Significance of renin-angiotensin system during and after surface-induced simple hypothermia in open-heart surgery.

The present studies were performed to investigate the metabolic role of the lungs in the renin-angiotensin system under hypothermia by measuring plasma renin activity, plasma angiotensin I (A I), plasma angiotensin II (A II), plasma aldosterone and plasma angiotensin converting enzyme activity on 14 patients who underwent open-heart surgery with surface-induced simple hypothermia. In addition, dog experiments were performed, in which changes of renal blood flow and angiotensin metabolism in lungs and kidneys under hypothermia were studied in vivo. The following results were obtained: 1) During and after open-heart surgery with surface-induced simple hypothermia, the homeostasis in the renin-angiotensin system is still maintained. 2) An increase of A II may play an important role in maintaining blood pressure under hypothermia. 3) Under hypothermia, the conversion of A I to A II in the kidneys may contribute to an increase of plasma A II.

Angiotensin II-induced myocardial damage with a special reference to low cardiac output syndrome.

The effects of large doses of angiotensin II on the rabbit kidney and heart and or the ability of perfused canine kidney to inactivate angiotensin II were examined to clarify the role of angiotensin II in the low cardiac output syndrome after open-heart surgery. Intravenous infusion of angiotensin II at a rate of 1.1 to 1.4 microgram/Kg/min for 48 hours caused multifocal myocardial necrosis and renal mononuclear cell infiltration and necrosis. Isolated canine kidney preparations inactivated 76% of angiotensin I and 79% of angiotensin II. The results indicates that the kidney can inactivate angiotensin and that high doses of angiotensin II can produce myocardial and renal lesions. It is suggested that an increased concentration of angiotensin II may result in the low cardiac output syndrome through myocardial damage, and that decreased inactivation of angiotensin II by the kidney accelerates the myocardial damage.

Synthetic cephalosporins. The synthesis and antibacterial activities of 7-[2-(2-(amino-1,3,4-thiadiazol-5-yl)acetamido]-cephalosporins.

Synthesis and antibacterial activities of 7-[2-(2-amino-1,3,4-thiadiazol-5-yl)acetamido]-cephalosporins and their derivatives having the methoxyimino group at the 2-position of the 7-acyl moiety are described. These compounds are of interest as structural analogues of potent antibiotics, 7-[2-(2-aminothiazol-4-yl)acetamido]cephalosporins. 7-[2-(2-Amino-1,3,4-thiadiazol-5-yl)acetamido]cephalosporins showed comparable activity with cefazolin. Introduction of methoxyimino group to the 7-side chain resulted in a lowering of activity.

[Chronic encapsulating fibrous peritonitis induced by carcinoma of the pancreas].

A 70-year-old man was admitted complaining of dull pain in the epigastrium and umbilical region. ERCP revealed a carcinoma of the pancreas; cytological examination of the pancreatic juice and ascites showed no tumor cells. Diagnostic laparotomy was performed for lasting abdominal pain and fever. Exploration revealed ascites and adhesions. The small and large intestine were covered with a gray-white membranous substance. A large tumor invading the retroperitoneal space was seen in the head of the pancreas. Histological examination of the membranous substance showed hyalinizing fibrinoid tissue; the tumor contained carcinoma cells of the pancreas. We posit that the tumor disturbed the flow of the portal vein and lymphatic vessels, leading to the patient's clinical symptoms.

Antiallergic properties of an orally effective agent, [[3-(1H-tetrazol-5-yl)-phenyl] amino] oxoacetic acid n-butyl ester.

A newly synthesized compound, [[3-(1H-tetrazol-5-yl)-phenyl] amino]oxoacetic acid n-butyl ester (MTB) has been demonstrated to be an orally active antiallergic agent. This compound inhibited the 48-hr passive cutaneous anaphylaxis (48-hr PCA) induced by IgE in rats. In guinea pigs, MTB also inhibited the 8-day passive cutaneous anaphylaxis (8-day PCA) and the 8-day passive systemic anaphylaxis induced by IgE. The compound partially inhibited the IgG-mediated 3-hr PCA in rats and guinea pigs, but failed to have any effect on the rabbit IgG-mediated 3-hr PCA in these animals. In the rat, MTB was not an antagonist of histamine or serotonin. The antiallergic effect of MTB was not mediated via any adrenergic mechanisms. MTB significantly inhibited histamine release from rat peritoneal cells induced by rat IgE in vitro.

Hypoglossia: case report and discussion.

A case of hypoglossia was observed between the ages of 3 and 15 years in a patient with micrognathia, congenital absence of three lower incisors, and a telescopic occlusion accompanied by an extremely narrow lower arch and severe overbite. Orthodontic intervention was begun when the patient was 8 years old. Craniofacial growth changes that occurred before and after orthodontic treatment are discussed. Cephalometric study of this case suggests the distinctive features of the postnatal growth pattern similar to other cases with the syndrome.
