Conservative surgery for regional lymphadenectomy in the treatment of early gastric carcinoma.

The relationship between lymph node metastases and postoperative prognosis in 209 patients with early gastric cancer was studied. As to the postoperative prognosis in relation to the extent of lymph nodes dissection, no significant difference was observed among the age-corrected 5-year survival rates following three surgical procedures in patients with early gastric cancer. Age-corrected 5-year survival rates were 0.92 +/- 0.44 R1-resection, 0.95 +/- 0.44 in R2-resection, and 1.00 +/- 0.06 in R3-resection, respectively. In addition, in 71 patients including 33 with early gastric cancer and 38 patients with advanced but relative early gastric carcinoma, the relationship between the immunostatus and postoperative prognosis was investigated. Postoperative age-corrected 5-year survival rate (0.904 +/- 1.153) of the optimal responders with good immunostatus was significantly higher than that (0.582 +/- 1.153) of the suboptimal responders with impaired immunostatus (P less than 0.01). Thus, conservative surgery for regional lymphadenectomy may be an effective procedure for cure of early gastric carcinoma, particularly in cases of a carcinoma limited to the mucosal area of the stomach.

Identification and properties of peptidylarginine deiminase from rabbit skeletal muscle.

Peptidylarginine deiminase, which catalyzes the deimination of arginyl residues in protein, was identified and partially purified from rabbit skeletal muscle. The specific activity of the crude extract from the muscle was aboutt 120 times that of the extract from newborn rat epidermis. The enzyme was purified abou 250-fold over the initial extract of rabbit skeletal muscle in a yield of about 53%. The enzyme requires Ca2+ as an essential cofactor and the activity was not inhibited by monoiodoacetate or PCMB. The molecular weight of the enzyme was found to be about 115,000 by gel chromatography on Bio-Gel P-300. The enzyme catalyzed the deimination of arginyl residue when both the alpha-amino and alpha-carboxyl groups were substituted, and showed low activity when substrates had either a free alpha-amino or a free alpha-carboxyl group. The action of the enzyme on free L-arginine was negligible. Proteins such as protamine, histone, and ribonuclease A were better substrates than the other small synthetic peptides tested.

Electrocochleographic study of summating potentials in Menière's disease.

(1) The polarity of summating potentials (SP) and the SP/AP ratio in Menière's disease depend on the electrode position in the tympanic cavity, on the interval between sound stimuli and the method of recording, whether it is by transtympanic or extratympanic recording. (2) In a series of 42 patients with unilateral Menière's disease abnormal SPs were not very closely related to the time elapsed since the onset of vertigo, audiometric pattern, or the results of the glycerol test. (3) It was not possible to differentiate Menière's disease from sudden deafness on the basis of large SP/AP ratios alone. Such ratios were found frequently in both diseases. (4) Recovery or progression of hearing impairment was not always related to changes in the SP. (5) Positive SPs were seen more often in Menière's disease than in other types of sensorineural hearing loss.

[Follow-up study of preoperative oral administration of an antineoplastic agent as an adjuvant chemotherapy in stomach cancer].

Based on the propensity of fat emulsion to be absorbed mainly into lymphatic capillaries and regional lymph nodes, preoperative oral administration of 5-FU emulsion was attempted as an adjuvant chemotherapy to surgery for gastric carcinoma. In our previous studies, it was demonstrated that the mean 5-FU level in the regional lymph nodes was higher in patients who received the 5-FU solution. Since 1974, we have administered 5-FU emulsion preoperatively to 167 patients with gastric cancer (500 mg X 10 days) and examined histologically the effect of this regimen on the metastatic foci in the lymph nodes. A positive change, such as marked necrosis or marked degeneration, was found in 58% of the metastatic lesions. Sixty-four patients with advanced cancer who received the preoperative 5-FU emulsion also received a curative resection between 1974 to 1977 in addition to postoperative chemotherapy (MMC 40 mg and 5-FU more than 5000 mg) (Group A). Their survival rate was compared with that of the curatively operated advanced cancer patients from 1959 to 1973 who received the same postoperative chemotherapy only (Group B, N = 59) and with that of patients, from 1959 to 1970, who received no chemotherapy (Group C, N = 222). The 5 year survival rate of Group A was 0.53 +/- 0.07, which was higher than that (0.49 +/- 0.07) of Group B and that (0.40 +/- 0.10) of Group C. Comparing the 5-year survival rates of the 3 groups from several points of view, such as a stage of cancer progress absence of serosal invasion, the 5-year survival rate of group A was higher than that of other groups. Although these differences between Group A and B were not statistically significant, but those between Group A and B were significant. From these results it is suggested that preoperative oral 5-FU emulsion might be effective as an adjunct to surgery for gastric cancer.

Steroid-responsive bilateral sensorineural hearing loss and immune complexes.

We have recognized in recent years that some cases with idiopathic bilateral sensorineural hearing loss which showed acute progression in either ear responded to steroid and that the improved hearing level could be maintained by longterm administration of steroid only. Forty cases of bilateral sensorineural hearing loss were selected for study. In most of these cases hearing deterioration of either ear was confirmed by audiometry. Eight of fifteen cases with marked improvement have been proved to respond to steroid treatment. These eight cases include three cases with syphilitic deafness and one case with aortitis syndrome. In four other cases the causes are still unknown, but could be due to autoimmune mechanisms. In four of 25 cases the immune complexes (IC) value was higher than normal and three among them responded to steroid. Since a high IC value has been reported in lupus nephritis, immunologic examinations are necessary for patients over 30 years of age with bilateral sensorineural hearing loss.

Mite fauna and fungal flora in house dust from homes of asthmatic children.

Mite fauna and fungal flora in the house dust from homes of asthmatic children with positive and negative skin test to house dust allergen and non-asthmatic controls were examined. There was no conspicuous difference in mite species distribution among the three groups. Pyroglyphid mites dominate the mite fauna in house dust more than half of which being Dermatophagoides: D. pteronyssinus and D. farinae. There was no statistically significant difference in numbers between the two species and either species could dominate depending on the conditions of the individual houses. The average number of acarina in 0.5 g of fine dust did not differ statistically among the three groups; however, mite number per square meter floor differed between patients with positive skin test and negative skin test. The results suggest that house-cleaning might influence the possible sensitization of children. The genetic distribution of mould fungi in house dust was largely similar to that of airborne fungi. The average number of fungal colonies detected in 0.5 g of dust did not differ statistically among the three groups. Wallemia with its minute spores may cause sensitization but has so far been insufficiently investigated.

The effect of iodoacetamide-induced fundic ulcers on gastric carcinogenesis produced by N-methyl-N'-nitro-N-nitrosoguanidine in rats.

This study was undertaken to determine the effect of ulcer induced by iodoacetamide on the development of gastric carcinoma by N-methyl-N'-nitro-N-nitrosoguanidine in male Wistar rats. Fifty-six of the 62 ulcers induced by IAM were located in the fundic gland area along the limiting ridge. The incidence of fundic carcinoma was 16% in the groups treated with IAM and MNNG, while no fundic carcinoma was found in the group treated with MNNG alone. This difference was statistically significant. All the carcinomas in the fundic gland area were confined within the ulcer itself or its scar tissue, produced by IAM. These findings indicate that if an ulcer is present, carcinoma develops even in the fundic mucosa which is, if intact, resistant to the carcinogenic stimulation of MNNG. It was concluded that gastric ulcer predisposes the development of gastric carcinoma.

A new analgesic testing method using ultrasonic stimulation. I. Effects of narcotic and nonnarcotic analgesics.

A quantitative method for measuring pain threshold by the use of ultrasonic stimulation in mice has been designed. The method had the advantage of precision, simplicity of technique, rapidity of measurement, and the fact that the stimuli is innocuous upon repeated application. The nature of the senstaions induced by ultrasonic stimulus is somewhat like that felt with a prick type of pain. Pentazocine (30, 100, 150 mg/kg i.p.) aminopyrine (15,50, 100, 150 mg/kg i.p.), phenacetin (100,150, 200, 250 mg/kg i.p.) sodium salicylate (150, 200, 250 mg/kg i.p.) and other antipyretic analgesics were active in a wide range of doses indicating that this technique is sensitive to the narcotic antagonist and to the weak analgesics as well as to the narcotic analgesics as well as to the narcotic analgesics such as morphine (2.5, 5, 10, 15 mg/kg i.p.), codeine (10, 20, 25, 30, 50 mg/kg i.p.) and pethidine (5,10, 15, 20, 25 mg/kg i.p.). The ultrasonic method is, therefore, applicable in screening procedures when attempting to evaluate the analgesic potency of a wide variety of chemical agents.
