[Epidemiological notes on nasal bleeding].

The incidence of epistaxis is difficult to calculate because only a 10% of them look for medical attention. In this study the nasal bleeding means the 13.3% of the ENT emergencies (279 patients). Its incidence was 6 cases/10,000/year. The majority were over 50 years old (57.3%). There was season variations. The anterior epistaxis were more frequent but he posterior ones increased over the 40. An 18% had rebleeding more significatively with age and localization. We discuss the mechanisms that can explain the male dominance, the season incidence, and the old ages of the patients with hospital epistaxis.

[Pleomorphic adenoma of the tongue base. A new case of big size].

The pleomorphe adenomas (PA) of the tongue base are rare (there are only 6 cases published until now) We present one case of a male 42 years old with a PA of the tongue base very big but with few symptoms. The FNAB and MRI were the key for the diagnosis and therapy. The treatment was surgical with a trans oral resection. The patient is free of tumor after seven years.

[An immunohistochemistry study of p53 protein in squamous cell carcinoma of the larynx].

P53 protein detects damages in DNA, stopping the cellular cycle to repair it and stimulates "apoptosis" or programmed death cell when it cannot do it. We present an immunohistochemistry study of 58 squamous cell carcinomas of larynx operated between 1991 and 1996 and in which we detected the overexpression of P53 in 31 cases (53.44%). We did not find a statistic relationship between survival and the detection of P53 protein using Log Rank statistic analysis; the survival media for the wild-type protein cases was 64 months (very similar to native cases).

[Epidemiological aspects of otosclerosis (1). Its frequency in comparison with other ear pathologies, incidence and prevalence].

A planned study was made with the group of patients seen for first time at Otologic External Consulting Department of ENT Service of the universitary Hospital Complex of Santiago de Compostela. The total amount 1,050 patients, 83 of them suffering otosclerosis. Most frequently diagnosed was sensorineural deafness (29.1%), in second place were chronic middle otitis (27.8%) and as last one was otosclerosis (8.7%). Clinic otosclerosis amounted for 19.20 cases of each 100,000 inhabitants yearly and stapedectomies performed 7.78. Prevalence was evaluated at 7.13 by 1,000 (10.86 women and 3.14 men). Higher value compared with other series published previously are linked to differences on sanitary development as well as health concept and the type of community where people lived through.

[Epidemiological aspects of otosclerosis (II): sex, hereditary factors, its course before diagnosis, age at diagnosis and its beginning].

Retrospective study done with a group of 401 patients enduring otosclerosis. Seventy percent were women. 59% with positive familial antecedents. Illness was recognized as midpoint, at 44.5 years and symptomatology started at 35.3 years. Lasting time of condition till diagnosis assured was 8.5 years (higher than 5 years the half of cases and between 1 an 5 years the third part of them), these numbers considerably decreasing regarding other older series. Figures lower in those cases without familiar otosclerotic antecedents, the same than in unilateral specimen. The only significative influence of sex where tinnitus aurium among women (70.7% to face up 51.7%).

[Surgical treatment of laryngotracheal stenoses. Our experience].

Laryngo-tracheal stenoses (LTS) treatment is a problem of difficult management for the otolaryngologist. There are many and varied techniques in order to solve the conundrum. The aim of this article is to analyze retrospectively the outcomes of surgical treatment of LTS in our Institution. We have checked 11 cases (9 men and 2 women, arithmetic mean 30.7 years) operated through surgical procedures during a 6 years lapse of time. All patients were endotracheally intubated prior the stenosis appearance. Stenosis location most encountered were subglottic-tracheal combination. To the end of the study 44 percent of the series was successfully cured and the 33 percent only partially recovered. The employed techniques (endoscopic or open surgery) are revised at the light of today's literature.

[Computerized analysis of craniography: new limits of the norm for static and dynamic forms].

Literature about normal parameters of Craniocorpography (CCG) is scant and poorly documented. Its results are variable according to the series. Our aim is to reach a criteria of normality for CCG in our reference population. We have prospectively made Romberg and Unterberger tests to 72 healthy volunteers (mean age 34.7 years old, SD 17.07, 55.6% men) using a computerized method of measurement and making its estimation by means of a statistical model widely accepted. All the studied variables observe the conditions of Gaussian distribution. The normal limits for lateral and antero-posterior oscillation were 9 centimeters, 12 centimeters for lateral sway and 59 degrees to the right and 46 degrees to the left for angular deviation. The computerized method of analysis has facilitated the detection of the limits of the light tracings, which together with the careful design of our study guarantees the validity of the results.

[A study of cervical metastases from unknown primary tumor].

Cervical metastases of cancer of an unknown primary tumour (CMUPT) are infrequent but they represent botha diagnostic and therapeutic challenge for ENT physicians. We present a retrospective study over 22 patients with CMUPT diagnosed in our hospital during 13 years (3.4% of the patients with head and neck cancer). The sensitivity of the fine-needle aspiration technique was 94.7%. 54% were N2a, 36% N3 and 9% N2b. 73% were squamous cell carcinoma and 27% were undifferentiated carcinoma. We established our protocol on diagnosis and management: panendoscopy with biopsies of nasopharynx, vallecula and pyriform sinus and ipsilateral tonsillectomy. The primary source was identified in 23% of the series. All squamous cell carcinoma were treated with radical neck surgery plus radiotherapy and the undifferentiated carcinoma with chemotherapy and radiotherapy. The overall survival was 64% at 3 years. The N-stage affected the survival rate.

[Prognostic factors influencing anatomic and functional outcome in myringoplasty].

The objectives of myringoplasty are essentially two: restore the eardrum integrity and improve hearing level. We have carried out a retrospective study of myringoplasties performed in our Department, between 1996 to 2000, corresponding to 197 surgical interventions, in 162 patients. The characteristics more frequently seen for operating ears were central localization of tympanic membrane perforation, dry ear, good eustaquian tube function and underlay technique placing fascia graft. Endaural or retroauricular approach were similar in number. We obtained 73.6% of anatomic success, though in some cases we needed to reoperate the patient. When we analysed risk factors of anatomic success, we noticed than only the tympanic perforation site had influence, with better rate in posterior perforations and lower rate in subtotal perforations. We analysed functional results in ears with anatomic success, finding 75.6% of hearing level improvement with a gap of less than 20 dB. Factors influencing hearing improvement were surgeon experience and the presence of middle ear alterations.

[Can we computerize our archive of clinical histories?].

Case-histories have unquestionable legal importance, as it is shown by High Court Jurisprudence. This is due to the fact that it must report all the patient's process and disease, being thus the relationship doctor-patient completely transcribed (or at least all its relevant issues). So, in case of accusation, the most important evidence deciding doctor's sentence or acquittal will be within case-history. That is why the sentence "Case-history can be doctor's best allied, but also doctor's worst enemy" has been coined. In this setting one can understand perfectly how important the issue posed by this work is, since nowadays computers development offers to us a great number of advantages when collecting case-histories. This has made many health workers- and hospitals-think about keeping them in software. Actually, many of them have already been doing it for quite a long time. For this reason we are going to analyse this topic from a legal point of view, since legal standards provided by Law must be followed: preserve citizens' right to privacy and confidentiality. Security guidelines provided also by Law must be observed, which will allow to guarantee who the author of the document is (authentication) and that there has not been any later manipulation of data (integrity).

[Practical considerations of surgical treatment of otosclerosis].

The ENT-specialist after determine the diagnosis, the deafness degree and the requirements of each otosclerotic individual must consider the therapeutic possibilities and the end results, also the complications, the indications and the contraindications. These considerations are fundamental under the forensic medicine viewpoint and a good medical practice and even in order to avoid failures provided preestablished prospectives and the outcomes fails to adhere to the proposed schedule. Surgical option is promising in cases of severe deafness when our aim is to improve the enlargement and recognition of environmental sounds when an acoustic prosthesis is not employed. In these cases possibly recruitment is present at the initial stages, which requires and adaptation period. Also must be analyzed the influence for surgery of environment factors and life-style (pressure changes, ambient noise, etc.), the existence of tinnitus, equilibrium disorders, hard of hearing features and the age. Results procured with surgery in unilateral otosclerosis are lesser compared with bilateral, which in association with the fact that in the majority od unilateral the degree of impairment (percent of binaural deficit) is minimal, compel us to be very cautions in counseling these patients.

[Otosclerosis surgery: hearing results and complications].

A retrospective study was made of 183 stapedectomies performed at the Santiago de Compostela Hospital from 1989 to 1996. Sixty-seven percent (67.3%) of patients were women and 32.7% were men. Good results were obtained in 87% of cases (79% had an air-bone gap closure difference under 10 dB, and 8% 10-20 dB). Ten pefect had not change in air threshold and the rest deteriorated. There were no significant differences between stapedectomy and stapedotomy. The 4. 000-Hz improvement obtained was inferior to those in 500, 1,000 and 2,000 Hz. In 8.7% of patients with obliterated footplate, heaving outcome was significantly worse. In patients with exposed or overhanging facial nerve, floating footplate, middle ear fibrosis, and necrosis of the long incus process had better results. Total sensorineural hearing loss occurred in 1.1% of cases.

[Treatment of epidermoid carcinoma of the oral cavity and oropharynx: our result (1991-1995)].

In a study of 79 patients diagnosed as squamous cell carcinoma of the oropharynx and oral cavity, we compared the results obtained with two different types of initial treatment (surgery versus irradiation). Patients were grouped by tumor size: small tumors (T1 and T2) and bigger tumors (T3 and T4). Thirty-one patients had T1-T2 tumors:17 treated surgically with an 83% cure rate, 14 treated by irradiation with a 71% cure rate. Forty-three patients had T3-T4 tumors: 38 treated by irradiation with an 32.5% cure rate and 5 treated by surgery, with a 25% cure rate.

The relationship between survival and cell proliferation index in squamous cell carcinoma of larynx.

Cell proliferation index is one of the most important factors in assessment of growth and progression of tumours. At present, the most suitable method of determining cell proliferation is by means of immunohistochemical techniques which use the monoclonal anti-body MIB-1 which reacts against Ki-67, a protein released only in the proliferation phases of the cell cycle. In this study, 44 patients diagnosed with squamous cell carcinoma of the larynx were analysed retrospectively and different macroscopic and microscopic parameters were assessed, such as MIB-1 in relation to the current status of the patient. A statistically significant relationship was found between the cell proliferation index, the histological grade of differentiation and the clinical evolution of the illness.

[Epidemiologic and descriptive study covering one year of consultation in otoneurology department].

The spread on Otoneurologic Units are increasing in number, because of the specific pathologic subjects it deals. But there are scanty epidemiologic studies on patients concurrent to these Unities. We report the data from a descriptive statistic pertaining to the Otoneurological Unit of our Hospital, in the wholly 1996. During this period of time 914 patients were attended, middle-aged 49.55, and the relation men/women being 0.61. The majority housewives, living in rural districts, prevailing the married women patients. The two ways of sanitary access more employed in so cases were: the otologist of the Social Security or reporting to the Urgency-Units of Hospitals. And the more complaining trouble was imbalance. Diagnosis more repeated was labyrinthine pathology due to ischemia and secondary vestibular conditions because cervical muscles contractures. We consider the profile of our patient group enhances the special risk in order to endure otoneurological pathology. And also we point out the fact that the most diagnosis are not related specially to vestibular pathology, but to other clinical entities not directly linked to an Otoneurologic dispensary.

[Classic Kaposi's sarcoma in the ORL area].

Kaposi's Sarcoma is a malignant neoplasty originated in vascular endothelial cells. Such medical case appears as its classical type located in the tongue and inferior limbs. The therapy consisted of intralesional injections of interferon alfa, leading into better clinic and macroscopic conditions of the injuries.
