[Basic studies on ultrasonic surgery. I. Principles, status and perspectives of ultrasonic surgery].

The ultrasonic power with regard to its medical use for resection, treatment, and joining of soft and hard biologic tissues is investigated after the formation of the ultrasonic diagnostics and therapy. The situation of the ultrasonic surgery as an unconventional method for the ingenious complementation of tested operative techniques is valued after the representation of the mode of operation and the present equipment. The trend of development and prospective problems of the research are deduced from them.

[Basic studies on ultrasonic surgery. II. Studies on the stability after ultrasonic osteosynthesis in various forms of fractures in vitro].

Investigations on the load capacity of an ultrasonic weld in transverse fractures, oblique fractures, and multiple fragment fractures by means of homologous bone fragments and Fimomed were taken up in vitro on 84 human femur diaphyses. It was possible to demonstrate the essential influence of the form of the fracture on the stability of the weld. The mechanical density was the highest in transverse fractures, small in oblique fractures, and the smallest in multiple fragment fractures. Conclusions were drawn up for the clinical use.

Regulation of pancreatic and gallbladder function by absorption of intrajejunal phenylalanine in man.

The effects of varying loads of intraduodenal phenylalanine on pancreatic and gallbladder function were investigated in 32 healthy volunteers using a triple lumen perfusion system. L-phenylalanine absorption rates in the proximal jejunum were measured simultaneously. Intraduodenal L-phenylalanine produced a dose related increase in pancreatic secretion reaching the maximum at the concentration of 100 mM. Bilirubin outputs too increased significantly but did not parallel enzyme secretion. In contrast to the effect of L-phenylalanine, D-phenylalanine 50 mM did not stimulate pancreatic enzyme secretion and gallbladder emptying. The absorption rates of L-phenylalanine increased with rising luminal concentrations. There was a significant correlation between pancreatic secretion and L-phenylalanine absorption rates. This relationship suggests that the release of cholecystokinin, a hormone stimulating pancreatic and biliary function is dependant of the absorption of nutrients like amino acids.

A highly sensitive and precise radioenzymatic assay for plasma epinephrine and norepinephrine.

A modification of the procedures for simultaneous radiometric determinations of plasma epinephrine (E) and norepinephrine (NE) was developed. High performance liquid chromatography (HPLC) is excellently suited to separate the radioactive products of the enzymatic methylation of catecholamines. The time-saving procedure shows significant advantages as against the conventional method of isolating normetanephrine and metanephrine by thin layer chromatography. The high sensitivity (detection limit = 1 pg), specifity, and good precision (cv = 6%) of this assay are demonstrated.

[Enflurane anaesthesia and plasma cortisol (author's transl)].

The influence of enflurane on adrenocortical function was investigated by determining the concentration of plasma cortisol by a radioimmunoassay during general anaesthesia and abdominal operations. Thirty minutes after the start of anaesthesia with enflurane 0.8--2.0%, N2O:O2 (2:1) and pancuronium, the plasma cortisol decreased slightly in the preoperative period; it increased markedly and continuously during the intra- and postoperative period. There was no correlation between plasma concentrations of enflurane, determined by gas chromatography, and cortisol.
