[Prognostic value of the stapedius muscle and electroneurography in facial paralysis a frigore, or Bell's palsy].

A retrospective study was made of 170 patients diagnosed as Bell's palsy in a 7-year period (August 1990 to April 1997). We evaluated two prognostic factors: stapedial reflex and electroneurography findings. The condition was classified by nerve affection into three types: less than 10%, 10 to 30%, and over 30%. The evolution of our patients was: complete recovery in 86.4%, slight impairment in 2.9%, major impairment in 3.5%, and unknown in 7%. The stapedial reflex was present in 87 patients, absent then present in 26 patients, and absent in 5 patients. Electroneurography revealed less than 10% nerve involvement in 5 patients, 10-30% in 18 patients, and over 30% in 63 patients.

[Informed consent in ear, nose and throat practice].

Changes in society's mentality have made health professionals get involved in legal aspects. In order to perform a correct medical proceeding, two premises must be fulfilled (Spanish Health General Law, 25th April 1986). First that it is carried out according to lex artis and second patient's consent. Consent needs as a prerequisite the adequate information; it is temporal, it can be revoked and it must be freely given and in writing (although it can be guaranteed by other means). Consent includes, on the patient's side, his/her acceptance of those risks inherent to the treatment. Nevertheless, it does not assure its results since doctor-patient's agreement is just related to means. If it were necessary, it is the doctor the one responsible to prove the consent was appropriately given. This would not be necessary when there is any kind of risk for public health or when the urgency allows no delay. Likewise, the assumption of the patient being under age or not able to take any decision will also be analysed. Features to be fulfilled by informed consent models are also analysed and a prototype is proposed.

Epidemiological study of salivary gland tumours.

Tumours located in the salivary glands form the most heterogeneous group in all human oncological pathology. They show various epidemiological, clinical and evolutionary characteristics which separate them from other neoplasms of the head and neck. In this paper, we have carried out a study on their epidemiological aspects, collecting 80 cases diagnosed in the ENT Service of the University Hospital Complex of Santiago over 17 years. The incidence was 1.22 cases per 100,000 inhabitants per year. The frequency was higher in males (58.75%) and in the 7th decade of age. A predominance was noticed in females under 40 years of age and in males over this age, but the differences were not statistically significant. The most frequent site was the parotid gland, and we could not find any case in the sublingual gland. In 52.5% of cases the tumour was benign, pleomorphic adenoma being the most prevalent. Among malignant tumours, the epidermoid carcinoma stood out in our series. The prevalence of benign tumours in females and of malignant tumours in males was clear, with significant differences. We compare our results with the data published in the literature.

[Tympanic chemodectoma. A report of 2 cases].

Report of 2 chemodectoma cases diagnosed in the ENT-Department (Prof. Labella) at C.H.U.S. The condition could be suspected because a tinnitus aurium and the transtympanic reddish image by otoscopy. And diagnosis settled through imaging techniques (computed tomography, magnetic resonance and selective angiography). Surgical treatment.

[Verrucous carcinoma of the larynx. An analysis of 20 years of experience].

Retrospective study about 10 verrucous carcinomas of the larynx surgically treated in a 20-year-term. This variety accounted por the 1.9 percent of the cancers seen in that period of time. Tabagism and alcoholism predominated in 8 and 6 patients respectively. Glottis was the localization and dysphony the paramount symptom. In the paper are emphasized the most important histologic features. Koilocytosis was present in 6 cases. Four patients developed a second tumor of epidermoid carcinoma type. No one exitus due to the verrucous growth. Only in 3 the death was attributed to the second malignancy.

[The detection of human papillomavirus in papillomas of the larynx and tonsils through immunohistochemistry and DNA in situ hybridization].

Presenting a study about Human Papillomavirus (HPV) determination by immunohistochemistry and "in situ" hybridization, in 25 samples of squamous cell papillomata (15 tonsilar and 10 laryngeal lesions). Five cases resulted positive for HPV: 2 of them for immunohistochemical probes, other 2 for "in situ" hybridization and only 1 cas showed its positivity for both techniques. All these samples belonging to laryngeal cases. No significant differences were found in antecedents, clinical and histological features related with results obtained. Two patients developed a laryngeal carcinoma after papilloma and they had positive "in situ" hybridization. Literature about diagnosis of HPV-infection was revised.

Waldeyer's ring lymphomas: an epidemiological study of 55 cases.

Waldeyer's ring is a relatively common location for non-Hodgkin's lymphoma. Here, we report an epidemiological study of Waldeyer's ring lymphomas based on data for 55 cases treated over a 19-year period at three centers in northern Spain. In most respects our results confirm those of previous studies. The average delay between first consultation and diagnosis was 3.8 months, and was longer for nasopharyngeal lymphomas than for tonsillar lymphomas.

Tumor-associated paralysis of the mastoid portion of the facial nerve: report of three unusual cases.

Tumors of the mastoid portion of the facial nerve are rare and often are misdiagnosed because they have subtle clinical manifestations. We have described three cases of tumor affecting the mastoid portion of the facial nerve seen in our hospital. In all cases, diagnosis proved problematic. Routine and careful use of imaging techniques and a certain amount of intuition are necessary to reach the correct diagnosis. Facial paralysis diagnosis requires histopathological examination following mastoidectomy.

Efficacy of prophylactic elective neck dissection in the treatment of cancer of the larynx.

The effectiveness of elective neck dissection for treatment of cancers of the head and neck is controversial. We report a restrospective study of 142 patients treated at our hospital between 1974 and 1994 for squamous cell carcinomas of the supraglottic larynx. At the time of surgery, none of the patients had palpable cervical adenopathy. All patients underwent tumour resection, while 55 additionally underwent functional elective neck dissection. Elective neck dissection had no significant effects on either the risk of subsequent development of ipsilateral cervical adenopathy or on survival. In view of these results, we do not consider elective neck dissection to be beneficial as a routine strategy for management of carcinomas affecting the supraglottic larynx.

[Wegener granulomatosis. 5 clinical case reports].

Wegener's granulomatosis has progressively acquired more significance from the otolaryngologist point of view. Its usual debut at ENT-territory, the occurrence of limited forms to the upper airway and the conviction that the nose is the best site for biopsies, makes the otolaryngologist the first line specialist for the actual diagnosis. We report the last 5 cases seen at our Department and furthermore review the nowadays knowledges on the Syndrome.

[Intracranial complications of otitis: the present-day situation].

The increasing number of intracranial complications of otic origin seen by the AA. in the previous 6 years, prompted them to make a survey of the respective files. In all, 9 patients were included in the group, with ages ranging from 18 to 72 years. Everyone showed marginal tympanic perforations with cholesteatoma. At their first visit to the office neurologic symptoms were present (mean time from beginning was 12 days) as an aftermath of chronic otitis. Meningitis, brain (temporo-parietal, temporal) and cerebellar abscesses of the implicated side were the diagnosis. Decisive for diagnosis was the CAT examination, being much better than the NMR (done once). Cases were managed in cooperation with the neurosurgical team. The mean time of hospitalization was 1 month. Eight cases did well (2 with neurologic sequels); the follow-up of the ninth patient is unknown.

[Results of tympanoplasty with antrum exclusion in the treatment of middle ear cholesteatoma].

INTRODUCTION: Exclusion of the antrum was designed to avoid the problem of mastoid ventilation by means of insufficient eustachian tube. MATERIAL AND METHODS: We retrospectively reviewed 72 ears with cholesteatoma who underwent antrum exclusion tympanoplasty (57% in men, mean age 40.9 years). Mean follow-up was 31.2 months (range, 12-64 mo). RESULTS: Exclusively epitympanic-antral cholesteatoma location (28.4%), and the necrosis of the long branch of the incus (54%) were the most common. The most used material to reconstruct the ossicular chain was Torp prosthesis (73.6%), followed by incus autograft (11.1%). We reported the following complications: postoperatory otorrhea (5.5%), perforation of neotympanum (16.7%), prosthesis extrusion (23.6%, time average of 15.6 months), and recurrence of cholesteatoma (4.2%). DISCUSSION/CONCLUSION: The technique with antrum exclusion has reported acceptable results in the control cholesteatoma recidivism. Although the functional results are similar to those of the open techniques, the resulting cavity with the antroexclusion is more anatomic.

[The usefulness of craniocorpography in the diagnosis of patients with dizziness and increasing muscle tension in the neck].

By the moment, craniocorpographic record of the Romberg test has been scarcely used in clinical practice. Since craniocorpography is a method for recording oscillations of the head and shoulders, it can show characteristic patterns in patients with vertigo caused by increased muscle tension in the neck. In order to confirm this hypothesis, a prospective comparative study was performed with 329 patients. They were included in three different groups: patients with cervical vertigo, patients with vestibular pathology and a healthy control group. Movements of the head (with static shoulders) are more usual in patients with cervical vertigo (67%) than in those with vestibular pathology (27%) and healthy subjects (31%). With regard to the kind of head oscillations, front-back flexions and oscillations in all directions were the most frequent (36% and 34%, respectively). Nevertheless, the only movements previously reported (cervical rotation or torsion) only were 3% of total head movements in our study. We concluded that static craniocorpography is an useful method in diagnosing vertigo secondary to increased muscle tension in the neck.

[Cell proliferation in salivary gland tumors].

Previous studies on cell proliferation in salivary gland tumors have shown the utility of immunostain with MIB1 in the differential diagnosis and prognosis of these neoplasms. We have carried out a study of 39 salivary gland tumors (17 benign), from different histological lineages. The immunocytochemical method used was the streptavidin--biotin--peroxidase complex which used the MIB1 monoclonal antibody. Benign tumors showed a low cell proliferation rates, below 5% with an overall average of 1.9%. The malignant tumors presented higher rates, with a middle value of 17.85%. Epidermoid carcinomas had the higher cell proliferation rates, with an average of 43%. In adenoid cystic carcinomas, we have observed that proliferation was greater at the peripheral level of tumor nests and cell surrounding the cystic structures. Neoplasms of low grade of malignancy presented lower cell proliferation rates. The MIB1 immunostain allowed to reach a differential diagnosis between pleomorphic adenoma and adenoid cystic carcinoma, specially in those cases in which there could be any doubt.

[Asymptomatic acoustic neurinoma associated with hypophyseal macroadenoma].

UNLABELLED: The widespread use of gadolinium-enhanced MRI (Gd-MRI) in our hospitals has supposed an increase in detection of asymptomatic acoustic neuromas (AN) as incidental finding. Concurrent primary intracranial tumors of different cell types are extremely uncommon. CLINICAL CASE: A 23 year-old woman complained of irregular menstrual cycles and galactorrhea for two years. A Gd-MRI scan showed a pituitary macroadenoma and an AN of 1.4 cm. with intracanalicular and cisternal extension. The patient did not have any otologic or neuro-otologic symptoms or signs. Audiograms revealed bilateral normal hearing. The results of acoustic reflex test and electronystagmographic caloric tests were normal. The AN underwent radiosurgery treatment. DISCUSSION: The literature about atypical AN is reviewed, giving a particular concern on asymptomatic cases. We only have found one case of association between AN and pituitary adenoma. The real prevalence of AN in general population is discussed.

[Possible factors influencing rehabilitation of the total laryngectomy patient using esophageal speech].

One of the handicaps of patients that have had a total laryngectomy is the loss of oral speech. There are three possibilities to rehabilitate these patients; surgery, voice prosthesis or esophageal speech. This last one appears as an inexpensive, non-invasive rehabilitation method; it does not need a complex learning and in spite of new technics, it continues to be an effective method in the social rehabilitation of laryngectomized patients. We have made a retrospective study of patients who underwent TL between 1992-1998 and that were rehabilitated by esophageal voice learning. Three factors have been evaluated: first, those related to the patient (gender, age, study level...); second, those related to the treatment itself [TL, pharyngolaryngectomy (PTL), TL plus radiotherapy...] and third, those related to the rehabilitation (technics, delayed on starting, number of sessions...). Data were compared with rehabilitation outcome: good (usually using esophageal voice), medium (speaking sometimes) and bad (not speaking). 74.1% had an acceptable outcome and they use esophageal voice with more or less difficulty to communicate, and this outcome increases to 95.8% when we do not take into account 7 patients who gave-up rehabilitation before finishing the first five sessions.

[Historical analysis of otosclerosis surgery].

In the present work main surgical techniques used along the history to solve hypoacusis caused by otosclerosis (opening of the timpani box internal wall, < >, puncture of cephalorachidium liquid, etc.) are studied. Those which have been more spread due to their results are analyzed in more depth (fenestration of the horizontal semicircular ductus, stapes mobilization and stapedectomy).
