[The dispensary care of mental diseases with a long-term evolution].

It is the authors belief that mental diseases follow-up is essential for the outpatient care and a requisite to inpatient care. The 5-year follow up of group of patients with longstanding mental diseases is analysed from the viewpoint of nosological groups prevalence, age and sex distribution, family and professional integration and type of care. The study in dynamics of the incidence and prevalence shows an increase of followed up cases and underlines the usefulness of follow up actions in estimating both the morbidity and the possibilities of prophylactic and curative actions.

Sociopathy: genesis and development.

A complex study of a sample of 1,029 children and young people cared for in orphanages revealed the increased incidence of the disorders of behaviour in these categories (33%) with great difficulties of post-school integration (70%). Factors like: affective frustration, noxious family climate intellectual handicap, socializing defect, school failure, etc. are deeply involved in the genesis of juvenile maladjustment and it further on can be found within the nucleus of the sociopathic personality and in adult's deviance.

[The social aspect within the dimensions and exigencies of contemporary psychiatry].

The social psychiatry is a necessary medical dimension in our contemporary world. The analysis of the concepts fundamenting the social psychiatry draws the attention upon certain different considerations regarding its value, limits and methods of medical care delivery. In this respect the author comments on the contribution brought about by the School of Psychiatry of "Socola" in the sequel of several periods of time according to certain social mutations and the dynamics of health status of population. The author analyses within this context his own contribution to the competence of contemporary relational psychiatry for the present day problems of mental health and inherently the level and the forms of participation of population according to its structure and community motivations.

[Trends in the classification of depressive states].

The evolution of the clinical concept of depression from the viewpoint of actual epidemiological data and classification systems is presented. The classification difficulties, due to the reference criteria and absence of some concordant data, which lead to the elaboration of some systematizations prevalently syndromic, are revealed. The uncertainty of the nosological framing of the depressive states is reflected in the diagnostic disagreements of the various doctrinaire orientations in psychiatry. Starting from the Kraepelinian nosological conception, the revision of the dichotomy endogenic-psychogenic or the systematization in primary and secondary depressions, the I.N.S.E.R.M., I.C.D. 9 and I.C.D.-10 (WHO), D.S.M. III and D.S.M.III-R classification are discussed.

[Antidepressive medication in older patients with cardiovascular ailments].

The study of depression in the old-aged entails a thorough knowledge of the peculiarities of aging process in a somatopsychic correlation. The epidemiologic, etiopathogenic, semiologic, clinic and therapeutic aspects of depressive states in the old-aged are discussed. Caution is needed when administering antidepressants in the aged patients with cardiovascular affections, the organic, affections and side-effects restricting their use. The authors' experience in the clinical use of antidepressants underlines the need of a through somatic evaluation, by repeated biomedical investigations according to a well defined program, aimed to ensure a correct individualization of psychology and treatment duration, as well as of the treatment of choice in relation with this indications, counterindications and side-effects.

[Biological markers in the accurate diagnosis of chronic alcohol intoxication--the significance of immunoglobulin A].

This paper is part of a study aiming to identifying some objective clinical and laboratory markers for the late diagnosis of chronic ethylism, often after the somatic affections and mental disturbances have occurred, fact requiring further investigations for the early and doubtless determination of alcohol even after variable withdrawal periods. The significance of IgA as a marker is similar to that of gamma GT, demonstrating that in time and at doses in excess alcohol can lead to increased serum IgA concentrations, proved by the IgA deposits in the liver detected immunohistologically, values that could be diagnostic. It seems the IgA as a marker is not superior to gamma GT, its diagnostic relevance being partially restricted by it also being influenced directly by the severity of the histopathologic changes.

[Biological markers in the accurate diagnosis of chronic alcohol intoxication--the significance of metabolic disorders (I)].

Continuing the series of papers dedicated to determining some objective markers for chronic alcohol poisonings, this time we insist on the significance of metabolic disturbances in the series under study. The hepatic changes are useful in the diagnosis, in determining the consequences and in following up the therapeutical management. The study was based on the marked alterations in the lipid and protein metabolisms, these alterations being strongly connected with ethanol metabolization stages.

The authors review the historical development of the concept of "depression" and the terminology used by psychiatrists in the description and limitation of various forms of depression. They bring into relief the contribution brought about by the authors who delineated and classified the syndromes of depression, constituting in this way the historical evolution of the concept of depression.

[A glossary of the terms used in psychoanalysis].

The psychoanalytic direction in psychopathology is considered to be necessary as an explanation of concept and methodology for the medical training concerning the study and the care of mental diseases. Irrespective of certain controverted opinions on the fund of concepts of psychoanalysis the consideration of a strict terminology belonging to this direction is necessary. In this respect a concise explanation of the meanings of a set of terms in the comments on some psychopathological states is useful.

[Trends in the differential diagnosis of acute psychogenic states].

The approach of the concept of psychogenesis and its significances for the explanation of certain psychic disorders allows a clinical estimation of the positive and differential diagnosis of acute psychogenic states. The variety and polymorphism of the psychogenies, clinically expressed in variable degrees of conscience disorders as well as their frequency in the practice of general medicine underline the importance of the clinical examination, case history and usual paraclinical investigations. The estimation of symptomatology, motivation and the evolution of psychogenic states allows the avoidance of some severe diagnosis errors with tumours and cerebral vascular accidents, acute intoxications, comatose states or the debut of certain psychoses giving in this way the possibility of a correct directions in setting the diagnosis and therapy.

[Lithium carbonate (Contemnol--Spofa). A clinical study].

The estimation of the present day stage in the therapy with lithium salts allowed the authors some considerations on the mechanisms of action and pharmacodynamics of lithium ion, as well as on the indications and counterindications, side effects and therapeutical management. Thus, the authors have analysed the findings of a simple-blind clinical study on lithium carbonate (Contemnol--Spofa) in a series of 15 inpatients. The research methodology included the thorough examination of the patients, blood lithium screening, psychological investigations and laboratory tests on the basis of an itemized investigation card. The analysis of the results confirmed the efficacy of this substance in bipolar hyperthymic psychosyndromes, depressions and hyperthymic psychosyndromes with dissociative elements, its tolerance and low incidence of side effects. The selection of the cases, the clinical and paraclinical examinations, the observance of the indications and counterindications permit the administration of this substance as a curative and prophylactic treatment, providing the blood lithium is screened and clinical control and biological investigations are carried out.

[Biological markers in the accurate diagnosis of chronic alcohol intoxication. The significance of gamma-glutamyl transpeptidase].

The late diagnosis of severe alcoholism--often after the onset of somatic complaints and the appearance of psychic disorders needs extensive research able to identify new objective clinical and paraclinical markers to allow early and accurate identification of alcoholism also after various periods of abstinence. The scores obtained are the consequence of alcohol abuse and not of nutritional deficiencies or some other associated visceral-humoral sufferings. The authors underline the significance of gamma-GTP in assessing early severe alcoholism state and the necessity and importance of finding certain specific biological markers--early and sensitive for etillic intoxication--that would considerably simplify the manner of diagnosis and an efficient therapy. The identification of certain significant and specific markers before the onset of visceral-humoral disorders would improve the psychoprotective measures before the onset of tolerance and withdrawal--determinant factors of severe etillic intoxication.

[The psychoprophylactic importance of treatment with depot neuroleptics in schizophrenia].

The authors refer to the analysis of a sample of 104 schizophrenic inpatients treated with depot neuroleptics. The study relates to the duration of the illness, the number of admissions, social and professional integration and the patient's and family attitude to the long term treatment. The analysis and the interpretation of the data allow the estimation of the efficiency of the depot neuroleptics in outpatient schizophrenia reflected by social integration, the prophylaxis of recurrences and severe developments, the stabilization of symptoms and shortening of hospitalization span. The psychoprophylactic significances of the treatment with depot neuroleptics in schizophrenia confirms chemical correlations with pharmacological data and brings into relief the unsubstituting value of the fashion of psychoprophylactic and recovering care delivery on the background of existential relationships.

[Clinical observations on treatment with flufenazin dihydrochloride (Lyorodin--Jenapharm)].

According to the psycho-pharmacological characteristics, indications, counterindications and side-effects of fluophenazine dihydrochloride, the authors present the results of a simple-blind study of Lyorodin--Jenapharm in a series of 30 in-patients of both sexes, aged between 30 and 65 years. Nosologically, there were cases of schizophrenia and post-process states, delirious hallucinatory syndromes of involution and asthenodithymic syndromes. The drug under the form of pills of 1 and 4 mg was given at average doses of 2-4 mg up to 6-10 mg for 28 days under clinical surveillance and laboratory tests. The estimation of the clinical results revealed the improvement of the symptoms in all investigated cases, a favourable evolution, low incidence and intensity of its side-effects, good tolerance and clinical efficacy comparable to that of Lyogen.

[Comparative clinico-therapeutic study of amitriptyline hydrochloride (Tryptizol/amitriptyline)].

As compared to the psychopharmacological profile of amitriptyline hydrochloride the authors analyse the findings of a comparative clinical trial amitriptyline versus Tryptizol regarding their efficacy in neurotic and presenile depression. 170 inpatients divided into four comparative series were investigated. The drugs were administered according to the same scheme in monotherapy for 30 days. The efficiency and tolerance of these drugs were estimated through clinical observations, the recording of the first ameliorated state and the maximum one, psychological check-up by using Hamilton's scale for depressions, paraclinical investigation, recording of side effects, clinical and paraclinical screening for 0-10-20-30 days. The analysis of clinical findings in the investigated series reveals for both drugs their easy administration, tolerance, incidence and low intensity of the side effects. The comparative estimation of the treatment with amitriptyline and Tryptizol in neurotic and presenile depressions attests a similar therapeutic efficiency.

Adolescent
