged duration of hematuria, however, produced no clinical problems. Hypofibrinogenemia and poor conditions of drug storage lowered its efficacy. These points should be remembered when using thrombin.

Formation of thiosulfate [35S] by neutron irradiation of potassium chloride and preparation of elementary sulfur [35S].

Potassium chloride was subjected to various kinds of pretreatments and irradiated in a nuclear reactor. Irradiated potassium chloride was dissolved in deaerated aqueous solution of several sulfur-salts. A portion of the solution was chromatographed on a thin layer chromatographic plate and the distribution of 35S-chemical species was determined. Irradiation of potassium chloride degassed at 3x10(-4)Torr resulted in the formation of about 60% of 35S in the form of thiosulfate and remainder was distributed among sulfide, sulfate and elementary sulfur. Thiosulfate[35S] was converted to elementary sulfur[35S] first by reducing to hydrogen sulfide [35S] with nascent hydrogen and then by oxidizing the latter to elementary sulfur[35S] with hexacyanoferrate(III). Elementary sulfur[35S] was finally extracted into benzene. Radiochemically pure elementary sulfur[35S] could be produced by simple procedures with a yield of about 60%. Thiosulfate[35S] was decomposed with acid to elementary sulfur[35S] and sulfur[35S] dioxide. The former was separated by centrifuging and dispersed in boiling water to form colloidal suspension of elementary sulfur[35S]. Approximately 40% of 35S was recovered as colloid.

Detection of HSV mRNA using reverse transcription-polymerase chain reaction for diagnosis in murine herpetic keratitis model.

Reverse transcription polymerase chain reaction (RT-PCR) was applied in the detection of herpes simplex virus-1 (HSV-1) mRNA from tear film and corneal epithelium in a murine herpetic keratitis model. The diagnostic value of this new technique for acute herpetic keratitis was evaluated in comparison with direct PCR for genomic DNA and viral culture. On day 2 postinfection (PI) of HSV, all mice showed dendritic keratitis, and PCR, RT-PCR, and viral culture were positive in all samples. On day 8 PI, no dendritic keratitis was observed in any mouse, PCR was positive in all samples, while RT-PCR was positive in only 5 of 12 samples and viral culture in only 2 of 12. The sensitivity of RT-PCR was lower than that of PCR, and approximately the same as viral culture; however, the findings of RT-PCR more closely concurred with clinical observations than the findings of PCR. These results show the potential of RT-PCR for rapid, specific diagnosis of acute herpetic keratitis.
